  • Distant metastasis to the spleen is extremely rare. To the best of our knowledge, metastasis to the accessory spleen based on pathological findings has only been reported in four patients in the English literature, including one each of ovarian cancer, transitional cell carcinoma, breast cancer, and uterine carcinosarcoma after surgery. Furthermore, among these reported cases, only two reports (one each of transitional cell carcinoma and uterine carcinosarcoma) presented imaging findings. In this study, we report a case of colon cancer metastasis to the accessory spleen without involvement of the spleen in a 58-year-old male patient, providing imaging findings. This case emphasized the importance of considering the possibility of metastasis to the accessory spleen in patients with malignancy.

  • A 10 months old, male Poongsan dog was referred with a history of right forelimb lameness due to hit by a car a few hours before presentation. On the physical examination, the dog showed pale mucous membrane, respiration distress, and skin abrasions. The main laboratory finding was leukocytosis, while hematocrit was normal. Abdominal radiography revealed the loss of abdominal serosal detail, which was diagnosed as peritoneal hemorrhage on ultrasonography (US) and aspiration. On computed tomography (CT), hepatic and splenic injuries were seen characterized by the discontinuity of the capsule with irregular margin and lower density lesions than parenchyma. No contrast enhancing area were shown in the laceration lesion. The accessory spleen was incidentally found by US and CT. In this case, acute intra-abdominal hemorrhage from splenic and hepatic laceration was diagnosed using CT.

  • We report our experience with a case of performing laparoscopy-assisted total gastrectomy along with pancreas-preserving splenectomy for treating early gastric cancer. laparoscopy-assisted total gastrectomy was planned for a 62-year-old male patient with a double early gastric cancer located in the upper and lower third of the stomach. Five trocars were placed and we used a harmonic scalpel to dissect the greater curvature. Enlarged splenic hilar lymph node was encountered and they were proved to be metastasis by frozen section biopsy. We then performed total gastrectomy with pancreas-preserving splenectomy for the purpose of completely dissecting the lymph nodes along the splenic artery and splenic hilum. We created a 4 cm sized longitudinal mini-laparotomy below the xiphoid process to remove the specimen, and anastomosis was done via the Roux-en-Y method. The patient was discharged on the 9th postoperative days after an uneventful recovery. Our experience shows that laparoscopy-assisted total gastrectomy with pancreas-preserving splenectomy is a relatively safe procedure for treating upper third early gastric cancer with metastatic splenic hilar lymph nodes.

  • Objective : The purpose of this study was to conduct a comprehensive survey of children's play in parents of disabled and non-disabled children prior to commencing school. This study aimed to further understanding play recognition and to present a specific direction of play necessary to each parent. Methods : A questionnaire based on prior studies was sent to 700 people who had previously agreed to the take part. A total of 596 questionnaires were analyzed. Uncollected and insincere surveys, of which 106 were questionnaires for parents of disabled children, were exclused from analysis. The SPSS Window 23 program was used for data analysis and frequency analysis and the independent sample T test were performed. Results : Disabled children's parents perceived playing with their children as more important than that of non-disabled children's parents(p<0.01). There was no statistical difference between disabled children and non-disabled children's parents, but there were some differences(p=0,053). Both disabled and non-disabled children had more time to participate with their mothers than with their fathers. Mothers with disabilities had more time to play however, fathers with no disabilities had more time. Both disabled and non-disabled children's parents had the most "ordinary" frequency of buying toys and there was very littele difference between the 2 groups. Both disabled and non-disabled children's parents primarily used the internet to acquire play information, and consideration when buying fun was followed by interest inducement, development level and safety. Conclusion : Through this study, it was possible to compare the status of play recognition and participation by parents of children with or without disabilities. Based on this study, parents will be able to find out what they really need to play and will be provided as a basis for future play studies for children.

