  • Talon cusp is cusp-like structure that develops additionally in crown of anterior tooth. And it is one of dental anomalies. Talon cusp can cause various diagnostic, functional, aesthetic problems depending on the size and configuration of cusp. there is a tendency for caries to occur in the developmental grooves, and advanced attrition, periodontal problems, irritation the tongue and temporomandibular pain, occlusal interference and displacement of affected tooth may result. Therefore, early diagnosis and appropriate treatment of each case is important so that it minimize local problems, such as caries, periodontal diseases, and malocclusion. These cases which are in permanent and primary anterior teeth are about gradual reduction forming reparative dentine and complete reduction of talon cusp and root canal therapy which is an alternative and effective form of treatment when gradual reduction of talon cusp may not be possible.

  • The purpose of this study was to investigate the effect of salivary gland on the calcification of dentin in rats. 80 Sprague-Dawley male rats that weighed approximately 120gm were used in this study. 5 rats among them were shared as controls. 75 rats received sialoadenectomy were divided into submaxillary adenectomy group, parotidectomy group, and submaxillary-parotid gland combined removal group. In experimental groups, 25 rats in each of the 3 groups were sacrificed at the following intervals; 3 days, 1, 2, 3 and 4 weeks. All animals were sacrificed by vascular perfusion with 10% formalin. The maxillary incisors including periapical tissues were removed and defatted in 20% KOH solution at $0^{\circ}C$ for 24 hours, and dehydrated with acetone. Each tooth specimen was attached on the stab for scanning electron microscopic study. Gold was coated on the each specimen in the thickness of 300${\AA}$ at D.C. 1400V, 6mA for 6 minutes with coating machine (Eiko IB-3). Inner dentinal surfaces of the specimens were observed with SEM (Hitachi S-450). The results were as follows, 1. Parotidectomy groups were found to be inhibited the formation of dentinal calcification compared to submaxillary adenectomy groups in the eady stages. 2. Combined removal of submaxillary and parotid gland was appeared to cause more severe inhibition effect on the dentinal calcification than that of each salivary gland separately. 3. Inhibition of the calcification and mineralization of dentin caused by sialoadenectomy was more extreme from 3 day to 2 weeks after beginning of the experiments. However it was tended to be normalized after that. 4. Salivary gland was responsible for alterations in calcification and mineralization of dentinal growth.

COMPARISON OF BOND STRENGTH OF A FIBER POST CEMENTED WITH VARIOUS RESIN CEMENTS (다양한 레진시멘트로 합착한 섬유포스트의 결합강도 비교)

  • The purpose of this study was to compare the push-out strength of a fiber post cemented with various resin cements. Newly extracted 36 human mandibular premolars which had single root canal were selected and their crown portions were removed. The root canal was instrumented using $PROTAPER^{TM}$ system and obturated using continuous wave technique. In each root, a 9-mm deep post space was prepared. #2 translucent fiber post (DT Light post, Bisco Inc., Schaumburg, IL, U.S.A.) was cemented using injection technique with Uni-dose needle tip (Bisco) and six different resin cements. The tested resin cements were Duo-Link (Bisco Inc., Schaumburg, IL, U.S.A.), Variolink II (Ivoclar-Vivadent AG, Schann, Liechtenstein), Panavia F (Kuraray Medical Inc., Okayama, Japan), Multilink Automix (Ivoclar-Vivadent AG, Schann, Liechtenstein), RelyX Unicem (3M ESPE Dental Products, St. Paul, MN, U.S.A.), and Maxcem (Kerr Co., CA, U.S.A.). After storage in distilled water for 24 hours, each root was transversally sectioned into approximately 1-mm thick sections. This procedure resulted in 6 serial sections per root. Push-out test wasperformed using a universal testing machine (EZ Test, Shimadzu Co.) with a crosshead speed of 1 mm/min. The data were analyzed with one-way ANOVA and Tukey HSD (p=0.05). The push-out strength of the groups which cemented fiber post with Panavia F and Multilink Automix were lower than those of the other groups. But, there were no statistically significant difference among groups at a probability level of 0.05.

