Minimally Invasive Cardiac Surgery -Lower half sternotomy- (최소 침습적 심장수술 -흉골하부절개술에 의한-)

  • Background: There are several advantages to the ministernotomy approach. The skin incision is much smaller than the traditional median sternotomy incision. This approach allows the patients to return to normal life more quickly and provide them with good self-image. Material and Method: From April to July 1998, we performed a ministernotomy via lower half sternum in 25 patients. There were 10 males(40%) and 15 females(60%) with a mean age of 30${\pm}$16 years(range 3 to 55 years). The body surface area ranged from 0.58 to 1.9 m2(mean 1.5 to 0.4 m2). A vertical skin incision of 11cm in mean length was made in the midline over the sternum extending inferiorly from the third intercostal space. The sternum was divided vertically in the midline from the xyphoid process to the level of second intercostal space using a standard saw and then transversely to the left(n=17) or to both sides(n=4) of the second intercostal space using an oscillating saw. The sternum was divided vertically only in children (n=4). Result: The ministernotomy was used in 25 consecutive patients undergoing mitral valve replacement(n=10), repair of ventricular septal defect(n=4) and atrial septal defect(n=11). There was no significant complication related to ministernotomy. The mean ICU stay time 20 hours. Patient and family acceptance was very high. Conclusion: We concluded that minimally invasive cardiac surgery via ministernotomy can be done safely. These methods may benefit the patients with lesser discomfort, smaller incision, and earlier ICU discharge than the traditional incision.

A HISTOPATHOLOGIC STUDY ON THE PULPAL RESPONSE IN DOGS AFTER PULPOTOMY WITH FERRIC SULFATE (Ferric sulfate를 이용한 치수절단술 후 성견 치수의 조직반응에 관한 연구)

  • The purpose of this study was to compare the effect of ferric sulfate and formocresol as pulp dressing agents in pulpotomized teeth of dogs. 40 teeth of 5 dogs, weighting 10kg. were used in this study. The animals were sacrificed at the intervals of 3, 7, 14, 28 and 56 days for histopathologic evaluation. The specimens were observed by the light microscope. The results were as follows : 1. Inflammatory response was observed in both groups, but pulp tissue of ferric sulfate group was showed lesser inflammatory degree and more rapid recovery than that of formocresol group. 2. In ferric sulfate group odontoblasts showed irregular arrangement pattern at initial stage and returned to regular pattern after 2 weeks. But in formocresol group. continued irregular pattern of odontoblast was observed during experimental period. 3. Reparative dentin was produced widely along the canal in one specimen of formocresol group at 8 weeks and dentinal bridge was formed in two specimens of ferric sulfate group at 8 weeks.

Vitamin C Promoted Liver Regeneration Following Partial Hepatectomy-induced Hepatic Injury in Senescence Marker Protein-30-deficient Mice (비타민 C 투여는 간 부분절제술에 의한 간 재생을 촉진 시킴)

  • The capacity for liver regeneration involves a variety of nutritional factors. Vitamin C has multiple metabolic and antioxidant functions. In this study, we investigated the role of vitamin C in liver regeneration following hepatectomy in senescence marker protein (SMP)-30 knockout (KO) mice. Partial hepatectomy was performed by resecting the median and left lateral lobes of mice. Vitamin C accelerated liver recovery in SMP30 KO mice treated with vitamin C (KV). The livers of the KV mice exhibited lower levels of aspartate aminotransferase and lower injury than those of the KO mice. Increased type II transforming growth factor-β receptor (TGF-βRII)-mediated regeneration signaling was accompanied by HGF and cMet in the KV but not the KO mice. Consistent with this, the expression of cell cycle regulatory proteins, including cyclin D1 and proliferating cell nuclear antigen (PCNA), increased rapidly in the KV mice. Enhanced activation of ERK and GSK-3β proteins and a significantly increased number of binuclear hepatocytes were also detected in the livers of the KV mice. Moreover, the KV mice synthesized the highest levels of albumin. These data suggest that treating SMP30 knockout mice with vitamin C resulted in earlier recovery and liver regeneration by activation of the regeneration system.

