  • 기존 산업에서 많이 쓰여져오고 있는 저압 플라즈마에 비해 여러가지 장점을 지닌 대기압 플라즈마는 수년 전부터 많은 연구가 되어 왔으며 폭넓은 응용분야에 있어서 활발히 이용되고 있고, 특히 온도가 거의 상온과 비슷하다는 장점으로 대기압 저온 플라즈마는 바이오메디컬 분야에서 활발하게 응용되어지고 있다. 본 연구에서는 대기압 저온 플라즈마 젯 장치를 사용하여 치아 표면에 불소를 도포하고 법랑질 표면의 불소 원소를 검출함으로써 플라즈마가 치아표면 불소도포에 있어서 어떠한 효과가 있는지 정량적으로 비교분석하였다. 또한 대기압 플라즈마 젯 장치의 방전개시전압과 가스유량에 따른 플라즈마젯 길이의 변화 및 OES(Optical Emission Spectroscopy) 장치를 사용하여 플라즈마에 대한 광학적 진단을 진행하였다. 치아표면에서 검출된 불소량은 플라즈마를 사용했을때가 그렇지 않을때에 비해 더 높게 관찰 되었다.

Efficiency of ceramic bracket debonding with the Er:YAG laser (세라믹 브라켓의 제거 시 Er:YAG 레이저의 효능)

  • Objective: The aim of this study was to find out whether Er:YAG laser can aid in debonding ceramic brackets, and to see what kind of method will be the most appropriate for debonding. Methods: One hundred and ninety teeth, monocrystalline brackets ($MISO^{TM}$, HT, Ansan-Si, Korea), polycrystalline brackets ($Transcend^{TM}$ series 6000, 3M Untek, Monrovia, CA, USA) and the KEY Laser3 (KavoDental, Biberach, Germany) were used. Experimental groups were classified according to the type of ceramic brackets, and the amount of laser energy (0, 140, 300, 450, 600 mJ). After applying laser on the bracket at two points at 1 pulse each, the shear bond strength was measured. The effect of heat caused by laser was measured at the enamel beneath the bracket and pulp chamber. After measuring the shear bond strength, adhesive residue was evaluated and enamel surface was investigated using SEM. Results: All ceramic bracket groups showed a significant decrease in shear bond strength as the laser energy increased. The greatest average temperature change was $3.78^{\circ}C$ on the enamel beneath the bracket and $0.9^{\circ}C$ on the pulp chamber. Through SEM, crater shape holes caused by the laser was seen on the enamel and adhesive surfaces. Conclusions: If laser is applied on ceramic brackets for debonding, 300 - 450 mJ of laser energy will be safe and efficient for monocrystalline brackets ($MISO^{TM}$), and about 450 mJ for polycrystalline brackets ($Transcend^{TM}$ series 6000).

THE CHANGE OF THE CONFIGURATION OF HYDROXYAPATITE CRYSTALS IN ENAMEL BY CHANGES OF PH AND DEGREE OF SATURATION OF LACTIC ACID BUFFER SOLUTION (유산 완충용액의 pH 및 포화도 변화에 따른 법랑질 내 수산화인회석 결정 형태의 변화)

