Pediatric Renal Transplantation in Asia (아시아 소아 신장이식 현황)

  • Purpose : Renal transplantation which allows children normal growth and development and a return to normal life. is now proven to be the best modality for children with ESRD Up to Recently, the number of renal transplantations in Asia has rapidly increased and the outcome has also improved. This investigation was planned to estimate the current status of pediatric renal transplantation in Asia and to find the keys for better improvement of outcome in pediatric renal allograft in Asian countries. Material and methods : The participating countries and institutions for this investigation were China, Hong Kong, India, Indonesia, Japan, Malaysia, Pakistan, Philippines, Singapore, Thailand, Korea, KSPN (Korean Society of Pediatric Nephrology), KONOS (Korean Network for Organ Sharing). Results : Many countries in Asia still do not have a well organized nation wide renal transplantation registration system independently in the pediatric field. So it's very difficult to evaluate the real state of pediatric transplantation among Asian countries. According to the estimation with fragmented data from each countries, in the front running countries of pediatric renal transplantation in Asia, about 40 or more transplants were performed in each country per year and the five year actuarial renal allograft survival was around 80% which is similar to that of western countries. But there were large gaps among the behind groups. Conclusion : Vigorous attempts to perform renal transplantation for children especially younger than 5 years old would be encouraged as well as organ donation from brain dead donor and non heart beating cadaveric donor also should be activated to cope effectively with the shortage of living donor supply. Large number of recent reports shows the favorable outcome of pre-emptive renal transplantation, we should make more efforts toward pre-emptive renal transplantation. First of all, in order to improve the outcome and to narrow the gap between Asian countries in pediatric renal transplantation, effective and continuous efforts to establish nationwide pediatric renal transplantation registration program as well as official, nation-to-nation data sharing program should be needed.

Aprotinin Therapy for Cardiac Operation in Adult -Full dose regimen- (개심술에서 아프로티닌의 효과 (고용량 요법))

  • Background: Recently, many cardiac centers have been using aprotinin to reduce operative bleeding in cardiac operations using cardiopulmonary bypass. A variety of reports have confirmed the effectiveness of the drug in cardiac operations. In addition to the operations which could be considered to cause severe operative bleeding such as redo operation, long cardiopulmonary bypass operation and etc, the use of aprotinin is increasing in the field of primary cardiac operations. Varying doses of regimen have been introduced since the first report by Royston et al, and also various opinions on the effectiveness and safeness of the each regimen have been reported. We reviewed our own experience of the full dose aprotinin regimen(Hammersmith regimen) retrospectively. Material and Method: From October 1994 to February 1998, 40 cases of cardiac operative patients were randomized into two groups: aprotinin group(20 patients) which received a full dose aprotinin regimen and control group(20 patients) which did not receive aprotinin. To evaluate the degree of bleeding decrease, we analysed and compared the amount of postoperative 6 hours and 24 hours bleeding in the each group. To confirm the renal dysfunction, we measured the postoperative creatinine level. Result: In the amount of postoperative 6 hours bleeding, a statistically significant bleeding decrease was demonstrated in the aprotinin group compared to the control group(aprotinin group: 186${\pm}$40cc, control group:409${\pm}$69cc, P=0.010). Similar result was observed in the postoperative 24 hours(aprotinin group:317${\pm}$53cc, control group: 671${\pm}$133cc, P=0.024). Conclusion: We concluded that full dose regimen of aprotinin can remarkably reduce postoperative bleeding in cardiac operations without significant renal dysfunctions.

A Comparative Study of Stenting versus Surgical Bypass in Gastric Outlet Obstruction Caused by Gastric Cancer (위암 날문부 폐쇄 환자에게 시행한 치료의 비교고찰: 위-공장 우회술과 금속성 자가팽창성 스텐트 삽입술)

