A Study on Establishing the Sasang Constitution Acupuncture Method through Reviewing Research (체질침법 연구 경향을 통한 사상체질침법 정립에 관한 연구)

  • Objectives Sasang Constitutional Medicine(SCM) is uniquely established in the field of Korean Medicine. Taegeuk Acupuncture(TGA) and Eight Constitution Acupuncture(ECA) which are created upon SCM are applied in clinics. However, there exists no established theory but hypothesis with regard to the composition of acupoints of constitution acupuncture. The purpose of this study was to help establish the Sasang constitution acupuncture(SCA) method. Methods Thirties studies were found on various Oriental medicine journals and 17 literatures with the key words relevant to constitution and acupuncture. Results For the principle of acupoint selection, 12 studies were included which were classified into TGA, ECA, SCA, other constitution acupuncture. Among 17 literatures, most of them were relevant to SCA. For the clinical studies, eight of total 13 using ECA were for musculoskeletal diseases. Conclusions Among studies and literatures chosen, the application of Saam's acupuncture method was most frequently found in SCA method. For establishing the SCA method, the two main concepts need to be considered, which are the consideration of four areas namely Lungs, Spleen, Liver and Kidneys and upward-downward-loosening-tightening movement of the Qi according to Sasang constitution type.

A Case of Cold Hypersensitivity from Hypothyroidism of Soeumin Diagnosed as Greater Yang Disease Reverting Yin Pattern (소음인 태양병궐음증으로 진단한 갑상선 기능저하증으로 인한 수족냉증 치험 1례)

  • Objectives The purpose of this case study was to report that cold hypersensitivity caused by hypothyroidism was alleviated by diagnosing and treating as Greater Yang Disease Reverting Yin Pattern. Methods The patient was administered Insamosuyu-tang and Sammisamyu-tang and treated with acupuncture. Global Assessment Scale (GAS) and Digital Infrared Thermal Imaging (DITI) was used to assess the improvements of symptoms. Results and Conclusions Cold hypersensitivity of hands and feet was improved to GAS 20, and hours of sleep were increased from one to six. On DITI, the increase of the temperature were 1-3 degree celsius on the chest and the abdomen, 0.2-1 degree on the forearms and the hands, 1-2 degree on the thighs, and 0.2-0.5 degree on the shins and the feet.

  • 댐이나 홍수방지시설과 같은 대규모 수공구조물의 설계 및 평가에는 주로 가능최대강수량(Probable Maximum Precipitation, PMP)가 적용되고 있다. 이러한 PMP의 산정은 관측자료의 정상성 가정을 기반으로 하기 때문에 기후변화와 같은 비정상성을 고려할 수 없다. 본 논문에서는 이러한 문제를 극복하기 위해 대기 프로세스의 비정상성 효과를 반영할 수 있는 물리적 기반의 수치 기상 모형(Numerical Weather Model)을 이용하여 최대강수량(Maximum Precipitation, MP)을 산정하는 접근법을 제시하고자 한다. 사례 연구로 대상 극한 강우사상을 식별하고, 식별된 사상들은 지역 대기 모형 중 하나인 WRF를 이용하여 재현된다. 이때, 한국 내의 약 650개의 AWS 자료와 NCEP에서 제공하는 전세계 기상관측자료 및 해수면 온도 자료를 사용하여 초기조건과 경계조건을 개선하고, 총 강수량과 강우의 공간적인 분포를 재현하기 위한 최적 물리옵션을 찾기 위해 다양한 수치실험이 수행된다. 최종적으로 재현된 극한 강우사상은 모형의 경계조건과 수분 최대화의 통해 최대화되어 물리적으로 가능한 최대 강수량을 산정하게 된다. 본 연구는 제한된 강우사상을 대상으로 최대 강수량을 산정하였기 때문에 추후 다양한 강우사상에 대한 연구와 강우의 최대화에 대한 보완이 필요하지만, 정상성 가정에 의존하지 않는 극한 강우사상 산정에 잠재적인 대안이 될 것이라 기대된다.