  • Since it was reported that incipient enamel caries can be recovered, previous studies have quantitatively evaluated that enamel artificial caries have been, remineralized with fluoride showing simultaneously the increase of width of surface layer and the decrease of width of the body of legion. There is, however, little report which showed that remineralization could occur without fluoride. In addition, the observations on the change of hydroxyapatite crystals also have been scarcely seen. In this study, enamel caries in intact premolars or molars was induced by using lactic acidulated buffering solutions over 2 days. Then decalcified specimens were remineralized by seven groups of solutions using different degree of saturation(0.212, 0.239, 0.301, 0.355) and different pH(5.0, 5.5, 6.0) over 10 days. A qualitative comparison to changes of hydroxyapatite crystals after fracturing teeth was made under SEM(scanning electron microscopy) and AFM(atomic force microscopy). The results were as follows: 1. The size of hydroxyapatite crystals in demineralized area was smaller than the normal ones. While the space among crystals was expanded, it was observed that crystals are arranged irregularly. 2. In remineralized enamel area, the enlarged crystals with various shape were observed when the crystals were fused and new small crystals in intercrystalline spaces were deposited. 3. Group 3 and 4 with higher degree of saturation at same pH showed the formation of large clusters by aggregation of small crystals from the surface layer to the lesion body than group 1 and 2 with relatively low degree of saturation at same pH did. Especially group 4 showed complete remineralization to the body of lesions. Group 5 and 6 with lower pH at similar degree of saturation showed remineralization to the body of lesions while group 7 didn't show it. Unlike in Group 3 and 4, Group 5 and 6 showed that each particle was densely distributed with clear appearance rather than crystals form clusters together.

COMPARATIVE STUDY ON THE RATE OF DENTAL ENAMEL DEMINERALIZATION USING A QLF (Quantitative Light-induced Fluorescence를 이용한 법랑질 탈회 속도에 관한 비교 연구)

  • The objective of this study was to compare the rate of in vitro demineralization of bovine permanent (BP), human deciduous (HD) and human permanent (HP) enamel. Twenty aye flattened and polished enamel samples for each group (BP, HD, HP) were immersed in a demineralizing solution (0.1 mol/L lactic acid, 0.2% Carbopol 907, and 50% saturated hydroxyapatite) for 1, 2, 4 or 8 days. All 25 samples from each group were subjected to Quantitative light induced fluorescence analysis (QLF) and 5 samples from each group were randomly selected for Transverse Microradiography analysis (TMR). Integrated mineral loss (IML) and lesion depth (LD) were determined by TMR. The fluorescence radiance (FR) of sound enamel $(FR_S)$, demineralized enamel $(FR_D)$ were determined by QLF and FR ratio $(FR_D/FR_S)$ was calculated. Bovine enamel samples showed significant correlation between FR ratio and lesion depth(p<0.05) and deciduous enamel samples does not showed significant correlation between FR ratio and lesion depth(p>0.05). Permanent enamel samples showed significant correlation between FR ratio and lesion depth(p<0.05) The constant of demineralization time between FR ratio from regression analysis were as follows: bovine enamel was -4.643(p<0.05) deciduous enamel was -5.421(p<0.05) and permanent enamel was -4.435(p<0.05).

STUDY ON THE SHEAR BOND STRENGTH OF ALL-IN-ONE DENTIN BONDING SYSTEM (우치에 대한 All-in-one 상아질 결합제의 전단결합강도 비교 연구)

  • This study was performed to compare the shear bond strength of AQ Bond $Plus^{TM}$ with AQ $Bond^{TM}$ and Single $Bond^{TM}$. Also by observing the fractured interface under scanning electro-microscope, the fracture pattern and the quality of hybrid layer were analyzed. The possibility of clinical application of all-in-one system which has an advantage to reduce chair time for children with difficult behavior pattern was evaluated, The results obtained are as follows ; 1. There was no significant difference between AQ $Bond^{TM}$ and AQ Bond $Plus^{TM}$ in shear bond strength and Single Bond showed the highest bond strength with statistical significant difference (p<0.05). 2. Adhesive fracture pattern was mainly observed in both enamel/dentin in AQ $Bond^{TM}$ and AQ Bond $Plus^{TM}$ group while Single Bond group showed equal numbers for cohesive and adhesive pattern. 3. Under scanning electro-microscope, resin tags observed in AQ $Bond^{TM}$ and AQ Bond $Plus^{TM}$ were very weak and tangled while strong and thick tags were shown with many lateral branches in Single Bond. Careful case selection and accurate clinical application is recommended when using AQ $Bond^{TM}$ and AQ Bond $Plus^{TM}$considering the result showing its weaker strength than Single $Bond^{TM}$.

