The Effects of the Integrated NICU Hospitalization Education on Maternal Attachment, Maternal Self-Esteem, and Postpartum Depression in the Mothers of High-Risk Infants (통합적 NICU입원교육이 고위험신생아 어머니의 모아애착, 모성자존감, 산후우울에 미치는 영향)

  • Purpose: The quasi-experimental study was conducted to investigate the effects of the integrated NICU hospitalization education on maternal attachment, maternal self-esteem, and EPDS in the 26 mothers of high-risk infants(HRI). Method: The subjects were divided into the experimental group to received the integrated NICU hospitalization education included the enforced direct education of NICU and HRI to the mothers using a booklet and the provision of the neomaternal exposure as soon as possible after delivery. The control group received the routine education of the NICU hospitalization, mostly including the admission process and the NICU rules delivering to the husband. The research variables were maternal attachment, maternal self-esteem, and EPDS. Result: The results showed the increases in these variables after the integrated education in experimental group compared to the control group. Conclusion: It supports the benefits of the early direct NICU education and the neomaternal exposures on maternal adaptation process in the mothers of HRI.

The Educational Needs and Satisfactory about Infant Care of the Mothers in the Post-partum Care Center (산후조리원의 산모의 신생아 돌보기 교육 요구도와 교육 만족도)

  • This study was designed to compare the educational needs and satisfactory about infant care of the mothers in the Post-partum Care Center for development of the educational program. The subjects for this study were 100 mothers in the Post-partum Care Center in Seoul and Buchon. The data were collected during the period from Feb. to March, 2000. The Educational Need and satisfactory for Infant Care was measured by questionnaire that has developed by researchers. The data were analysed by descriptive statistics, paired t-test using SPSS PC+ WIN. The results were as fellow; 1) The educational needs of the mothers in the Post-partum Care Center were high (mean=4.32). 2) The satisfactory of the mothers in the Post-partum Care Center of educational program of infant care were middle range (mean=3.60). 3) The educational needs of infant cares in the Post-partum Care Center were higher than satisfactory of the mothers in the Post-partum Care Center of educational program of infant care(t=7.71 p=.000). 4) The educational need by content was signs, symptoms and management of disorders(i.g. cyanosis, seizure, fever, etc.) were higher than general care of baby(i.g. feeding, diaper change etc.). In conclusion, when the infant caregiver in the Post-partum Care Center teaching the method of infant care to mother, there should be in consideration of educational need of mother. And its educational contents must be included of mental and emotional development and signs, symptoms and management of disorders.

The Effect of a Breastfeeding Educational Program on Feeding Behavior (산후 모유수유 교육프로그램이 수유행위에 미치는 효과)

  • The purpose of this study is to investigate the effect of an educational program on parturient women's feeding behavior One hundred and twenty seven mothers who delivered in one general hospital in Seoul participated in the study, 66 mothers of whom were assigned to experimental group and 61, to control group. The data collection was done from April 20 to June 30 in 1997 and the educational program was applied to the experimental group on the third postpartum day and telephone calls were made on the fourth and the twelfth postpartum week to investigate feeding behavior after discharge. The collected data were analyzed by means of Chi-square test and descriptive statistics. The results are as follows : 1. Breast-feeding rate of the experimental group was higher than that of the control group($x^2$=6.578, p=.043). 2. Background variables affecting feeding behavior were age and feeding plan(age: $x^2$=8.660, p=.070, feeding plan : $x^2$=25.762, p=.000). 3. The insufficient milk supply was the main cause of discontinuing breast-feeding and others were mother's job, baby's jaundice and diarrhea, baby's refusal to suck breastmilk. In conclusion, the breast-feeding educational program which was applied in this study is effective in the promotion of breast-feeding.

