The Effect of Mibaeksan(MB) on Melanin Synthesis and Gene Expression (미백산(美白散)이 멜라닌 생성 및 유전자 발현에 미치는 영향)

  • Purpose: This study was performed to elucidate the inhibitory effect of Mibaeksan (MB) on melanin synthesis in B16F10 mouse melanoma cell. Methods: To demonstrate the inhibitory effects of MB on melanin synthesis, we measured the amount of released and produced melanin in B16F10 melanoma cell. Also, we evaluated tyrosinase-activity in vitro as well as in B16F10 melanoma cell. And to investigate the action mechanism, we assessed the gene expression of tyrosinase, TRP-1, TRP-2, MMP-2, PKA, $PKC{\beta}$, ERK-1 ERK-2, AKT-1 and MITF in B16F10 melanoma cells. Results: 1. MB decreased the release and production of melanin in B16F10 melanoma cells. 2. MB decreased tyrosinase activity in vitro and in B16F10 melanoma cells. 3. MB decreased the expression of tyrosinase, TRP-1, TRP-2, PKA, $PKC{\beta}$ and MMP-2 in B16F10 melanoma cells. 4. MB increased the expression of ERK-1, ERK-2 and AKT-1 in B16F10 melanoma cells. 5. MB decreased the expression of MITF in B16F10 melanoma cells. Conclusion: From these results, it may be concluded that MB has the antimelanogenetic effects.

Two Cases Report of the Primary Dysmenorrhea Treated with Gamisoyosan-gagambang (가미소요산가미방(加味逍遙散加味方)으로 호전된 원발성월경통(原發性月經痛)환자 치험 2례)

  • Objectives: This paper was aim to report the effects of Gamisoyosan-gagambang (加味逍遙散加味方) on the Primary Dysmenorrhea patients. Methods: Two patients who had been suffering from primary dysmenorrhea were enrolled in this study. They received oriental treatment such as herbal medicine. We gave them Gamisoyosan-gagambang several times. During the treatments, we requested them to visit hospital once every two weeks. If they could not come to hospital, we called them to take follow-up. The progress of symptoms was evaluated by visual analogue scale(VAS). Results: After treatment, most symptoms were improved and also VAS was reduced. Conclusions: This clinical cases indicate that Gamisoyosan-gagambang is effective in treatment of the primary dysmenorrhea patients. More clinical data and studies are requested for the treatment of primary dysmenorrhea.

Two Case Report of Spontaneous Pregnancy Treated with Jokyung-san-gagam after Failure in a Intrauterine Insemination (자궁내 인공수정 실패 후 조경산가감(調經散加減) 복용을 통해 자연임신에 성공한 난임환자 2례의 임상보고)

  • Objectives: The purpose of this paper is to report the spontaneous pregnancy of Jokyung-san-gagam treatments on two infertile patients who were failed to in intrauterine insemination (IUI) by three times. Methods: Two patients, in this case, were who failed three times IUI were treated with Jokyung-san-gagam, acupuncture and moxibustion. Results: The two infertile women achieved spontaneous pregnancy after taking Jokyung-san-gagam without assisted reproductive technology. Conclusions: These cases suggest that Jokyung-san-gagam is effective in treating infertile female after failure in IUI and can be an effective option for patients who were failed in IUI. For further study, the clinical approach on infertile patients that is based on Korean medicine treatment including Jokyung-san-gagam would be sustained.

  • JuJinHyoung(朱震亨) is a well-known physcian of the four schools in the Jin(金) and Yuan(元) Dynasties. He lived in ZhejiangSheng(浙江省) which located in the southeast region of China. There was humid and heatful climate, this kinds of environments influenced to his medical thoughts. He persisted in the theory that Yang is ever in excess while Yin is ever deficient(陽常有餘陰不足論) and exhorted the therapy of subduing excessive Yang and making up deficient Yin by nourishing it, and he said about the theory of ministerial fire(相火論). So he was regarded as a representative of the nourising Yin school(滋陰派). And he said that the method of "Clearing away heat and nourishing blood" is suitable in pregnancy. So Stucellaria baicalensis GEORGI and Atractylodes macrocephala KOIDZ. Can be apply in case of treatment on Anti-abortion. Thus these theories have influenced on Obstetric & Gynecology in Korea.

  • Comparisons of enzymatic method, gas chromatographic method, and the Iatroscan method for the determination of cholesterol in each parts of Korean squid were undertaken. The Iatroscan method was the most suitable procedure for the rapid and simple determination of net cholesterol concentration in parts of Korean squid. 5$\alpha$-cholestane is used as a good internal standard. The cholesterol contents in body, leg, viscera, eye, skin, and liver part of Korean squid, Todarodes pacificus STEEN STRUP by Iatroscan method were 178.9, 321.4, 168.9, 159.5, 608.8 and 634.2 mg%, respectively.