A BIOACTIVITY STUDY OF PORTLAND CEMENT MIXED WITH β-GLYCEROPHOSPHATE ON HUMAN PULP CELL (β-glycerophosphate 혼합시 인간 치수 세포에 대한 Portland cement의 생활성에 관한 연구)

  • The purpose of this study is to investigate the response of human pulp cell on Portland cement mixed with $\beta$-glycerophosphate. To investigate the effect of $\beta$-glycerophosphate and/or dexamethasone on human pulp cell, ALP activity on various concentration of $\beta$-glycerophosphate and dexamethasone was measured and mineral nodule of human pulp cell was stained with Alizarin red S. MTS assay and ALP activity of human pulp cell on Portland cement mixed with various concentration of $\beta$-glycerophosphate (10 mM, 100mM, 1M) was measured and the specimens were examined under SEM. Addition of $\beta$-glycerophosphate or dexamethasone alone had no effect however, the addition of 5 mM $\beta$-glycerophosphate and 100 nM dexamethasone had the largest increasement in ALP activity. There was no toxicity in all samples and the data showed that Portland cement mixed with 10 mM $\beta$-glycerophosphate had more increase in ALP activity compared with control. In conclusion, Portland cement mixed with $\beta$-glycerophosphate has no toxicity and promotes differentiation and mineralization of pulp cell compared with additive-free Portland cement. This implicated that application of Portland cement mixed with $\beta$-glycerophosphate might form more reparative dentin and in turn it would bring direct pulp capping to success.

PERIPHERAL ODONTOMA IN A CHILD: CASE REPORT (소아에서 발생한 변연성 치아종의 증례보고)

  • The odontoma is a hamartoma of odontogenic origin and consists of enamel, dentin, cementum and pulp tissue. Two types of odonotma are histologically recognized: complex and compound. Odontoma may be the cause of noneruptoin or impaction of teeth, formation of cyst and resorption of adjacent bone. The recommended treatment for an odontoma is conservative surgical excision. No propensity for recurrence has been noted. Peripheral odontoma is relatively rare lesion and shows the histological characteristics of an intraosseous odontoma. This report described a 3-year-old Korean girl with a firm gingival mass in the posterior mandibular area, which had been gradually enlarging over 18months. Radiographic examination showed a radiopaque mass but no evidence of underlying intraosseous lesion. Excisional biopsy was performed for the mass removal and histopathological examination. The result of histopathological study was identified as hamartous supernumerary tooth germ. Based on the clinical presentation, radiographic and histopathologic findings, we concluded that the final diagnosis was peripheral odontoma. For three years after surgery, there is no clinical sign of recurrence.

  • Dens invaginatus is a rare malformation resulting from invagination of the enamel before calcification has occurred. It is mostly found in permanent maxillary lateral incisors and mandibular teeth are rarely affected by this anomaly. The malformation is estimated to affect between 0.04 % and 10 % of people and has been associated with other abnormalities such as taurodontism, microdontia, gemination and dentinogenesis imperfecta. Dens invaginatus is classified in three types with respect to the depth of invaginatus and has a broad spectrum of morphologic variations. Invagination frequently allows the entry of irritants and microorganism, which usually lead to caries, pulp infection and pulp necrosis. Root canal treatment on such invaginatus tooth may present severe problems because of its complex anatomy of the tooth. Therefore, the early diagnosis of such malformation is crucial and preventive approach is strongly recommended.

DIFFERENCE OF CALCIUM FLUORIDE FORMATION BETWEEN THE ENAMEL AND DENTIN AFTER FLUORIDE APPLICATION IN VITRO (불소적용시 법랑질과 상아질에서 불화칼슘형성의 차이에 관한 실험적 연구)

