3-D CT Imaging of Pathological Bone Changes in a Rat Model of Adjuvant-Induced Arthritis

  • Computed tomography (CT) is a medical imaging method employing tomography. CT is a 3-Dimensional (3-D) radiographic imaging technique, which is not suited for assessment of inflammation, but can be considered a reference method for assessment of bone damage, due to its direct 3-D visualization of calcified tissue. In this study of pathological joint changes in a rat model of adjuvant-induced arthritis (AIA) and quality analysis of bone destructions were performed by 3-Dimensional computed tomography images. These data demonstrate that the destructive progression of disease in a rat AIA model can be quantified using 3-D CT image analysis, which allows assessment of arthritic disease status and efficacy of experimental therapeutic agents.

  • 본 연구에서는 무릎 보조도구를 착용하지 않은 상태와 착용한 상태에서 앉았다 일어서기 동작 시의 무릎 근육의 활성도 차이를 비교하여, 착용형 무릎 보조도구가 무릎 뼈관절염을 가진 노인들의 하지 근육의 근 활성도에 미치는 영향을 알아보았다. 그 결과, 무릎 보조도구를 착용하지 않은 상태에 비해 보조도구를 착용한 상태에서 안쪽넓은근과 가쪽넓은근의 근 활성도가 통계적으로 유의하게 감소한 것으로 나타났다. 이를 통해, 무릎 보조도구가 앉았다 일어서기 동작 시 하지 근육을 보조하여 근 활성도를 감소시키는 등 무릎의 뼈관절염을 가진 노인들의 일상생활 보조에 유용할 수 있는 가능성을 확인할 수 있었다.

  • 골관절염은 관절연골의 퇴행성 변화로 연골기질의 분해로 인하여 연골 강도와 cushion으로서의 능력이 감소되는 질환이다. 골관절염의 대부분 약물치료는 통증과 염증을 감소시키는 목적으로 사용되며, 근본적인 치료효과를 주는 약물은 현재까지 개발되어 있지 않다. 영양약학 제품에 대한 질병의 예방적, 치료 보조적 차원에서 인체에 대한 기초적인 생리활성의 중요성을 인식하게 됨으로써 그 역할이 중요하게 인식되고 있다. 특히, 골관절염 치료에 쓰이는 glucosamine은 proteoglycan (PG)와 glucosaminoglycans(GAGs)의 합성의 전구물질로서 연골세포 생성을 자극하며, 통증, 염증의 경감 및 진행과정을 억제시키고, 관절기능 회복을 촉진하는 것으로 알려져 있다. 그러나 glucosamine이 어떤 기전에 의해 관절연골세포에 직접적인 영향을 주는지 밝혀져 있지 않다. 본 연구에서는 glucosamine sulfate가 연골세포에 미치는 영향을 규명하고자 하였다.

Physical disability, perceived dependence and depression in women with osteoarthritis (관절염을 앓고 있는 여성들의 신체활동 장애와 우울감 사이 의존성의 매개효과)

  • Objective: This study assessed whether both physical disability and perceived dependence relate to depression or whether perceived dependence in personal care activities, household activities, community mobility and recreational activities is an intermediate step in the relationship between physical disability and depressive symptoms. Methods: Data come from a cross-sectional survey of 209 women, aged 55 or older, with osteoarthritis (OA). Mediation analyses occurred in four step and included logistic regression analyses as outlined by Baron & Kenny. Results: The results from step 1 showed that greater difficulty with each domain of physical disabilities was significantly related to greater perceived dependence. In step 2, greater perceived dependence was significantly associated with greater depressive symptoms in all domains. Step 3 analyses showed that greater physical disabilities in each domain were significantly associated with depression. The final step testing mediation indicated that personal care activities, household activities and community mobility were fully mediated by perceived dependence. That was, once dependence was taken into account, the relationship between physical disabilities and depression was no longer significant. Partial mediation was found for dependence and recreational activity limitations. Conclusion: It is important to take into account the experience of perceived dependence as a mediator in understanding the relationship between disability and depression in the domains of personal care, household, and community mobility. To conclude, these findings point to the importance of taking into account an individual's reaction to their disability rather than just focusing on the severity of disability.

