Adamantinoma on Tibia - A Case Report - (경골에 발생한 법랑종 - 증례 보고 -)

  • Adamantinoma is a rare malignant tumor and approximately 85% of cases occur in the tibia. In 1985, Moon and Mori reviewed the world literature and identified 195 cases. A 59-year-old woman was presented with nonspecific pain on left leg after trauma. Radiographic finding showed well-circumscribed soap bubble appearance with areas of sclerosis in diaphyseal region of the tibia. Magnetic resonance imaging and biopsy was performed for diagnosis. Histologically, the lesion was compatible with adamantinoma. Wide resection with bone graft was performed. There was no recurrence at 3 years follow-up. We report a case of adimantinoma that arose in diaphyseal region of the tibia with review of literature.

Conservative therapy of extensive unicystic ameloblastoma: a case report (낭종성 법랑아 세포종의 치험례: 증례 보고)

  • Ameloblastoma is a benign neoplasm originating from odontogenic epithelium. It is the most common neoplasm in the jaws and is characterized by aggressive behavior and local invasion. Unicystic ameloblastoma (UA) has a unilocular feature in radiologic examination and a cystic feature histologically. Decompression and marsupialization are conservative method of treatment of large UA. The purpose of decompression and marsupialization are size reduction of the mass, which makes it easy to handle at total enucleation with protection of nerve damage and facial deformity. Here we report successful conservative treatment of extensive UA using decompression and marsupialization with a review of literatures.

  • Ameloblasotma is slowly growing, locally invasive neoplasm with a potentially destructive behavior. The epithelium of ameloblastoma is thought to have an intrinsic growth potential and has been shown to present a higher rate of proliferation as compared to odontogenic cysts with low local recurrence rate. The molecular mechanisms that regulate the cell growth and invasion of ameloblastoma cells are unknown. Bcl-2 protein, which prevent apoptosis, is expressed in immortalized ameloblastoma cell line(AM-1)(Harada et al 1998). Expression of bcl-2 protein occurs in tooth germs, whose epithelial component may act as the histogenic precursor of ameloblastoma. Bax is considered as a main effector of apoptosis. Bax forms homodimers and also heterodimers with bcl-2. p53 tumor supressor gene participates not only in cell proliferation control but also in induction of apoptosis. The objective of the present study was to evaluate the apoptosis related protein expression in odontogenic cyst and ameloblastoma. A total of 10 dentigerous cysts and 16 ameloblastomas were used in the present study. Dentigerous cyst showed negative or slight positive for p53 and bcl-2 but strongly positive for bax, ameloblastoma, on the other hand, strongly positive for p53 and bcl-2 but weekly positive for bax. Bcl-2 was expressed for ameloblastoma mainly in outer layer or whole layer of epithelium and for dentigerous cyst mainly in basal layer. The difference in expression of apoptosis related protein in dentigerous cyst and ameloblastoma might explain the peculiar aggressive growth pattern of ameloblastoma.

  • Ameloblastic fibro-odontoma is a rare benign odontogenic tumor that mainly occurs at mandibular molar area. Radiography usually shows a well-defined radiolucent area containing various amounts of radiopaque material of irregular size and form. In histiologic aspect, Ameloblastic fibro-odontoma composed of connective tissue characteristic of an ameloblastic fibroma and calcined tissue identifying the tumor as a complex odontoma. Thirteen years two months aged boy of this case visited with chief complaint of delayed eruption of mandibular left canine. After enucleation of lesion, biopsy was done and diagnosed by ameloblastic fibro-odontoma. In continous follow-up check, canine erupted normally after operation.

  • 랫드의 치아에서 발생하는 종양에는 에니멜모세포종 (ameloblastoma), 에나멜모세포성 치아종 (ameloblastic odontoma), 치아종 (odontoma) 이 있다 . 에나멜모세포종은 상피종양으로서 법랑질(enamel), 상아질 (dentin) , 시멘트질 (cementum) 을 형성하지 않는다 . 대부분의 에나멜모세포종은 소포성 (follicular) 성장이나 얼기상 (plexiform) 성장방식을 띠게 된다. 에나멜모세포성 치아종(ameloblastic odontoma) 은 치조직 종양으로서 에나멜모세포종 같이 연조직으로 된 부분과 치아종 (odontoma) 처럼 상아질, 법랑질, 시멘트질과 같은 단단한 부분으로 구성되어 있다.   • The purpose of this case report is to provide the information of the treatment of the impacted-displaced teeth associated with the disease in the jaw. A 10-year-old boy presented with a large radiolucent lesion accompanying the displacement of the second premolar and first molar in the left mandibular body area. The lesion was diagnosed as unicystic ameloblastoma. As a surgical procedure for the present case, marsupialization was executed. For the eruption of displaced impacted teeth, no orthodontic intervention was done for second premolar while a helical spring was used for the forced eruption of first molar. Goof occlusion was established by simple orthodontic intervention.

