  • Ovarian hyperstimuation syndrome (OHSS) is the most serious complication of controlled ovarian stimulation. It causes symptoms such as, ovarian enlargement, ascites, pleural effusion, pericardial effusion, hemoconcentration, electrolyte imbalance, and thromboembolism. Although proper management is done, thromboembolism could occur and is difficult to predict. Moreover it can cause death. Consequently thromboembolism is the most dangerous complication of OHSS. We experienced a OHSS patient with thromboembolism of the brain after having IVF-ET.

A Case of Ruptured Ovarian Cyst in a Newborn (신생아에서 발생한 파열된 난소 낭종 1례)

  • Ovarian cysts are the most common cystic abdominal masses in female newborns. It is believed to be derived from an overstimulation of the ovarian follicles by maternal, placental, and fetal hormones. Although most ovarian cysts resolve spontaneously, surgical management is mandatory for life-threatening, complicated cases, including torsion, intracystic hemorrhage, and rupture of the cyst. Rupture of ovarian cysts is thought to be exceedingly rare, but can lead to severe hemorrhagic ascites or peritonitis. We managed a case of a ruptured ovarian cyst in a female newborn who presented with mild abdominal distension and two episodes of gross hematuria. Exploratory laparotomy revealed a right ovarian cyst with torsion and rupture. She was successfully treated with a right salpingo- oophorectomy with no sequelae.

A Study on the Spatial Distribution Patterns of Co-authoring Activities in the Korean Cadastral Research Field (한국 지적학 연구분야 공동저술활동의 공간분포패턴연구)

  • The primary purpose of this study is to identify spatial distribution patterns of co-authoring activities in Korean cadastral science. The analysis showed that a small number of researchers played an essential role in the Korean cadastral co-authorship network. In particular, some authors not only had a significant influence on other nodes in the network but also served as intermediaries between researchers. Moreover, the distance between researchers influenced co-authorship decisions to a limited extent. This study differs considerably from previous studies in that it used spatial analysis techniques to identify spatial distribution patterns of co-authoring activities. However, this research is limited in that it applied only 2019 data to determine the spatial distribution pattern of co-authoring activities. We can overcome this limitation if we analyze the spatial distribution patterns of co-authoring activities using multi-year data in future studies.

Diagnostic Values of Abdominal Ultrasonography in Patients with Fever and Abdominal Symptoms (발열과 복부 증상을 주소로 하는 환아에서 복부 초음파 검사의 진단적 의의에 관한 연구)

  • Purpose: Acute febrile illness in children frequently accompanies with abdominal symptoms such as vomiting, diarrhea, and abdominal pain, even if its etiology is not occured from the gastrointestinal tract. If the etiology of fever was unknown and the fever was accompanied with abdominal symptoms, we should be concerned about whether the etiology of fever was originated from the gastrointestinal tract or interpretated from the abnormality in the gastrointestinal tract. This study was performed to evaluate the diagnostic value of abdominal ultrasonography in patients with fever and abdominal symptoms. Methods: We reviewed retrospectively the medical records of abdominal ultrasonographic (US) findings of 60 cases of acute febrile illness with abdominal symptoms at department of Pediatrics, Ulsan Dongang General Hospital during the period from January 1994 to June 1995. Results: The abnormal abdominal US findings obtained were as follows. 1) The abnormal US findings were seen in 56 cases (93.3%). 2) The most common abnormal US finding was the enlarged mesenteric lymph nodes in 52 cases (86.7%) 3) The enlarged mesenteric lymph nodes with or without intra-abdominal fluid were seen in 30 cases (50.0%) of a wide variety of illnesses, so their diagnostic values were absent. 4) The enlarged mesenteric lymph nodes and splenomegaly with or without ileocecitis were seen in 7 cases. Among them, 4 cases (6.7%) were confirmed as typhoid fever. 5) The US findings in 6 cases of typhoid fever were the enlarged mesenteric lymph nodes in all cases (100%), splenomegaly in 4 cases (66.7%), ileocecitis in 1 case (16.7%), enlarged mesenteric lymph nodes and splenomegaly in 4 cases (66.7%), enlarged mesenteric lymph nodes, splenomegaly and ileocecitis in 1 cases (16.7%). 6) The enlarged mesenteric lymph nodes and the abnormalities around the appendix were seen in 7 cases (11.7%), which were confirmed as appendicitis all. 7) The thickening of wall in urinary bladder was seen in 2 cases (3.3%) of acute cystitis and acute hemorrhagic cystitis. 8) The subtle thickening of wall in colon was seen in 1 cases (1.7%) of shigellosis. Conclusions: The enlarged mesenteric lymph nodes, splenomegaly and ileocolitis on the abdominal ultrasonography in patients with fever and abdominal symptoms aree suggestive findings of typhoid fever. The enlarged mesenteric lymph nodes and the abnormalities around the appendix on abdominal ultrasonography make the rapid diagnosis of acute appendicitis and its complications, when physical examination is difficult in small children and diagnosis of their illnesses is obscure in patients with fever and abdominal pain.