  • This study was attempted to investigate the regional distribution and shapes of pancreatic endocrifle cells in the hedgehog, Erinaceus koneanus by the immunocytochemical PAP methods (Nalane, 1968; Stemberger, 1979). The tissue specimen taken from the splenic and duodenal regions of pancreas(proximai, middle and distal portions, respectively) were fixed with Bouin solution and the sections(5$\mu$ m) were followed by simple and double staining with 2 substrates, DAB and 4-CI-1-naphthol. The results were as following: Glucagon(A) cells, 13 $\mu$ m x 9.5 $\mu$ m in size, were found in the islets periphery and among the exocrine parenchyma. A cells were abundant in all the portions of splenic region and distal portion of duodenal region in contrast to a few in the proximal and middle portion of duodenal region. The shapes of the A cells were round, oval and pyramidal types. Insulin(B) cells, 11.6$\mu$m x 9.4$\mu$m in sise, were round or oval in shape and located throughout the islets. B cells were the most numerous cell types in all portions of splenic region and distal protion of duodenal region as compared with the other portions. Somatostatin(D) cells, 12.6$\mu$m x 9.1 $\mu$m in size, were round or oval in shape and found in the islets periphery and scattered in the exocrine parenchyma. These cells were rare in all the portions of splenic and duodenal region. Pancreatic polypeptide(PP) cells of various type, 12.8$\mu$ m x 8.5 $\mu$ m in size, were found in the islets periphery and among the exocrine parenchyma. PP cells were numerous in the proximal and middle portion of duodenal region, but rarely scattered in the other portions.

  • Splenic marginal zone lymphoma (SMZL) is a rare type of non-Hodgkin's lymphoma (NHL). We report here on a patient who displayed the synchronous occurrence of SMZL and early gastric cancer (EGC). The patient was a 74 year-old male with liver cirrhosis. An EGC in the gastric antrum was diagnosed and the preoperative abdomen computed tomography scan revealed splenomegaly and intra-abdominal lymphadenopathy. We performed subtotal gastrectomy and the postoperative pathologic examination revealed adenocarcinoma limited to the gastric mucosa and SMZL in the lymph nodes. The patient recovered from the surgery without complications and is now awaiting chemotherapy. SMZL has an indolent clinical course with good long-term survival and so there is the possibility of the occurrence of a second primary malignancy. Rare cases of a second primary malignancy being diagnosed along with SMZL have been described in the literature. Patients with SMZL should be carefully followed after treatment to detect the possible occurance of a second primary malignancy.

  • Isolated left-sided portal hypertension is a rare clinical syndrome caused by splenic veno-occlusive disease. Splenic vein thrombosis and extrinsic compression causes proximal splenic vein hypertension, and the splenic blood flows into the superior mesenteric or portal vein through the upper stomach's collateral vessels, such as the short gastric, coronary, and gastroepiploic veins. Open splenectomy is recommended to treat gastrointestinal bleeding caused by isolated left-sided portal hypertension. Interventional management could be a clinically useful option for selected patients who want to avoid surgical corrections. The report presents two cases of left-sided portal hypertension with gastric variceal bleeding.

  • The distributions and relative frequencies of the serotonin-immunoreactive cells were studied in dorsal, ventral, third and splenic lobes of developing chicken pancreas during embryonic periods (10 days of incubation to hatching) by immunohistochemical methods. The regions of pancreas were subdivided into three regions, exocrine, light and dark islets. Round and/or oval shaped serotonin-immunoreactive cells were detected in all four lobes. According to developmental stages, the types of lobes and the regions of pancreas, these immunoreactive cells were showed various distributions and relative frequencies. In exocrine portions, serotonin-immunoreactive cells were found in the splenic lobes at 13-14 days of incubation, in the third lobes from 10 days to 19 days of incubation, in the ventral lobes from 10 days of incubation to hatching and in the dorsal lobes from 11 days of incubation to hatching. In pancreatic islets, these cells were detected only in the dark islets of splenic lobes at 15 and 16 day of incubation with rare frequency. In conclusion, serotonin-immunoreactive cells decreased with developmental stages in all four lobes and their relative frequencies decreased with developmental stages.

  • This study investigated the distribution of scuticociliates in the intestine, spleen, kidney, testis, brain, pericondrial bone and muscle layer of the tiger puffer, Takifugu rubripes, infected with scuticociliates. Scuticociliates were infiltrated in the connective tissues of the outer layer of the intestine, spleen, kidney, testis, brain, pericondrial bone and muscle layer. In brain, membranous tissue and optic lobe cortex separation were accompanied by the infection of scuticociliates. Other internal tissues and organs did not show any lesions expect for heavy deposition of hemosiderin in spleen.
