Operation of Ascending Aorta and/or Aortic Arch (상행대동맥 및 대동맥궁의 수술)

  • From Dec. 1993 to May 1995, 9 male and 5 female patients ranging in age from 25 to 65 years, were operated on for ascending aorta and/or aortic arch diseases. Six patients had acute aortic dissection, type A(ruptured in 4 cases); four had ruptured ascending aortic aneurysm; three had annuloaortic ectasia(ruptured in 1 cases); one had aortic arch aneurysm. The diagnostic procedures were echo cardiography and dynamic CT scan in all patients having acute dissection or rupture. The aortic angiography was performed in two cases. Indications for operations were rupture in five cases, acute aortic dissection in five cases, severe congestive heart failure in two cases, progressive aortic insufficiency in one case and impending rupture in one case. The emergent repair was performed in ten cases(71%). The surgical treatment consisted of 6 Cabrol operations, a Cabrol operation combined with arch replacement, a modified Bentall operation, 4 replacement of ascending aorta, a replacement of aortic arch, and a replacement of ascending aorta and aortic arch. Complications were a hypoxic encephalopathy, two atrial fibrillations, a sternal deheiscence, and a mediastinitis. Two early mortality(14%) were due to intractable bleeding and multiple organ failure, and one late mortality(7%) was due to ventricular arrhythmia. In eleven survivors, follow-up period was from 2 months to 12 months and the course was uneventful.

Congenital Left Atrial Appendage Aneurysm - A case report- (선천성 좌심방이류 - 1례 보고 -)

  • Congenital left atrial appendage aneurysm is a very rare congenital cardiac abnormality. That is postulated to arise from a developmental weakness in the atrial wall in utero. Clinically, patients are often asymptomatic and are diagnosed incidentally, but supraventricular arrhythmias and systemic thromboembolism have also been reported in some cases. Surgical resection at the time of diagnosis is recommended because of the propensity for thromboembolic complications. A 13-month-old female, who was suspected preoperatively as having partial absence of pericardium with left atrial herniation through the defect, underwent surgical resection of the left atrial appendage aneurysm. Exposure through a median sternotomy showed an intact pericardium. The postoperative course was uneventful.

  • Primary cardiac lymphoma is an extranodal malignant lymphoma of any cell type involving only heart and pericardium without dissemination. Patients usually present symptoms of heart failure, arrhythmias, pericardial effusion and cardiac tamponade. Diagnosis of primary cardiac lymphoma can be performed by echocardiogram, CT and MRI and cytologic examination of pericardial effusion or transvenously biopsied cardiac tissue. Prognosis of primary cardiac lymphoma is poor. Because of poor prognosis, early diagnosis and complete surgical excision is essential and postoperative systemic chemotherapy or radiotherapy is useful. In this case extensive tumor infiltration on the right ventricle and atrioventricular groove preclude surgical excision. Tissue biopsy revealed primary cardiac lymphoma. After postoperative chemotherapy and radiotherapy, the size of intracardiac mass is decreased in follow up chest CT scan and echocardiogram and symptoms of patient are relieved. Therefore, we report a case of primary cardiac lymphoma with review of literatures.

Abnormality Detection of ECG Signal by Rule-based Rhythm Classification (규칙기반 리듬 분류에 의한 심전도 신호의 비정상 검출)

  • Low misclassification performance is significant with high classification accuracy for a reliable diagnosis of ECG signals, and diagnosing abnormal state as normal state can especially raises a deadly problem to a person in ECG test. In this paper, we propose detection and classification method of abnormal rhythm by rule-based rhythm classification reflecting clinical criteria for disease. Rule-based classification classifies rhythm types using rule-base for feature of rhythm section, and rule-base deduces decision results corresponding to professional materials of clinical and internal fields. Experimental results for the MIT-BIH arrhythmia database show that the applicability of proposed method is confirmed to classify rhythm types for normal sinus, paced, and various abnormal rhythms, especially without misclassification in detection aspect of abnormal rhythm.

  • Although left Left ventricular hypertrophy (LVH) is not only an adaptive response of the heart to increased cardiac workload in hypertension, it surelybut also is the most potent risk factor of overt cardiovascular complications such as coronary heart disease, heart failure, arrhythmia and stroke in the hypertensive population. Also it has become generally accepted that subclinical cardiovascular disease begins in childhood and LVH is the most readily assessed marker for that. As LVH can be seen in children and adolescents with even mild blood pressure elevation with the reported prevalence of 10 to 47%, aggressive antihypertensive treatment is critical in preventing the development of hypertensive heart disease in that those cases.

