Application of Computer-Aided Diagnosis for the Differential Diagnosis of Fatty Liver in Computed Tomography Image (전산화단층촬영 영상에서 지방간의 감별진단을 위한 컴퓨터보조진단의 응용)

  • In this study, we are using a computer tomography image of the abdomen, as an experimental linear research for the image of the fatty liver patients texture features analysis and computer-aided diagnosis system of implementation using the ROC curve analysis, from the computer tomography image. We tried to provide an objective and reliable diagnostic information of fatty liver to the doctor. Experiments are usually a fatty liver, via the wavelet transform of the abdominal computed tomography images are configured with the experimental image section, shows the results of statistical analysis on six parameters indicating a feature value of the texture. As a result, the entropy, average luminance, strain rate is shown a relatively high recognition rate of 90% or more, the control also, flatness, uniformity showed relatively low recognition rate of about 70%. ROC curve analysis of six parameters are all shown to 0.900 (p = 0.0001) or more, showed meaningful results in the recognition of the disease. Also, to determine the cut-off value for the prediction of disease six parameters. These results are applicable from future abdominal computed tomography images as a preliminary diagnostic article of diseases automatic detection and eventual diagnosis.

  • Recently, There has been a growing interests in exposure dose to the patient who take a examination using radiation. The radiological technologists should be concerned about the exposure dose to patients and make an efforts to reduce the patient dose without decreasing the image quality. In the case of foreign, the exposure dose of general X-ray examination have been managed by standard value of exposure dose using dose area product (DAP) and entrance surface dose (ESD) dosimeter. This study is to compare DAP and ESD in skull anterior posterior (AP), chest posterior anterior (PA), and abdomen AP projections of phantom by using DAP and ESD dosimeter. In the results, there were no differences between DAP and ESD dosimeter.

  • In diagnostic X-ray examinations, dose assessments for pregnant female and fetus are realistically difficult, and related research is also lacking. Therefore, in this study, the purpose of the simulation was to analyze the dose and fetal dose for pregnant female during abdominal X-ray examination. Based on the data presented in ICRP 89, this study produced phantom reconstructed of the existing prenatal phantom, which was used to analyze the evaluation of the organ dose and fetal dose of pregnant female according to pregnancy week and the difference between the dose of the existing phantom and the reconstructed phantom. As a result, the abdominal X-ray test showed a tendency to show higher doses for organs close to the direction of the source joining. In addition, it was confirmed that fetal doses in posteroanterior position were reduced by more than 65% compared with anteroposterior position.

Usefulness of Ultrasonographic Examination by a Pediatrician in Children with Abdominal Pain (소아 복통에서 소아과 의사에 의한 초음파 검사의 유용성)

  • Purpose: Ultrasonography (US) is widely used as a screening test in patients with abdominal pain (AP). We investigated the usefulness of US by a pediatrician in children with AP. Methods: We retrospectively analysed the medical records of children with AP who undertook US from December, 2008 to July, 2010. Results: A total of 628 patients (325 male, 303 female) were enrolled in this study. The mean age of patients was $8.08{\pm}4.61$ years. Duration of AP was acute in 427 and chronic in 201 patients. Localization of AP was diffuse (36.9%), periumbilical (24.4%), epigastric (21.0%), and right lower quadrant (8.1%). On the examination, there were no abnormal findings in 327 patients (52.1%). Abnormal ultrasonographic findings were mesenteric lymphadenitis (27.1%), intestinal mural thickening (10.0%), intussusception (3.0%), appendicitis (2.6%), choledochal cyst (1.6%), and pancreatitis (0.3%). We performed additional imaging studies such as computed tomography (CT) or magnetic resonance imaging (MRI) in 39 patients who showed obscure findings on the US. In 33 patients (84.6%), the same results were obtained from CT or MRI. Two cases of appendicitis, one case of pancreatitis and one case of Henoch-Sh$\ddot{o}$nlein purpura were diagnosed by the CT examination. However, there were two cases of appendicitis diagnosed by US thathad no evidence of appendicitis on the CT. Diagnostic accuracy of initial US in children with abdominal pain was 99.4%. Conclusion: US by a pediatrician as a screening test in children with AP provides a rapid and accurate diagnostic indication and has non-invasive and radiation-free advantages.