  • The purpose of this study was to compare the amount of calcium fluoride deposited on the enamel and dentin surface and to obtain information on the morphological change and crystallographic details of mineral deposition after 12,000ppm APF application in vitro. The bovine enamel and dentin blocks were randomly assigned to eight groups according to artificial caries lesion formation and difference of fluoride application time. The fluoride concentration and morphological characteristics on the treated enamel and dentin surface were investigated by using fluoride quantitative analysis and SEM. The powdered enamel and dentin of the intact bovine incisors were prepared for the X-ray diffraction analysis. The following results were obtained. 1. The amounts of KOH-soluble fluoride on the carious enamel and dentin surface after 24h APF application were higher than after only 5min APF application(p<0.05), but in the case of the sound enamel and dentin surface were similar after 5min and 24h application (P>0.05). The fluoride content was highly increased in the carious dentin as compared with sound dentin after APF application(P<0.05). 2. The carious enamel surface after APF application, the demineralized enamel surface were recovered a more dense enamel surface and precipitation of crystal was observed a distintive surface layer of spherical globules of about 1 m diameter. In the case of the fluorided carious dentin surface, precipitation of calcium fluoride-like material was deposited both inside the dentinal tubules as well as in the intertubular regions. 3. The crystallographic structure of powdered enamel and dentin after 24h APF application had large crystallities of apatite and CaF2 diffraction peaks in the enamel as compared with dentin. The diffraction data collected from the 27.50-29.50(2) angular range of the powdered enamel, the (105) apatite, (225) apatite and (111) CaF2 peaks of the enamel crystallities were detected after 24h APF application.

  • Traumatic injuries in the young peranent dentition are common, but root fractures, defined as fractures in volving dentin, cementum and pulp, are relatively uncommon. Case 1 is a 9-year-old boy who had a horizontal root fracture of his maxillary right central incisor in the apical third. Root canal therapy was performed in coronal segment and calcium hydroxide therapy was initiated. Six months after treatment, a periapical radiograph showed calcific tissue formation and normal root development. 1 year and 3 months later, the canal was permanently obturated with gutta-percha. Case 2 is a 7-year-old girl who had a vertical root fracture of her maxillary right central incisor. Fractured tooth was intentionally extracted atraumatically, and then the separated fragments are bonded with resin cement. the restored tooth was replanted into the original socket. Recalls up to 8 months showed normal mobility and no periapical pathosis. In these cases, we performed conservative treatment. Clinical and radiographic examination showed no pathosis or abnormality of the teeth and periodontal tissue.

  • The purpose of the present study was to evaluate the patterns of hybrid layer according to the application time and the frequency and its effects on the shear bonding strength of All-In-One system in primary teeth. A single bonding agent(Scotchbond Multi-Purpose Plus, 3M) and an All-In-One system(Prompt L-pop, 3M ESPE) were applied on the dentin varying the application time and the frequency in primary teeth. Shear bond strength was measured and the patterns of hybrid layers were observed by SEM. The following results were obtained ; 1. The shear bonding strength of single bonding agent was significantly higher than that of All-In-One system(P<0.05). 2. The shear bonding strength of All-In-One system applied twice or 3 times were higher than that of applied once (P<0.05). And thickness of the hybrid layer was increased when applied twice or 3 times compared to once. 3. The shear bonding strength of All-In-One system when applied for 15 second and 30 second were higher than that of 7 second (P<0.05). And the hybrid layer thickness of 15 second and 30 second's application time were higher than that of 7 second. 4. Thickness of hybrid layer applied with single bonding agent was $2-4{\mu}m$ and that of All-In-One system was $1-2{\mu}m$.

  • Hypophosphatemia rickets, also known as Vitamin D-resistant rickets(VDRR) and refractory rickets, is a form of rickets which is resistant to the usual doses of vitamin D. VDRR is characterized by decreased renal tubular reabsorption of inorganic phosphate and is easily diagnosed by a normal blood calcium, hypophosphatemia, and slightly elevated alkaline phosphatase. Clinical features of Hypophosphatemia rickets included lateral bowing deformities of the legs, short stature, scoliosis, and enlargement of wrist and ankles. Dental finding in patient with VDRR were spontaneous dental abscesses in caries free teeth and other dental findings included delayed eruption, delayed apical closure, thin and hypoplastic enamel, absent or poorly defined lamina dura, enlarged pulp chambers, and numerous accessory canals and pulp horns that extend up and into the dentinoenamel junction. we reported the clinical feature and treatment of VDRR child who was referred from the department of pediatrics for early loss of primary teeth and its treatment.
