Bivenrticular Repair of Double Outlet Right Ventricle with Remote Ventricular Septil Defect. (비상관성 심실중격결손중을 동반한 양대혈관우심실기시증 환자에서의 양심실성 교정)

  • Understanding of the surgical anatomy of patients with double outlet right ventricle (DORV) is important in the planning of biventricular repair From May 1995 to September 1996, 7 patients underwent biventricular repair for DORV with remote ventricular septal defect. There were 5 males and 2 (tamales. Age at operation varied from 2 to 9 years(mean 3.4$\pm$ 2.7years). Preoperative diagnostic assessment was made by two-dimensional echocardiography and cardiac catheterization. Ventricular septal defect was perimembranous inlet type in all patients. Associated cardiac anomalies were pulmonary atresia in two, pulmonary stenosis in five and tricuspid chordal attachment to zonal septum in five. The operations were performed intraventricular repair and pulmonary enlargement in two, REV operation in two, and Rastelli operation in three. There was no early postoperative deaths and complications. The follow-up period war from 1 month to 18mon1hs, averaging 10: 6.1 months. In the past, we considered the Fontal operation indicative as primary choice when DORV was associated with abnormal tricuspid chordal attachment to the zonal septum, but now we believe that biventricular repair is feasible for those cases by making conal flap or reattachment method. Biventricular repair has theoretic advantages because it estabilishes normal anatomy and physiology, and it was concluded that the precise preoperative evaluation using both echocardiography and cardiac catheterization was essential to the successful surgery.

Effect of Gami-sopungsan on Inflammation and DNCB-induced Dermatitis in NC/Nga in Mice (가미소풍산(加味消風散)이 염증 및 아토피피부염 동물병태에 미치는 영향)

  • Gami-Sopungsan (GS) is one of the traditional korean remedy. We investigated the anti-inflammation and anti-atopic dermatitis (AD) effect of GS. No cytotoxicity of GS was observed in the range of $1{\sim}100{\mu}g/m{\ell}$ on Raw 264.7 cells. The Inflammatory response of Raw 264.7 cells were induced by lipopolysaccharide (LPS), followed by GS treatment at indicated concentrations (0, 1, 10 and $100{\mu}g/m{\ell}$). At $100{\mu}g/m{\ell}$ concentration, GS showed inhibitory effect on LPS-induced nitric oxide production by 20%. Production of IL-$1{\beta}$, IL-6 and TNF-${\alpha}$ was decreased by approximately 56%, 36% and 79%, respectively upon GS treatment at $100{\mu}g/m{\ell}$. 200 mg/kg of GS was orally administered to NC/Nga mice, where AD was induced by 1-chloro 2,4-dinitrobenzene. There were no significant difference between GS treated group and the control group on body weight and food intake changes during growth. The back skin of GS group showed decrease in erythema, pruritus, dry skin, edema, excoriation, erosion and lichenification level through naked eye observations. In addition, leukocyte infiltration and the thickness of epidermis were significantly decreased in the skin tissues (back and ear). The serum IgE levels were decreased by 28.8% in the GS treated group. The GS treated group showed remarkable inhibition of IL-4 (83%), IL-5 (95%), IL-6 (62%) and TNF-${\alpha}$ (84%) in serum, indicating that GS has similar or higher efficacy than those of the dexamethasone treated group. From the results above, we conclude that GS has significant anti-inflammation and anti-AD effects on Raw 264.7 cells and NC/Nga mice. The results should provide fundamental and valuable data for the research on natural products being developed against atopic dermatitis.

Effects of Phellinus linteus-Barley corn Water Extract in Triton WR-1339(TX)-Induced Hiperlipidemic Rats (상황버섯보리의 기능성에 관한 연구)

