IL-1 gene polymorphisms in Korean periodontitis patients (한국인 치주염 환자에서의 IL-1 유전자 다변성 연구)

  • 중증 만성 치주염과 1L-1B+3954 및 1L-1A+4845 유전자의 대립유전자 2 보유 유전자 다변성이 관련된다고 보고되었다. 그러나 이러한 1L-1 복합유전자 다변성과 만성 치주염 및 급진성 치주염과의 관련성에 대해서는 상반되게 보고되고 있는데 이는 인종적 배경과 질환특성의 차이에 기인한 것으로 보인다. 이 연구는 한국인에서 경도, 중등도와 중증의 만성 치주염 그리고 급진성 치주염 환자를 대상으로 하여 1L-1A+4845, 1L-1B+3954, 1L1B-511, 1L-1 RN intron 2 (VNTR) 유전자 다변성의 분포를 평가하고, 치주질환의 심도와 유형에 관련되는지 알아보고자 시행되었다. 전남대학교 병원 치주과에서 검진과 치료를 받은 100명의 치주질환자를 대상으로 하였고 질환군은 치주낭 깊이, 부착 소실, 골 소실을 기준으로 하여 경도, 중등도, 중증의 만성 치주염, 급진성 치주염군으로 분류하였다. 대조군으로는 전남대학교 병원 소아치과에 내원한 전신적으로 건강한 92명의 아동을 포함하였다. 각 대상 환자에서 채취된 협점막상피에서 genomic DNA를 얻어 1L-1A+4845, 1L-1B+3954, 1L-1B-511 genotype은 중합효소 연쇄반응을 시행한 후 제한 효소분해과정을 거쳐 전기영동 후 분리한 결과를 해석하였으며 1L-1 RN(VNTR) 유전형은 중합효소연쇄반응 후 분리한 결과를 해석하여 다음의 결과를 얻었다. 대립유전자 2 보유자 비율은 치주질환자에서 1L-1A+4845, 1L-1B+3954, 1L-1B-511, 1L-1 RN이 각각 61%, 13%, 76.6%, 34%였으며 대조군에서는 76.9%, 7.7%, 62.2%, 19.1%였다. 1L-1B+3954과 1L-1A+4845 대립유전자 2 보유자인 양성유전자형 비율은 경도, 중등도, 중증의 만성치주염, 급진성 치주염환자에서 각각 10%, 7.9%, 22.2%, 12% 였으며 치주질환자의 13%, 대조군의 7.7%에서 양성 복합유전자형(positive genotype)을 보였다. IL-1B-511 유전자 다변성은 치주질환자에서 대조군에 비하여 높았으며 급진성 치주염환자에서 대립유전자 2 보유자율이 유의하게 높았다(p<0.01). IL-1 RN intron 2 유전자 다변성은 중등도 및 중증 만성 치주염환자에서 대립유전자 2 보유자율이 유의하게 증가하였다. 이러한 결과는 IL-1 gene cluster의 유전형이 한국인에서도 치주염의 유형과 질환 심도에 관련될 수 있음을 시사하였다.

The Immune Response and Protective Efficacy of Hepatitis B Vaccine in Neonates Born from Hepatitis B Carrier Mothers (B형 간염 바이러스 보유 산모로부터 분만된 신생아의 B형 간염 백신의 면역반응 및 방어효과)