Effect of Wood Carbonization Products on Growth and Body Composition of Flounder, Paralichthys olivaceus (목재 탄화물이 넙치의 생육 및 체 조성에 미치는 영향)

  • This study was examined to investigate the effect of wood carbonization products on growth and body composition of oliver flounder(Paralichthys olivaceus) as a part of fishery utilization of charcoal and pyroligneous acid. Weight gain in oliver flounder fed with wood carbonization products was higher than that of the control group. Feed efficiency between the groups fed with charcoal and Mogchotan(the mixture of charcoal and pyroligneous acid, 80:20, w/w) and the control group showed no significant difference(p>0.05) and was significantly higher in group fed with pyroligneous acid than the control group(p<0.05). All groups fed with wood carbonization products were also higher than the control group in hemoglobin and hematocrit. Especially, the amount of both components was significantly higher in group fed with Mogchotan(p<0.05). The result indicates that the addition of Mogchotan can improve the health of oliver flounder, and was excellent in the mortality and the recovery rate. In conclusion, Mogchotan treatment would be suitable way for improving the growth and health of oliver flounder.

Effect of Insoluble Dietary Fiber Extracted from Salicornia herbacea L. on Large Intestinal Function in Rats (불용성 함초 식이섬유의 섭취가 흰쥐 장기능에 미치는 효과)

  • This study investigated the effect of insoluble dietary fibers extracted from Salicornia herbacea L. (S. herbacea) on the improvement of intestinal function in rats. Sprague-Dawley rats were fed on diet containing 5% and 10% S. herbacea dietary fiber (SHDF) for four weeks. Rats receiving the SHDF diet showed a significant decrease in their triglyceride levels and an increase in HDL-cholesterol levels. In addition, compared with the control group, the SHDF group showed a significant increase in the total quantity of the feces and its moisture content. The intestinal transit time of the feces was also shorter in this group. The pH of the feces decreased in all the other experimental groups. Particularly, the bile acid content of the feces and the thicknesses of the mucus layers showed significant recovery on SHDF intake. These results suggest that dietary fiber isolated from S. herbacea has a marked effect on the improvement of bowel function in rats with loperamide (2 mg/kg)-induced constipation.

Dissolution behavior of octacalcium phosphate added hydroxyapatite (수산화아파타이트가 첨가된 옥타칼슘포스페이트의 분해거동)

  • Octacalcium phosphate(OCP, Ca8H2(PO4)6·5H2O) is one of biodegradable calcium phosphate materials with osteoconductivity and biocompatibility. It has the advantage of rapid bone formation and resorption due to the property of stimulating stromal cells to differentiate into osteoblasts. However, if OCP is inserted in body, it is immediately decomposed without maintaining of its shape as scaffolds due to their weak cohesive force between powder. On the other hand, hydroxyapatite (HA, Ca10(PO4)6(OH)2), which has a crystal structure similar to that of OCP, remains in the body without decomposition until the bone defect is restored. In this study, the degradation behavior of OCP/HA disc with different amount of HA in SBF (simulated body fluid) solution was characterized in terms of the weight loss, pH variation and microstructure change with immersion duration in SBF solution. As a result, the OCP/HA disc was not quickly decomposed and maintained its own shape for 2 weeks regardless of HA content. In particular, the surface of 40HA specimen was uniformly dissolved and then CDHA (calcium deficient hydroxyapatite) phase were formed onto the surface of disc after 7 days in SBF solution. It would be suggested that the 40HA specimen would be suitable candidate material as the scaffolds for the restoration of bone defect.

  • Purpose: By categorizing opinions by subject in the safety certification ecosystem, we want to identify weaknesses in system operation and suggest improvement plans so that the safety certification system can have quick resilience against future variability. Method: Through literature research and data analysis, similar domestic and foreign safety certifications and related cases, as well as the current status of international standards and national standards, etc. were confirmed, and a fact-finding survey was conducted for each stakeholder in the safety certification ecosystem, and problem types and improvement measures were established. Result: We conduct a fact-finding survey of the overall system, such as quality satisfaction with safety certification target products, obstacles in the development, manufacturing and use process, and safety certification-related improvements, targeting workplaces that manufacture, import or use safety certification target machines By discovering and categorizing problems and weaknesses in system operation, detailed implementation tasks were derived to establish improvement directions and improve operability. Conclusion: For the advancement and internationalization of the safety certification system, it is necessary to efficiently carry out the detailed promotion tasks derived from this study. In addition, in order to strengthen the resilience to the variability of the safety certification ecosystem, the operating system of a virtuous cycle structure by improving the mutual relationship between each subject construction is considered important.