  • This study was designed to observe the effects of the Phellinus linteus-Barley corn water extract (PLBC) on the improvement of the blood biochemical aspects; serum AST, ALT, LDH, ALP, Phospholipid, Trigryceride, Total Cholesterol, HDL, LDL, of Triton WR-1339(TX)-induced hiperlipidemic rats. Experimental groups were designed to normal control group(Normal), hiperlipidemic control group(Control), hiperlipidemic control group treated with Barley corn water extract(1000mg/1kg ; BC), hiperlipidemic control group treated with Phellinus linteus-Barley corn water extract (500, 1000, 1500mg/1kg; PLBC-1, PLBC-2, and PLBC-3, respectively). Serum AST levels of BC and all PLBC treated groups were significantly decreased (p<0.005) compared to Control group. Serum ALT levels of BC and PLBC-2 groups were decreased (p<0.05), PLBC-1 and PLBC-3 groups were significantly decreased (p<0.005) compared to Control group. Serum LDH levels of BC group was decreased but no significant, PLBC-1 and PLBC-2 groups were decreased (p<0.05), PLBC-3 group was significantly decreased (p<0.025) compared to Control group. Serum ALP levels of BC and PLBC-1 groups were decreased (p<0.05), PLBC-2 group was decreased but no significant, PLBC-3 group was significantly decreased (p<0.005) compared to Control group. Serum Phospholipid levels of BC group was decreased but no significant, PLBC-1 group was decreased (p<0.025), PLBC-2 and PLBC-3 groups were significantly decreased (p<0.005) compared to Control group. Serum Triglyceride levels of BC group was increased but no significant, PLBC-1 group was decreased but no significant, PLBC-2 and PLBC-3 groups were significantly decreased (p<0.005) compared to Control group. Serum Total cholesterol levels of BC group was decreased (p<0.05), PLBC-1, PLBC-2 and PLBC-3 groups were significantly decreased (p<0.005) compared to Control group. Serum HDL levels of BC group was increased (p<0.05), PLBC-1 and PLBC-2 groups were significantly increased (p<0.005), PLBC-3 group was increased (p<0.025) compared to Control group. Serum LDL levels of BC, PLBC-1 and PLBC-2 groups were decreased but no significant, PLBC-3 group was significantly decreased (p<0.005) compared to Control group. These results suggested that Phellinus linteus-Barley corn water extract improve Triton WR-1339(TX)-induced hiperlipidemic symptom in rats.

Effects of Sungsimjihwang-tang Hot Water Extract & Ultra-fine Powder on the Alzheimer's Disease Model (성심지황탕(醒心地黃湯) 열수추출물과 초미세분말제형이 Alzheimer's Disease 병태 모델에 미치는 영향)

  • This experiment was designed to investigate the effect of the SSJHT hot water extract & ultra-fine Powder on Alzheimer's Disease Model Induced by ${\beta}A$. The effects of the SSJHT hot water extract on expression of IL-1RA, $IL-1{\beta}$$, IL-6, IL-10, $TNF-{\alpha}$, NOS-II, COX-2 mRNA and production of $IL-1{\beta}$, IL-6, $TNF-{\alpha}$ in BV2 microglial cell line treated by lipopolysacchaide(LPS). The effects of the SSJHT hot water extract & ultra-fine powder on (1) the behavior (2) expression of $IL-1{\beta}$, $TNF-{\alpha}$, MDA, CD68, CD11b and AChE (3) and the infarction area of the hippocampus in Alzheimer's diseased mice induced with ${\beta}A$ were investigated. The SSJHT hot water extract suppressed the expression of $IL-1{\beta}$, IL-6 and $TNF-{\alpha}$, NOS-II, COX-2 mRNA and increased IL-1RA, IL-10 in BV2 microglia cell line treated with LPS. The SSJHT hot water extract suppressed the production of $IL-1{\beta}$, IL-6, $TNF-{\alpha}$ significantly in BV2 microglial cell line treated with LPS. The SSJHT hot water extract & ultra-fine powder a significant inhibitory effect on the memory deficit was shown for the mice with Alzheimer's disease induced by ${\beta}A$ in the Morris water maze experiment, which measured step-through latency. The SSJHT hot water extract & ultra-fine powder suppressed the expression of $TNF-{\alpha}$$, $L-1{\beta}$ protein significantly in the microglial cell of mice with Alzheimer's disease induced by ${\beta}A$. The SSJHT hot water extract & ultra-fine powder reduced the MDA and suppressed the over-expression of CD68, CD11b in the mice with Alzheimer's disease induced by ${\beta}A$. The SSJHT hot water extract & ultra-fine powder significantly decreased AChE activity in the serum of the mice with Alzheimer's disease induced by ${\beta}A$. The SSJHT hot water extract & ultra-fine powder reduced infarction area of hippocampus. and controlled the injury of brain tissue in the mice with Alzheimer's disease induced by ${\beta}A$. The results suggest that the SSJHT hot water extract & ultra-fine powder may be effective for treatment of Alzheimer's disease. Investigation into the clinical use of the SSJHT hot water extract & ultra-fine powder for Alzheimer's disease is suggested for future research.

