Orthodontic Traction of the Permanent Molar Using Skeletal Anchorage: A Case Report (골성 고정원을 이용한 영구 대구치의 교정적 견인 : 증례 보고)

  • Treatment options for impacted permanent molars include orthodontic traction, surgical repositioning, transplantation, and extraction of the impacted teeth. Orthodontic traction is recommended because it is the most conservative method. However, it has limitations, such as loss of tooth anchorage. In an effort to overcome these limitations, skeletal anchorage devices tailored for orthodontic use were developed. In this case report, 3 patients were diagnosed with impacted permanent molars. The impacted teeth of these patients were surgically exposed, the orthodontic devices were attached, and the skeletal anchorage devices were implanted for the successful traction of the impacted teeth.

Association between Oral Health Status and Rheumatoid Arthritis (구강건강상태와 류마티스 관절염의 관련성)

  • The purpose of this study was to examine the association between oral health status and rheumatoid arthritis (RA). The study used a nationally representative sample of Koreans (2013 Korea National Health and Nutrition Examination Survey) aged 19 years over (n=6,113). Dependent variable was RA, which was assessed with oral health status. Independent variable was oral health status (periodontal status, missing tooth). The chi-square test and logistic regression analysis were performed to identify the association between oral health status and RA. Results of logistic regression analysis for association between periodontal status and RA was no significant. Results of logistic regression analysis for association between missing tooth and RA was statistically significant. The odds ratio (OR) for RA participants was 3.03 (95% confidence interval [CI], 1.47~6.23) in missing tooth 19~28 than missing tooth 0~8. The OR for RA participants was 2.08 (95% CI, 1.06~4.08) in missing tooth 9~18 than missing tooth 0~8. After adjustment for confounders (socio-demographic factors, health behaviors), results of logistic regression analysis was no significant. More missing tooth among adults was greater the risk of RA. By promoting the improvement of oral hygiene and oral health would contribute to reduce the risks associated with systemic diseases. Future study is needed to examine the detailed causal relations between oral health status and RA bidirectionally.

A Study of Oral Health Status in Handicapped Students Comparing with Normal Students (장애학생과 일반학생의 구강건강상태 조사연구)

  • This study was carried out to evaluate the status of oral health in physically or mentally handicapped students. The status of oral health in 597 handicapped students of six special education institutes and 731 normal students in Chonbuk province were inspected by a dental hygienist from August to November of 1999. 1) The DT index(Decayed Teeth index) was significantly higher in handicapped students comparing with those of normal students. The proportions of children having gingivitis and malocclusion were also significantly higher in handicapped group (p<0.01). 2) The incidences of dental caries, gingivitis and malocclusion were significantly higher in mentally retarded students comparing with those of physical or sensory impairment groups(p<0.01). 3) Handicapped students participating in the regular school teeth-control program showed significantly low incidences of gingivitis and malocclusion comparing with non-participants. However, dental caries had no relationships with teeth control program. There were no differences between commuting and domitory-resident students in the incidences of dental caries, gingivitis and malocclusion.

  • Distal displacement of the tooth germ of the mandibular second premolar (MnP2) leads to its impaction and obturation of the eruption path of the mandibular first molar delaying its eruption. The present case report describes the treatment of 2 cases of eruption guidance for distally displaced developing MnP2 that caused delayed eruption of the mandibular first molar. Intentional extraction of primary predecessor results in the mesial shift of the displaced MnP2. However, unfavorable distal ectopic eruption of the mandibular first premolar after the premature loss of primary second molar has been previously reported. Hemisection and sequential extraction of the mandibular primary second molar were performed to mesially shift the distally displaced MnP2, while preventing unfavorable distal ectopic eruption of the mandibular first premolar.