EFFECT OF ULTRASONIC VIBRATION ON ENAMEL AND DENTIN BOND STRENGTH AND RESIN INFILTRATION IN ALL-IN-ONE ADHESIVE SYSTEMS (All-in-one 접착제에서 초음파진동이 법랑질과 상아질의 결합강도와 레진침투에 미치는 영향)

  • The objective of this study was to apply the vibration technique to reduce the viscosity of bonding adhesives and thereby compare the bond strength and resin penetration in enamel and dentin achieved with those gained using the conventional technique and vibration technique. For enamel specimens, thirty teeth were sectioned mesio-distally. Sectioned two parts were assigned to same adhesive system but different treatment(vibration vs. non-vibration). Each specimen was embedded in 1-inch inner diameter PVC pipe with a acrylic resin. The buccal and lingual surfaces were placed so that the tooth and the embedding medium were at the same level. The samples were subsequently polished silicon carbide abrasive papers. Each adhesive system was applied according to its manufacture's instruction. Vibration groups were additionally vibrated for 15 seconds before curing. For dentin specimen, except removing the coronal part and placing occlusal surface at the mold level, the remaining procedures were same as enamel specimen. Resin composite(Z250. 3M. U.S.A.) was condensed on to the prepared surface in two increments using a mold kit(Ultradent Inc., U.S.A.). Each increments was light cured for 40 seconds. After 24 hours in tap water at room temperature, the specimens were thermocycled for 1000cycles. Shear bond strengths were measured with a universal testing machine(Instron 4465, England). To investigate infiltration patterns of adhesive materials, the surface of specimens was examined with scanning electron microscope. The results were as follows: 1. In enamel the mean values of shear bond strengths in vibration groups(group 2, 4, 6) were greater than those of non-vibration group(group 1, 3, 5). The differences were statistically significant except AQ bond group. 2. In dentin, the mean values of shear bond strengths in vibration groups(group 2, 4, 6) were greater than those of non-vibration groups(group 1, 3, 5). But the differences were not statistically significant except One-Up Bond F group. 3. The vibration group showed more mineral loss in enamel and longer resin tag and greater number of lateral branches in dentin under SEM examination.

Effect of applying adhesive after enamel etching on the shear bond strength of orthodontic brackets using light curing resin cements (광중합형 레진시멘트를 사용한 치열교정용 브라켓 접착 시 접착제 사용 유무가 산 부식한 법랑질의 전단접착강도에 미치는 영향)

  • Purpose: The purpose of this study is to compare the shear bond strength of resin cement for orthodontic brackets without applying an adhesive primer, to the case of applying an adhesive primer. Materials and Methods: The specimens were divided into three experimental groups, Transbond XT, GC Ortho Connect and Orthomite LC, and the enamel surface was divided into two sections, one with 37% phosphoric acid and the other with 37% phosphoric acid and an adhesive primer or universal adhesive. Each of three types of cement was applied to orthodontic bracket, and after bonding, the shear bond strength was measured. Results: Transbond XT and Orthomite LC significantly increased shear bond strength when orthodontic brackets were bonded after applying an adhesive primer and universal adhesive, respectively. Conclusion: It is expected that application of an adhesive primer or universal adhesive after acid etching will improve shear bond strength of orthodontic brackets in Transbond XT and Orthomite LC.

Effects of the Enamel Erosion Caused by Certain Antipyretic and Analgesic Medicines for Children (일부 어린이 해열·진통제의 유치 법랑질 부식효과)