  • Purpose : We performed this study to evaluate the immune responses and protective efficacies of the HBV vaccine in infants born from hepatitis B virus(HBV) carrier mothers. Methods : Seventy eight infants born from HBV carrier mothers, who were able to follow up for 12months in the Catholic University St. Vincents hospital, were involved in this study from July 1995 to December 1996. Samples were collected at birth, 4, 8 and 12months after injection of HBIG and HBV heat-inactivated plasma derived vaccines. We evaluated the changes and relationships of viral markers detecting by enzyme immunoassay and radioimmunoassay between HBV carrier mothers and their infants. Results : 1) A total of 5.0%(106/2,117) of pregnant women were found to be a HBV carrier. The rates of HBeAg positive and negative were 38.5%(37/96) and 61.5%(59/96), respectively. 2) The seroconversion rates of anti-HBs with infants of HBV carrier mothers at 4, 8 and 12 months were 85.9%(67/78), 75.6%(59/78) and 73.1%(57/78), respectively. Although these were statistically significant differences(P<0.05), they were not related to HBeAg status of the mothers. The geometric mean titers of anti-HBs at 8 and 12 months were significantly higher than at 4 months, statistically(P<0.05). The protective efficacy of the HBV vaccine and HBIG at 12 months in infants from HBeAg positive and negative mothers were 89.8% and 100%, respectively. 3) Five of 78(6.4%) infants became infected by HBV from only HBeAg positive mothers during the follow up period of 12 months. Three of 5 infected infants became HBV carriers. HBsAg positive at birth from HBeAg positive and negative mother were 4 infants, respectively. Three of 4 infants became infected by HBV from only HBeAg positive mothers. Conclusion : We confirmed that the seroconversion rate of HBV heat-inactivated plasma derived vaccine which was one of other vaccines manufacturing in Korea was 85.9%. The protective efficacy of this HBV vaccine and HBIG at 12 months in infants from HBeAg positive and negative mothers were 89.8% and 100%, respectively.

Understanding policies regarding intangible cultural treasures and directions for improvement to promote the continuing tradition of Pansori (판소리 전승 활성화를 위한 무형문화재 제도의 이해와 개선 방향)

  • Pansori has been passed down over several generations and over time have undergone continued change in accordance with the times, as well as the skills and ability of the singer. Policies regarding intangible cultural treasures were established to preserve and promote the continuing tradition of art forms including Pansori and thus must spare no effort in supporting and preserving the genre. As such, for proper implementation of the newly legislated law, it is necessary to review the agents who pass down the tradition of Pansori and whether there are any areas that need to be changed in terms of our perception of culture in general. Pansory in the $21^{st}$ century features both contemporary aspects and mass appeal and have undergone many changes in how it is enjoyed. It is our responsibility therefore, to establish how the art and universality of Pansori should be promoted. From this perspective, this study reviewed the evolution of law on intangible cultural treasures, the current status of intangible cultural treasures being passed down with a focus on national treasures and those of Jeonbuk Province to shed light on issues. Diversification is needed in the number of those who carry this intangible cultural treasure, as well as the number of categories. To that end, a survey index or practical ability index must be taken into account for the application and designation of intangible cultural treasures. The study also noted issues of the categories for designation as intangible cultural treasures and discussed directions for improvement. In the case of Pansori, suggestions for improvement were presented for the designation of skilled artists by school, regular surveys and regular application, increased role of artists for increased mass appeal, survey of regional singers, supervision and monitoring of skilled artists and establishment of a manual for the education on how to pass down the art form. In doing so, efforts should be made to make the passing down of Pansori more active and related education more systematic. Since we are in the early years of the law on intangible cultural treasures being implemented, areas of improvement will continue to be identified. It is however certain that the proper support for the art form to be handed down should be done in a way where law and culture are complementary given that Pansori is not just a Korean tradition, but a tradition of mankind.

The Application of the Principle of "Preserving the Original Form" to Intangible Heritage and Its Meaning (무형문화재 '원형규범'의 이행과 의미 고찰)

