Bilateral chylothorax with diffuse pulmonary Iymphangiomatosis (미만성 폐림프관종증과 동반된 양측성유미흉 -수술치험 1례 보고-)

  • Chylothorax is denned by extravasation of the milky fluid to pleural cavity from the thoracic duct or it's main branches due to operative trauma, congenital lesions, diagnostic procedures, tumor, etc. Another rare cause is diffuse pulmonary Iymphangiomatosis which is uncommon and not well charact rized. We experienced a case of the bilateral chylothorax caused by the diffuse pulmonary Iymphangiomatosis. The patient was at years old girl with symptoms of coughing and febrile sensation, and the chest radiographs showed bilateral pleural effusion and interstitial infiltrates. The laboratory data of the pleural effusion was identified as chile. Uncontrollable with closed tube thoracostomy, division of tHe thoracic duct and biopsy were decided. Biopsy showed anastomosing endothelial lined spaces along the pulmonary Iymphatic routes especially in pleural and interlobar septum, and smooth muscle at the proliferative interstitium of the Iymphatic duct was observed. Postoperatively, chylothorax was controlled with several trial of chemical pleurodesis. Af'leer discharge from the hospital, she was well for ten months follow up.

Pulmonary Giant Cell Carcinoma (폐에 발생한 거세포암종 -1 Case-)

  • The pulmonary giant cell carcinoma is classified as a variant of a large cell carcinoma and is diagnosed by the minimum component of 10% huge, pleomorphic and multinucleated giant tumor cell and emperipolesis of the neutrophils into the tumor cells. This tumor is characterized by local recurrences and early metastasis with extremely short patient survival. However, there are some reports that state that the survival time was extended by the operative resection and postoperative adjuvant chemotherapy and radiotherapy. A 46-year old male was admitted with complaint of hemoptysis for 2 months. Through chest X-ray and chest CT, a 5cm sized mass was found in the apical segment of the right upper lobe. During the preoperative evaluation, stenotic lesion in the left anterior descending coronary artery was found and treated by percutaneous transarterial coronary angioplasty. Four weeks later, right upper lobectomy was performed and the mass was proven to be a giant cell carcinoma. The patient received adjuvant chemotherapy and radiotherapy.

Immunohistochemical Expression of Galectin-3, Cytokeratin 19 and HBME-1 in Papillary Microcarcinoma of the Thyroid Gland (갑상선 미세 유두암종에서 Galectin-3, Cytokeratin 19와 HBME-1의 발현)

  • With the wide use of ultrasonography and fine needle aspiration of the thyroid gland, the incidence of papillary microcarcinoma of the thyroid gland is rapidly increasing nowadays. To improve the diagnostic accuracy of histopathologic findings of papillary thyroid carcinoma, various molecular markers have been used recently. We analysed the expression of galectin-3, cytokeratin 19 and HBME-1, using immunohistochemical technique in 37 cases of papillary microcarcinoma of the thyroid gland to evaluate the diagnostic value of these molecular markers. Immunohistochemically, galectin-3 expression was found in 37 cases of papillary microcarcinoma. Its localization was mostly cytoplasmic. Cytokeratin 19 expression was found in 36 cases. It was mostly localized to the cytoplasm and membrane. HBME-1 expression was found in all cases. Its localization was plasma membrane. The expression of these three molecular markers was negative in the adjacent normal thyroid tissue and accompanying benign lesions, although there are scattered foci of incomplete positive staining in cases of Hashimoto's thyroiditis. Our findings suggest that the immunohistochemical staining using antibodies for galectin-3, cytokeratin 19 and HBME-1 is an useful adjunctive method for the histopathological diagnosis of a papillary microcarcinoma of the thyroid gland.

Prostatic Adenocarcinoma in a Schnauzer Dog (슈나우져 개에서 발생한 전립선 선암종)

  • An eight-year-old, castrated male, a Schnauzer dog was presented with dysuria, hematuria and intermittent vomiting. Retrograde positive contrast urethrography was performed under fluoroscope. On the contrast examination, the leakage was observed in the prostatic urethra region and irregular filling defect was also shown from the prostatic urethra and the level of the trigon of the bladder. Surgical treatment was selected because of partial urethral obstruction and dysuria. The metastasis at the time of surgery was suspected, and the owner decided euthanasia. We detected lymph metastasis with immunohistochemical staining for pan-cytokeratins (CK) AE1/AE3, confirming their epithelial origin. This report described typical clinical findings, diagnostic imaging and histopathological examination of a prostatic adenocarcinoma.

Endobronchial Leiomyoma Combined with Pulmonary Sequestration -A case report - (폐격리증을 동반한 기관지 내 평활근종 -1예 보고-)

  • A 42-year-old female was admitted to our hospital complaining of a dyspnea. Chest X-ray showed left atelectasis. A mass was detected in left main bronchus by computed tomography and bronchoscopy. The mass was diagnosed as a endobronchial leiomyoma by biopsy exam. After open thoracotomy and bronchotomy, mass removal was done and middle lobe was ventilated normally. Aberrant arterial supply from descending aorta to left lower lobe of the lung was detected and left lower lobectomy was done. The lower lobe of the left lung was pathologically diagnosed as intralobar pulmonary sequestration. Herein we report a rare coexistent case of endobronchial leiomyoma and intralobar pulmonary sequestration.

