Clinical Studies of Acupuncture Treatment for Alzheimer's Disease Using Neuroimaging Method: A Review of Literature (알츠하이머병의 신경영상 기법을 이용한 침치료 임상연구: 문헌고찰)

  • The purpose of this article was to investigate the current state of studies on clinical trials of acupuncture treatment for Alzheimer's disease using neuroimaging method. We searched for clinical trials of acupuncture treatment for Alzheimer's disease(AD) and mild cognitive impairment(MCI) using neuroimaging method in the MEDLINE (Pubmed) database on March 18, 2020. Once the online search was finished, studies were selected manually by the inclusion criteria. Finally, we analyzed the characteristics of selected articles and reviewed the neural substrates of acupuncture treatment in AD. Total ten studies were included in this study. The most frequently applied modality for AD was functional MRI. The most frequently selected acupoints for AD were KI3, LR3 and LI4. One of studies showed that acupuncture treatment could improve the symptoms of MCI. Through the analysis, we demonstrated that neuroimaging method could capture the neural substrates associated with AD. Moreover, acupuncture may induce differential response according to the disease status. Finally, real acupuncture could produce more extensive activation/deactivation than sham acupuncture. We hope that neuroimaging method can contribute to the clinical research of acupuncture treatment for AD through large-scale RCT and diverse imaging modality.

Osteochondroma at the Cervicothoracic Junction (경흉추 이행부에 발생한 골연골종)

  • Osteochondroma is a common benign bone tumor, but relatively rare in the spine. Spinal osteochondroma originates mainly from the posterior column and sometimes invades the spinal canal causing cord compression and myelopathy. A 36-year-old man was admitted to the authors' institution with a two-year history of neck pain. A radiology examination revealed osteochondroma, arising from the left superior articular process of the T1 vertebra with significant central canal narrowing. Most cases of the spinal osteochondroma originate in the cervical vertebra, but there are few reports of an origin of the cervicothoracic junction. This paper reports a case of osteochondroma at the cervicothoracic junction with a review of the relevant literature.

  • Functional brain imaging techniques have been used to diagnose psychiatric disorders such as dementia, depression, and autism. Recently, these techniques have also been actively used to study hearing loss. The present study reviewed the application of the functional brain imaging techniques in auditory research, especially those focusing on hearing loss, over the past decade. EEG, fMRI, fNIRS, MEG, and PET have been utilized in auditory research, and the number of research studies using these techniques has been increasing. In particular, fMRI and EEG were the most frequently used technique in auditory research. EEG studies mostly used event-related designs to analyze the direct relationship between stimulus and the related response, and in fMRI studies, resting-state functional connectivity and block designs were utilized to analyze alterations in brain functionality in hearing-related areas. In terms of age, while studies involving children mainly focused on congenital and pre- and post-lingual hearing loss to analyze developmental characteristics with and without hearing loss, those involving adults focused on age-related hearing loss to investigate changes in the characteristics of the brain based on the presence of hearing loss and the use of a hearing device. Overall, ranging from EEG to PET, various functional brain imaging techniques have been used in auditory research, but it is difficult to perform a comprehensive analysis due to the lack of consistency in experimental designs, analysis methods, and participant characteristics. Thus, it is necessary to develop standardized research protocols to obtain high-quality clinical and research evidence.

A Narrative Literature Review on the Effectiveness of Qigong Therapy for Cognitive Impairment: Focusing on the Neuroimaging Studies (인지장애에 대한 기공요법의 유효성에 관한 서술적 문헌 고찰: 신경영상 연구를 중심으로)

  • The purpose of this study was to investigate the current state of clinical studies of qigong therapies for cognitive impairment using neuroimaging method. We searched for clinical trials of qigong therapies for cognitive impairment using neuroimaging method in the MEDLINE (Pubmed) and CNKI on October 18, 2024. Once the online search was finished, studies were selected manually by the inclusion criteria. Finally, we analyzed the characteristics of selected articles and reviewed the neural substrates of qigong therapies in cognitive impairment. Total six studies were included in this study. The most frequently utilized modality was functional MRI. The qigong therapies applied in the study were Tai chi and Baduanjin. Looking at the selected studies, there were many cases that the control group included controls who did not perform any interventions other than health education and an active control group that performed brisk walking. It was confirmed that Qigong therapy can improve cognitive function more broadly, including MoCA scores, compared to the general control group or walking group. Moreover, several studies revealed that qigong therapies could regulate brain activation centered on medial temporal lobe, frontal areas, parietal lobes, attention-related brain networks, and brainstem including neurotransmitter-related nuclei. Through the analysis, we demonstrated that neuroimaging techniques could explore the neural mechanisms of qigong therapies on cognitive impairment. We hope that this perspective on the effectiveness of qigong therapy using neuroimaging methods would be helpful in performing future clinical researches on the mechanisms of Alzheimer's disease in the traditional Korean medicine.

