Problematized obesity and standardization of treatment: Multiple translation in lapband surgery network (문제화된 비만과 치료의 표준화 과정: 랩밴드 수술 연결망에서의 다중번역)

  • Globally, awareness about obesity is increasing rapidly. In Korea, obesity is recognized as a disease and steps are being taken to treat it. From the health governance point of view, such standardized measures amplify the risk of obesity and thus play an important part in the prevention of the disease. In this context, various obesity treatments act as a medium for the problem-solving process. In recent years, obesity surgery has been viewed as a rational solution to the problem of obesity. In the context of standardization of treatment, Callon's "Process of Translation" in STS theories highlights the importance of the central actor (Obligatory Passage Point; OPP). However, in the case of obesity, it is difficult to identify a single OPP to project different perspectives of an actor's needs. "Lapband surgery" often acts as a "boundary object" in this context. This article assesses this absence of central actors in the process of problem solving through a case study of adoption of Lapband surgery in Korea. Further, we attempt to suggest an analytical framework with a boundary object and multiple translation concepts to aid solving the problem of obesity.

Weight loss effects of Bariatric Surgery after nutrition education in extremely obese patients (고도비만환자에서 베리아트릭 수술 (Bariatric Surgery) 후 영양교육이 체중감량에 미치는 효과)

  • Purpose: This study was planned to determine the characteristics of extremely obese patients during Bariatric surgery and to evaluate how the difference in the number of postsurgical personal nutritional educations they received affected the weight loss. Methods: This is a retrospective study on the basis of the medical records of extremely obese patients for 15 months after receiving gastric banding. A total of 60 people were selected as the study subjects and they were divided into the Less Educated Group and the More Educated Group according to the average number of personal nutritional educations they received. We investigated both groups to determine the general characteristic, health related lifestyle habits, obesity related complications and symptoms in possession, and eating habits before their surgery, the body composition measurement result, obesity determination indices at 1, 3, 6, 9, 12, and 15 months before and after their surgery, and the biochemical parameters at 6 months before and after their surgery. Results: Body fat and weight showed rapid reduction until 6 months after the surgery, but thereafter reduced slowly depending on the result of body composition measurement. Regarding body fat and weight, the More Educated Group, who received nutrition education more often, showed significantly lower levels than the Less Educated Group at 15 months after surgery. Regarding BMI and degree of obesity, the More Educated Group showed significantly lower levels than the Less Educated Group at 15 months after surgery. Here, we were assured that BMI is reversely proportional to the number of personal nutritional educations at 15 months, which is more outstanding after surgery than before surgery. Conclusion: Long-term nutritional education is a key factor for the extremely obese patient in maintaining the effects of Bariatric surgery on weight and body fat reduction onwards. In the next stage, considering the characteristics of the study subjects, adoption of individual nutrition education is recommended for postsurgical prospective arbitration of obesity in order to monitor blood pressure, obesity related complications, symptoms in possession, and how eating habits and health related life habits change, and to judge the actual effect of the nutritional education method at the same time.

  • Several reports have described the increased perioperative risks as well as surgical techniques for performing various procedures in obese patients. However, few reports have addressed the effects of obesity on routine arthroscopic procedures known to be common and safe. Describing the case of a partial meniscectomy performed in a 17-year-old male with a body weight of 120 kg, we review literatures published to the present and provide technical problems and possible complications that can occur when performing routine knee arthroscopy in bariatric patients.

  • The obesity is a medical condition in which excess body fat has accumulated to the extent. It may have an adverse effect on health, leading to reduced life expectancy and increased health problems such as lifestyle disease, hypertension, and diabetes. Furthermore, it may cause the mental illness such as depression, bipolar disorder, anorexia, hyperphagia, and so on by shrinking psychologically. So, it is regarded as the serious worldwide social problem. Recently, although there are many general obesity care methods with various approaches, these can't be the radical obesity care because of the side effects such as surgery aftereffect, drug addiction, and so on. For these reasons, a methodical approach with a consideration of individual obesity pattern is strongly demanded. In this study, complex obesity care and management system which is overcome the weakness of non-surgical obesity treatment is proposed. And the individually adapted obesity treatment system with the far-infrared therapy module, oxygen therapy module, color therapy module, and microwave therapy module was developed. The complex obesity treatment system which is proposed in this study, could improve the efficiency of treatment by suggesting the various treatment protocols. And proposed system could be the foundation of the high quality obesity treatment system.