Extragonadal Intraabdominal Mixed Germ Cell Tumor with Liver Metastasis: Successful Treatment and Long-term Follow-up (간 전이를 동반한 복강 내 성선 외 혼합 생식세포종양: 성공적인 치료 및 장기간 추적관찰)

  • 성선 외 생식세포종양은 비교적 드물어 모든 생식세포종양의 5% 미만을 차지한다. 주로 천미부생식세포종양이 골반부나 후복막으로 확장된 경우가 대부분이며, 후복막이나 복강 내에 발생하는 생식세포종양은 매우 드물다. 저자는 복부 종괴를 주소로 내원한 18개월 된 여아에서 간 전이를 동반한 복강 내 혼합 생식세포종양을 치료하였다. 복부 초음파 및 전산화 단층촬영에서 복부 좌상사분역에 석회화, 고형 및 지방 성분을 포함하고 있는 낭성 종괴와 간에 1cm 크기의 결절이 관찰되었다. 종괴는 개복 후 제거되었으며, 병리조직학적으로 대부분의 내배엽동 종양과 적은 성숙 기형종으로 구성된 혼합 생식세포종양으로 진단되었으며, 간의 전이병변도 동일하게 진단되었다. 수술 후 항암화학요법을 시행하였으며, 환자는 현재 수술 후 10년이 경과하였으나 재발의 소견은 없다.

  • In order to reduce the irresistible radiation exposure of patients who perform periodic examinations using a CT among various scan parameters a method to reduce patient dose was investigated through changes in the tube voltage close to X-ray penetrating power. As a result of the experiment 100 kVp was applied instead of 120 kVp which is commonly used in clinical practice and CTDI decreased by about 41% during scan. In addition the degree of change in image quality was measured as 1046.1±3.7 HU for CT value and 71.4±7.9 for Pixel value and statistical analysis showed no significant difference (0.05

  • A 53-year-old woman presented with dyspnea. She had undergone extended thymectomy for an invasive thymoma two months prior. CT revealed numerous small nodules in the lung. After that, she deteriorated owing to acute respiratory distress syndrome (ARDS), and the vascular surgeon planned veno-venous extracorporeal membrane oxygenation (ECMO). During percutaneous cannulation through the left femoral vein, a vascular injury was suspected, and the patient's vital signs became unstable. Diagnostic angiography showed a ruptured left common iliac vein, and the bleeding was stopped by placement of a stent-graft. May-Thurner syndrome was diagnosed on abdominal CT. Here, we report a rare case of ECMO-related vascular injury in a patient with an unrecognized anatomical variant, May-Thurner syndrome.

  • Purpose: We aimed to assess the role of positron emission tomography using fluorodeoxyglucose (FDG-PET) in the diagnosis of recurrence or the assessment of therapeutic response in cervical and ovarian cancer patients through making a comparison between FDG-PET, abdominal computed tomography (CT) and serum tumor marker. Materials and methods: We included 103 cases (67 patients) performed FDG-PET and abdominal CT. There were 42 cervical cancers and 61 ovarian cancers. We retrospectively reviewed the interpretations of PET and CT images as well as the level of tumor marker. We calculated their sensitivity, specificity, positive predictive value and negative predictive value for these three modalities. And then we analyzed the differences between these three modalities. Results: Tumor recurrences were diagnosed in 37 cases (11 cervical cancers and 26 ovarian cancers). For PET, CT and tumor marker, in cervical cancer group, sensitivity was 100% (11/11), 54.5% (6/11) and 81.1% (9/11), respectively. And specificity was 93.6% (29/31), 93.6% (29/31) and 100% (31/31). In ovarian cancer group, sensitivity was 96.2% (25/26), 84.6% (22/26) and 80.8% (21/26), and specificity was 94.3% (33/35), 94.3% (33/35), 94.3% (33/35), PET was highly sensitive to detect the intraperitoneal and extraperitoneal metastasis with the help of the CT images to localize the lesions. However, CT had limitations in differentiation of the recurrent tumor from benign fibrotic tissue, identification of viable tumors at the interface of tissues, and detecting extraperitoneal lesions. Conclusion: FDG-PET can be an essential modality to detect the recurrent or residual tumors in gynecologic cancer patients because of its great field of the application and high sensitivity.

  • Acute abdominal Aortic occlusion is rare but it is a vascular emergency with high mortality and morbidity. There­fore, delay in diagnosis can have severe impact on the prognosis. A 60-year-old women complained of paresthesia, paralysis, and severe pain in bilateral lower extremities on 13th day after open heart surgery for mitral stenosis, atrial fibrillation, coronary arterial stenosis, tricuspid regurgitation, and atrial septal defect. Her skin was mottled and cool from the umbilicus to the feet, and there were no palpable pulses in the lower exteremities. We diagnosed an acute abdominal aortic occlusion using the Multi-Detector Row Spiral Computed Tomography and successfully treated the problem with emergent thrombo-embolectomy and Aortobifemoral bypass.

  • An inflammation of an epiploic appendage is a relatively rare cause of acute abdominal pain. It can be clinically misdiagnosed as either acute appendicitis or diverticulitis. Judicious interpretation of imaging with ultrasound or computed tomography may lead to an early diagnosis and prevent unnecessary surgical or medical treatment. Conservative treatment of symptoms is usually sufficient. We have experienced a case of epiploic appendagitis treated by conservative management and report this case with the review of the literature.