  • Purpose: In gastric cancer patients with gastric outlet obstruction, there are several complications such as malnutrition and vomiting. Palliative enteral stenting is a less invasive procedure as compared with a gastrojejunostomy. The aim of this study was to determine whether there was a significant difference between patients that undergone palliative enteral slanting and patients that had received a bypass gastrojejunostomy. Materials and Methods: One hundred patients underwent palliative entering stenting and 3f patients were subjected to a surgical bypass gastrojejunostomy. We reviewed the medical records of the patients with gastric outlet obstruction secondary to far advanced gastric cancer that were diagnosed using a gastrofibroscope, UGI and abdominal CT, and were admitted to our institution between January 2000 and August 2006. The outcome of stent placement for gastric outlet obstruction was compared with palliative gastrojejunostomy during the same period. We excluded patients with recurrent gastric cancer and double cancer from this study. Results: There were significant differences between the group of patients that underwent slanting and the group of patients that received a gastrojejunostomy regarding the age of patients ($67{\pm}12$ vs. $57{\pm}9$, P<0.001) but not between the sex of the patients (M : F, 2 : 1 vs. 2 :1, P=0.637). The most common complication of stenting was tumor ingrowth (16/100, 16%) and the second most common complication was stent migration (14/100). Failure of the procedure occurred in only three patients. Twenty-three patients underwent re-slanting and one patient required open conversion with a gastrojejunostomy. The median time to the first meal was $4{\pm}2$ days in the stent group of patients and $6{\pm}2$ days in the gastrojejunostomy group of patients (P=0.001). The median postoperative hospital stays were 9 days in the stent group of patients and 15 days in the gastrojejunostomy group of patients (P=0.003). The mean survival periods were 11 months in the stent group of patients and 10 months in the gastrojejunostomy group of patients (P=0.937). Conclusion: There were no significant differences In the mean survival rates. An earlier first meal and a shorter hospitalization stay were found in the slanting group of patients compared to the bypass gastrojejunostomy group of patients. However, re-slanting was a concern due to tumor ingrowth and stent migration.

An Empirical Comparison and Verification Study on the Containerports Clustering Measurement Using K-Means and Hierarchical Clustering(Average Linkage Method Using Cross-Efficiency Metrics, and Ward Method) and Mixed Models (K-Means 군집모형과 계층적 군집(교차효율성 메트릭스에 의한 평균연결법, Ward법)모형 및 혼합모형을 이용한 컨테이너항만의 클러스터링 측정에 대한 실증적 비교 및 검증에 관한 연구)

  • The purpose of this paper is to measure the clustering change and analyze empirical results. Additionally, by using k-means, hierarchical, and mixed models on Asian container ports over the period 2006-2015, the study aims to form a cluster comprising Busan, Incheon, and Gwangyang ports. The models consider the number of cranes, depth, birth length, and total area as inputs and container twenty-foot equivalent units(TEU) as output. Following are the main empirical results. First, ranking order according to the increasing ratio during the 10 years analysis shows that the value for average linkage(AL), mixed ward, rule of thumb(RT)& elbow, ward, and mixed AL are 42.04% up, 35.01% up, 30.47%up, and 23.65% up, respectively. Second, according to the RT and elbow models, the three Korean ports can be clustered with Asian ports in the following manner: Busan Port(Hong Kong, Guangzhou, Qingdao, and Singapore), Incheon Port(Tokyo, Nagoya, Osaka, Manila, and Bangkok), and Gwangyang Port(Gungzhou, Ningbo, Qingdao, and Kasiung). Third, optimal clustering numbers are as follows: AL(6), Mixed Ward(5), RT&elbow(4), Ward(5), and Mixed AL(6). Fourth, empirical clustering results match with those of questionnaire-Busan Port(80%), Incheon Port(17%), and Gwangyang Port(50%). The policy implication is that related parties of Korean seaports should introduce port improvement plans like the benchmarking of clustered seaports.

Accuracy of implant digital scans with different intraoral scanbody shapes and library merging according to different oral exposure height (구내 스캔바디의 형태에 따른 임플란트의 디지털 스캔 정확도 및 구강 내 노출 높이에 따른 라이브러리 중첩 정확도 비교 연구)

  • Purpose: The purpose of this study is to compare the accuracy of digital scans of implants according to different shapes of scanbodies, and to compare the accuracy of library merging according to different oral exposure height. Materials and methods: A master model with a single tooth edentulous site was prepared. For the first experiment, three types of intraoral scanbodies were prepared, divided into three groups, and the following experiments were conducted for each group: An internal hex implant was placed. The master model with the scanbody connected was scanned with a model scanner, and a master reference file (control group) was created. 10 files (experimental group) were created by performing 10 consecutive scans with an intraoral scanner. After superimposing the control and experimental groups, the following values were calculated: 1) Distance deviation of a designated point on the scanbody 2) Angle deviation of the major axis of the scanbody. For the second experiment, the scanbody scan data were prepared in 6 different heights. Library files were merged with each of the scan data. The distance and angular deviation were calculated using the 7 mm scan data as control group. Results: In the first experiment, there were no significant differences between A and B (P=.278), B and C (P=.568), and C and A (P=.711) in the distance deviations. There were no significant differences between A and B (P=.568), B and C (P=.546), and C and A (P=.112) in the angular deviations. Also, the scanbody showed significantly higher library merging accuracy in the groups with high oral exposure height (P<.5). Conclusion: There were no significant differences in scan accuracy according to the different shapes of scanbodies, and the accuracy of library merging increased according to exposure height of the scanbody in the oral cavity.