A Case of Bariatric Surgery for an OSAS Patient with Severe Obesity (고도비만이 동반된 폐쇄성수면무호흡증 환자에서 시행된 비만대사수술 1례)

  • Obstructive sleep apnea syndrome (OSAS) has negative effects on health, including increased mortality, risk of cardiovascular disease, and neurocognitive difficulties. OSAS is common in obese patients and obesity is an important risk factor of OSAS. A 41-year-old female OSAS patient with severe obesity (body mass index [BMI] ${\geq}35$) who failed dietary weight loss underwent bariatric surgery. After surgery, there were improvements in BMI (from 36.9 to $31.7kg/m^2$) and polysomnographic data, including the apnea-hypopnea index (from 25.1 to 11.2 events/hr) and minimum SaO2 (from 69 to 82%). This case demonstrates that bariatric surgery may be an effective therapeutic option to reduce sleep-disordered breathing in severely obese patients with moderate OSAS. Bariatric surgery as a treatment option for OSAS should be considered in OSAS patients with severe obesity who failed dietary weight loss.

  • The contemporary peoples focus on treatment of obesity in order to prevent the adult disease and to manage the beauty. Although surgical treatment of obesity shows the reliable cure effect, it could cause side effects and has a disadvantage that postoperative recovery period is long. Accordingly, this paper compares and analyzes the non-operative treatments which can be of help to treat obesity. Also, it considers the obesity therapy based on the Peltier cooling system. And finally a basic control circuit based on Peltier module is designed for Peltier cooling-based obesity therapy system.

The inhibitory effect of swimming exercise on inflammation in adipose tissue of obese mice after ovariectomy (수영 운동이 난소절제 후 비만 쥐의 지방조직에서 염증에 미치는 억제 효과)

  • This study investigated whether swimming exercise suppresses(: inhibites) the inflammatory response in adipose tissue in obese mice induced by a high fat diet after ovariectomy in female mice. Experimental group and control group were divided into sham-opereated group (Sham), ovariectomized group (OVX), and ovariectomized group with swimming exercise (OVX/Ex). All female mice fed high fat diet (45% fat) for 8 weeks. Compared with the Sham, the OVX increased the weight of visceral adipose tissue including parametrial, retroperitoneal, and mesenteric adipose tissue. Compared to the OVX, the OVX/Ex demonstrated reductions the weight of visceral adipose tissue. Compared to the Sham, the OVX decreased the mRNA expression of IκBα and increased the mRNA expression of inflammatory cytokines (L-6, MCP-1, TNF-α and leptin) in adipose tissue. But the OVX/Ex appeared to increase the mRNA expression of IκBα and decrease the mRNA expression of inflammatory cytokines in adipose tissue, compared with the OVX. This findings suggested that swimming exercise is useful for preventing adipose tissue inflammation, which will be effective in the prevention and treatment of obesity in obese mice induced obesity by a high fat diet after ovariectomy.

The Review on the Evidence : Effects on Non-surgical Localized Fat Treatments (비수술적 부분비만 치료 효과에 관한 근거 중심적 연구)

  • Localized fat and cellulate have not only esthetic but health problem, they are treated by surgical or non-surgical method. There are elecro-acupuncture, ultrasound, radio frequency, endermologie, mesotherapy, and carbon dioxide therapy in prevalent non-surgical treatments. We reviewed the studies on them to analyze their seicntific evidence. The studies are not many. They reported good results of circumference loss or appearance improvement of histologic change but most of them have poor study design. Before marketing or using these modalities, it is ontcal to evaluate to their effectiveness through strict and scientific clinical trials.