Utilization Pattern of Complementary Therapy in Hypertension, Diabetes and Chronic Arthritis Patients Visited to Local Health Center (일개 보건소를 방문하는 고혈압, 당뇨 및 관절염환자의 보완요법 이용실태)

  • Objectives: The objective of this study is to investigate the use rate and some aspect of complementary therapies used by patients with chronic illness(hypertension, diabetic mellitus and chronic arthritis). Methods: 600 patients visiting the health center for one month(Jan. 2001) were interviewed on their complementary therapies used by the subjects for the previous year. Results: About fourteen-eight percent of the respondents used therapies; 35% of patients with hypertension, 44.6% of patients with diabetic mellitus and 62.9% of patients with chronic arthritis, which shows the highest rate among patients with three chronic disease. The use rate of complementary therapies indicates few meaningful differences according to the general characteristics of the interviewees. Hypertension patients used herb medication(31.0%) acupuncture(29.6%) and most of all the other therapies. Diabetic patients used dietary therapy(57.5%) and herb medication(35.1%). Chronic arthritis patients used acupuncture(85%) and herb medication(34.7%). 36.8% of all the patients who used complementary therapies tried more than two therapies. 18.3% of hypertension patients, 24.1% of diabetic patients and 55.9% of chronic arthritis patients used more than two therapies. Acupuncture(47%) was used most frequently, followed by herb medications(26.3%), health assistance utensils(21.8%). oriental therapy(21.8%), physical therapy(9.5%), health assistance food(8.4%), herb(7.7%), Korea hand acupuncture(3.2%), abdomen respiration(1.1%), and pore therapy(0.7%) Oriental clinic was visited most frequently(42.8%), which was used to cure diseases(61.8%), and to relieve symptoms(26.0%). (p<0.001) The cost spent on complementary therapies last year was 90,000 won(40.3%) and there are some cases of more than 500,000 won(31.2%). Most of the patients(56.1%) were satisfied with the complementary therapies, with 6% of them having side effects. 74% of the patients used complementary therapies answered that they would continue them and 56.1% of them also answered that they would continue them and 56.1% of them also answered that they would advise other patients to do them. Advantages(compared with those of orthodox medical treatment) are psychological comfort(28.1%), body protection(26.0%), effectiveness(20.0%). 34% of the patients using complementary therapies wanted to have informational orientation on complementary therapies. These findings reveal that a considerable number of patients with chronic illness(47.5%) tried a variety of complementary therapies. Though 6% of the patients using therapies had side effects, most of the subjects seemed satisfied with them and they are supposed to continue them. Conclusions: In conclusion, health center personnels and medical doctors should pay more attention to the complementary therapies used by patients with chronic illness. They also have to try their best to advise more scientific and informative complementary programs with less side effects and more help to improve their conditions.

A Study on Flow Characteristics around Foot to Investigate Principle of Underwater Exercise for Rheumatoid Arthritis Patients (류마티스 관절염 환자의 수중운동 원리 규명을 위한 발 주위 유동 특성 연구)

  • There are positive effects, such as pain reduction, when rheumatism patients exercise in water, but the cause of the pain reduction is unclear, and research on this is inadequate. This study examined the flow of the surface of the foot and the principles of pain relief. Unsteady simulations were conducted to analyze the flow, which was performed by repeatedly setting the movement of the foot raising and descending three times. Pressure fluctuations and frequencies were analyzed by designating pressure points at the painful location. The results showed that a positive and negative pressure of approximately ±500Pa was applied overall. A pressure of approximately ±2000Pa was applied when the direction of movement was changed. A frequency of approximately 35 to 80Hz was generated in the area where rheumatoid arthritis pain frequently occurs. The effects of reducing pain could be predicted when continuous pressure fluctuations and frequencies are applied repeatedly to the painful location, blood circulation promotion. The results could be used as basic data to understand the principles of aquatic exercise and support the development of underwater exercise programs and developing related medical equipment.

