Carpal Tunnel Syndrome Caused by Pseudogout (가성통풍으로 인한 수근관 증후군)

  • Carpal tunnel syndrome (CTS) caused by pseudogout is an uncommon disease. The authors report a 65-year-old female who complained of sudden pain and neurological symptoms on her left hand. Surgical decompression was performed. In the histologic study, a calcium pyrophosphate dihydrate crystal deposit was confirmed. Her pain and neurological symptoms were relieved. Because CTS caused by pseudogout is rare, it is difficult to differentiate it from other diseases. This paper reports an uncommon case of CTS caused by pseudogout.

  • Myxoid liposarcoma is an extremely rare malignant breast tumor. We report the case of a 44-year-old woman who had myxoid liposarcoma of the breast with a history of phyllodes tumor and describe the imaging findings on US, mammography, and MRI. Before surgery, the mass was considered to be a recurrent phyllodes tumor. However, using US, we retrospectively identified some differences between myxoid liposarcomas and phyllodes tumors.

The Diagnostic Usefulness of Endoscopic Findings and Detection Rates of CLO and HpKit Test for Helicobacter pylori Infection in Children (소아의 Helicobacter pylori 감염에 있어서 위내시경 소견의 진단적 의의와 CLO 및 HpKit 검사의 검출률)

  • Purpose : We performed this study to evaluate the diagnostic usefulness of endoscopic finding of nodular gastritis, CLO and HpKit test for H. pylori infection in children. Methods : Gastroduodenal endoscopy and mucosal biopsy were performed on 212 children who visited our hospital between Jul. 1999 and May 2000 due to abdominal pain. We performed CLO and HpKit test for H. pylori with the time interval of 15, 30 minutes, 1, 2, 3, 24, 48, 72, 96, 120 and 144 hours. Histological examination of H. pylori was made by H-E or Alcian yellow stain with biopsy specimens. Sensitivity, specificity, positive predictive and negative predictive value of nodular gastritis, CLO and HpKit test were calculated from the analysis of above data. Results : Sensitivity and specificity of 3 hour-CLO test was 68.4% and 100% respectively. Sensitivity and specificity of 3 hour-HpKit test was 65.8% and 100% respectively. No significant difference in sensitivity and specificity was found between in 3 hour-CLO and HpKit test(P>0.05). Sensitivity of CLO test increased as time lapsed, but corresponding specificity did not decrease as time lapsed(sensitivity and specificity at 144 hours : 89.5% and 94.8% respectively). However, sensitivity of HpKit test increased as time lapsed, but specificity markedly decreased. Sensitivity and specificity of the nodular gastritis was 78.9% and 93.7% respectively. Conclusion : Both CLO and HpKit test have relatively low sensitivity and specificity for the detection of H. pylori in 3 hours of testing in children. The endoscopic finding of nodular gastritis is another good standard in the diagnosis of H. pylori infection in children.

Comparison Between Antimicrobial Susceptibility Test and mecA PCR Method for Reading of Methicillin-Resistant Staphylococcus aureus (메티실린 내성 황색포도알균 판독에 있어 항균제 감수성 검사와 mecA PCR법의 비교)

  • Methicillin-resistant Staphylococcus aureus (MRSA) is one of major pathogen causing hospital infection and several diseases such as purulent infection, bacteremia. The isolation ratio of MRSA is gradually increased up to 80% in the hospital, which makes a limitation for treatment of antibiotics because the isolated MRSA show resistance to methicillin as well as other antibiotics. This study proposes that mecA detecting methods which are not commonly used because of cost in the hospital is a more accurate method than Susceptibility Testing to detect a MRSA. We compared Staphylococcus aureus ATCC 29213 as a negative control and 20 MRSA strains isolated from patients by these two methods. We amplified mecA gene by polymerase chain reaction (PCR) and confirmed the PCR products by sequencing. All of the MRSA showed oxacillin and cefoxitin resistance whereas 85% (16/19) of the strains had mecA wildtype. These results suggest that some of the MRSA are mecA mutants therefore mecA genotyping reinforces the MRSA detection by antibiotic susceptibility test.

