Indications and Estimations of the Needs for Direct Medical Control in the Patients Transported by 119 Rescuers (119 구급대에 의해 이송된 환자들 중 직접적 의료지도가 필요한 범위와 그에 따른 수요 추정)

  • Direct medical control by medical doctors is an essential part of emergency medical services system (EMSS). However, the indications are not specifically defined, even in 119 system with their own medical control team. The Seoul Metropolitan Fire and Disaster Management Department has operated internal medical consultation services on its own since January 2004. Based on the experiences from these services, we reviewed the cases of the direct medical consultation and establish the indications for direct medical control. And we presumed the demand of direct medical control with the established indications. The crews of 119 in Seoul made 793 calls to Medical Control Team during November 2004. We reviewed all of the calls according to the level of consciousness (AVPU), the kinds of emergency care done by crews during transport (10 categories), and the mechanisms of injuries (9 categories). The need for direct medical control was judged by authors with reviewing the records reported by the crews and control teams. Among 23 items, 14 items assigned as the indications, which were abnormal level of consciousness (VPU), 6 kinds of emergency care, and 5 mechanisms of injures. The sum of the three of them, 7,782 cases (45.9%), was in need of direct medical control. In conclusion, about half of the patients transported by 119 crews in Seoul require direct medical control. The need for the direct medical control in Seoul was estimated as many as 260 calls per day. To fulfill the need for direct medical control and to provide a effective medical control, the direct medical control should be accomplished through the communications between the crews and the medical staffs in the local hospitals.

Production of Transgenic Animals by the Testis-Mediated Gene Transfer II. Production of Transgenic Korean Native Goats (정소실질내 유전자 도입에 의한 형질전환동물의 생산 II. 형질전환 한국재래산양의 생산)

  • The totipotential spermatogonial stem cells of adult testis which give rise to mature sperm cells is well-known to incorporate foreign DNA as well as those of somatic cells. Also, the integration rates of foreign DNA after haploid stages are generally known to decrease and /or is simply bound foreign DNA into the sperm plasma membrane. To overcome these problems, liposome and DNA complexes were used to determine how direct injection of these complexes into testis were integrated into sperm genome and resulted in transgenic offspring. To study this purpose, cation liposome was gently mixed with WAF/hGH DNA (1 : 2) and the complexes were injected into testis. At 10, 20, 40, 60, and 80 days after direct injection into testis, mature sperm cells were recovered by using artificial virgin method from two goats and each semen except a part of semen used for DNA analysis such as PCR or Southern blotting was cryopreserved for the artificial insemination. The results obtained in this study are summarized as follows. 1. By PCR, the presence of exogenous DNA was confirmed up to 80 days after injection with liposome/DNA complexes. The highest integration rates were obtained at day 40 after direct injection. This results suggested that spermatogonial stem cells were integrated exogenous DNA into their genome. 2. Among 23 Korean Native Goats which were artificially inseminated, 4 goats resulted in pregnancy and produced 7 young goats. 3. Two young goats were confirmed as a transgenic by PCR and Southern blot analysis. Therefore, our results suggested that testis-mediated gene transfer can be used as a feasible tools for the production of transgenic livestock.

Vitamin A content of human milk at 7 and 60 days postpartum (분만후 7일과 60일에 분비된 모유의 Vitamin A 함향)

  • The changes of vitamin A contents of transitional (7th day postpartum) and mature (60th day postpartum) milk were comparatively studied from 20 Korean lactating women to provide the basic information for recommended dietary allowances. The mean retinol contents($\mu\textrm{g}$/100$m\ell$) of transitional milk in primiparae and multiiparae were 79.71 (${\pm}$50.61) and 73.43(${\pm}$31.02) and those of mature milk in primiparae and multiparae were 44.73(${\pm}$19.03) and 38.25(${\pm}$17.63) respectively. The mean ${\beta}$-carotene contents ($\mu\textrm{g}$/100$m\ell$) of transitional milk in primiparae and multiparae were 10.44(${\pm}$7.43) and 14.15(${\pm}$11.81) and those of mature milk in primiparae and multiparae were 4.97(${\pm}$2.22) and 5.36(${\pm}$4.23) respectively. The total content of vitamin A($\mu\textrm{g}$/100$m\ell$) in transitional milk was 81.44(${\pm}$ 51.47) in primiparae and 75.79(${\pm}$31.99) in multiparae, and those of mature milk in primiparae and multiparae were 45.56(${\pm}$19.12) and 39.14(${\pm}$17.73) respectively. The vitamin A content decreased with increase of lactation period. The mean content of vitamin A between primiparae and multiparae during lactation was not significantly different (p>0.05) However, there was a significant difference in the total vitamin A contents according to lactation period. The average content($\mu\textrm{g}$/100$m\ell$) of retinol in lactating mothers delivered boys and girls were 74.45 (${\pm}$43.84) and 78.96(${\pm}$35.98) in transitional milk, 40.27(${\pm}$16.61) and 42.51(${\pm}$21.22) in mature milk, respectively. However, no significant difference was found between retinol contents contentscontentsin lactating mothers delivered boys and girls. The average contents($\mu\textrm{g}$/100$m\ell$) of ${\beta}$-carotene in lactating mothers delivered boys and girls were 11.34(${\pm}$4.27) in mature milk. The contents($\mu\textrm{g}$/100$m\ell$) of total vitamin A was significantly different (p<0.05) between transitional and mature milk the difference of total vitamin A content between lactating mothers delivered boys and girls. and primiparae and multiparae was not significant(p>0.05) As a result the contents of vitamin A, retinol and ${\beta}$-carotene in transitional and mature milk decreased according to the increase of lactation period.