  • With the introduction of the system of recognizing masters of craft and performance skills in 1970, the principle of "preserving the original form," which was already in general use, was adopted as a legal principle in the Cultural Heritage Protection Act. While the concept "original form" can be related to tangible elements of heritage through the Act, the intangibility of craft and performance skills does not allow their pinpointing at a particular temporal period or the identification of a particular master from the past as the basis of an original form. Therefore, those craft or performance skills that are available at the point of recognition of relevant masters must serve as the basis of the original form for the intangible heritage concerned. This means that the principle of preserving the original form of intangible heritage has been implemented not based on a fundamental form of materiality, but rather on the craft or performance skills that may be held by a master at the time of his/her recognition as a "temporary original form." This principle has been observed through intangible heritage transmission and education policies for recognized masters and their trainees, contributing to establish an elitist transmission environment in which public were denied to join the education on intangible heritage. Even with policies guided by the principle of preserving the original form, designated craft and performance skills have been transformed contingent upon given social and environmental conditions, thus hindering the preservation of the original form. Despite the intrinsic limitations of the principle of preserving the original form when applied to intangible heritage, this principle has served as a practical guideline for protecting traditional Korean culture from external influences such as modernization and Westernization, and also as an ultimate goal for the safeguarding of intangible heritage, engendering actual policy effects. The Act on the Safeguarding and Promotion of Intangible Cultural Heritage that comes into effect in March 2016 takes the constantly evolving nature of intangible heritage into consideration and resultantly adopts a concept of "essential form" (jeonhyeong) in place of "original form" (wonhyeong). This new concept allows for any transformations that may take place in the environment surrounding the intangible heritage concerned, and is intended to mitigate the rigidity of the concept of "original form." However, it should be noted that "essential form," which is manifested as the unique significance, knowledge, and skills delivered by the intangible heritage concerned, should be maintained according to the guidelines and principles related to heritage conservation. Therefore, the new concept can be understood not as a rupture, but more as a continuum of the concept of "original form."

A Clinical Study of HBV Markers in Various Liver Diseases Carriers and Controls (간기능 검사상 이상을 보인 환자에서의 HBV 표식자 발현 양상)

  • Serum HBsAg, AntiHBs, HBeAg, AntiHBe and AntiHBc were detected by radioimmunoassay in 39 patients with acute viral hepatitis, 79 patients with chronic hepatitis, 30 patients with liver cirrhosis, 16 patients with primary hepatocellular carcinoma, 14 patients of HBsAg carriers and 129 cases of controls:78 cases of normal level of SGOT, SGPT, and 51 cases of elevated level of SGOT, SGPT. Following results were obtained: 1. HBsAg was detected in 66.7% of acute viral hepatitis, 63.3% of chronic hepatitis, 36.7% of liver cirrhosis, 81.3% of primary hepatocellular carcinoma and 27.1% of controls. 2. AntiHBs was positive in 0% of acute viral hepatitis, 21.5% of chronic hepatitis, 36.7% of liver cirrhosis, 31.3% of primary hepatocellular carcinoma, 0% of carrier and 44.2% of controls. 3. HBeAg was detected in 45.6% of chronic hepatitis, 23.3% of liver cirrhosis and 31.3% of primary hepatocellular carcinoma. 4. Among chronic liver diseases, antiHBe was positive in 56.3% of primary hepatocellular carcinoma, 23.3% of liver cirrhosis and 20.3% of chronic hepatitis. 5. AntiHBc was detected in most of all examines and the significance of presence of AntiHBc does not seem to represent liver disease itself but the evidence of infection of HBV. 6. Among 14 HBV carriers, 6 cases presented with abnormal SGOT, SGPT. 7. All HBV markers were negative in 5.1% of acute viral hepatitis, 5.1% of chronic hepatitis and 14.7% of controls: 17.6% of subjects with abnormal SGOT, SGPT and 12.8% of subjects with normal SGOT, SGPT. 8. Beside of HBV, other causes, such as non A, non B virus, Delta-agent, other viruses or related factors should be excluded among the patients with evidence of HBV infection associated with elevation of SGOT & SGPT.

The Association between Types of Usual Source of Care and Medication Adherence among the Elderly (고령자의 상용치료원 유형이 복약순응에 미치는 영향)

  • This study investigates the effect of types of usual source of care on medication adherence among the elderly. Using the Korea Health Panel Survey in 2017, this study includes 3,623 elderly people 65 years of age or older taking prescription drugs for treatment of chronic diseases. The type of usual source of care was considered in three aspects: whether having a usual source of care or not, whether having a regular site only or having a regular site and doctor, size of the regular site. The types of usual source of care are heavily associated with medication adherence. Compared to those who have no usual source of care, those who have a regular site only are 1.4 times, and have both regular site and doctor are 1.8 times more likely to comply with the medication guidelines, respectively. The finding shows usual source of care has a positive effect in improving medication adherence of the elderly. The results can provide an evidence for policies for effective management of chronic diseases and efficient use of medical resources.