Effects of Breast-feeding Adaptation, Attitude and Practice of Primipara Depending on Method of Postpartum Breast-feeding Education (산후 모유수유 교육 방법에 따른 초산모의 모유수유 적응, 태도 및 실천효과)

  • Purpose: This study was done to identify the effects of individual postpartum breast-feeding education on adaptation, attitude and practice of breast-feeding in primiparas. Methods: A quasi-experimental nonequivalent design was used to examine the effects of individual postpartum breast-feeding education among primiparas. The data were analyzed using $x^2$-test, Fisher's exact test and paired t-test. Results: There was a significant difference in breast-feeding adaptation (t=-12.94, p<.001), breast-feeding attitude (t=-2.71, p=.011) and breast-feeding practice ($x^2$=16.47, p<.001) between the group education (experimental group I) and individual education after group education (experimental group II). Conclusion: The results indicate that individual education on breast-feeding compared to group education has more positive effects on increasing breast-feeding adaptation, attitude and practice in primiparas. This result also indicates that many primiparas should be given opportunities of various types of education to improve breast-feeding practice such as individual approaches and postpartum breast-feeding education programs.

Comparison of self-care and self-efficacy before and after childbirth in Pregnant women with Gestational Diabetes Mellitus (당뇨병을 가진 임부의 자가 간호와 자기효능감에 대한 출산 전후 비교)

  • The aim of this study was to evaluate the difference of self-care and self-efficacy before and after childbirth in pregnant women with diabetes mellitus. Data were collected from 24 May 2018 to 23 May 2020 for 40 pregnant women over the 24th week of pregnancy. The collected data were analyzed by matched paired t-test using the SPSS 27.0 software program. 27 subjects (67.5%) were under the age of 35, and 13 subjects (32.5%) were over the age of 35. Self-care and self-effectiveness scores were lower in postpartum than in prenatal, which was statistically significant (p< .000). Therefore, it is necessary to develop a nursing intervention program to enhance self-care and self-efficacy after childbirth.

  • 지난 10년 동안 구미에서는 부모-아동 관계 영역에 혁신을 가져왔다. 특히 부모-아동 애착이론의 획기적인 변화로 가족 중심 출산 경험을 강조하고 있다. 과거에는 남편이 아내의 출산에 참여하던 것이 타부시되어 왔지만 애착의 개념이 대두되면서 남편도 출산에 함께 참여하도록 고려하여 임신에서부터 부부가 함께 이 과업을 완수하도록 모든 환경을 변화시키고 있다. 애착이론은 다양하여 정신분석이론, 학습이론, 인지발달이론과 동물 행동학적 측면에서 이해되고 있다. 다양한 이론적 배경과 개념들을 가지고 아버지-아동의 애착 및 유대관계에 대한 연구가 지난 10여년 이상 되어오고 있다. 가장 관심의 촛점이 되어 연구 되어온 것으로는 애착형성을 위해 중요하다고 하는 민감한 시기(Critical period)를 중심으로 애착의 특성인 시각, 청각, 촉각, 상호호혜적 관계, 긍정적 애정을 바탕으로 연구되어졌다. 이 연구는 부아 애착(Paternal Infant Attachment)의 연구를 연구방법, 측정도구 및 방법상문제와 변수들을 중심으로 재정리하여, 이 분야에 있어서 앞으로의 연구 방향을 살펴보았다. 17편의 연구들을 각각 년도, 표본수, 방법 및 측정, 결과 및 비고난을 비교 분석하여 도표를 작성하였다. 이론적 기틀과 문헌고찰에 근거하여 다음과 같은 적용과 제언을 한다. 1. 출산전 아버지의 특성에 대한 사정이 필요하다. 2. 연구자가 집단간 비교를 용이하게 하기 위해 연구전에 표본의 특성을 정확히 기술해야 한다. 3. 부모와 아버지의 과거력에 대한 상세한 정보수집이 강조되어야 한다. 4. 앞으로의 연구는 이론적 모델에 근거하여 이루어져, 대중교육 뿐 아니라 정책 결정에 기여해야 한다. 5. 연구결과 평가와 해석을 위해 정확한 이론적 근거가 필요로 된다. 6. 간호연구는 부적절한 부아 애착 형성에 있어서 아버지의 특성을 확인하고 부아유대와 애착 증진의 요소를 파악해야한다. 7. 부아 유대에 대한 위협요소 확인을 위한 도구개발과 그들에 대한 효과적인 간호전략이 필요 된다. 8. 가족에 있어서 모든 부모행위가 하나의 독립변수로서 연구되어야 하고 부아유대 증진에 관한 연구가 시도되어야겠다. 오늘날 부모들은 임신기간동안 많은 정보에 접하기를 원한다. 산전, 산후의 교육과 지식은 긍정적인 부아 관계를 증진시키고, 이것은 아동의 발달에 크게 기여할 수 있다. 긍정적으로 이러한 관계는 가족단위를 강하게 통합시키게 되므로 건강관리자(Health care workers)들은 애착에 대해 높은 관심을 갖어야 하겠다.

