하악전치부 분절골절단술식기의 연조직가변화에 대한 고려;치험 2례 (CONSIDERATION OF THE SOFT TISSUE CHANGES IN ANTERIOR SEGMENTAL OSTEOTOMY OF THE MANDIBLE;REPORT OF TWO CASES)

  • Facial balance is the primary detevminant of good facial esthetics and is expressed externally by the shape of facial soft tissues. Balance of the facial skeleton is most important in prediction of orthognathic surgery, however, it is not alwags coincided to soft tissue balance because the soft tissue drapes overlying hard tissue varies in thickness and tones from case to case. So, soft tissue facial balance and esthetics also should always be considered in prediction of hard tissue changes preoperatively. The chin has a paramount importance in the overall appearance of the face and facial profile because it may express individual charactor or image. Therefore positional change of the chin must be considered in any cases as the last and important option to give an overall soft tissue balance. Two cases were referred from orthodontists only for anterior segmental of teortomuy of the chin. Pre-operative evaluation showed poor soft tissue chin profiles which were not coincided to hard tissue chin balance. We altered surgical plans to fulfill balancing soft tissue profile and then could improve overall esthetics after surgery.

성인의 상하악 치조 전돌증의 교정 - 외과적 악교정 2례 - (Surgical-Orthodontic Correction of Adult Bimaxillary Protrusion - Report of 2 cases -)

  • 저자들은 영남대학교 의과대학 부속병원 치과에 상하악 치조전돌을 주소로 내원한 2명의 환자에게 상하악의 전치부 분절골절단술과 교정치료를 병행하여 심미적으로 좋은 결과를 얻었기에 치료결과를 아래와 같이 요약했다. 1. 전치부를 후방 위치시키는 외과적 술식으로 제 1소구치 발치 공간을 이용해 Wunderer, Kole방법을 사용했다. 2. crowding이 있었던 증례 2에서는 술전 교정으로 이를 해소했고 intrusion시키는 단계에서 외과적 시술을 시행하였다. 3. 술전 paper surgery, cast surgery에 의해 술후 결과를 어느 정도 예상할 수 있었다. 4. cast상에서 resin splint를 제작하여 6주 동안 악내고정하였고 이에 따라 악간고정은 필요치 않았다. 5. 증례 2에서 수술후 남아있는 deep bite를 개선하기 위해 술후교정을 시행했다. 6. 각각 3개월, 5개월 관찰하였는 바 아직까지는 relapse와 합병증이 나타나지 않았으며 심미적인 면에서 개선된 양상을 보였다.

양악 전방분절골절단술과 이부 성형술을 통한 개방교합의 치험례 (TREATMENT OF ANTERIOR OPEN BITE WITH BIMAXILLARY ANTERIOR SEGMENTAL OSTEOTOMY AND GENIOPLASTY)

  • Skeletal anterior open bite is a difficult problem to correct in orthodontic treatment. To treat adult patients who have skeletal anterior open bite, we considered two methods. Combination treatment of orthodontics & surgery and camouflage orthodontic treatment. In adults, treatment of severe skeletal anterior open bite consists mainly of surgically repositioning the maxilla or the mandible. However, camouflage therapy is often the treatment of choice for skeletal open bite patients who have mild to moderate skeletal discrepancies when growth modification is no longer possible. But excellent results generally require careful coordination of the orthodontic and surgical phases of treatment. This is a case report of a skeletal anterior open bite patients who were treated with orthodontic treatment and orthognathic surgery. First case was diagnosed as skeletal class I malocclusion & bimaxillary protrusion with anterior open bite, and finally treatment ended for removal of open bite with orthodontic procedure and bimaxillary anterior segmental osteotomy surgery. Second case was diagnosed as skeletal class II malocclusion with open bite & mandibular retrusion, and was treated with only camouflage orthodontics because she feared to have a surgery. In a regular follow up visit after debonding we proposed to the patient advanced genioplasty, and in her agreement her facial esthetics was improved through the surgery.

양악전돌증 환자에서 하악 6전치 후방 이동 시 치료 방법에 따른 하악 경조직과 연조직의 변화: 하악 전방분절골절단술과 발치 교정 치료 (The Evaluation of Soft and Hard Tissue Change for Retraction of Lower Anterior Tooth in Bimaxillary Protrusion Patients according to Two Different Therapeutic Methods: Mandibular Anterior Segmental Osteotomy and Orthodontic Treatment with Teeth Extraction)

  • Purpose: The purpose of the study is to evaluate the effectiveness of anterior segmental osteotomy (ASO) in bimaxillary protrusion (BP) patients by comparison between the mandibular soft and hard tissue changes from orthodontic treatment and ASO. Methods: All 44 patients were diagnosed with BP in Kyung-Hee Medical Center. Orthodontic treatment with teeth extractions were underwent by 23 patients (Group A) and 21 patients underwent ASO (Group B). Mandibular soft and hard tissue changes were measured and evaluated, which were based on the vertical and horizontal reference line in lateral cephalometric radiographs. Statistical significance between the changes and correlation between each measurement were analyzed. Results: The amount of B point movement was lesser than that of the lower incisal tip (LIT) retraction, and LIT was tilted lingually in group A. The posterior movement discrepancy between LIT and B point was insignificant, and the inclination of lower incisor was not changed in group B. The soft tissues, including the lower lip, showed a posterior movement and reduction in the depth of mento-labial groove. According to the correlation analysis, the movement of the lower incisor was significantly related to the movement of the lower lip in group A, and the movement of the lower incisor was significantly related to that of the movement of lower lip, B point and Pog in group B. Conclusion: The orthodontic treatment in BP patients results in posterior tilting movement of the lower incisor, but ASO results in the bodily movement of the lower incisor. Consequently, ASO is more effective in BP cases because it ensures the controlled movement of the lower incisors.