Effect of Magnetic Field Therapy Applied to Acupuncture Point on Dysmenorrhea Experienced by High School Girls (경혈점에 적용한 자장요법이 여고생의 월경통에 미치는 효과)

  • This study was investigate the effects of the magnetic field therapy applied to the acupuncture point on dysmenorrhea experienced by high school girls and to verify the possibility to use this therapy as an effective in-home nursing intervention. Data were collected from Feburary to April, 2006. The research procedure was that female students who visited in public health center had to fill in questionnaires with dysmenorrhea Grapic Rating Scale(GRS) and Adjective Labor Pain Rating Scale(ALPRS), the gauge of pain, before participating the experiment. And then each group was tested for 3 hours. Participants before and after applying magnetic treatment filled each questionnaires with dysmenorrhea GRS and ALPRS. The GRS points and ALPRS points in the experimental group, applied by Magnetic Field therapy on acupuncture point is lower than that in the comparison group. The above result has proved that the magnetic field therapy on acupuncture has an effect on the pain alleviation of dysmenorrhea of female students in high school. Therefore, propose this therapy to dysmenorrhea of female students. In this research, ALPRS indicated validity on dysmenorrhea, so for verifying validity, further research is necessary.

Influence of Maternal Diet on Mineral and Trace Element Content of Human Milk and Relationships Between Level of These Milk Constituents (수유부의 식이섭취가 모유의 무기질 및 미량원소 함량에 미치는 영향과 모유의 각 무기질 농도 사이의 상관성 연구)

  • This study was conducted to assess the relationships between maternal dietary intakes and milk contents of minerals and trace elements, and the correlation among levels of these milk constituents. Maternal dietary intakes were measured and milk samples were collected at 2∼5 days, 2, 4, 6 and 12 weeks postpartum from 29 lactating women. The results obtained are sumarized as follows: 1) The overall mean nutrient intakes of lactating women in this study were below the recommended allowances and there were extensive individual variations between subjects. 2) Concentrations of minerals and trace elements in matured human milk showed the same range reported from different countries with the exception of calcium, magnesium, manganese and molybdenum which were relatively high. 3) There were no significant relationships between maternal dietary intakes of minerals and the corresponding mineral levels of human milk. In addition, no significant correlations were found between maternal vitamin C intake and the iron contents of milk. These were significant positive correlations between maternal calcium intake and the magnesium level of milk ; between maternal protein intake and the contents of zinc and copper in human milk. Maternal energy intake was negatively correlated with milk sodium level. 4) Pearson correlation coefficient showed positive significant relationships between levels of 17 pairs of various mineral and trace elements : sodium and potassium, iron ; potassium and calcium, phosphors, magnesium, iron, copper, manganese ; calcium and magnesium, iron manganese, molybdenm, nickel ; magnesium and iron, molybdenum ; iron and copper ; nickel and manganese.