Study on the Shapes of Jung-kwa, Gi-kwa, Sin-kwa, Hyul-kwa (정기신혈과(精氣神血科)의 형상(形象)에 대한 연구)

  • After study about the shapes of Jung, Gi, Sin, Hyul-kwa advocated by Master Jisan, the conclusions are drawn as follows. It pursues the rules of universe being changed from formless to concrete objects that the Body Essence, Vital energy, Mentality, and Blood are materialized to Jung, GE, Sin, Hyul-kwa. It can be drawn an inference from the next three theories that Jisan considered Jung, Gi, Sin, Hyul-kwa as round, square, reverse triangle, and triangle or oval shape. First, it was taken to following the shapes of viscera. Namely Jung-kwa was taken as Kidney, Gi-kwa as Lung, Sin-kwa as Heart, Hyul-kwa as Liver. Second, it was adopted from five kinds of constitutions according to the five elements in [Yeongchu Yin and Yang twenty five shapes] that is Jung-kwa is purchased with Water-earth phases, Gi-kwa with metal phase Sin-kwa with fire phase, Hyul-kwa with Wood phase, Third, it is taken in order to eliminate contradictions. Hyul-kwa disliking to move is assumed to round shape to easily move. Gi-kwa tending to disperse is chosen to square in order to be unmoved. Sin-kwa declining to stagnate of seven emotions is pursued to reverse triangle to compose oneself. Hyul-kwa inclining to gather is preferred to acreage phase to harmonize ascending tendency. Among shapes which Jisan was divided Hado, Nakseo, Bokhee pal kwoe, Moonwang pal kwoe, Jung-kwa as round is Hado, Gi-kwa as square is Nakseo, Sin-kwa as reverse triangle is Bokhee pal kwoe order shape, Hyul-kwa as acreage phase or oval shape is accord with Moonwang pal kwoe order shape. The reason being classified into Jung, GE, Sin, Hyul-kwa from the shapes of the face is that the face is the place to reflect the whole conditions of the body connected with all of the viscera and meridian system such as semen of plants. Above contents are drawn an inference from the transcription of Jisan's lectures and his writings. And it is considered that insufficient parts of the Jisan's theory must be demonstrated continuously.

Relationship of Inhibitory Effects of Dichroa febrifuga and $IKK{\gamma}$ on the Activation of $NF-{\kappa}B$ (상산의 $NF-{\kappa}B$ 활성억제작용과 $IKK{\gamma}$의 연관성 연구)

  • Activation of $NF-{\kappa}B$ is known to be a trigger of various cellular disorders including inflammatory and autoimmune diseases such as rheumatoid arthritis. Numerous approaches are ongoing within laboratories to identify potential therapeutic agents which inhibit the $NF-{\kappa}B$ activation. In this study, we have tested the inhibitory effects of five traditional medicines on the activation of $NF-{\kappa}B$ by NIK. Among three medicines which exhibited inhibitory effect on the expression of $NF-{\kappa}B$ repoter plasmid, we investigated further the inhibitory mechanism of Dichroa febrifuga in connection with IKKY activity. Wild type $IKK{\gamma}$ inhibited the $NF-{\kappa}B$ activation by NIK but the C-terminal deletion mutant of IKKY did not show the inhibitory effect, indicating that the C-terminal leucine zipper domain of $NF-{\kappa}B$ is important for the inhibition of $NF-{\kappa}B$ activation. The water extract of Dichroa febrifuga(DFE) also strongly inhibited the $NF-{\kappa}B$ activation by NIK. The inhibitory activity of DFE appeared to be independent of the expression of $IKK{\gamma}$, suggesting that the pathways of inhibition by Dichroa febrifuga and $IKK{\gamma}$ are different. Our results suggest that Dichroa febrifuga can be used as a medicine for inhibition of the $NF-{\kappa}B$ activation in a wide range of cells without relation to the expression of $IKK{\gamma}$.

Effects of Electroacupucture on NMDA Receptor-dependent Spinal ERK MAPK Expression in CFA-induced Pain Model (전침에 의한 CFA유발 통증모델의 NMDA 수용체 의존적 ERK MAPK 발현 변화)