Combined use of direct and indirect digital impression in temporary denture fabrication (직접 및 간접 디지털 인상을 병용한 임시 의치 제작 증례)

  • Through the use of intraoral scanners, it is possible to obtain intraoral scan impressions and produce prostheses. This approach is also being attempted not only in dentate patients but also in edentulous patients. However, obtaining scans of edentulous areas can be more challenging than scanning dental areas, and there may be limitations, especially in capturing the details of the mucosal tissues. On the other hand, when obtaining impressions with intraoral scanners, simultaneous recording of the occlusal relationship of the maxilla and mandible can reduce the number of patient visits and expedite the restoration process. In this case, we aimed to combine the advantages of direct intraoral scanning and indirect digital impressions obtained after traditional impression-taking by merging two types of scan files. Consequently, in patients with partially edentulous arches, we sought to provide effective interim prostheses through direct and indirect digital model impressions and report our findings accordingly.

Effects of Hydraulic-hydrological Changes by Monsoon Climate on the Zooplankton Community in Lake Paldang, Korea (몬순기후로 인한 수리수문학적 변화가 팔당호 동물플랑크톤 군집에 미치는 영향)

  • The zooplankton community of Lake Paldang, Korea, was investigated on a weekly basis from 2004 to 2006. The seasonal succession of zooplankton community structure was influenced by hydrological factors such as rainfall pattern and efflux in Lake Paldang. According to the monsoon climate, spring, fall and winter had reduced precipitation, so that zooplankton dynamics of the lake showed a typical succession pattern. In spring, small sized and faster growing rotifera rapidly increased, and copepods and cladocera noticeably increased thereafter. Rotifera dominated the zooplankton community, occupying more than 90% of total zooplankton abundance. Among rotifera, Keratella cochlearis was extremely dominant in spring. Copepoda were mainly present as Copepodid and Nauplius. Among cladoceran species, Bosmina longirostris was dominant. In summer, during the rainy season, zooplankton were flushed out by an associated dam. After the rainy season, rotifera increased rapidly when the water column of the lake was stable. During the fall, zooplankton abundance gradually reduced in accordance with decreasing water temperature. However, the occupation rate of copepod (Copepodid, Nauplius) increased relatively. Zooplankton dynamics were influenced by meteorological changes and hydraulic-hydrological factors, because Lake Paldang is a completely closed ecosystem.

Applicability of Pulse Tests to Estimate Transmissivity in Crystalline Rock (결정질 암반의 투수량계수 도출을 위한 펄스시험의 적용성 연구)

  • In this study, pulse tests were undertaken at an underground research facility, as part of in-situ hydraulic tests, to derive the hydrogeological characteristics of crystalline rock. The applicability of pulse tests for estimating the transmissivity of a fractured rock mass was evaluated by comparing the results to those from a slug test. Results from the pulse and slug tests were very similar for the test section, with both tests indicating low transmissivity. A slight difference between the results of pulse and slug tests, however, was observed in the section with the transmissivity larger than 1 × 10-8 ㎡/s, which is likely due to the difference in the radii of influence of the tests. Furthermore, when the pulse test was conducted in permeable zones where transmissivity was larger than 1 × 10-7 ㎡/s, it was difficult to produce accurate results. This lack of accuracy was due to the rapid recovery of the hydraulic head in these permeable test zones. When performing pulse tests, it was important to accurately measure the pressure when valves were opened and closed in order to apply the head change in the test section. Although it is difficult to derive the hydrogeological characteristics from pulse tests in areas with high permeability, these tests can be used as an economical test method for identifying hydrogeological characteristics in a relatively short time, especially when deriving the transmissivity of rocks with low permeability.
