  • Cerebellar Hippocampal and Basal Nuclei Transient Edema with Restricted diffusion (CHANTER) syndrome is characterized by an altered mental status. The acute MRI lesions show abnormal restricted diffusion imaging bilaterally and symmetrically in the cerebellum, hippocampus, and basal nuclei. This syndrome is an unknown syndrome and is presumed to be mainly an opioidinduced toxidrome. Here, we present a case study wherein we show that it can also be caused by an antidepressant overdose.

  • Perineal involvement by metastatic renal cell carcinoma (RCC) is very rare, and there are only few reports on its radiological findings in the literature. Here, we present a case of a 76-year old female who presented with perineal pain caused by metastatic clear cell RCC. We discuss the radiological changes of the tumor before and after targeted therapy.

  • Objective: The purpose of this study was to report the effect of acupuncture on the splenius muscle and the effect of combined Korean medicine treatment on acute dizziness in patients due to whiplash injury. Methods: Three patients with acute dizziness were treated with acupuncture of the splenius muscle and combined Korean medicine. A numeric rating scale (NRS) and the Korean dizziness handicap inventory (K-DHI) were checked every day from admission to discharge to confirm symptom improvements. Results: In the acute stage of dizziness caused by whiplash injury, acupuncture of the splenius muscle and combined Korean medicine improved the NRS and K-DHI scores. Conclusions: For patients with acute dizziness caused by whiplash injury, a combination of acupuncture on the splenius muscle and Korean medicine treatment may be a solution.

  • This study was to investigate the effectiveness of combined Korean medicine treatment on a patient who was diagnosed on lumbosacral transitional vertebra (LSTV) with localized bone marrow edema. The patient was treated with acupuncture, pharmacopucture, cupping and herbal medicine. We evaluated patient's progress using range of motion (ROM), EuroQol-5 dimensions (EQ-5D), Oswestry disability index (ODI), numerical rating scale (NRS), patient global impression of change, self-walkable distance. NRS of back pain decreased from 7 to 5, at the same time NRS of lower limb decreased from 4 to 3. EQ-5D score increased from 0.24 to 0.646. ODI score improved 80 to 62. Self-walkable distance and lumbar spine ROM also increased. As a result, clinical improvements were found in a patient. In conclusion, this study shows that Korean medicine treatment can be considered as effective conservative care for LSTV with localized bone marrow edema.

  • Objectives: This study investigated the latest clinical studies on Korean medicine treatment of idiopathic edema in the Chinese National Knowledge Infrastructure Database (CNKI). Methods: We searched the last 9 years of clinical studies discussing Oriental medicine-based treatments for idiopathic edema in the CNKI database. The search focused on the authors, publication year, type of study, purposes of study, method and duration of treatment, evaluation criteria, and results of the selected articles. Results: The search identified 7 randomized controlled trials (RCTs) and 9 others. All studies used oral herbal medicine for treatment. The most commonly used herbs were Poria Sclerotium (茯苓), Atractylodis Rhizoma Alba (白朮), and Alismatis Rhizoma (澤瀉), which are the constituent herbs of Oryeong-san (五苓散). All 16 studies confirmed the efficacy of Oriental medicine treatments. Conclusion: More varied and scientifically designed clinical studies are required to develop treatments for idiopathic edema. The results of this study could be used as basic data for further studies on idiopathic edema.

  • We report a case of neurolymphomatosis of lumbosacral plexus. A 63-year-old man, who had no past history except for diabetes mellitus, complained of severe pain and weakness on left lower extremity. Idiopathic lumbosacral plexopathy was diagnosed by electromyography. There were no abnormal findings except for FDG-PET/CT and MRI. They showed high uptake and thickening lesion in sciatic nerve and sacral plexus. However, about 7 months later, mass like lesion in left thigh was detected by FDG-PET/CT and MRI. Also, multiple hypermetabolic lesions were found in brain. Through brain biopsy, diffuse large B-cell lymphoma was confirmed. When a patient with idiopathic lumbosacral plexopathy complains of severe pain, it is necessary to consider FDG-PET/CT and MRI to differentiate neurolymphomatosis, even in patients who have no past history of lymphoma before. Especially, if FDG-PET/CT and MRI show sciatic and/or lumbosacral plexus lesion, neurolymphomatosis of lumbosacral plexus should be considered.

  • Objective: This study reported the effectiveness of Korean medicine for a patient with Type-1 diabetes mellitus after pancreatectomy with a compression fracture of the thoracic spine. Methods: A patient diagnosed with Type-1 diabetes mellitus after a pancreatectomy was treated with acupuncture, pharmacopuncture, Gami-hwanggigyejiomul-tang (modified Huangqi Guizhi Wuwu decotion), Jindang-won, etc. The conventional treatment was maintained using an insulin pump and insulin injection. Results: Blood glucose (FBS/PP2hrs), ESR and CRP decreased after one month of treatment. Conclusion: Gami-hwanggigyejiomul-tang and Jindang-won may be effective treatments for Type-1 diabetes mellitus. Further studies are needed to confirm the findings of this case.

  • Objectives: This study was conducted to find out the general public's perception of Korean Medicine (KM) treatment for traffic accident (TA) patients during pregnancy. Methods: A self-developed questionnaire was distributed to outpatients and inpatients between the ages of 19 and 70 at KM hospitals located in Bucheon. The subjects were asked whether they thought KM treatment could be applied to TA patients during pregnancy. The perceptions of acupuncture, pharmacupuncture, herbal medicine, and Chuna treatment during pregnancy were also investigated. Results: 87.50% answered that KM treatment could be applied to TA patients during pregnancy. Most respondents thought that musculoskeletal symptoms of TA could be treated with KM (85.71%). 93.75%, 87.50%, 56.25%, and 52.08% of the respondents said they were willing to recommend acupuncture, pharmacupuncture, herbal medicine, and Chuna treatment to patients after TA during pregnancy. The respondents answered that acupuncture (95.83%), pharmacupuncture (91.67%), herbal medicine (62.50%), and Chuna (66.67%) is effective for pain controlling during pregnancy, and acupuncture (8.33%), pharmacupuncture (12.50%), herbal medicine (45.83%), and Chuna (39.58%) could be affect the risk of premature birth, miscarriage or birth defects. Conclusions: Acupuncture and pharmacupunture treatment are perceived positively, whereas herbal medicine and Chuna treatment are perceived negatively by the general public for the TA patients during pregnancy. In order to establish the correct awareness of KM treatment for TA patients during pregnancy, more clinical studies and case reports on the efficacy and safety of KM treatment during pregnancy are required.

  • Objectives: The purpose of this case report was to propose Yangshimtang-Gamibang on three patients diagnosed with acute stress disorder. Methods: Yangshimtang-Gamibang was treated on three patients diagnosed with acute stress disorder after experiencing a traffic accident. The effects of Yangshimtang-Gamibang were evaluated through BAI (Beck Anxiety Inventory), NRS (Numerical rating scale), NDI (Neck Disability Index), ODI (Oswestry Disability Index), respectively, and quality of sleep was assessed with total sleep time and number of times waking up during sleep. Results: After the treatment, there was significant decrease in the value of BAI score, a measurement of anxiety, as well as the value of NRS, NDI, ODI, a measure of pain. Also, the sleep quality of three patients improved. Conclusions: The results indicate that Yangshimtang-Gamibang may have an effect on people with acute stress disorder who recently experienced a traffic accident.