  • Lung cancer is the leading cause of cancer death in Republic of Korea. After their initial diagnosis, only 10-20% of patients with advanced non-small cell lung cancer (NSCLC) survive for 5 years of longer. Given enormous advances in therapeutics such as novel targeted therapies and immunotherapies, survival rates are improving for advanced patients with NSCLC; 5-year survival rates range from 15% to 50%, contingent upon the biomarker. Detection of the specific molecular alteration as biomarker is thus crucial for identifying subgroups of NSCLC that contain therpapeutically targetable oncogenic drivers. This review examines the process of diagnosing lung adenocarcinoma with dominant biomarkers in order to customize treatment with appropriate targeted therapy.

Diagnostic Imaging of Liver Cirrhosis in a Shih-Tzu Dog (시츄견에서 발생한 간경화의 영상 진단)

  • A 5-year-old, intact female Shih-Tzu dog was presented with 1 year history of icterus, ascites and anorexia. The serum biochemistry revealed elevated liver enzyme levels. Microhepatica and decreased serosal detail were detected in abdominal radiography. Abdominal ultrasonographic findings included irregular liver margins, multifocal hypoechoic nodules in the liver parenchyma, and ascites. Computed tomography (CT) showed multifocal hypodense nodules with ring-like contrast enhancement. Cytologic and histopathologic examination by liver core biopsy revealed fibrosis. Cirrhosis was diagnosed based on above results. This report focuses on the imaging characteristics of ultrasonography and CT for liver cirrhosis in a dog.

Arthroscopic Excisional Debridement of Cyst-like lesion in juxta-articular Knee Joint (관절경을 이용한 슬관절주위 낭종유사 병변의 절제제거술)

  • Purpose : A cyst-like lesion within and around the knee joint is very rare, with very few articles available in the literature. The aim of this study is to evaluate effectiveness of arthroscopic treatment of cyst-like lesion within and around the knee joint which reported rarely. Materials and Methods : We are reporting 14 cases of cyst-like lesion around the knee joint. Our cases include 3 ganglion cyst in infrapatellar fat pad, 1 fibroma, 1 giant cell tumor and 1 epidermoid cyst which have not yet been reported. The diagnosis of cyst-like lesion in Juxta-articular knee Joint was made only by MRI study and we confirmed pathology. Male was 9, female was 5 cases, average age was 24(11-43)year old. Follow up was average 45(12months-8years)months. Minimum follow up was 12 months. Results : All the patients were treated arthroscopic excisional debridements successfully. All the cases were excellent result in last follow up by functional criteria in Lysholm knee score average 98.9(95-100). All examimation was normal except 3 cases in last follow up. But all patients satisfied in arthroscopic treatment. Conclusion : Arthroscopic excisional debridement of the cyst-1ike lesion in juxta-arthcular knee joint is excellent method, but long term follow up needed in the future.

Concentration and Distribution of Tumor Associated Antigens, TAG-72 and CEA, in Stomach Cancer (위암조직에서 종양관련항원 TAG-72와 CEA의 농도 및 분포)

