MAXILLARY FLOATING TEETH IN A CHIARI MALFORMATION PATIENT (Chiari malformation 환아에서 상악 구치부의 부유치)

  • The Chiari malformation is a deformation within the central nervous system which the lower brain stem and the cerebellum migrate into the foramen magnum causing herniation. In 1891, Arnold Chiari classified such symptoms into 3 categories. This case report is of a 8-year-old female with the complaint of a slight facial swelling and pain on the upper right molar during tooth brushing since 10 days before. Clinical examination showed gingival pocket formation on distal of the upper right first molar with pain and mobility of the tooth. Radiographic examination showed generalized low bone density in the upper molar area, and especially no bone support above the upper right and left first molars were noted. With a temporary diagnosis of Early-onset periodontitis, consultations with medical doctors for the possibility of an underlying systemic disease were made during periodontal treatment. 3D CT was taken with after a final diagnosis of Chiari malformation. Generalized thinning and defect of the cranial bone was noted and the foramen magnum was slightly enlarged. The occipital and maxillary bone was low in density, and the alveolar bone of maxillary posterior teeth was especially almost non-existing causing the upper right and left first molar to be floating. For this, the patient went under consultation with the department of neurosurgery and is still under observation. Periodontitis in childreren is very rare. When symptoms of periodontitis appear in a child, due to the possibility of an underlying systemic disease such as leukemia, histiocytosis X, and hypophosphatasia, proper examinations should be carried out so that the primary factor the symptoms can be treated.

Stimulation of bone formation by direct electrical current in an orthopedically expanded suture in the rat (백서의 악정형적으로 확장된 봉합부에서 직류 전류 자극의 골형성 촉진에 관한 연구)

  • Objective: The aim of this experimental study was to evaluate the effects of direct electrical current stimulation (DECS) on bone regeneration in response to an expansion of the inter-premaxillary suture in the rat. Methods: Sixteen 50 - 60 days old Wistar male rats were separated into two equal groups (control and experimental). Both groups were subjected to expansion, and 30-gram of force was applied to the maxillary incisors with helical-spring. In the experimental group, two metallic-screws were placed at lateral parts of the maxillary segments. Electrodes were connected to the screws. The device was activated with current adjustment to measure $10{\mu}A$ continuously and the current was monitored daily during the expansion and early-retention phase. Bone regeneration in the sutural area was histomorphometrically evaluated including new-bone area (${\mu}m^2$), bone perimeter (${\mu}m$), feret's diameter (${\mu}m$) and newly formed bone (%) parameters. Kruskal-Wallis rank and Mann-Whitney U tests were used for statistical evaluation at p < 0.05 level. Results: Statistical analysis showed significant differences between groups for all investigated histomorphometric parameters. New bone area (p = 0.002), bone perimeter (p = 0.004), feret's diameter (p = 0.002) and newly formed bone percentage (p = 0.002) measurements were significantly higher in the experimental group than the control group. Bone histomorphometric measurements revealed that bone architecture in the DECS group was improved. Conclusions: The application of DECS to an orthopedically expanded inter-premaxillary suture area during the early retention phase stimulated the formation of new bone.

Maxillary protraction effects of TTBA (Tandem Traction Bow Appliance) therapy in Korean Class III children (한국인 성장기 III급 부정교합의 TTBA 치료 후 상악골 견인 효과)

  • Objective: The purpose of this study was to investigate the maxillary protraction effects of the Tandem Traction Bow Appliance (TTBA), a new appliance devised several years ago for the treatment of growing skeletal Class III patients. Methods: Participants were 88 Korean children (42 boys, 46 girls) with skeletal Class III malocclusion treated with TTBA at the orthodontic clinic of Ewha Womans University Mokdong Hospital. Mean age at the start of treatment was 7.5 years${\pm}$1.5 years. Mean treatment periods were $13{\pm}3$ months. Pretreatment and posttreatment lateral cephalograms were traced and superimposed by the same investigator and analyzed by modified McNamara analysis and pitchfork analysis. Changes were evaluated with paired t-tests at a significance level of p<0.05. Results: The maxilla and maxillary dentition moved forward. The mandible moved backward, although not significantly; and the mandibular dentition moved forward. The net dental changes combined with the apical base change resulted in a favorable total molar relationship correction. Net dental movement was 26% and the apical base change 74% of the total molar relationship correction. Conclusion: These results suggest that TTBA has a maxillary protraction effect that can be useful in the treatment of growing skeletal Class III malocclusion with maxillary deficiency.

  • The maxillary defects lead to anatomical and functional deformity of the maxillofacial region. As far as functions are concerned, such defects can produce difficulty in speech, mastication, and deglutition. Obturator prostheses play a very important role in functional recovery for post-maxillectomy patients. To achieve rigidity of the obturator, appropriate retention should be given, and the weight of the prosthesis be reduced. There are two types of hollow bulb obturator: open and closed. A closed type has many advantages. Nevertheless, some problems, including complexity of fabrication and water leakage into the bulb, have the closed hollow obturator not be widely used. The one-step polymerization technique described in this case overcomes the shortcomings by easily constructing a small hollow bulb with two thermoplastic resin sheets.