The Solution to the Limitation of the Conventional Digital X-ray System and Its Feasibility Test (디지털 X선 촬영장치의 한계 검사 개선 및 유용성 평가)

  • In this study, we conducted a survey on the limited examinations of the DR system to the 30 radiologic technologists engaging in the clinical sites and university hospitals in Daejeon metropolitan city from Oct. 15, 2012 to Nov. 15, 2012. Based on the survey results, we propose the conceptual design of a supplementary table for the conventional DR system to extend the examination capability and overcome the limitation of DR system. And, we evaluated the convenience and usefulness of the proposed table by applying it to five subjects. Ten experts were involved to evaluate the clinical usefulness of the obtained images. In this study, the experimental results show the feasibility to overcome the limited examination of the conventional DR system, and the proposed supplementary table may be beneficial to patients and hospitals.

  • According to the development of information processing technology and computer hardware, PACS systems have been installed in many hospitals. They can increase the efficiency and the convenience remarkably for handling medical images using digitalized data. After we compare the generation images with other cases, we can read the images correctly and decide how to treat the patients. If the results, included test method and specialist's opinion, are represented dynamically on homepage in hospital. then visitors can get their experience in directly and understand the field of examination and the area of medical treatment. In this thesis, we display the effective images such as MR of the abnormal cases according to parts and diseases, the movie and still images such as Angio image, the other multimedia materials such as the sound and text of doctor's opinions, in SMIL based on XML, concerning the problem of concurrency.

Frequency of Candida Strains Isolated from Candidiasis Patients at A Tertiary Hospital over the Last 10 Years (최근 10년 동안 일개 상급종합병원의 칸디다혈증 환자에서 분리된 칸디다 균종의 빈도)

  • Candidemia is a major cause of nosocomial infections resulting in increased morbidity and mortality. It remains a serious risk in inpatients and increases medical treatment costs. From 2009 to 2018, Candida strains (3,533) isolated from blood culture tests at the S Hospital were analyzed according to the period, year, sex, age, ward, etc. During the entire period, 54,739 of 717,996 blood culture tests showed a positive rate (7.6%) and the Candida isolation rate was 3,533 (6.4%) out of 1,036 patients. Among the Candida isolates, C. albicans was most common (33.8%), followed by C. tropicalis (28.6%), C. glabrata (19.8%), C. parapsilosis (7.8%), and C. krusei (4.0%). In early (2009~2013)/late (2014~2018) isolation, C. tropicalis decreased by 3.8% and C. glabrata increased by 3.4%. After 50 years of age, the higher the separation frequency. C. parapsilosis (31.3%) in 1~10s, C. tropicalis (30.3%) and C. glabrata (27.6%) in 41~50s, and C. tropicalis (28.6%) in 80s are relatively frequent. has been separated C. krusei was isolated in a relatively high proportion from females (60.9%). Therefore, a systematic and continuous nosocomial infection control system should be established for appropriate treatment as per antifungal treatment guidelines. The system should continuously monitor the distribution of Candida species and provide rapid identification results.

  • Biliary atresia is an unknown etiology of extrahepatic bile duct obstruction with a 'fibrous ductal remnant,' which represents the obliterated ductal remnant in the porta hepatis. The sonographic 'triangular cord' (TC) sign has been reported to indicate a fibrous ductal remnant in the porta hepatis. In this review, we discuss the correlations among surgicopathological and sonographic findings of the porta hepatis and the definition, objective criteria, diagnostic accuracy, and differential diagnosis of the TC sign in biliary atresia.

  • Maple syrup urine disease (MSUD, OMIM#248600) is a rare and autosomal recessively-inherited metabolic disorder that is caused by mutations in the branched-chain ${\alpha}$-ketoacid dehydrogenase (BCKDH) genes. It prevents the normal breakdown of branched-chain amino acids (BCAAs), such as leucine, isoleucine, and valine, and leads to poor feeding, lethargy, abnormal movements, seizure, and death if untreated. Here, we report the case of a Korean newborn of biochemically- and genetically-confirmed MSUD manifesting lethargy and central apnea, the acute state of which was successfully treated. The molecular genetic investigation revealed two novel heterozygous mutations (p.Ala32Phefs*48 and p.Val 130Phe) in BCKDHB, and both parents were confirmed as carriers. We emphasize the importance of early diagnosis and prompt introduction of specific treatment for MSUD in life saving and prognosis.