  • 악성종양의 진단 및 치료에 있어서 특정 종양에 대한 항체를 이용하는 연구가 활발히 진행되고 있다. 단세포군항체를 이용한 방사면역신티그라피로 암의 조기 진단 및 영상화가 가능하고 나아가 방사면역치료는 암의 선택적 치료에 도움이 될 수 있다. 위암은 우리나라에서 가장 흔한 악성종양으로 방사면역신티그라피와 방사면역치료법이 새로운 방법으로 모색되고 있다. 이러한 진단 및 치료법의 성공여부를 결정하는 중요한 인자의 하나가 종양조직내에서 종양관련 항원의 농도와 분포이다. 따라서 본 연구에서는 단세포군 항체를 이용한 방사면역학적 방법의 임상 이용을 위한 기초 연구의 일환으로 in vitro quantitative autoradiography를 이용하여 종양 관련항원인 TAG-72와 CEA의 위암조직내 농도 및 분포를 측정하였다. 33예의 위암조직에서 얻은 동결절편을 $1.3\sim83.3$ nmol/liter의 $^{125}I$ 표지 항 TAG-72 단세포군 항체 B-72.3과 항 CEA 단세포군 항체 CEA-79로 반응시킨 후 이 표본들의 자가방사법 디지탈 영상을 H & E 염색과 immunoperoxidase염색 표본과 비교하였으며, 특정 단세포군 항체의 결합에 대한 컴퓨터 분석으로 조직내 항원의 농도와 분포를 측정하였다. TAG-72는 25예(75.7%)의 조직에서 검출되었으며 그 농도는 $8.4\sim525.3$ pmol/gram이었고, CEA는 32예 (96.9%)에서 검출되었으며 그 농도는 $8.8\sim592.9$ pmol/gram 이었다. CEA의 위암 조직내 발현농도는 중앙치가 101.7 pmol/gram 으로 TAG-72의 중앙치인 27.9 pmol/gram 보다 높았다. TAG-72의 조직내 분포는 41.4%에서 병변 부위의 암세포 분포와 일치하였고, CEA의 분포는 병변 부위의 80.5%에서 암세포와 일치하는 소견을 나타내었다. TAG-72의 농도는 점액성 선종(mucinous adenocarcinoma)과 점액함유 선종(mucin containing adenocarcinoma)에서 다른 선종보다 더 높았다. CEA의 농도는 위암의 병리학적 종류에 따른 유의한 차이가 없었다. 이상의 결과로 위암조직에서 TAG-72와 CEA항원이 다양하게 발현됨을 알 수 있었고 CEA는 TAG-72 보다 더 빈번하게 균일한 분포로 발현하는 것으로 나타났다.

Diagnostic Imaging of Lipoma in the Retroperitoneum and Pelvic Cavity in a Dog (개의 후복강과 골반강에 발생한 지방종의 진단영상 증례)

  • An 11-year-old, spayed female, weighing 10 kg miniature Schnauzer was admitted for abdominal distension, anorexia, pollakiuria, and constipation. A large, homogeneous fat opacity mass was identified in the retroperitoneum and pelvic cavity on the abdominal survey radiography. The mass displaced the descending colon peripherally and ventrally, and the urinary bladder ventrally. On the abdominal ultrasonographic examination, the mass was uniformly hyperechoic with a coarse internal echo texture and had outer hyperechoic capsule, and it showed homogeneously same attenuation (-180 ~ -110 HU) as adjacent fat on the computed tomography. There was no evidence of invasion into the surrounding structures or organs. Cytological findings from fine needle aspirates were numerous sheets and clusters of adipocytes. The dog showed complete resolution of clinical signs after surgical resection of the mass. The mass was confirmed as simple lipoma through histopathological examination.

Seizure Propagation on Ictal Brain SPECT : A Pitfall in the Localization of the Seizure Focus (발작기 뇌혈류 스캔에서의 간질 확산에 관한 연구 : 간질 병소 국소화의 오류)

  • 본 연구는 간질 병소의 국소화에 있어 발작기 및 발작간 뇌스캔의 정확도를 알아보고 발작기 뇌 스캔에서 나타나는 간질 확산이 정확한 간질 병소를 국소화 하는데 어떠한 영향을 미치는가에 대하여 알아보고자 하였다. 15명의 복잡 부분 발작 환자를 대상으로 하였으며, 간질 병소의 최종적인 국소화는 두피 및 발작 뇌파, 피질 뇌파, 자기 공명 영상, 임상양상 및 병리 소견을 종합한 근거로 하였다. 발작기 뇌스캔은 뇌파상 발작 중 또는 환자가 aura를 호소할 때 Tc-99m HMPAO 20mCi(740 MBq)를 정맥주입후 시행하였으며 발작간 뇌스캔은 발작기 뇌스캔 후 3일 이내 임상적으로 발작 증상이 없는 기간에 시행하였다. 간질 병소는 우측 측두엽이 8예, 좌측 측두엽이 6예, 측두엽 이외의 기원이 1예 였다. 발작기 뇌스캔상, 모두 11예(73.3 %)에서 단발성 또는 다발성 섭취 증가가 간질 병소 및 간질확산 부위에서 관찰되었으며, 간질 병소에만 국한된 섭취 증가는 4예(26.7%)에 불과 하였다. 발작간 뇌스캔에는 모두 11예(73.3 %)에서 간질 병소에만 섭취가 감소되었다. 자기 공명 영상에서는 8예(53.3 %)에서 hippocampal sclerosis를 포함한 간질 병소가 확인되었다. 본 연구로 복잡 부분 발작 환자에서, 간질 확산이 발작기 뇌 스캔 중 자주 관찰됨을 알 수 있었으며, 이러한 간질 확산에 따른 다발성 방사능 섭취가 발작기 뇌스캔상, 간질 병소의 국소화에있어 한계가 있음을 결론 내릴 수 있었다.
