COVID-19 감염병전담병원 간호사의 조직몰입에 미치는 영향요인 (Factors Influencing Organizational Commitment of Nurses at an Infectious Disease Hospital of COVID-19)

  • Purpose : This study investigated coronavirus disease-19 (COVID-19) related stress, resilience, and organizational commitment, and determined the factors influencing nurses' organizational commitment at an infectious disease hospital of COVID-19. Methods : A cross-sectional descriptive survey was conducted with 138 nurses. Data analysis, including descriptive statistics, independent t-tests, one-way ANOVA, Pearson's correlations, and multiple regression analysis, were performed using SPSS 26.0 program. Results : Factors influencing organizational commitment included resilience (𝛽=0.31, p<.001), position (𝛽=0.31, p<.001), COVID-19 related stress (𝛽=-0.26, p<.001), and COVID-19 nursing period (𝛽=-0.19, p=.012). These variables explained 29.6% of the organizational commitment. Conclusion : In order to enhance the organizational commitment of nurses in infectious disease hospitals of COVID-19, active program development and intervention are required at the organizational level to improve nurses' resilience and relieve stress related to nursing infectious disease patients..

방사선사 조직의 조직유효성에 영향을 미치는 변수에 관한 연구 (A Study on the Variables Affecting on the Organizational Effectiveness of Radiological Technologist)

  • Medical institutions are abundant in human resources and have a unique structure with diverse professions and various organizational cultures coexist. This organizational culture influences organizational effectiveness and influences the effectiveness of the organization as a factor of leadership and organizational culture. Leadership is an important factor in linking employee behavior to organizational commitment to achieve job satisfaction. The purpose of this study was to analyze the effect of organizational effectiveness on organizational culture, leadership perception, and job stress in a special organization called radiologist. As a result, organizational culture and transformational leadership showed high correlation with 0.627. Among the organizational effectiveness variables, organizational commitment showed the highest influence with transformational leadership(${\beta}=0.284$, p<0.001), and job satisfaction was also the highest with transformational leadership (${\beta}=0.440$, p<0.001). The results showed that the expectation of transformational leadership affects organizational commitment and job satisfaction. In order to expect a positive change in the effectiveness of the organization, the radiologist culture requires an organizational culture that can fully demonstrate autonomy and creativity, and transformational leadership is required for this.

  • 심한 하악 치조골 흡수룰 보이는 완전 무치악 환자에서 의치는 환자의 저작 효율이 극히 저하되며 특히 치조골의 형태가 불규칙한 경우 의치의 안정성은 더 떨어질 수 있다. 이때 임플란트를 통한 지지를 얻는 피개의치를 통해 편안감 회복과 향후 지속적인 치조골 흡수를 줄이는데 도움을 줄 수 있다. 임플란트 피개의치를 위한 하방의 attachment는 크게 bar attachment와 solitary attachment로 나뉘며 임플란트 고정체의 위치가 하악 전방 치조골에 있으면서 의치 전체가 견고한 지지를 얻기 위해서는 bar attachment가 유리하고 구치부에도 고정체가 위치하며 쉬운 착탈과 유지관리의 편의를 얻고자 하면 solitary attachment를 고려할 수 있다. 본 증례는 환자의 상황에 따른 임플란트 식립 위치 결정, attachment를 선택을 통하여 임플란트 피개의치를 다양하게 적용하여 만족할만한 결과를 얻어 이를 보고하는 바이다.

Phosphor Particle 혼합형 Hybrid 선량계의 방사선치료 Quality Assurance에 대한 적용가능성 평가 (Feasibility Study of Phosphor Particle Blended Hybrid Dosimeter for Quality Assurance in Radiation Therapy)