Postnatal Breast-Feeding Knowledge, Techniques and Rates of First-Time Mothers Depending on a Prenatal Breast-Feeding Education Method (산전 모유수유 교육 방법에 따른 초산모의 산후 모유수유 지식, 방법, 실천율)

  • Purpose: To investigate the impacts of individualized versus group prenatal breast-feeding education on postnatal breast-feeding knowledge, this study addressed techniques and rates of first-time mothers as part of data collection for developing breast-feeding education programs. Methods: A quasi-experimental nonequivalent control group design was applied to compare the effects of prenatal breast-feeding individual vs. group education among first-time mothers. Collected data was analyzed using $x^2$-test, t-test, paired t-test, and ANOVA. Results: There was no difference on the knowledge of prenatal breast-feeding between individual and group education. However, individual education was a more effective method than group education on post-natal knowledge. In the 4th and 8th weeks of breast-feeding, individual education was more effective than group education. However, there was no difference between the two education methods at Week 12. Conclusion: This study indicates that many outpatient mothers should be given various individual approaches and prenatal breast-feeding education programs to provide them with opportunities for education to improve breast-feeding rates. However, there was no statistically significant difference in the breast-feeding rates at Week 12, which may be attributed not to education methods but to changes in external environmental factors. Further research is needed to address the changes.

Psychosocial Impacts of Newborn Bloodspot Screening on Parents: A Scoping Review (신생아 스크리닝이 부모에게 미치는 사회심리적 영향: 주제범위 문헌고찰)

  • This study synthesized the literature to identify parental psychosocial impact and related factors on newborn bloodspot screening (NBS) process. A scoping review, using the framework proposed by Arksey and O'Malley (2005), was conducted. A literature search was performed of electronic databases (PubMed, CINAHL, RISS, KISS, DBpia) from 2000 to 2023. In total 749 published articles were identified, and 32 articles were included for the final analysis. Almost studies were descriptive studies using survey or interviews and only one intervention study to reduce negative psychosocial impacts was identified. Parents were experiencing negative psychosocial impacts, particularly related to how positive results were initially communicated and had difficulties accessing timely and reliable information. Findings identified that knowledge, quality and satisfaction of education and communication, information sources and providers as related factors of parental psychosocial impacts. Prenatal and postpartum repeated education, providing timely and reliable information, effective communication between health care providers and parents were key to mitigate negative psychosocial impacts. Nurses can play important roles to improve quality on NBS.

Effects of a Newborn Care Education Program on Newborn Care Confidence and Behavioral Accuracy of Primiparas in a Postpartum Care Center (신생아 돌보기 교육프로그램이 산후조리원 이용 초산모의 양육자신감과 양육행위 정확도에 미치는 효과)

  • Purpose: The study aimed to identify the effects of a newborn care education program on newborn care confidence and behavioral accuracy of primiparas in a postpartum care center. Method: This was a quasi-experimental study with a non-equivalent control group non-synchronized design. Subjects were 29 primiparas selected from a postpartum care center in D and P hospitals in Ulsan. The intervention was a newborn care education program which was composed of education with a booklet, demonstration, watching a video, and verbal encouragement. The outcome variables were newborn care confidence and behavioral accuracy. Data was collected from Feb. 1 to Mar 22 in 2005 with self-administered questionnaires and observation by researchers. Data was analyzed using the $X^2-test$, Fisher's exact test, t-test and ANCOVA. Result: The experimental group showed significantly higher score changes between the pre-test and post-test in confidence and behavioral accuracy than the control group. Conclusion: The newborn care education program showed positive effects on newborn care confidence and behavioral accuracy of primiparas in a postpartum care center. Therefore, we recommend that this program should be applied in postpartum care centers.