Accuracy of bite registration according to the buccal bite scan range of intra-oral scanner (구강 스캐너의 협측 교합 스캔 부위에 따른 교합 인기의 정확도)

  • Purpose: The aim of this study was to determine which scan range would provide the most accurate bite registration when performing a bite scan after scanning an upper and lower arch using an intraoral scanner. Materials and Methods: The occlusal contact points were recorded using articulating paper for 30 adults, and the results of various ranges of buccal bite scan were compared based on this. Buccal bite scan of 5 ranges (1st premolar to 2nd premolar, 1st premolar to 1st molar, 1st premolar to 2nd molar, 2nd premolar to 1st molar, and canines to another side canine of the maxillary teeth) was performed, and then the buccal bite scan file was used in a CAD program to confirm the occlusal area in the scan file through data editing and alignment, leaving the buccal area of the teeth. Afterwards, the degree of agreement between the occlusal contact points obtained from the articulating paper and the occlusal area obtained from the scan file was compared, and statistical analysis was performed using the homoscedastic T-test (α = 0.05). Results: The alignment success and alignment failure rates among each group were 77.23% and 40.85% in canine to another side canine, 68.23% and 28.89% in bilateral first premolar to second premolar, 63.76% and 29.97% in bilateral first premolar to first molar, 61.31% and 32.04% in bilateral first premolar to second molar, 67.55% and 27.46% in second premolar to first molar. The results of the anterior scan of both canines showed higher alignment success and failure rates compared to the scan results of all maxillary posterior teeth. In the alignment success rate, statistical significance was not found depending on the scan range of the posterior teeth, but in comparing the results of the posterior teeth and both canines, statistical significance was observed except for the scan results of the second premolar to the first molar. There was no statistical significance in the alignment failure rate depending on the scan range of the posterior teeth, and statistical significance was observed in the results of the posterior teeth and both canines. Conclusion: When taking a buccal bite scan, in the case of scanning the anterior teeth, more occlusal area appear than when scanning the posterior teeth, and in the case of scanning the posterior teeth, there is no significant difference in the bite registration depending on the scan range.

  • Background : Idiopathic pulmonary fibrosis(IPF) is a devastating illness for which there is little effective treatment. The key cytokines currently implicated in the fibrotic process are the transforming growth factor-${\beta}_1$(TGF-${\beta}_1$), tumor necrosis factor-$\alpha$(TNF-$\alpha$), endothelin-1(ET-1) and interferon-$\gamma$(IFN-$\gamma$). The rat model for paraquat-induced pulmonary fibrosis was chosen to investigate the role of ET-1 in this disease. Both ET-1 and TGF-${\beta}_1$ expression in lung lesions were examined using immunohistochemical staining. After $Bosentan^{(R)}$ administration, an orally active ET-$l_A$ and ET-$1_B$ receptor antagonist, the degree of pulmonary fibrosis and ET-1 and TGF-${\beta}_1$ expression were analyzed. Method : Sprague-Dawley rats were divided into three groups, the control group, the fibrosis group, and the fibrosis-$Bosentan^{(R)}$-treated group. The animals were sacrificed periodically at 1, 3, 5, 7, 10, 14 days after administering saline or paraquat. The effects between groups were compared with the results of light microscopy and immunohistochemical staining for ET-1 and TGF-${\beta}_1$. The degree of fibrosis was evaluated by H&E and Masson's trichrome staining, which were graded by a computerized image analyzer. The degree of immunohistochemical staining was categorized by a semi-quantitative analysis method. Results : The lung collagen content had increased in the paraquat instillated animals by day 3, and continued to increase up to day 14. A daily treatment by gavage with $Bosentan^{(R)}$ (100mg/kg) did not prevent the increase in collagen deposition on the lung that was induced by paraquat instillation. There were increased immunohistochemical stains of ET-1 on the exudate, macrophages, vascular endothelial cells and pneumocytes in the paraquat instillated group. Furthermore, TGF-${\beta}_1$ expression was higher on the exudate, macrophages, some inflammatory cells, pneumocytes( type I, and II), vascular endothelium and the respiratory epithelial cells around the fibrotic area. After Bosentan treatment, there were no definite changes in ET-1 and TGF-${\beta}_1$ expression. Conclusion : Fibrosis of the Paraquat instillated group was more advanced when compared with the control group. In addition, there was increased ET-1 and TGF-${\beta}_1$ expression around the fibrotic area. The Effects of a School Zone Improvement Project on Crash Reduction Regarding Land Use Characteristics (토지이용 특성별 어린이 보호구역 개선사업의 교통사고 감소효과 분석)