A Case Report of San-syndrome (疝症) Patient Diagnosed as Soeumin Greater Yang Disease Reverting Yin Pattern (소음인 태양병 궐음증으로 진단한 산증(疝症) 환자 치험 1례)

  • Objectives The purpose of this case study was to report that San-syndrome patient was treated by diagnosing as Greater Yang Disease Reverting Yin Pattern. Methods The patient was administered with Insamosuyu-tang. Numeral Rating Scale (NRS) and National Institutes of Health Chronic Prostatitis Symptom Index (NIH-CPSI) was used to assess the improvements of symptoms. Results and Conclusions The perineal region discomfort was reduced from NRS 4 to NRS 1. NIH-CPSI score was reduced from 23 to 11, which means symptom relief. Nocturia and sleep disorder were resolved, and general weakness was relieved accordingly.

A Systematic Review and Meta-analysis on Constitutional Herbal Medicine Treatment for Obesity (비만의 체질한약치료에 대한 체계적 문헌고찰 및 메타분석)

  • Objectives The purpose of this study was to systematically review previous studies on constitutional herbal medicine treatment for the obesity. Methods A total of 11 databases were searched in English, Chinese, and Korean. Standard methodologies for the systematic review were used to search, analyze and synthesize data for undertaking review and meta-analysis for intervention. The risk of bias was assessed by two independent researchers using the Cochrane risk of bias. Results A total of 2 RCTs were selected and analyzed. The results of the meta-analysis showed a significantly positive effect in the body weight and body mass index for the patient group treated with Taeeumjowi-tang compared to the placebo control group. Conclusions Our results suggested that Taeeumjowi-tang could be considered for improvement of symptoms in obese patients, without serious adverse events. Due to the low quality of the clinical evidence in this study, further studies are needed to evaluate efficacy and safety of constitutional herbal medicines for obesity.

A Case Study of Soyang Patient with Post COVID-19 Syndrome Treated with Hyeongbangjihwang-tanggamibang (형방지황탕가미방으로 호전된 코로나-19 후유증 소양인 환자 치험 1례)

  • Objectives This study was aimed to report a significant improvement of patient with post COVID-19 syndrome, diagnosed as 'Soyang type lesser yang wind damage patterns' based on Sasang Constitutional Medicine. Methods The patient was diagnosed as Soyang type and had treatment with Sasang Constitutional Medicine three times everyday from June 13th to July 16th. Clinical improvement was evaluated with the degree of dizziness, migraine, palpitation and nausea caused by COVID-19. Results The symptoms of dizziness, migraine, palpitation, nausea caused by COVID-19 were improved after treated with Hyeongbangjihwang-tanggamibang. Conclusions After the treatment with Hyeongbangjihwang-tanggamibang, the symptoms of dizziness, migraine, palpitation, nausea caused by COVID-19 were disappeared. This suggests that Hyeongbangjihwang-tanggamibang is effective in recovering post COVID-19 syndrome for patient who diagnosed as 'Soyang type lesser yang wind damage patterns' based on Sasang Constitutional Medicine.

A Case-Study of Taeeumin Patient with Atypical Parkinsonian Disorders Having Dizziness and Gait Disturbance (비정형파킨슨증후군 태음인의 어지럼증과 보행장애 치험례)

  • A 52-year-old female diagnosed with Atypical Parkinsonian Disorders (APD) about one year ago suffered from dizziness and gait disturbance. The patient was identified as Taeeumin esophagus-cold lung-dry symptomatology pattern and was treated with Jowisengcheong-tang. The patient's symptoms of dizziness and night sweats were assessed using Global Assessment Scale (GAS). And, the Unified Multiple System Atrophy Rating Scale (UMSARS) and the Unified Parkinson's Disease Rating Scale (MDS-UPDRS) were used to assess the overall function of the patient. The patient's symptoms of dizziness and night sweats were significantly improved, and UMSARS and MDS-UPDRS scores were decreased. This case showed that Sasang constitutional medicine treatment can improve APD symptoms and improve the quality of life of the patient.

A Case Study of a Taeeumin Patient with Chronic Diarrhea treated with Cheongsimyeonja-tanggamibang (삼차신경통 태음인 환자의 만성 설사 치험례)

  • Objectives This study was aimed to report a significant improvement of chronic diarrhea patient, diagnosed as 'Taeeum type liver heat-lung dryness pattern' based on Sasang medicine. Methods The patient had treatment with Sasang constitutional medicine. The treatment was executed three times everyday from August 27th to September 3rd and two times everyday from September 3rd to September 16th of 2021. Clinical improvement was evaluated with Bristol stool form scale, the degree of abdominal discomfort and nausea compared to the day of hospitalization and the number of defecations and nocturia per day. Results The symptoms of diarrhea, abdominal discomfort and nausea were improved. The number of night urination was decreased from 3-4 times to once a night. Conclusion After the treatment with Sasang constitutional medicine, diarrhea, abdominal discomfort, nausea and the general condition of the patient also improved.