산업간호사의 근무조건 - 서울 경인지역을 중심으로 -

  • 본연구는 보건관리자로서의 산업간호사의 역할을 뒷받침 할 수 있는 근무조건을 파악하기 위한 기초조사연구이며 산업간호사의 근무조건 개선에 기여코저 한다. 연구방법은 1991년 3월부터 4월까지 서울, 인천, 경기 지역에 위치한 산업장의 건강관리실에 근무하는 산업간호사 131명을 대상으로 설문지를 이용하여 전화 또는 산업간호사회를 통해 자료를 수집하였다. 조사 결과는 다음과 같다. 1) 조사대상의 일반적 특성 서울, 경기, 인천지역에 상시근로자수가 300인 이상에 근무하는 산업간호사 배치율은 49.3%로 나타났다. 조사대상의 지역별 분포는 서울이 61개사업장이었고 인천 경기의 순이었다. 또한 산업간호사의 교육수준은 전문대졸이 84.0%이었고, 연령별 분포는 20대가 65.7%이었으며, 규모는 500~999사이가 42.7%였고, 노조가 있는 사업장은 73.3%인 것으로 나타났으며 산업간호사 1인이 근무하는 곳이 90.7%였고 보건관리자가 간호사뿐인 곳이 63.3%이며 1000명이상 근로자를 가진 산업체중 31.9%가 전임산업의나 산업위생사 없이 간호사 혼자서 근무하는 것으로 나타났다. 2) 근로조건 산업간호사는 주당근무시간이 44시간이하인 경우가 59.6%였고, 1부제 근무만하는 경우가 93.1%였으며, 월차휴가는 94.7%가 사용할 수 있었으며, 보수는 기본급이 25~40만원 사이가 69.5%였고 면허수당은 43.8%만 받는 것으로 나타났으며 상여금은 년 600%를 받는 산업장이 42%였고 총급여액이 35~55만원을 받는 경우는 전체의 67.3%를 차지하였다. 또한 연장근로인정여부는 일부만 인정하거나 인정하지 않는 경우가 61.8%였고, 과거경력 은 34.4%가 인정하지 않았고 승진은 66.4%가 불가능한 것으로 나타났다. 산업간호사가 별정직이나 촉탁으로 취업한 경우가 35.1%였고 일반 고졸이나 전문대졸 대우를 받고 있는 경우가 68.9%였다. 또한 전문단체활동 지원정도는 대한간호협회나 산업간호사회지원이 95 %를 넘었으며 산업간호사가 원하는 것 모두 지원하는 경우가 68.7%였으며 회비와 참석 모두를 지원하는 경우가 83.7%였다. 3) 모성보호 산업간호사중 81.7%가 결혼 후 근무가 가능했으며, 94.7%가 생리휴가를 사용할 수 있는 것으로 나타났다. 그리고 산전산후휴가는 76.3%가 사용할 수 있었다. 그러나 결혼후 근무나 산전산후휴가는 명문화되어 있었으나 실제로 이용한 경우는 없는 것으로 나타났다. 본 연구결과는 산업간호사의 근무시간, 전문단체활동지원은 비교적 좋은 조건이나 보수를 비롯 승진이나 대우, 모성보호에 대한 개선여지가 있음을 보여주었다. 따라서 결혼후 지속근무, 산전산후 휴가제 등이 보장되어야 전문직으로서의 산업간호사 인력확보, 활용이 용이할 것으로 기대된다. 더불어 전문단체의 조직적 활동과 연구, 행동적인 지원이 계속되어져야 할 것을 제언하는 바이다.