Long-Term Clinical Results of Tricuspid Valve Replacement (삼첨판막 대치술의 장기간 임상성적)

  • There are only limited numbers of reports about long-term results of tricuspid valve replacement(TVR) with bioprosthetic and mechanical prostheses. We analyzed risk factors for tricuspid valve replacement and compared long-term clinical results of bioprosthetic and mechanical valves in tricuspid position. Material and Method: We reviewed 77 cases of TVR, which were performed between October 1978 and December 1996. Mean age was 38.8 15.9 years. Bioprostheses were implanted in 26 cases and mechanical prostheses were implanted in 51 cases. Result The operative mortality was 15.6% and late mortality was 12.3%. Survival for bioprosthetic and mechanical valve group at 5, 10 and 13 years was 81.3% vs. 100%, 66.1% vs. 100%, 60.6% vs. 100% (p=0.0175). Free from valve related re-operation for bioprosthetic and mechanical valve group at 5, 10 and 13 years was 100% vs. 93.9%, 100% vs. 93.9% and 58.3% vs. 93.9% (p=0.3274). Linealized incidences of valve related re-operation for bioprosthetic and mechanical valve group was 2.27 %/patient-years and 1.10 %/patient-years. Risk factor analysis showed that presence of preoperative ascites, hepatomegaly larger than 2 finger breaths, poor preoperative NYHA functional class and number of tricuspid valve replacement were risk factors for early mortality, and the use of bioprosthetic valve and number of open heart surgery were risk factors for late mortality. Conclusion: Long-term survival of mechanical valve was superior to bioprosthetic valve in tricuspid position. We recommend mechanical valve in tricuspid position if the patient can be closely followed up.

A Case of Abdominal Tuberclosis Mimicking Ovarian Cancer (난소 종양의 임상적 양상을 보인 소아 복막 결핵)

  • We report the case of a girl with peritoneal tuberculosis (Tb) who presented with mimicking features of ovarian cancer including massive ascites, elevation of CA-125 and ovarian masses. These ovarian masses cannot be ruled out malingnacy with image study. Gram stain, acid-fast stain, bacteriological and mycological culture of centrifuged ascites cells were negative. Tumor markers revealed an elevated CA-125 level of 112 U/mL and adenosine deaminase level were significantly high with 118 U/L. Diagnostic laparoscopy was done. The histology of the peritoneum and the ovary showed multiple granulomatous nodules with epithelioid cells and multi-nucleated giant cells. And there were no evidence of malignancy. Furthermore acid-fast bacilli (AFB) was identified in this specimen. Peritoneal Tb was difficult to diagnosis in our patient because it mimicked ovarian cancer. And this is the first case of abdominal Tb mimicking ovarian cancer in children in Korea.

  • Classical galactosemia (OMIM# 230400) is an autosomal recessive disorder of carbohydrate metabolism, due to a complete loss in galactose-1-phosphate uridyltransferase (GALT; E.C.2.7.7.12) enzyme activity. It caused by mutations in the GALT gene (OMIM$^*$ 606999) that is located at chromosome 9p13. The GALT enzyme deficiency results in a build-up of galactose and galactose-1-phosphate, causing life threatening complications such as feeding problems, failure to thrive, hepatocellular damage, bleeding and sepsis. However, Duarte galactosemia, a variant form of GALT deficiency, has residual GALT enzyme activities in erythrocytes and do not have manifest the symptoms of classical galactosemia. Since the advent of newborn screening (NBS) for galactosemia, we rarely encounter such overwhelmingly ill newborns. The positive NBS with no symptoms indicates the possibility of Duarte galactosemia besides a simple false positive and it has to be differentiated from classical galactosemia which is a medical emergency. In Korea, detection rate of Duarte galactosemia is very low and its genetic information is restrictive, too. We report a case of monozygotic twins with D/G galactosemia compound heterozygote in proven by the mutational analysis of GALT gene, which revealed N314D polymorphism and -119 to -116 delGTCA.