  • ECG signals are recorded for diagnostic purposes in many clinical situations. Also, In order to permit good clinical interpretation, data is needed at high resolutions and sampling rates. Therefore In this paper, we designed to compression structure using multiple wavelet basis function(SWBF) and compared to single wavelet basis function(SWBF) and discrete cosine transform(DCT). For experience objectivity, Simulation was performed using the arrhythmia data with sampling frequency 360Hz, resolution lIbit at MIT-BIH database. An estimate of performance estimate evaluate the reconstruction error. Consequently compression structure using MWBF has high performance result.

  • 기술의 발전에 따라 소형 디바이스에서도 데이터를 수집하고 전송하는 것이 가능해졌다. 따라서 최근 IoT와 헬스케어가 부각되고 있으며 여기서 발생한 데이터에 대한 많은 연구가 진행되고 있다. 그 중에서도 헬스케어 장비에 내장된 심전도 센서를 이용하여 시계열 데이터를 수집할 수 있고, 여기서 수집한 데이터는 부정맥 등의 심장질환 진단의 중요한 지표로서 사용될 수 있다. 시계열 데이터는 시계열 분석 방법을 사용하여 정상 패턴과 비정상 패턴으로 분류할 수 있지만, 대량의 시계열 분석 방법은 수행시간이 많이 소요되기 때문에 이를 단축 할 필요성이 있다. 본 논문에서는 시계열 데이터 분석 기법 중 하나인 Shapelet을 사용하여 심전도 데이터의 패턴을 정상 및 비정상으로 분류하였고, 병렬처리 기법을 적용하여 수행시간을 단축하였다. 실험 결과, 각각의 심전도 데이터는 87%의 정확도로 분류되었고, Shapelets을 탐색하는 구간의 병렬처리를 통하여 수행 시간이 약 60%로 감소하였음을 확인하였다.

  • Digital electrocardiogram (ECG) recorders are rapidly replacing to conventional paper ECG recorders. Digital ECG recorders have several advantages over conventional type ECG recorders, including better capability for data storage, better portability and better applicability in clinical settings. In this study, we presented 4 case studies related to cardiac rhythm disturbances, which were diagnosed and managed using a digital ECG recorder.

Idiopathic Atrial Standstill in an Old English Sheepdog Cross Dog (Old English Sheepdog Cross Dog에서의 특발성 심방정지)

  • An 1 year old intact male mixed dog (weighing 20 kg) was presented with primary complaints of abdominal distension and severe exercise intolerance. Electrocardiogram found profound bradycardia (46-79 beats/min) with atrial standstill. Laboratory studies found no particular abnormalities. Serum cortisol and T4 concentration were within normal range. Diagnostic imaging studies revealed generalized cardiomegaly, ascites, dilation in all cardiac chambers, absence of atrial contraction, absence of transmitral A-peak, mitral and tricuspid valve insufficiency and normal left ventricular systolic dimension. Based on diagnostic findings, the case was diagnosed as idiopathic atrial standstill. The dog was treated with conventional therapy for heart failure.

Early Detection and Successful Treatment of Vertically Transmitted Fulminant Enteroviral Infection Associated with Various Forms of Arrhythmia and Severe Hepatitis with Coagulopathy

  • Enteroviral infections are common in neonates. One important infection pathway is vertical transmission from an infected mother to her neonate. Here, we report the early detection and successful treatment of a vertically transmitted fulminant enteroviral infection associated with myocarditis and hepatitis. The patient had a sudden onset of high fever on the fourth day of life and developed severe, rapidly progressing symptoms of disseminated intravascular coagulopathy (DIC), hepatitis, and myocarditis accompanied by tachyarrhythmia. As it was the peak season for enteroviral infections and both the mother and the patient's 36-month-old sibling had a high fever around the time of delivery, we suspected an enteroviral infection. Thus, we initiated prompt evaluation of enteroviral infection, as well as close observation and intensive care of the neonate. We strongly recommend evaluation for the possibility of vertical enterovirus infection in neonates when the mother is suspected of having a viral infection (e.g., high fever and negative results from bacterial infectious studies) around the time of delivery and when the neonate shows some early symptoms of infectious diseases such as thrombocytopenia, DIC, hepatitis, and myocarditis. Early detection of enteroviral infections and prompt implementation of proper treatment are key to reduce the risk of complications and mortality associated with enteroviral infections in neonates.