  • Ameloblastic fibroma is rare true benign mixed odontogenic tumor. Most of these tumors occur in the posterior region of the mandible under 20 years of age. It develops generally associated with unerupted tooth and grows slowly on the surface of alveolar bone, therefore interferes normal tooth eruption. These lesions rarely showing a little bony expansion, are usually asymptomatic and are discovered incidentally on routine dental exam. It is similar to amleoblastic fibroodontoma and ameloblastic fibrodentinoma clinically and roentgenographically but represents no dental hard tissue formation histologically. Enucleation and curettage of surrounding bone are generally recommended options for treatment. Even though there are some reports of recurrence and malignant transformation and more aggressive treatment options like block resection are suggested sometimes, but in most cases, recurrence is unusual because it is well encapsulated and easily separated from adjucent bony socket. In these cases, we did conservative treatment such as enucleation and curettage to the patients who were visited for ameloblastic fibroma associated with delayed eruption of lower first molar. After regular check-ups, we found relatively natural eruption process of combined teeth.

  • Calcifying odontogenic cyst(COC) is a rare developmental odontogenic cyst, which shows diverse classification and terminology. Cystic epithelial lining of COC is composed of basal cell layer of columnar cells and overlying layer of stellate reticulum. In the epithelium, ghost cells that might induce adjacent mesenchymal tissue to develop dental organ are shown characteristically. In spite of low rate of recurrence, we have to get a histopathological examination so that odontogenic lesions may recur without fully curettage of lining epithelium. 7-year-old male child came pediatric dentistry in wonkwang university dental hospital in order to check the delayed eruption of left maxillary central incisor. Radiographic examination revealed a well-defined radiopaque mass, overlapping impacted left central and lateral incisor crown. Enucleated mass was tooth-like features and also had epithelium lining. Results of histopathologic procedure, we saw the lots of ghost cell and proliferating hard dental tissues. Also we saw the cystic epithelium cells. It revealed diagnosis of the COC associated complex odontoma. For this reason one should consider of COC when patients present odontoma-like lesion with impacted tooth.

WHITENING EFFECT OF BLEACHING AGENTS ON TETRACYCLINE-STAINED RAT TEETH (테트라싸이크린에 의해 변색된 백서 치아에 대한 표백제의 표백 효과)

  • 본 연구는 테트라싸이클린에 의해 변색된 백서 치아에 대한 3 종 표백제의 표백효과를 비교하고 표백제의 침투 깊이 (법랑질 두께)에 따른 표백효과의 차이를 판별하기 위해 시행되었다. 30 마리의 웅성 백서에 매일 2주 동안 테트라싸이클린 용액을 복강내 주입하였다. 디스크 형태의 32개 백서 치아 표본을 16 시간 동안 자외선으로 조사하여 변색을 야기시켰다. 대조군은 생리 식염수에 보관하였으며 실험군은 3종 표백제(Opalescence, Rembrandt, NiteWhite)에 하루 5회씩 2주간 노출시켰다. 치아 색상을 표백전, 표백 1일, 3일, 5일, 7일, 9일, 11일, 및 14일에 각기 기록하여 치아내 변색부와 정상 색상간의 색상차 (${\Delta}$E)를 판독하였다; 대조군 - 9.78, 9.17, 9.36, 9.65, 9.40, 9.99, 10.57, 11.36; Opalescence - 10.08, 7.63, 6.72, 6.04, 5.10, 4.87, 4.89, 4.27; Rembrandt -9.83, 11.27, 9.55, 8.36, 7.75, 6.94, 7.11, 7.04; NiteWhite - 10.44, 9.92, 7.58, 6.80, 5.45, 5.05, 4.73, 4.01. 표백된 모든 치아들의 색상이 밝아졌다. (p<.01) 테트라싸이클린이 주입된 56개 백서 치아를 3 일간 자외선에 조사한 다음, 3가지 유형의 표백제 침투깊이를 설정하였다. 설측 상아질과 순측 법랑질을 통한 침투 (DN), 순측 법랑질만을 통한 침투 (RE), 1.0 mm 두께의 사람 법랑질로 피개된 순측 법랑질을 통한 침투 (HE). 표본들을 Opalescence로 하루 5회씩 1주일 및 4 주간 표백한 후 횡단하여 치아 색상을 측정하였다. 표백되지 않은 치아들을 대조군으로 이용하였다. 치아내 변색부와 정상 색상간의 색상차 (${\Delta}$E)는 다음과 같았다. 대조군 - 11.67; 1주 DN 군 - 13.55; 1주 RE군 - 12.80; 1 주 HE군 - 12.07; 4주 DN군 - 7.48; 4주 RE군 - 7.50; 4주 HE군 - 11.69. 4주 DN군과 4주 RE군의 색상 변화가 가장 크게 나타났다.(p<.01)

  • Odontoma is defined as a benign odontogenic tumor containing enmel, dentin as well as cementum. It has come to mean a growth in which both the epithelial and the mesenchymal cells exhibit complete differentiation. Most authorities accept the view today that the odontoma represents a hamartomatous malformation rather than a true neoplasm. The etiology of odontomas is uncertain but hypothesized to involve local trauma, infection, inheritance or mutant gene. The odontomas often cause various disturbances in the eruption and position of the teeth. The steps in removal of an odontoma in close relation to an adjacent impacted normal tooth should comprise 1) removal of odontoma and 2) exposure of the impacted tooth. Orthodontic therapy may be applied. Before treatment, the necessary space for the impacted tooth should be evaluated. If there is lack of space in the dental arch, orthodontic treatment should be carried out before operation.