Comparison of climate extremes under different RCP-SSP scenarios (RCP 및 SSP 기후변화 시나리오 기반 극한기후사상 전망 비교)

  • 기후변화로 인한 변동성의 증가는 돌발 홍수, 홍수량 증가로 이외에도 강우 사상의 변화, 가뭄의 빈도 및 강도의 증대 등의 문제를 이어질 수 있다. 이러한 기후변화에 대응하기 위하여 기후변화 시나리오를 제시하고 이를 정책적으로 반영할 수 있도록 하고 있다. 기존 IPCC 5차 보고서에 활용한 RCP(Representative Concentration Pathway) 시나리오에서는 온실가스 농도변화만을 반영하고 있으나, 최근 IPCC 6차 보고서에서는 사회적인 노력과 경제적 구조 등 전반적인 기후정책, 사회 불균형 등을 고려한 SSP(Shared Socio-economic Pathways) 시나리오를 제시하였다. 본 연구에서는 2가지 기후변화 시나리오의 차이점과 유사점을 강수 중심으로 평가하였다. 기존의 RCP 시나리오에 비하여 극한 강우 사상의 변화를 비교 및 평가하기 위하여 CORDEX-EA에서 제공하는 지역기후모델(Regional Climate Model; RCM) 기반에 시나리오를 수집하여 극한기후지수를 산정하였다. 극한기후사상을 비교하기 위하여 WMO에서 활용하는 ETCCDI(Expert Team on Climate Change Detection and Indices) 지수 중 강우 관련 지수인 R10mm, RX1day, RX5day, RD95P, RD99P, SDII를 선정하여 시나리오 별로 결과를 비교하여 제시하였다. 또한, 기존의 연대기 기준의 평가방식에서 탈피하여 동일한 기온 상승 시점에 따라 변화를 확인하기 위한 분석절차를 수립하였다. 즉, 1.5℃, 2℃, 3℃ 및 4℃ 상승한 시점의 ETCCDI 지수를 산정하여 극한기후사상을 비교 및 평가하였다.

Intensity-persistence day-frequency analysis of future extreme heat wave event using Bayesian method and uncertainty assessment (베이지안기법을 이용한 미래 폭염사상의 강도-지속기간-발생빈도 해석 및 불확실성 평가)

  • 극한 폭염사상은 지난 20세기 이후 점점 더 빈번하게 발생하고 있으며, 더 광범위한 지역에서 발생하고 있다. 이러한 폭염사상은 다가오는 지구 온난화 시대에서 그 강도가 더 강해지고 지속기간이 길어질 것으로 예상되고 있다. 본 연구에서는 극한강우에 대한 강우강도-지속기간-빈도(intensity-duration-frequency, IDF)곡선의 개념을 폭염사상에 적용하여 미래의 극심한 폭염사상에 대한 발생확률, 강도 및 지속날짜(heat wave intensity-persistence day-frequency, HPF) 간의 관계를 확인해보고자 한다. 또한 해당 모델의 불확실성은 베이지안 기법을 이용하여 분석하였다. 우리나라 6개 주요 지역(대관령, 서울, 대전, 대구, 광주, 부산)에 대해 16개의 미래 일 최대 기온 앙상블 자료를 이용하여 비정상성 HPF곡선을 적용하였다. 미래 극한 폭염 앙상블 결과를 분석한 결과, 2050년을 기준으로 지속기간 2일에 대해 극한 폭염의 강도가 RCP 4.5 이하 시나리오 기준 1.23 ~ 1.69 ℃ 범위에서 상승할 가능성이 높은 것으로 나타났으며, RCP 8.5 이하 시나리오 기준의 경우 1.15 ~ 1.96 ℃ 범위로 나타났다. 또한 HPF 모델의 매개변수 추정으로 인한 불확실성의 경우, 다양한 기후 모델의 변동성으로 인한 불확실성보다 크게 나타났다. 모델의 매개변수 추정에 따른 불확실성을 반영한 결과, 2010~2050년에 해당하는 폭염의 강도에 대한 delta change의 95% 신뢰구간은 RCP 4.5 이하에서 0.53 ~ 4.94 ℃, RCP 8.5 이하에서 0.89 ~ 5.57 ℃로 나타났다.