The Experimental Study on Antioxidant, Anti-inflammatory, Antipruritic and Antibacterial Effects of the Banchong-san (BCS) (반총산의 항산화, 항염증, 항소양증, 항균효능에 관한 실험 연구)

  • Objectives: Banchong-san (BCS) is a herbal formula composed of 13 korean medicinal herbs and is traditionally used to treat inflammatory diseases and pain. The object of this study was to research the antioxidant, anti-inflammatory, antipruritic and antimicrobial effects of the BCS in lipopolysaccharide (LPS)-stimulated RAW 264.7 macrophages. Methods: In this experiment, effects of BCS on the following four were measured as follows: (1) Anti-oxidative effects were evaluated by 1,1-diphenyl-2-picryl-hydrazyl (DPPH) Radical scavenging activity, 2,2-azino-bis-3-ethylbenzothiazoline-6-sulfonic acid (ABTS) Radical scavenging activity. (2) Anti-inflammatory effects were evaluated by the production amount of Reactive oxygen species (ROS), Nitric oxide (NO), Interleukin-1β (IL-1β), Interleukin 6 (IL-6), tumor necrosis factor-α (TNF-α), Prostaglandin E2 (PGE2), inducible nitric oxide synthase (iNOS), cyclooxygenase-2 (COX-2)(the previous two are "mRNA"), extracellular signal-regulated kinase (ERK), c-Jun N-terminal kinase (JNK), p38 mitogen-activated protein kinases (p38), inhibitor of nuclear factor kappa B (IκBα), nuclear factor kappa B (NF-κB) (the previous five are "Protein") in LPS-Stimulated RAW 264.7 cells. (3)Antipruritic effects were evaluated by the production amount of histamine, Leukotriene B4 (LTB4), LeukotrieneC4 (LTC4) Levels in phorbol 12-myristate 13-acetate(PMA)/ionomycin-stimulated MC/9 mast cell. (4) Anti-microbial effects were evaluated by the growth suppression of Escherichia coli, Staphylococcus aureus, Pseudomonas aeruginosa and Aspergillus niger. Results: The following results were obtained through each measurement: (1) DPPH Radical Scavenging Activity, ABTS Radical Scavenging Activity evoked a significant concentration-dependent increase. (2) ROS, NO, IL-1β, IL-6, TNF-α, PGE2 production amount, iNOS, COX-2 mRNA expression were significantly reduced in the BCS extraction group compared with the control group and significantly decreased the amount of ERK, JNK, p38, NF-κB Protein expression. The amount of IκB-α Protein Expression have increased significantly. (3) The amounts of histamine, LTB4, LTC4 were significantly decreased. (4) The antibacterial efficacy, BCS inhibited the growth of Escherichia coli, Pseudomonas aeruginosa at concentrations of 5 ㎍/ml, but did not suppress the growth of staphylococcus aureus and aspergillus niger. Conclusions: The experimental results show that BCS has anti-oxidant, anti-inflammatory, antipruritic and antimicrobial properties.

Combination Treatment of Korean Medicine with Ohrim-san for Lower Urinary Tract Symptoms (LUTS) after Laparoscopic Myomectomy Patient : A Case Report (복강경하 근종적출술 후 발생한 하부요로증상에 대한 오림산을 포함하는 복합한의진료의 경과 : 증례보고)

  • Objectives: This case report aims to report the clinical effectiveness of the combination treatment of Korean medicine on a post-laparoscopic myomectomy patient's lower urinary symptoms (LUTS). Methods: The patient who diagnosed with uterine myoma had Laparoscopic myomectomy. After the operation, the patient complained of dysuria, nocturia, frequent urination, urgent urination and urinary retention. The patient received combination treatment of Korean medicine during 14 days of hospitalization. The treatment included Ohrim-san, acupuncture, moxibustion, and pharmacoacupuncture. The effects were evaluated through the Numeric Rating Scale (NRS), Overactive Bladder Symptom Score (OABSS), Frequency-Volume chart, and 5-Level version of EuroQol-Five Dimension (EQ-5D-5L). Results: After the treatment, the clinical symptoms such as dysuria, nocturia, frequent urination, urgent urination, urinary retention were improved. Also, the quality of life was enhanced. Conclusions: This case report shows that the combination treatment of Korean Medicine with Ohrim-san may be effective for treating lower urinary tract symptoms (LUTS) after laparoscopic myomectomy.

Korean and United States: Comparison of Costs of Nursing Interventions (NIC과 연계된 산부인과 환자 간호중재에 대한 한국 건강보험 수가체계와 미국 ABC 코드체계와의 수가 비교 분석)

  • Purpose: This study was performed to compare the costs of nursing interventions implemented for the obstetrical and gynecological patients using Korean Reimbursement System and ABC codes system developed in the US for costing out interventions performed by health care professional. Methods: First, the narrative data on nursing interventions were extracted from electronic medical record system of a tertiary university and mapped with Nursing Intervention Classification (NIC) by two researchers until 100% consent was reached. Narrative nursing interventions mapped with NIC were then remapped with ABC codes system using the electronic program developed in the research. The mapping data were analyzed with real numbers, frequency, percentage, mean, and standard deviation. Results: More nursing interventions were mapped with ABC codes than Korean reimbursement system. Total of 97 different types of narrative interventions could be mapped with NIC, 43 NIC interventions could be reimbursed by ABC code but only 16 NIC interventions were reimbursed by Korea Reimbursement System. Conclusion: Korean medical insurance fee system needs amendment to include more comprehensively interventions performed by nurses which are very important to patient outcomes. Further study is needed to develop strategies to costing out nursing interventions.

  • Purpose: The purpose of this study was to report the clinical application of traditional korean medicine(TKM) treatment to hyeolheopungyeol type of dermatoses gravidarum. Methods: We experienced four patients who had been suffering from itching, plaque, erythema during pregnancy. They had already been treated with antihistamine or ointment, but they were not improved. We diagnosed their case as hyeolheopungyeol type of dermatoses gravidarum, and treated with sopungsan-gaml. Results: After TKM treatment, itching, plaque and erythema were alleviated or resolved. And they gave birth to a healthy baby. Conclusion: The results indicate that TKM, sopungsan-gami can be effective for the management of hyeolheopungyeol type of dermatoses gravidarum.
