Trigeminal neuralgia is a condition characterized by excruciating paroxysms of pain in lips, gums, cheek, or chin, and very rarely, in the distribution of the ophthalmic division of the trigeminal nerve. There are many treatments of trigeminal neuralgia, such as, medical treatment, electrical stimulation, radiation therapy and ablative procedures. Infraorbital nerve block with pure ethyl alcohol is an ablation procedure for trigeminal neuralgia. We injected pure ethyl alcohol into the infraorbital foramen for pain control. The results were as follows; 1) The infraorbital nerve block with pure alcohol was an a simple and an effective method. 2) Complication, included a mild sensory deficit and mild edema over the infraorbital area.

The trigeminal nerve, one of the cranial nerves, innervates the maxillofacial area and has three branches: the ophthalmic, maxillary, and mandibular nerves. Paresthesia, due to damages to the inferior alveolar nerve and mental nerve (branches of the mandibular nerve), is quite frequent in dental implants and third molar extractions. As medical disputes are increasing, it is necessary to formulate an objective and reasonable disability evaluation. When evaluating the frequent rate of impairment for inferior alveolar nerve damage, it may be reasonable to follow the criteria for the rate of maxillofacial impairment of the American Association of Oral and Maxillofacial Surgeons (AAOMS) - the most scientific and reputable criteria based on the American Medical Association (AMA). Therefore, the Committee of Guides for Maxillofacial Impairment Ratings, in the Korean Association of Oral and Maxillofacial Surgeons (KAOMS), is trying to suggest more reasonable and realistic guidelines for evaluating impairments by reviewing the current evaluation criteria and those of AMA and AAOMS.
본 논문은 맞춤형 신발제작을 위하여 맞춤형 신발에 필요한 화형제작용 데이터를 위한 3차원 측정 장치를 통해 획득한 발의 형상을 인공지능 기법을 기반으로 하는 최적화된 형상을 복원하는 방법을 제시하고자 한다. 본 연구를 위해 개발된 시스템은 PC를 기반으로 하는 기존의 3차원측정 방식을 이용하여 상, 하, 좌, 우로 각각 장착된 8대의 CCD 카메라와 4대의 레이져를 통해 화형 및 발의 형상 데이터를 획득한다 획득된 데이터들은 인공지능 기법을 이용한 영상처리 알고리즘으로 처리되며, 처리 결과는 기존의 지능 기법을 도입하지 않은 시스템에 비해 노이즈제거 특성이 향상되었고, 후처리과정을 간소화 할 수 있다. 따라서 본 논문에서는 3차원 측정을 위해 기구적인 부분과 하드웨어적인 부분의 시스템을 구성하고, 데이터 처리용 소프트웨어에서 입력영상의 전처리 과정 중 영상의 이진화 단계에서 임계값을 결정하기 위하여 간단한 신경망을 사용하였으며, 이에 대한 결과를 제시하고자 한다.

Trigeminal neuralgia is a unique disease entity to be dealt with at pain clinic due to it's severe pain attack. A 33-year old male patient had severe pain on left cheek area usually initiating from first upper molar tooth area for three years. We successfully treated this patient with maxillary nerve block using pure alcohol by a lateral approach. Three months after maxillary block the patient is still pain free.

We blocked the mandibular nerve via lateral extra-oral route with pure ethyl alcohol in patient with localized gingival pain. The results were as follows; 1) The mandibular nerve block with pure ethyl alcohol was effective. 2) No complications were noted.
The purpose of this study was to review the results of the neurolytic trigeminal nerve block in 41 patients from April 1992 to March 1994. Most common site of trigeminal neuralgia was the second division(27 patients, 68.3%). Nineteen patients of these were treated with infraorbital nerve block. Another 4 patients had cerebello-pontine angle tumor close to TREZ in MRI findings therefore they were excluded from this study. Thirty nine patients (95.1%) remained free of pain. Only 2 patients treated with infraorbital nerve block experienced recurring pain. Although the follow-up period was short, alcohol blocks proved effective and safe for treatment of trigeminal neuralgia.
Background: Trigeminovascular system is implicated in the pathophysiology of the headache in migraine. This study was designed to evaluate the pattern of Fos protein expression in trigeminal nociceptive central pathway after meningeal stimulation of rats by capsaicin. Methods: The expression of Fos protein was examined by immunohistochemistry in thalamus, brainstem and upper cervical cord (at three levels corresponding to obex, 0.8 mm and 2 mm below obex) 2 hours after intracisternal injection of either diluted capsaicin solution (0.1 ml, $61{\mu}g/ml$) or normal saline (0.1 ml) through a catheter placed in the cisterna magna, or following epidural instillation of diluted capsaicin solution in urethane-anesthetized Sprague-Dawley rats. Results: Fos immunoreactivity was strongly expressed within lamina I, II of bilateral trigeminal nucleus caudalis (TNC) after cisternal capsaicin injection and magnitude of expression was greatest at level 2.0 mm below obex. Epidural capsaicin caused much less labelling than cisternal capsaicin. Fos positive cells were also observed in area postrema, nucleus of the solitary tract, medullary reticular nucleus and midline nuclear groups of the thalamus with similar intensity between capsaicin and control group. Conclusions: These results indicate that the injection of capsaicin into the cisterna magna is an effective stimulus for the induction of Fos protein within TNC through activation of trigeminovascular afferents and this animal model can be useful for the evaluation of the pathophysiology and drug development in migraine and related headache.
In 1983, Sten H$\ddot{a}$kanson first reported the clinical safety and efficacy of retrogasserian glycerol injection as a treatment of typical trigeminal neuralgia in 96 of 100 patients during a follow-up period of 1~6 years. Since September 1987, we have injected sterile pure glycerol into the trigeminal cistern using an anterior percutaneous approach via the foramen ovale (H$\ddot{a}$rtel route) for treatment of tic douloureux in 12 patients who were suffering from attacks pain of following discontinuation of carbamazepine. The results were as follows; 1) Eight patients were completely free from pain attacks with a single dose of glycerol (0.4 ml). The remaining four patients needed a second dose (0.4 ml) several days later following the single dose. The degree of patient's subjective satisfaction by those injections was very good in 11 and fair in one. 2) During the follow-up period (1~13 months), persistent sensory deficit as determined by the pin prick test, appeared to be mild in 10 and moderate in one patient. There was no sensory deficit in one patient. further attacks of pain from those injections were still noted. 3) As a transient complication, there was headache in all patient, facial hematoma in 4, nausea and vomiting in two each, and vertigo and herpes simplex in one each. In conclusion, we confirmed that the above glycerol injections into the trigeminal cistern were clinically very effective as a treatment of tic douloureux even though the follow-up period was short.
