Magnetic Resonance Imaging Feature of Bacterial Meningitis in a Neonate Hanwoo Calf (한우 송아지에서 세균성 뇌막염의 자기공명영상)

  • A 1-week-old, male Korean native calf with acute clinical signs of depression, mild diarrhea, ataxia, recumbency and tremor was referred to Chonbuk Veterinary Medical Center of Chonbuk National University. Vision loss and cornea edema were also observed on physical examination. The patient had been deteriorated with nystagumus, strabisumus and opisthtonus. Blood cell count test and blood biochemistry test revealed remarkable leukocytosis, and hypoalbuminemia and increased blood urea nitrogen. No remarkable findings were observed on radiography. On magnetic resonance imaging study, there were enlarge lateral, third, and forth ventricles. The cortical grey and subcortical white matter of left temporal lobe showed hypointense on T1-weighted images and hyperintense on T2-weighted images, and slightly enhanced on contrast-enhanced T1-weighted images. Escherichia coli strain was identified from cerebrospinal fluid sample. Palliative treatment was attempted but the neonatal calve was expired three days after admission. Severe multifocal fibrino-suppurative meningitis with Escherichia coli infection was confirmed histopathologically.

Treatment of Malignant Peripheral Nerve Sheath Tumor Using Surgery and Metronomic Chemotherapy in a Dog (개에서 발생한 악성 말초 신경집 종양의 외과적 절제와 메트로놈 화학요법을 이용한 치료 증례)

  • A 6-year-old female Shih-tzu dog was referred with left elbow joint mass associated with weight bearing lameness. Ultrasonography demonstrated an encapsulated hyperechoic mass at the left elbow joint. Radiography was performed on elbow joint, chest, and abdomen, but there was no evidence of metastasis. Clinicopathologic examination revealed the existence of neoplastic cells with anisocytosis, pleomorphism and increased nuclear-cytoplasmic ratio. The left forelimb was amputated for cure. Histopathological examination diagnosed the mass as a malignant peripheral nerve sheath tumor. During the 4 weeks follow-up, the patient showed full remission and adopted to walk with remaining three legs. Thereafter, the tumor recurred after 5 months of first surgery at the operation site and other two cutaneuos regions. Cytology test revealed the mesenchymal originated tumor cells with malignancy. These tumors were surgically removed and histopathological examination of the resected tissue revealed the recurrence of primary tumor and metastasis. Metronomic therapy with cyclophophamide (10 mg/$m^2$, PO, sid) and piroxicam (0.3 mg/kg, PO, sid) had been adopted for 5 months. At 26 months of follow up after the first surgery, the dog alive with satisfactory quality of life. Aggressive surgical resection with metronomic chemotherapy should be the most effective treatment for malignant peripheral nerve sheath tumor.

Treatment of Recurrent Coxofemoral Joint Luxation by Total Hip Replacement in a Dog (대퇴관절 전치환술을 이용하여 개의 재발되는 대퇴관절 탈구증의 치료)

  • A 7-year-old, intact female Golden Retriever weighing 38 kg was referred for left coxofemoral joint luxation. On physical examination, pain and crepitus were noted atthe left hip joint during joint extension. Radiological examination revealed coxofemoral joint luxation and mild degenerative bone changes in the left hip joint. We performed minimally invasive arthroscopy-assisted reduction with TightRope$^{(R)}$ as a treatment option; however, the left coxofemoral joint was reluxated after one month postoperatively. We performed cementless total hip replacement (THR) of the luxated left coxofemoral joint. At tenmonths after surgery, the dog could sit, stand, walk normally and jump comfortably without lameness. The thigh girth measurement of the operated limb was 108.6% of the contralateral limb. Based on the present case, THR can be a successful treatment for coxofemoral joint luxation in dogs with failed coxofemoral joint reduction.

  • This paper presents the biomechanical analysis and evaluation technology of musculoskeletal system by multi-body human dynamic model and 3-D motion capture data. First, medical image based geometric model and material properties of tissue were used to develop the human dynamic model and 3-D motion capture data based motion analysis techniques were develop to quantify the in-vivo joint kinematics, joint moment, joint force, and muscle force. Walking and push-up motion was investigated using the developed model. The present model and technologies would be useful to apply the biomechanical analysis and evaluation of human activities.

A Case of Acquired Myasthenia Gravis in German Shepherd Dog (개에서 발생한 후천성 중증근육무력증 일례)

  • A three-year old intact female German shepherd dog weighing 32kg was presented with the history of regurgitation and vomiting. Physical examination revealed a thin body condition, hoarse voice, hypersalivation, and pelvic limb weakness. In radiograph, meagaesophagus was identified. The neostigmine challenge test was positive. The titer of acetylcholine receptor (AChR) antibody was 1.58 nmol/L (reference range, <0.6nmol). Therefore, the dog was diagnosed to be suffering with chronic generalized form of acquired myasthenia gravis and followed by treatment with pyridostigmine bromide, 1mg/kg, q12 hrs. Treatment improved the pelvic limb weakness, however intermittent regurgitation and vomiting persisted. This resulted in aspiration pneumonia f3r which antibiotics were administered However, the dog was euthanized due to poor management and owner's request.