Solitary Fibrous Tumor of the Diaphragmatic Pleura (횡격흉막에서 발생한 고립성 섬유성 종양)

  • We present a case of a 47-year-old woman with benign solitary fibrous tumor of the pleura originated in the diaphragm which was discovered incidentally on a chest radiograph. Chest radiograph, sonograph, computed tomographic scan and magnetic resonance image studies proved a well circumscribed and lobulated fibrous tumor of the pleura. During the subsequent right thoracotomy, the tumor was found to be encapsulated and consisted of firm mass. It was connected to the diaphragm with $5{\times}4$cm area by pedicle. Most of the tumor was free of adhesion. It was excised completely together with attached diaphragm. The tumor measured $23.5{\times}13.5{\times}8.0$cm and the pathologic diagnosis was benign solitary fibrous tumor and the attached diaphragm was free of disease.

Multiple Schwannomas in the Peripheral Nerve - 3 Cases Report - (말초신경에 생긴 다발성 신경초종 - 증례 보고 3예 -)

  • This journal reports three cases diagnosed with schwannomatosis in which no clinical symptoms of type 2 neurofibromatosis. The chief complain was pain. In adolescence and adult group, all masses were found. The locations were brachial plexus, popliteal fossa and hand. No hearing impairment, vertigo, tinnitus and visual disturbance was observed in any of the case. Family history was negative. In all cases, there was no evidence of vestibular schwannoma on cranial MRI imaging study. In all cases, Tinel sign was positive. Pathologic diagnosis was positive for schwannoma. Further study and case collection is needed to idenity the clinical manifestation, clinical course and genetic characteristic of schwannomatosis.

Carpal Tunnel Syndrome in Stroke Patients According to the Degree of Spasticity in Median Nerve Cross-Sectional Area and Nerve Conduction Velocity and Comparison of Upper Extremity Function (수근관증후군 뇌졸중 환자에서 경직정도에 따른 정중신경 단면적과 신경전도속도 및 상지기능의 비교)

  • The purpose of this study were carpal tunnel syndrome in stroke patients according to the degree of spasticity in the median nerve cross-sectional area, nerve conduction velocity, and to evaluate differences in upper extremity function. The subjects of this study was in adult patients with stroke 42 patients from 21 patients CTS group and 21 patients Non-CTS group were selected. Measurement of median nerve-cross sectional area, nerve conduction velocity, GST, FMAS, CTS-FSS was measured. The study results were each group between the unaffected side and the affected side CTS and Non-CTS group in each grade between groups unaffected side(p<.001), and affected side(p<.001) median nerve-cross sectional area, median motor and sensory nerve onset latency, there was a statistically significant difference. CTS and Non-CTS group between groups in each grade GST(p<.05), FMAS(p<.05), CTS-FSS(p<.001), there was a statistically significant difference. In this study, the carpal tunnel pathokinesiology ever presented by the contents of upper extremity functional training in stroke patients is one of the information that you need to consider when presented.

A Study on Deep Learning Binary Classification of Prostate Pathological Images Using Multiple Image Enhancement Techniques (다양한 이미지 향상 기법을 사용한 전립선 병리영상 딥러닝 이진 분류 연구)

  • Deep learning technology is currently being used and applied in many different fields. Convolution neural network (CNN) is a method of artificial neural networks in deep learning, which is commonly used for analyzing different types of images through classification. In the conventional classification of histopathology images of prostate carcinomas, the rating of cancer is classified by human subjective observation. However, this approach has produced to some misdiagnosing of cancer grading. To solve this problem, CNN based classification method is proposed in this paper, to train the histological images and classify the prostate cancer grading into two classes of the benign and malignant. The CNN architecture used in this paper is based on the VGG models, which is specialized for image classification. However, color normalization was performed based on the contrast enhancement technique, and the normalized images were used for CNN training, to compare the classification results of both original and normalized images. In all cases, accuracy was over 90%, accuracy of the original was 96%, accuracy of other cases was higher, and loss was the lowest with 9%.

THE EXPERIMENTAL STUDY OF EARLY IRRADIATION EFFECTS ON THE TEMPOROMANDIBULAR JOINT IN WHITE RAT (방사선 조사가 백서 악관절에 미치는 조직병리학적 조기변화에 관한 실험적 연구)

  • The purpose of this study was to investigate the early effects of irradiation on the temporomandibular joint in rats. Male rats were singly irradiated with the dose of 5 Gy or 10 Gy to their head and neck region by /sup 60/Co X ray. Experimental animals were sacrificed at each of the following time intervals -1, 2, 3, 5, 7 and 14 days. The specimens were examined with a light microscope, and treated with H & E staining and immuno-histochemical staining. The results were as follows, 1. By light microscopic findings, proliferative and hypertrophic zone were narrowed and hematopoietic cells were few in number at 5 days after irradiation. Repair signs were seen at 7 days after irradiation when decrease in osteoclast, increase in hematopoietic cells and increase of proliferative zone were noted. The 10 Gy irradiated group showed more severe histopathologic change than the 5 Gy group, and their repair was more slow. 2. In the S -100 antibody, positive cells were examined in the glenoid fossa. Positive cells of irradiated group showed more slight decrease in number than the control group. Low radiosensitivity and slow repair was noted in the glenoid foosa. 3. The interarticular disc was high radioresistant, and any histopathologic changes were not seen in disc. 4. Repair was examined clearly with the response to the antibodies. Especially by 5 days after irradiation 5 Gy group showed S-l00 positive cells in hypertrophic zone next to proliferative zone, chondroitin-4-sulfate positive cell in erosive zone next to hypertrophic zone, type-1 collagen positive cell in subchondral bone.