The Effect of Rice Straw and Tall Fescue Hay for Holstein Dairy Heifers on Growth, Age at First Calving and Feed Cost (볏짚 및 톨페스큐 건초 급여가 홀스타인 육성기의 성장, 초산월령 및 사료비에 미치는 영향)

  • Given the recent increases in feed cost, the demand for rice straw has been increasing on dairy farms because it is cheaper than expensive imported forages. Therefore, it is necessary to study the effect of rice straw feeding on the growth and productivity of dairy heifers. This study compared how feeding tall fescue and rice straw affects the growth, age at first calving, and feed cost of heifers. we compared the growth, age at first calving, and feed cost for each group during the breeding period. Body height was higher in the tall fescue-fed group than in the rice straw-fed group (p<0.05). The length of the growing stage was significantly shorter in the tall fescue group than in the rice straw group (p<0.001) and the age at calving also showed a significant reduction in the tall fescue group compared to the rice straw group (p<0.001). However, the feed cost was lower in the rice straw group because of the difference in the unit price of forage (p<0.001). Based on these results, during the growing period, the rice straw group had delays in growth and the age at first calving compared to the tall fescue group, but also had reduced feed costs in the short term.

Studies on Dairy Farming Status, Reproductive Efficiencies and Disorders in New Zealand (I) A Survey on Dairy Farming Status and Milk Yield in Palmerston North Area (뉴질랜드 (Palmerston North) 의 낙농 현황과 번식 및 번식장해에 관한 연구(I) Palmerston North 지역의 낙농 현황과 우유 생산량에 관한 조사 연구)

  • Eighty dairy farms in Palmers ton North area in New Zealand were surveyed on 1) general characteristics (10 Questions), 2) milk yield and feed supplementary (7 questions), 3) reproductive efficiencies (12 questions) and 4) reproductive disorders (12 questions) by mail questions from February to July, 1998. Among those 4 items from 38 dairy farms (47.5%), especially in items 1) and 2), overall dairy farming situation, supplementary feeding and milk yields were surveyed and analyzed for Korean dairy farmers (especially in Cheju island) to have better understanding or higher economical gains. The results were as follows. 1. In dairy experience, 21 (45%) among 38 dairy farms surveyed were answered that farming less than 15 years, 15~19 year, 20~25 years and over 26 years experience were 3 (7.9%), 7 (18.4%), 6 (15.8%) and 5 (13.2%) which generally showed longer experience compare to Korean dairy farming situation. In survey of labour input and business goal of dairy farming, self-managing farms, sharemilkers, unpaid family manpowering farms, manager running farms, farms with hired worker, farms with part time helper and other type was 21 (55.3%), 10 (26.3%), 2 (3.5%), 3 (5.3%), 18 (31.6%), 2 (3.5%), and 1 (1.8%), respectively. 2. Analyzing pasture and tillable land, pasture according to feeding scale (200, 300 and 400 heads) were 56, 90 and 165.3 ha, and tillable lands were 51, 78 and 165 ha which showed some differences among feeding scale. In recording methods in 38 farms replied, 36 (95%) dairy handbook and 23 (70%) dual methods taking farms were higher than that of 10 (26.3%) computer and 15(39.5%) well-recorder methods. 3. Dairy waste processing facilities in environmental field were almost perfect except of metropolitan area, and so no problem was developed in its control so far. Hence, 26 farm (68.4%) of pond system was higher rather than those in 8 (21.2%) of using as organic manure after storing feces of dairy cattle, 1(2.6%) bunker system and 3 (7.9%) other type farms. 4. In milking facilities, 33 farms (86.9%) of Harringbone types were higher than those in 3 (7.9%) of Walkthrough types, 1 (2.6%) of Rotary system and other types. Although the construction facilities was not enough, this system show the world-leveled dairy country to attempted to elevate economic gains using the advantage of climatic condition. 5. In milking day and yearly yield per head, average 275 milking days and 87 drying days were longer than that of 228 average milking days in New Zealand. Annual total milk yield per head and milk solid (ms) was 3,990 kg and approximately 319 kg. Dairy milk solid (ms) per head, milk yield, fat percentage was 1.2 kg, 15.5 kg and average 4.83% which was much higher than in other country, and milk protein was average 3.75%. 6. In coclusion, Palmerstone North has been a center of dairy farming in New Zealand for the last 21 years. Their dairy farming history is 6~9 year longer than ours and the average number of milking cows per farm is 355, which is much greater than that (35) of Korea. They do not have dairy barn, but only milking parlors. Cows are taken care of by family 0.5 persons), are on a planned calving schedule in spring (93%) and milked for 240~280 days a year, avoiding winter. Cows are dried according to milk yield and body condition score. This management system is quite different from that of Korean dairy farms. Cows are not fed concentrates, relying entirely on pasture forages and the average milk yield per cow is 3,500 kg, which is about 1/2 milk yield of Korean dairy farms. They were bred to produce high fat milk with an average of 4.5%. Their milk production cost is the lowest in the world and the country's economy relies heavily on milk production. We Korean farmers may try to increase farming size, decreasing labor and management costs.