Posterior and Posterolateral Instability of Knee Joint (후방 및 후외측 불안정성 슬관절)

  • Posterolateral instability of the knee occurs more commonly in association with an injury to anterior and posterior cruciate ligament and combined injuries are severe injuries that result in significant functional instability and articular cartilage degeneration. Reconstruction of the anterior and posterior cruciate ligament without an appropriate treatment of posterolateral corner injury result in failure of the reconstructed cruciate ligaments. Meticulous physical examinations, imaging studies, lower limb alignment and gait pattern should be evaluated. Acute grade III isolated or combined injury of the posterolateral corner is best treated within three weeks by direct repair, or augumentation, or reconstruction. The appropriate surgical method or combined methods are selected among the several methods of posterior and posterolateal reconstruction, and all injuried posterolateral and cruciate ligament structures are anatomically reconstructed simultaneously or by stages. If a varus alignment and varus thrust is disclosed in chronic posterolateral instability of knee, soft tissue reconstructions laterally are highly unlikely to be able to correct tile problem. It is appropriate that valgus osteotomy should be done before soft tissue reconstruction and reevaluate the posterolateral instability about 6 months later.

  • Scoliosis describes a lateral curvature of the spine and is often associated with cosmetic and functional impairments due to severe deformity of the spine. The incidence of adolescent scoliosis is 2-4% of children between 10 and 16 years of age. Eighty five percent of them are idiopathic, in which the most common type of scoliosis is right side-bending. In addition, it is classified into congenital, and secondary scoliosis such as neuromuscular disease and neurofibromatosis. Congenital scoliosis is associated with abnormalities of urinary system(20%), congenital heart diseases(15%), and other abnormalities(10%) such as syringomyelia. We experienced a case of scoliosis by a foreign body(a pencil) in the colon which has never been reported up to date. A brief review of the literature was made.

Treatment of an Occipital Fracture by Esquillectomy (골편적출술을 적용한 후두골 골절 치료)

  • A 4-year-old male Yorkshire terrier was referred to us with signs of vomiting and unconsciousness due to a blunt head trauma. Gross examinations detected facial edema, subcutaneous hemorrhage and hypersalivation. A survey radiograph located an occipital fragment which was displaced caudally. A three-dimensional computed tomographic reconstruction demonstrated that the ventral portion of the fragment was attached incompletely. Because of the instability of the fragment, it was decided to perform an esquillectomy. After removing the fragment, the defect was reinforced with a muscular flap originating from the splenius muscle. The patient's condition gradually improved except for a slightly ataxic gait. At 20 months follow-up, there was no evidence of ataxia. The neurological status did not deteriorate before starting surgical intervention, although the patient sustained a skull fracture with severe intracranial hemorrhage. It is likely that the fragment being displaced outwardly played an important role in preventing an increase in intracranial pressure which could have led to neurological deterioration.

Tibial Fracture Repair in a Black Gibbon (Hylobates concolor) (긴팔원숭이에서 발생한 경골 골절의 외과적 정복 1례)

  • A 5-year-old intact male black gibbon (Hylobates concolor) was referred for evaluation of the right pelvic limb lameness following a fight against other black gibbons. Fractures of the right tibia and fibula were suspected on physical examination and palpation of the right pelvic limb, but no other injuries or abnormalities were detected. While the black gibbon was sedated, pelvic limb radiographs were taken, which revealed diaphyseal oblique fractures of the right tibia and fibula. Open reduction of the fractures was performed. The tibial fracture was repaired by use of an internal fixation technique that included a tubular dynamic compression plate and cortical screws secured along the craniomedial aspect of the tibia. There were no complications during the postoperative rehabilitation period. At 9 weeks, radiographs revealed that bridging callus was well formed over the cortices of the tibial and fibular fracture area. The cast was removed 9 weeks after surgery. The black gibbon exhibited no evidence of lameness and was released back into the group. Presently, there are no published reports of internal fracture fixation in a black gibbon where a tubular dynamic compression plate and cortical screws provided excellent stabilization of the tibia and complete fracture healing allowing normal ambulation.

The effect of unstable plate on the ankle joint displacement and dynamic balance ability of female college students wearing high-heeled shoes (불안정 판을 이용한 훈련이 높은 굽 신발을 신은 여자 대학생의 발목관절 변위와 동적균형능력에 미치는 영향)

  • The purpose of this study was to investigate the effect of training using unstable plate on displacement and dynamic balance ability of ankle joints in women college students wearing high heel shoes. Subjects were randomly divided into experimental group and control group. 3D motion analysis was performed while walking 9cm high-heeled shoes and walking 5m. Dynamic balanced ability evaluation was performed. The intervention program was administered to experimental subjects three times a week for four weeks. They performed balance training using unstable plates. There was no intervention in the control group. The results showed that the displacement of the ankle joints in the experimental group after the intervention was decreased overall and the dynamic balance ability was significantly increased. The control group showed little change. As a result, the training using unstable plate stabilizes the ankle joints and improves the dynamic balance ability of the subjects wearing high-heeled shoes.