  • This study was conducted to provide basic understanding regarding possible enamel erosion by three kinds of fist-aid antipyretic and analgesic medicines over a period of time, with comparison and analysis of the resulting deciduous teeth surface and microhardness changes. The analysis was performed using energy dispersive X-ray spectroscopy (EDX) and scanning electron microscope (SEM) to examine the surface erosion and changes. The Kruskal-Wallis test show differences in surface erosion and changes after 3, 5 and 8 days of treatment as well as before and after the treatment in each group. According to the results, there was no significant difference in the early deciduous teeth enamel surface microhardness (p>0.01). However there were signigicant changes after 3, 5, and 8 days (p<0.01). Calcim (Ca) and phosphorous (P) analysis using EDX showed significant differences in the enamel characteristics according to each tissue area after 8 days (p<0.05), but there was no significant difference in any of the areas for P content (p>0.05). In the surface observation with the SEM treatment with Children's Tylenol$^{(R)}$ tablet, which has the lowest pH, looked the roughest, followed by Brufen syrup for children and Children's Tylenol$^{(R)}$ suspension. Based on these results, it should be considered that antipyretic and analgesic medicines for children, which have lower pH values, may cause tooth erosion. Hence, it is necessary to give special attention to oral hygiene in young children or infants by brushing their teeth after such drugs are administered.

THE REMINERALIZATION ASPECT OF ENAMEL ACCORDING TO CHANGE OF THE DEGREE OF SATURATION OF THE ORGANIC ACID BUFFERING SOLUTION IN pH 5.5 (pH 5.5에서 유기산 완충용액의 포화도 변화에 따른 법랑질의 재광화 양상)

  • The purpose of this study is to observe and compare the remineralization tendencies of artificial enamel caries lesion by remineralization solutions of different degree of saturations at pH 5.5, using a polarizing microscope and computer programs (Photoshop, Image pro plus, Scion Image, Excel). For this study, 48 sound permanent teeth with no signs of demineralization, cracks, or dental restorations were used. The specimens were immersed in lactic acid demineralization solution for 2 days in order to produce artificial dental caries that consist of surface and subsurface lesions. Each of 9 or 10 specimens was immersed in pH 5.5 lactic acid buffering remineralization solution of four different degrees of saturation (0.507, 0.394, 0.301, and 0.251) for 12 days. After the demineralization and remineralization, images were taken by a polarizing microscope (${\times}100$). The results were obtained by observing images of the specimens, and using computer programs, the density of caries lesions were est mated. While the group with the lowest degree of saturation (0.251) showed total remineralization feature from the surface to the subsurface of the lesion, the group with the highest degree of saturation (0.507) showed demineralization mainly on the surface of the lesion at the constant organic acid concentration 0.01 M and pH 5.5.

Influence of additional etching on shear bond strength of self-etching adhesive system to enamel (부가적인 산부식이 자가산부식 접착제의 법랑질에 대한 전단결합강도에 미치는 영향)

  • Recently, self-etching adhesive system has been introduced to simplify the clinical bonding proce- dures. It is less acidic compared to the phosphoric acid, thus there is doubt whether this system has enough bond strength to enamel. The purpose of this study was to investigate the influence of additional etching on the adhesion of resin composite to enamel. Ninety extracted bovine permanent anterior teeth were used. The labial surfaces of the crown were ground with 600-grit abrasive paper under wet condition. The teeth were randomly divided into six groups of 15 teeth each. Clearfil SE $Bond^{\circledR},\;Adper^{TM}$ Prompt L-Pop and Tyrian $SPE^{TM}$ were used as self-etching primers. Each self-etching primers were applied in both enamel specimens with and without additional etching. For additional etching groups, enamel surface was pretreated with 32% phosphoric acid (UNI-ETCH, Bisco, Inc., Schaumburg, IL. USA). Hybrid resin composite Clearfil AP-X, (Kuraray Co., Ltd., Osaka, Japan) was packed into the mold and light-cured for 40 seconds. Twenty-four hours after storage, the specimens were tested in shear bond strength. The data for each group were subjected to independent t - test at p < 0.01 to make comparisons among the groups. In Clearfil SE $Bond^{\circledR}$, shear bond strength of additional etching group was higher than no additional etching group (p < 0.01). In $Adper^{TM}$ Prompt L-Pop and Tyrian SPE, there were no significant difference between additional etching and non-etching groups (p > 0.01). In conclusion, self-etching adhesive system with weak acid seems to have higher bond strength to enamel with additional etching, while self-etching adhesive system with strong acid seems not.