  • Patients with maxillectomy defects predisposed to not only difficulty in deglutition, mastication, speech but also psychological depression from impaired facial esthetics that affect life quality. Obturator prostheses play a important role in restoring the lost form, function and the quality of life for patients with maxillectomy defects. This clinical report presents the simplified approach to predict the degree of adequate facial support by Artificial palate which reflected from a maxillary interim obturator during the stabilization period after maxillectomy.

  • The facial asymmetries include maxillary, mandibular, and chin asymmetries, although the most common deformity is primarily in the mandible. Common causes of this type of asymmetry can include asymmetric growth of the condyle or the mandible. In these patients, the location of the Me would be deviated to the shorter side because of the asymmetric growth of the mandible, and, commonly, the maxillary occlusal plane would be tilted toward the deviated side because the maxilla likely grows asymmetrically according to the pattern of asymmetric mandibular growth. Three-dimensional CT images are ideal for evaluating the size and location of anatomic structures, and such reconstructed images allow the use of software that can show anatomic structures from numerous angles, allowing actual measurements of distances and angles without problems of magnification, distortion, or superimposition caused by 2-dimensional imaging. In the present study using 3D-CT imaging, the 8 parameters, including measurements of the upper midline deviation, maxillary canting in the canine and first molar regions, width of the upper arch, width of the mandible at the Go, vertical length of the ramus, inclination of the ramus, and deviation of the Me were easily measured. The dentition should be orthodontically decompensated and dental midline should ensure incisor midlines positioned in the midline of each jaw before surgical correction. Surgical correction could be considered such as canting or yawing correction in the frontal or horizontal aspect, respectively.

Treatment effects of the Teuscher appliance in skeletal Class II division 1 malocclusion (골격성 II급 1류 부정교합에서 Teuscher 장치의 치료효과)

  • Various methods have been used on patients with skeletal Class II division 1 malocclusion. The activator, Frankel appliance, headgear, Herbst appliance, and Twin-block appliance are some examples. The ideal treatment effect using these appliances would be to inhibit horizontal and vertical growth of the maxilla while promoting mandibular growth and obtaining optimum dentition. The Teuscher appliance has a simultaneous combined headgear effect with maxillary growth inhibition and an activator effect with mandibular growth promotion. The purpose of this study was to examine how well these effects were clinically obtained and the results are as follows. 1. The forward growth of the maxilla was effectively inhibited. 2. The downward-forward growth of the maxillary dentoalveolar complex was inhibited. 3. Growth promotion of the mandible was not observed. 4. The overjet, overbite, molar key were effectively improved. 5. The protruded upper lip and facial profile were unproved.

  • When restoring edentulous patients with lower complete denture, the smaller supportive and retentive area of mandible can lead to poor support and stability, denture dislodgement and pain resulting discomfort. In this situation, implant prosthesis can improve esthetics, stability and occlusal force. Whereas, patients with a upper complete denture can adjust more easier because of palate. Therefore, it is suggested to rehabilitate fully edentulous patients with lower implant-supported, upper complete denture as one of the treatment options. So, we are going to report the case and literature review about how the lower implant prosthesis opposing to upper complete denture affects the bone resorption of maxillary residual ridge.

TREATMENT OF CLASS III MALOCCLUSION BY ORTHOPEDIC & ORTHODONTIC APPLIANCE (III급 부정교합의 정형적.교정적 치료)

  • The conventional treatment of skeletal class III malocclusion has been focused on the application of orthopedic forrce primarily to the mandible. Thus moderate Class III malocclusions can be corrected by the anterior displacement of the maxilla and maxillary dentition, possibly by restricting the growth of the mandible or by changing its direction. The patients having skeletal Class III malocclusion were treated with removable appliance & fixed appliance for detailed tooth movement and the following results were observed: 1. The anterior crossbite was corrected. 2. The forward and downward growth of the maxillary complex was obtained. 3. The lingual tipping of the mandiblar incisors was performed and the mandible was rotated in the clockwise direction.

  • 치원성 각화 낭종의 임상적 방사선학적 양상을 연구하였다. 32명의 환자(21명의 남자, 11명의 여자)를 이 연구를 위하여 사용하였다. 환자들의 기록과 방사선사진을 통하여 성별 및 연령의 분포, 호발부의 방사선사진의 양상, 재발율 임상적 소견들을 조사하였던 바 아래와 같은 결과를 얻었다. 1. 남성이 여성에 비해 약 2배 호발하였고, 10대와 20대에서 가장 많이 발생하였다. 2. 호발부위는 하악의 구치에서 하악지에 이르는 부위였고 하악골이 상악골에 비해 호발하였다. 3. 원기성 낭종에서 각화성 변화를 일으킨 것이 가장 많았다. 4. 치근의 흡수를 많이 보였으며, 하악골에서 경계는 원형과 부채형(scalloped)이 거의 동등히 나타났고 상악골에서는 대부분 원형의 단발형 낭종이었다. 5. 높은 재발율을 보였다. 6. 감염이 되어 있는 경우가 많았다.