Expression of Co-stimulatory Molecules and STAT/SOCS Signaling Factors in the Splenocytes of Mice Tolerized against Arthritis by Oral Administration of Type II Collagen (제2형 콜라겐으로 경구관용을 유도한 관절염 모델 마우스의 비장림프구내의 보조자극인자 및 STAT/SOCS 신호전달 인자의 발현 양상조사)

  • Oral administration of antigen has long been used in the induction of immune tolerance in various animal models of autoimmune diseases including rheumatoid arthritis (RA). Alleveation of arthritogenic symptoms has been reported from RA patients who received oral administration of type II collagen (CII) without side effects, however its rather inconsistent therapeutic efficacy and variation among patients calls for more detailed investigation on the mechanism of oral tolerance to be settled as regular treatment for RA. In an attempt to understand the immunogenic processes underpinning tolerance induction by orally administered CII, we analyzed changes in the expression of costimulatory molecules and STAT/SOCS signaling messengers in the mouse model of collagen induced arthritis (CIA). We found thatin the spleen of CIA mice, that has been undergone repeated oral feeding of CII prior to the induction of arthritis, showed increased promortion of CTLA4 expressing lymphocytes than in the spleen of PBS fed control. On the other hand, cells expressing CD28 or ICOS were decreased in the spleen of tolerized mice. Tolerance induction by oral CII administration also enhanced the expression of STAT6 in both RNA and protein level, while not affecting the expression of STAT3. The expression of SOCS3, which hasbeen known to transmit STAT-mediated signals from Th2 type cytokines, remained unchanged in the spleen of tolerized mice. Interestingly transcript of SOCS1, which has been associated with Th1 related pathways, was only visible in the spleen of tolerized but not of control mice, suggesting that as in the case of IL-6 signaling, it may exert a feed back inhibition toward the Th1 type stimulation.

A Study on how to Utilize the DR System using Assistive Devices (보조기구를 이용한 DR System 활용 방법에 대한 연구)

  • The hip joint device is an important immobilization device for internal rotation in the diagnosis of bone density test, microfracture, and arthritis. These fixing mechanisms have limitations in maintaining reproducibility due to the use of Velcro. In this study, we developed an improved hip joint device and evaluated its usefulness. For 30 students enrolled in the Department of Radiology, we evaluated the reproducibility through medical imaging and the time required for positioning by applying the conventionaling and improved device. Changes over time were also evaluated. As a result, compared to the conventional device, the improved device showed a 32% reduction in positioning time and improved reproducibility by about 6.2 times. As for the amount of change over time, the positioning time increased in the conventional device, but decreased in the improved device, and the reproducibility of the conventional device was lower than that of the improved device. Through this, it can be said that the use of the improved device improved the image quality and reduced the radiologist's workload.

Three Dimensional Printing Technique and Its Application to Bone Tumor Surgery (3차원 프린팅 기술과 이를 활용한 골종양 수술)

  • Orthopaedics is an area where 3-dimensional (3D) printing technology is most likely to be utilized because it has been used to treat a range of diseases of the whole body. For arthritis, spinal diseases, trauma, deformities, and tumors, 3D printing can be used in the form of anatomical models, surgical guides, metal implants, bio-ceramic body reconstruction, and orthosis. In particular, in orthopaedic oncology, patients have a wide variety of tumor locations, but limited options for the limb salvage surgery have resulted in many complications. Currently, 3D printing personalized implants can be fabricated easily in a short time, and it is anticipated that all bone tumors in various surgical sites will be reconstructed properly. An improvement of 3D printing technology in the healthcare field requires close cooperation with many professionals in the design, printing, and validation processes. The government, which has determined that it can promote the development of 3D printing-related industries in other fields by leading the use of 3D printing in the medical field, is also actively supporting with an emphasis on promotion rather than regulation. In this review, the experience of using 3D printing technology for bone tumor surgery was shared, expecting orthopaedic surgeons to lead 3D printing in the medical field.