Fluvial Processes and Vegetation - Research Trends and Implications (하천과정과 식생 - 연구동향과 시사점)

  • We've reviewed existing studies on the interactions among vegetation, hydrology, and geomorphology in the stream corridors, adding one more factor of vegetation in the traditional area of hydro-geomorphology. Understanding of the interactions among those three factors is important not only academically but also practically since it is related intimately to the restoration of river corridor as well as management itself. Studies of this area started from field investigations in the latter part of the 20th century and focused on the flume experiments and then computer modelling in the 1990s and 2000s. Now, it has turned again to the field investigations of specific phenomena of the vegetative-hydrologic-geomorphologic interactions in detailed micro scales. Relevant studies in Korea, however, seem to be uncommon and far behind the international status quo in spite that practically important issues related directly to this topic have been emerged. In this study, we propose, based on the extensive literature review and authors' own knowledge and experiences, a conceptual diagram expressing the interactions among vegetation, flow (water), sediment, and geomorphology. Existing relevant studies in Korea since the 1990s are classified according to the categorization in the proposed diagrams and then briefly reviewed. Finally, considering the practical issues of riparian vegetation that have emerged recently in Korea, we propose areas of investigation needed in near future such as, among others, long-term and systematic field investigations and monitoring at multiple river corridors having different attributes on vegetative-hydrologic-geomorphologic interactions, including vegetative dynamics for succession.

Trends and Prospects of Forest Meteorological Studies Based on the Publications in Korean Journal of Agricultural and Forest Meteorology (한국농림기상학회지 수록 논문에 기반한 산림기상 연구 추세와 전망)

  • This study was conducted to review the trends of forest meteorological studies based on the publications for last 20 years in Korean Journal of Agricultural and Forest Meteorology (KJAFM), and to provide insight for future prospect for researches in the field of forest meteorology. A total of 220 papers related to forest meteorology were published in KJAFM for the last 20 years. That corresponds to 33.5% out of all the papers including agricultural meteorology papers. To review the trends of forest meteorology studies, the 220 published papers were classified into seven categories. They are forest meteorology and forest fire, forest meteorology and tree physiology, forest meteorology and forest protection, micrometeorology in mountain area, climate and forest growth, climate and forest vegetation distribution, and climate change and forest ecosystem. Even if there were differences in paper numbers among the seven categories, it was found that various and very specific studies were conducted in the field of forest meteorology for the last 20 years. It was also expected that the accumulation and utilization of various and accurate forest meteorological information would bring remarkable progress of forest meteorological studies in the near future.

Serotypes and Antimicrobial Susceptibility of Streptococcus pneumoniae (폐구균의 혈청형 분포와 항생제 감수성에 관한 연구)

  • Purpose : Streptococcus pneumoniae is part of the normal flora but is also responsible for causing many invasive diseases such as pneumonia, meningitis, and sepsis in addition to noninvasive diseases such as otitis in children. Multi-drug resistant strains has raised a lot of concern worldwide and thus the importance of prevention has been emphasized. We have analyzed the current serotypes and antibiotic sensitivity of each serotype as a baseline study to estimate the efficacy of the pneumococcal vaccine in Korean children. Methods : One hundred sixteen cases of pneumococcus cultured at Yonsei Medical Center from September 2001 to January 2003 were analyzed. The serotyping was done with the Quellung reaction and penicillin resistance was tested using the oxacillin disc diffusion method. Results : Pneumococcus were cultured from the sputum in 76 cases(65.5%), from the blood in 13 cases(11.2%), from the ear discharge in 12 cases(10.3%), from the throat in 7 cases(6.0%), from the nasal cavity in 2 cases(1.7%), and one case(0.9%) each from the cerebrospinal fluid, eye discharge, peritoneal fluid, post-operational wound, brain abscess, and catheter tip. Serotyping was possible with 98 cases and the following serotypes were found; 15 cases of type 19F(15.3%), 11 cases of 19A(11.2%), 8 cases of 11A(8.2%), 7 cases each of 6A, 14 and 3(7.1%), 6 cases each of 35, 6B and 23F(6.1%). Eighty two cases(70.7%) out of 116 cases were penicillin resistant and serotypes 19F, 19A, 11A, 23F, 6A, 9V constituted the majority, 48 cases(59.8%). These serotypes showed resistance to cotrimoxazole (74.4%), tetracycline(69.5%), and erythromycin(90.3%) as well. In the 22 cases cultured from children, 19A and 19F were found in 25.0%, 6A, 6B, and 23F in 10.0%, 11A, 14, 19, and 29 in 5.0%. Fifty percent(10/20) of the clinical isolates were represented in the current 7-valent pneumococcal protein conjugate vaccine, and 85%(17/20) when the cross-reacting serotypes were included. Penicillin resistance was found in 86.4%(19/22). Conclusion : The percentage of serotypes included in the 7 valent pneumococcal protein conjugate vaccine found in our study was 40.8% which was less than other prior studies. In anticipation of a change of pneumococcal serotypes, a nationwide multicenter study is needed before the initiation of pneumococcal vaccines in Korea.