  • The present study aims to investigate a possible mechanism of electroacupuncture (EA) in the spinal dorsal horn that may underlie N-methyl-D-aspartate (NMDA) receptor-associated extracellular signal-regulated kinase (ERK) mitogen-activated protein kinase (MAPK) signaling pathways. The hot plate latency of the ipsilateral hindpaw of EA-treated rats was significantly decreased compared with complete Freund's adjuvant (CFA)-injected ones. The expressions of NR1 and NR2B subuint mRNA of NMDA receptor in the whole L4-5 segments are decreased by CFA treatment, but NR2B subunit was significantly recovered by EA treatment. When we detected the expression of ERK, there were no significant difference between normal and CFA-treated rats with EA or NMDA receptor antagonist MK801. But phosphorylated ERK expressions were markedly induced by CFA, but these inductions were significantly modulated by EA treatment. Although hosphorylation of ERK was also arrested by MK801, these inductions of CFA-injected rats was markedly inhibited only by co-treatment with EA and MK801. Phosphorylated cAMP response element-binding protein (CREB), ERK-related transcriptional factor, showed a significant increase in CFA-treated rats and this increase was slightly inhibited by EA and MK801 treatments. But immunoreaction for phosphorylated CREB were significantly increased by CFA treatment in the superficial laminae of the dorsal horn and these inductions were significantly arrested by co-treatment of EA and MK801. Consequently, the hyperalgesia induced by CFA are associated NMDA receptor and EA and MK801 may showed anti-hyperalgesia via same mechanism for inhibition of ERK and CREB phosphorylation in the dorsal horn.

The effect of antipsychotics and antidepressants on the TREK2 channel (TREK2 채널에 대한 항정신성약물 및 항우울제의 효과)

  • Fluoxetine and tianeptine are commonly used as antidepressants (AD), and haloperidol and risperidone are widely used as antipsychotic drugs (APD), and it modulates various ion channels. TREK2 channel subfamily is very similar to physiological properties of TREK1 channel which can play important roles in the pathophysiology of mental disorders such as depression and schizophrenia, therefore, the pharmacological effect of psychiatric and depression drug on TREK2 channel may be similar to those of TREK1. Using the excised inside-out patch-clamp technique, we have examined the effects of APD and AD on cloned TREK2 channel expressed CHO cells. Fluoxetine (selective serotonin release inhibitor, SSRI) inhibited the TREK2 channel in a concentration-dependent manner ($IC_{50}$ $13{\mu}M$), whereas selective serotonin reuptake enhancer (SSRE) tianeptine increased without reducing the TREK2 channel activity. Haloperidol also inhibited the TREK2 channel in a concentration-dependent manner ($IC_{50}$ $44{\mu}M$), whereas even higher concentration ($100{\mu}M$) of risperidone did not completely inhibit on the activity. This study showed that TREK2 channel was preferentially blocked by fluoxetine rather than tianeptine, and inhibited by haloperidol rather than risperidone, suggesting differential effect of TREK2 channels by APD and AD may contribute to some mechanism of adverse side effects.

SREBP as a Global Regulator for Lipid Metabolism (지질대사 조절에서 SREBP의 역할)

  • Sterol regulatory-element binding proteins (SREBPs) are a family of transcription factors that regulate lipid homeostasis and metabolism by controlling the expression of enzymes required for endogenous cholesterol, fatty acid (FA), triacylglycerol, and phospholipid synthesis. The three SREBPs are encoded by two different genes. The SREBP1 gene gives rise to SREBP-1a and SREBP-1c, which are derived from utilization of alternate promoters that yield transcripts in which distinct first exons are spliced to a common second exon. SREBP-2 is derived from a separate gene. Additionally, SREBPs are implicated in numerous pathogenic processes, such as endoplasmic reticulum stress, inflammation, autophagy, and apoptosis. They also contribute to obesity, dyslipidemia, diabetes mellitus, and nonalcoholic fatty liver diseases. Genome-wide analyses have revealed that these versatile transcription factors act as important nodes of biological signaling networks. Changes in cell metabolism and growth are reciprocally linked through SREBPs. Anabolic and growth signaling pathways branch off and connect to multiple steps of SREBP activation and form complex regulatory networks. SREBPs are activated through the PI3K-Akt-mTOR pathway in these processes, but the molecular mechanism remains to be understood. This review aims to provide a comprehensive understanding of the role of SREBPs in physiology and pathophysiology at the cell, organ, and organism levels.

Inhibitory Effect of Prunus mune Extracts on Physiological Function of Food Spoilage microorganisms (매실추출물이 변패미생물의 생리기능에 미치는 영향)

  • Moderate consumption of maesil(Prunus mune) was associated with pharmaceutical and physiological effects on human health. The object of this study was to determine the inhibitory effects of Prunus mune extracts(PME) on food spoilage microorganisms. PME was found to have an antibacterial effect on Colletotrichum fragariae. The hydrophilic fractions of PME showed more effective inhibition than did the hydrophobic fractions. In addition, the hydrophilic fractions of PME seemed to inhibit(12-40%) metabolic enzymes related to energy production, including glucose-6-phosphate dehydrogenase, succinate dehydrogenase, malate dehydrogenase, and hexokinase. Our data suggest that hydrophilic components of PME might control food spoilage microorganisms because of suppression of membrane enzymatic function.