  • 방사선치료 분야에서는 치료의 안전성을 검증하기 위한 Quality Assurance(QA) 절차가 매우 중요하게 여겨진다. 그러나 일반적으로 이에 사용되는 선량계들의 다양한 문제점 때문에, 이를 대체하기 위한 선량계 연구가 활발히 진행되고 있다. 본 연구에서는 형광체로부터 방출된 visible light(VL)에 의한 Sensitivity 극대화를 위해, 뛰어난 형광 효율을 가지는 형광체인 $Gd_2O_2S:Tb$를 요오드화납(Lead(II) Iodide; $PbI_2$)에 다양한 weight percent(wt%)로 혼합한 Blended hybrid sensor를 제작하였다. 이후 Blended sensor 및 Pure $PbI_2$ sensor의 고에너지 방사선에 대한 반응특성을 비교 및 평가하였다. 민감도 평가결과, 3wt%는 sensor에서 타 sensor들과 40% 이상 차이나는 최댓값이 나타났으며, 이를 제외한 센서에서 wt%의 증가에 따른 점차적 민감도 감소추세를 확인하였다. 또한, 재현성 평가에서는 Pure $PbI_2$ sensor가 coefficient of variation(CV)>0.015의 큰 편차를 보인 반면, blended sensor는 모두 CV<0.015 이하의 결과를 보였다.

응급실 간호사의 직무 스트레스, 피로, 직무 만족 및 조직 몰입 (Job Stress, Fatigue, Job Satisfaction and Commitment to Organization in Emergency Department Nurses)

  • Purpose: This research was done to identify relationships among job stress, fatigue, job satisfaction, and commitment to organization in emergency department nurses. Methods: Research participants were 513 nurses who worked in the emergency department of 17 tertiary centers and 28 secondary hospitals. Data collection for this descriptive correlational study was done from May to July 2010 using self report questionnaires, and analyzed using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient with the SAS 9.2 program. Results: There were statistically significant differences in organizational commitment according to age, religion, total clinical experience, emergency department experience, current position, frequency of night shift, average sleep hours, wage level, numbers of beds in unit, and reason for working. A positive correlation was found between job stress and fatigue, and between job satisfaction and commitment to organization for the emergency department nurses and a negative correlation between job stress and Job satisfaction, and fatigue and Job satisfaction. Conclusion: Results of this research indicate that job stress in emergency department nurses was the principal variable affecting job satisfaction and commitment to organization along with fatigue. Appropriate stress management strategies based on identification of job stress for these nurses should lead to development of improved management of nursing manpower and a reduction in stress.

Brain-expressed X-linked (Bex) 2와 heterotrimeric kinesin-2의 KIF3A와의 결합 (Brain-expressed X-linked 2 Binds to Kinesin Superfamily Protein 3A)

  • Kinesin-2는 heterotrimeric kinesin-2와 homodimeric kinesin-2의 두종류가 존재한다. 세포내 미세소관을 따라 이동하는 분자 모터 단백질인 heterotrimeric kinesin-2는 motor 활성을 가진 kinesin superfamily protein (KIF) 3A와 3B (KIF3A와 KIF3B) 그리고 kinesin associated protein 3 (KAP3)로 구성되어져 있다. 특히 섬모내에서 heterotrimeric kinesin-2는 미세소관의 plus방향으로 운반체를 운반하는 역할을 수행한다. Heterotrimeric kinesin-2는 다양한 운반체 그리고 단백질들과의 결합이 알려져 있지만 heterotrimeric kinesin-2와 결합하는 결합단백질에 대하여서는 아직 충분히 밝혀지지 않았다. 본 연구에서는 KIF3A의 C-말단 영역과 결합하는 단백질을 효모 two-hybrid system을 사용하여 탐색한 결과, brain expressed X-linked 2 (Bex2)를 분리하였다. Bex2는 KIF3A의 cargo-binding domain (CBD)을 포함하는 C-말단 영역과 결합하지만, KIF3B와 kinesin-1의 motor 단백질인 KIF5A와는 결합하지 않았다. 그리고 KIF3A는 Bex2의 다른 isoform인 Bex1과는 결합하지만, Bex3와는 결합하지 않았다. KIF3A은 GST-Bex1, GST-Bex2와는 결합하지만 GST와는 결합하지 않았다. HEK-293T세포에 Bex2을 발현시켰을 때 Bex2와 KIF3A는 세포 내의 같은 부위에 발현하며, Bex2 그리고 KIF3A을 면역침강한 결과 KIF3A와 KIF3B도 같이 침강함을 확인하였다. 이러한 결과들은 Bex2는 heterotrimeric kinesin-2와 운반체를 매개하는 단백질로의 가능성을 시사한다.