  • The first phase of School Zone Improvement Project introduced to enhance the safety of school-routes were completed by 2007 and the second phase of the project will be continued from 2008 to 2012. Since a huge budget is required for the project, there is a need to evaluate how many child-related crashes were reduced through the project, prior to the implementation of the second phase. Therefore, this study aims to analyze the influence of the School Zone Improvement Project on reduction in child-related crashes with data collected from the database of the National Police Agency. This study employed a Before-and-After study with comparison group to assess how many crashes were reduced. The results showed that the number of child-related crashes were decreased by 10, which is about 39% reduction in crash frequencies. With respect to the land use characteristics of school zones, the number of reduced crashes in residential zone, commercial zone, and mixed zone were 2.41, 3.8, and 0.5, respectively. The results may provide a significant implication for the second phase of the project that there is a need to establish different plans for improvement project with respect to land use characteristics.

A study on the difference analysis between an ideal and a clinical shape in case of manufacturing a metal-ceramic pontic substructure (금속-도재 가공치 하부구조 제작에서 이상적인 형태와 임상에서 사용되는 형태의 차이 분석)

  • Purpose: The purpose of this research is to determine whether pontic metal substructures, which are currently used in clinical surgeries, are designed appropriately and identify the problems that can occur due to their shape, size, and position. Then it aimed to emphasize the importance of making and designing pontic metal substructures based on basic principles. Materials and Methods: This research measured pontic basal surface (P1) used sample metal substructures in this study, gingiva margin (P2), and the porcelain thickness of maximum infrabulge of labial surface around 1/3 of cervix dentis (P3). One-way ANOVA analysis was carried out to test the differences among groups, Tukey Honestly Significant Difference Test was conducted for statistical analysis among groups. Results: For porcelain thickness and SD value, the P1 part was $1.2-1.8({\pm}0.17)mm$ for experimental group 1, $1.2-1.7({\pm}0.17)mm$ for experimental group 2, and $0.4-2.8({\pm}0.92)mm$ for experimental group 3. Next, the P2 part was $1.4-1.6({\pm}0.07)mm$ for experimental group 1, $1.3-1.8({\pm}0.07)mm$ for experimental group 2, and $0.5-2.7({\pm}0.67)mm$ for experimental group 3. The P3 part was $1.4-1.7({\pm}0.10)mm$ for experimental group 1, $1.5-2({\pm}0.10)mm$ for experimental group 2, and $0.9-3.1mm({\pm}0.90)$ for experimental group 3. There was no significance when One-way ANOVA analysis/Tukey Honestly Significant Difference Test was conducted for statistical analysis among groups (P > 0.05). Conclusion: The suggested metal substructures can be used clinically as they meet the requirements that pontic must have.

Ultrasonographic Findings of the Shoulder in Asymptomatic High School Baseball Players (무증상 고교 야구 선수의 견관절 초음파 소견)

  • Purpose: To evaluate the ultrasonographic (US) findings in both shoulders of asymptomatic high school baseball players and compare with healthy control. Materials and Methods: 42 individuals (age: $7.6{\pm}1.2$ years) participated in this study. We recruited two groups of high school male baseball players, 14 pitchers and 18 fielders without shoulder pain for recent 6 months as well as one control group of 10 untrained healthy high school students who were age-matched. Ultrasound measurements of thickness of the biceps and supraspinatus tendon and acromio-humral distance (AHD) at $0^{\circ}$ abduction were taken in dominant and nondominant shoulders. Results: On US examination, subacromial bursa effusion was observed in 7 of the dominant shoulders and in 2 of the nondominant shoulder of 32 baseball players and in none of the asymptomatic controls. The thickness of the biceps and supraspinatus tendons and AHD in both shoulders were significantly greater in the baseball players than in the controls (p<0.05). We also found that the biceps and supraspinatus tendon thickness and AHD of the dominant shoulder were significantly greater than the non-dominant shoulder in baseball players (p<0.05), but there were no differences between pitchers and fielders and between healthy controls. Conclusion: Subacromial effusion could be found even in the asymptomatic stage and the thickness of the biceps and supraspinatus tendons and AHD were significantly greater in the baseball players than in the healthy controls. Our data is useful reference guide for the ultrasonographic diagnosis of shoulder pathologies occurring in high school baseball players.