THE SPACE OF CONGENITALLY MISSING OF PRIMARY CANINE WITH ODONTOMA (치아종을 동반한 선천적 결손된 유견치의 공간)

  • Space loss of dental arch can appear when the proper position of teeth within the dental arch changes by a certain cause, because the balance of force makes changes about tooth position as well as alignment. The causes of space loss include proximal caries, early extraction, congenital missing of a tooth and hypodontia, etc. Among those causes of space loss, congenital missing of a tooth is more rarely observed in the primary dentition than in the permanent dentition. Congenital missing in the primary dentition is associated with that in the permanent dentition. Furthermore, it can cause space problem, such as mesial tilting or drift of adjacent teeth, space loss for permanent successors and dental arch constriction, etc. Primary lateral incisors is the most commonly involved, in the maxilla rather than in the mandible, but primary canine is rarely reported. In this patient, who visited the department of pediatric dentistry at Yonsei university dental hospital, it was observed that the maxillary right primary canine was congenitally missing and an odontoma was found insteadly. However, neither the space loss for the congenitally missing primary canine nor midline deviation is remarkable during the 2-year-10-month observation period. In addition, any clinical or radiographical symptom did not occur in spite of odontoma. Therefore, surgical enucleation of odontoma is planned according to the eruption of permanent lateral incisor or canine, unless eruption failure of permanent lateral incisor or canine nor cystic change around the odontoma is occurred. Through further evaluation, space maintainer or orthodontic treatment may be necessary.

  • The swallowing threshold of 40 subjects with bilateral missing of molars was tested. The results were as follows : 1) The swallowing threshold of the patients with bilateral missing of molars was higher than that of the normal control subjects. 2) The swallowing threshold of the patients with bilateral missing of molars was higher than that of the full denture wearer. 3) The swallowing threshold of the patients with bilateral missing of molars was different according to the quality and quantity of the test food.

  • 보철물이 제기능을 발휘하기 위해서는 건강한 치주조직이 필요할 분 아니라, 보철물 그 자체가 건강한 치주조직을 계속적으로 유지시켜 줄 수 있어야 한다. 보철물이 치주질환을 야기하거나, 치주질환을 가진 지대치에 아무런 처치없이 보철물을 장착한다면 결국 치아의 상실을 초래하게 된다. 이런 일로 인하여 외국에서는 치과이사와 환자가 소송에까지 이르게 되는 불행한 에가 많다. 이런 예는 먼 나라의 이야기가 아니라, 우리 나라에서도 문제가 되어가고 있는 것 같다. 필자의 조사(미 발표)에 의하면, 보철물이 치주조직에 미치는 영향에 관하여 재 조명이 있어야 될것으로 생각한다.

SURGICAL REPOSITIONING OF AN IMPACTED INCISOR IN MIXED DENTITION (매복된 중절치의 재식)

  • Delayed eruption of a maxillary incisor results in midline shift, the space occupied by adjacent teeth and different levels of alveolar height. Extraction or surgical/orthodontic therapy is the most common treatment for a impacted maxillary incisor. Surgical repositioning provides another option for treatment of this problem. The advantages of this approach include immediate esthetic improvement, use of a single and simplified surgical procedure, simple and short orthodontic therapy, a normal gingival margin and the possibility of the developing root adapting to the new position. Autotransplantation of an immature tooth provides for possible adaptation of the developing root apex to the new position. A root with an open apex has good chance of pulp revascularization after transplantation.

The frog appliance for upper molar distalization: a case report (Frog appliance를 이용한 상악 대구치의 원심 이동: 증례 보고)

  • The purpose of this article was to evaluate the effects of a new upper molar distalization system, the Frog Appliance, on dentofacial structures in a Class II, division 1 patient. An 11-year-old girl was referred to our clinic for orthodontic treatment. She had a mild skeletal Class II malocclusion with Class II molar and canine relationship on both sides. The treatment plan included distalization of the upper first molars bilaterally followed by full fixed appliance therapy. For the upper molar distalization, a new system, the Frog Appliance, was constructed and applied. Lateral cephalometric radiographs were used to evaluate the treatment results. Distalization of the upper first molars was achieved in four months successfully, and Class I molar relationship was obtained. Total treatment time was 16 months. According to the results of the cephalometric evaluation, a nearly bodily distal molar movement with a slight anchorage loss was attained. In conclusion, the Frog Appliance was found to be a simple, ef ective, non-invasive, and compliance-free intraoral distalization appliance for achieving bilateral molar distalization.

  • In order to solve the problem of repositioning the old hader bar attachment and to restore the non-esthetic facial profile and improper occlusion of the edentulous patient due to inadequate jaw relation, determination of vertical dimension of occlusion and tooth arrangement were performed similar to the natural teeth before the teeth loss. In addition to improving the esthetics by restoring the inadequate facial appearance, the composite resin was used to maximize the patient's secondary esthetic satisfaction with the denture.