외측 발목 불안정성을 치료하지 않으면 어떻게 되는가 (What Happens without Treating Chronic Lateral Ankle Instability)

  • The term chronic lateral ankle instability (CLAI) embraces a spectrum of conditions that involve functional and mechanical instability, both in athletes and lower-demand patients. The hallmarks of CLAI are recurrent sprains, persistent pain, and repeated episodes of the ankle giving way. CLAI is the main complication of acute ankle sprains, which can cause discomfort in daily and sports activity. Approximately 10% to 30% of patients with acute ligament ruptures will develop chronic instability over the course of a year from the index injury. An accurate diagnostic approach and successful treatment plan should be established based on a comprehensive understanding of the concept of functional and mechanical instability. Unfortunately, the optimal modality for the management of CLAI is unclear. Even after conservative treatment or surgical intervention, it could result in degenerative changes to the ankle joint in the long term. Thus far, the incidence of ligamentous posttraumatic ankle osteoarthritis was reported to be 13% to 78%. The mean latency time between injury and osteoarthritis was at least 10 to 34.3 years. CLAI is an important pathological condition that can cause discomfort or dysfunction in daily activity in the short term, resulting in joint destruction in the long term. Therefore, it is important to understand the various complications that can occur when CLAI is not treated properly.

신규간호사의 직무 스트레스, 자아존중감, 회복탄력성이 현장적응에 미치는 영향 (The Influence of Job Stress, Self-Esteem, and Resilience on the Field Adaptation in Newly Graduated Nurses)

  • Purpose: This study aims to determine the relation among job stress, self-esteem, resilience, and field adaptation of the newly graduated nurses, through which an effective mediation strategy toward field adaptation and basic data on improving nursing quality can be established. Methods: The subjects were 110 newly graduated nurses who have worked at a tertiary hospital and a general hospital for less than 12 months. The data were collected, using self-reported questionnaires, from July 9 to July 31, 2018, and were analyzed by descriptive statistics, t-test, one-way ANOVA, $Scheff{\acute{e}}$ test, Pearson's correlation coefficient and stepwise multiple regression using an SPSS/WIN 24.0 program. Results: The study result showed that the newly graduated nurses' job stress, resilience, job satisfaction, and desired department influenced their field adaptation, explaining 60.2% of the variance in the field adaptation (F=42.14, p<.001). The nurses' job stress had the biggest influence on their field adaptation, explaining 38.7% of the variance. Conclusion: Human resource management strategy considering those influencing factors may improve the filed adaptation of newly graduated nurses and help them to retain in their work field.

자살시도로 응급실에 내원한 환자의 자살 위험성과 양극성 장애 가능성의 연관성 (The Relationship between Possibility of Bipolar Disorder and Suicidal Attempt in Emergency Room)

  • 연구목적 본 연구는 자살 시도자를 대상으로 자살 생각의 심각성과 관련된 요인들을 찾아보고자 하였다. 방 법 일 대학병원 응급실에 자살 시도로 내원한 환자 중 50명을 대상으로 임상가가 면담, Columbia Suicide Severity Rating Scale(C-SSRS), Beck Depression Inventory(BDI), Korean Version of Mood Disorder Questionnaire(K-MDQ), Bipolar Spectrum Diagnostic Scale(BSDS)를 시행하였다. 결 과 자살 경향이 높은 고위험군은 C-SSRS의 자살 생각의 심각성 점수와 BDI 점수가 높았고(p<0.001), MDQ에서 양극성 장애 가능성의 양성으로 판정된 자는 자살생각의 심각성 점수가 높았다(p=0.033). 결 론 자살 시도자의 자살 생각의 심각성과 양극성 장애의 가능성은 서로 연관성이 있을 것으로 생각된다.

식도에 발생한 소세포암의 수술 치험 (Surgical Treatment of Esophageal Small Cell Carcinoma -1 Case Report-)

  • 식도 소세포암은 매우 드문 악성 종양으로 알려져 있다. 본 인제 대학교 부산백병원 흉부외과학교실에서는 식도에서 발생한 소세포암 1례를 수술 치험하였다. 68세된 남자로서, 술전 위내시경 조직검사에서 신경내분비종양이 의심되어 식도절제술 및 위-식도 문합술을 시행하였고, 술후 경과는 양호하여 합병 증없이 퇴원하였다.
