  • 랫드의 치아에서 발생하는 종양에는 에니멜모세포종 (ameloblastoma), 에나멜모세포성 치아종 (ameloblastic odontoma), 치아종 (odontoma) 이 있다 . 에나멜모세포종은 상피종양으로서 법랑질(enamel), 상아질 (dentin) , 시멘트질 (cementum) 을 형성하지 않는다 . 대부분의 에나멜모세포종은 소포성 (follicular) 성장이나 얼기상 (plexiform) 성장방식을 띠게 된다. 에나멜모세포성 치아종(ameloblastic odontoma) 은 치조직 종양으로서 에나멜모세포종 같이 연조직으로 된 부분과 치아종 (odontoma) 처럼 상아질, 법랑질, 시멘트질과 같은 단단한 부분으로 구성되어 있다. A Study of Ameloblastoma in Children and Adolescents (아동 및 청소년에 발생한 법랑모세포종)

  • A retrospective study of 15 cases of ameloblastoma in children and adolescents was performed. During the period of evaluation, 46 patients with ameloblastoma were seen, of which 15(32.6%) cases were in the patients aged 18 years or younger with the mean age of 14.5 years. There was more prevalence in male (1.5: 1). All the cases occured in the mandible, the molar and ramus area was the most frequent location(66.7%), and the most frequent sign was swelling of the face or jaw. Radiographically, 11(73.3%) cases manifested unilocular leion. Cortical expansion and root resorption were presented in 14(93.3%) and 11 (73.3%) cases respectively. Pexiform and plexiform unicystic types were common in the ameloblastoma occured in young patients.

  • The unicystic ameloblastoma deserves separate consideration on the basis of its clinical, radiologic, and pathologic features and its response to treatment. It refers to those cystic lesions that show clinical, radiographic, or gross features of a jaw cyst, but on histologic examination show a typical ameloblastomatous epithelium lining part of the cyst cavity. The lesion is most commonly found on the mandible posterior area, and often asymptomatic, although large lesions may cause a painless swelling of the jaws. The lesion typically appears as a circumscribed radiolucency that surrounds the crown of an unerupted molar. These are usually considered to be a dentigerous, residual cyst on the relationship of the lesion to teeth in the area. Three histopathologic variants of unicystic ameloblastoma may be seen. 1) Luminal type, 2) Intraluminal type, 3) Mural type. In this case, these tumor was treated as cysts by enucleation with iliac bone graft, and the diagnosis of ameloblastoma is made after microscopic examination of the presumed cyst.

  • Ameloblastoma is the most common benign odontogenic tumor, with approximately 80% arising in the mandible and the remainder in the maxilla. However, the infratemporal fossa is a rare site in which ameloblastomas occur. Although malignant transformations or metastasizing processes are extremely rare, tumor recurrence is common, if the tumor is not completely resected. Because reoperation could deteriorate quality of life in the patients and increase surgical morbidity, radical surgery is often recommended to minimize recurrence rates. In this report, we presented our experience of resection of ameloblastoma in the left infratemporal fossa with sufficient safe margin through a lower cheek flap approach and marginal mandibulectomy.

  • Adenomatoid odontogenic tumor(AOT) is an infrequent odontogenic tumor which arise in the jaw. It was considered as a variant of ameloblastoma. The adenomatoid odontogenic tumor is clearly benign and, in contrast to the ameloblastoma, present a very low recurrence. It most often appears in the canine region of the maxilla. The adenomatoid odontogenic tumor is frequently asymptomatic, however it may cause painless swelling. The radiological findings of adenomatoid odontogenic tumor frequently share characteristics of dentigerous cyst and unicystic ameloblastoma. Conservative surgical enucleation and curettage are the treatment of choice. In this case a 10-year-old child was presented with mandibular right lateral incisor in unerupted. Radiographically, the tooth was impacted and a radiolucency was seen in the area. The lesion was enuclated without extraction of the tooth. Bracket was attached on the tooth for orthodontic extrusion installed. Histopathologically adenomatoid odontogenic tumor was revealed.

  • Ameloblastoma is a benign odontogenic epithelial tumor with high recurrence rate and requires extensive resection of the surrounding tissue and reconstruction of defect site. Because of the anatomical limitation of the reconstruction site, prosthetic treatment with implants is the first recommendation. This is a case of prosthetic restoration of the reconstruction site with implant fixed prosthesis in patient who underwent mandibular resection and iliac bone reconstruction due to ameloblastoma. However 14 months after completion of implant prosthesis, adjacent natural tooth erupted unexpectedly, resulting in 1mm infra-occlusion occurred including posterior implant prosthesis and anterior natural teeth. In adults, implant infra-occlusion may occur due to residual growth after placement of the maxillary anterior implant. But this case, hypo-occlusion of molar implant and open bite of anterior natural teeth is occurred due to extrusion of adjacent tooth, is rare. Thus we report the treatment process including orthodontic treatment with intrusion of the posterior tooth, and investigate the causes of sudden, unexpected tooth extrusion.

  • 하악에 발생한 45예, 상악에 발생한 1예로 총 46예의 법랑모세포종이 연구되었다 환자의 평균연령은 34세였으며 성별에 따른 차이는 없었다. 가장 흔한 징후는 악골 또는 안면의 종창이었다. 소구치-대구치-하악지 부위를 침범한 예가 20예(43.5%)였다. 다방성 병소가 26예(56.5%)에서 발견되었다. 방사선학적 형태는 환자의 연령이나 조직학적 형태와는 상관관계가 없었다. 단방형 형태는 주로 젊은 환자에서 발견되었으며 10대에 호발하는 상관관계를 나타내었다(P<0.05). 근치 또는 보존적 수술로 치료를 받은 분석 가능한 18명의 환자에서 재발율은 33.3%를 나타내었다. 다방성 형태(36.4%)학적 행태를 명확하게 예측하기 위해서는 좀 더 상세한 임상 및 방사선학적 변수가 첨가되어야 할 것으로 사료되었다.

  • A case of an unicystic ameloblastoma of the mandible presenting as an apical periodontal cyst was reported. The lesion showed an unilocular radiolucency with well delineated border. Histologic examination revealed that a fibrous cyst wall with a lining that consists of partially of ameloblastic epithelium. The overlying epithelial cells are loosely cohesive and resemble stellate reticulum. The fibrous cyst wall of the cyst is infiltrated by typical follicular ameloblastoma. but devoid of inflammatory reaction in the cystic wall. Some considerations regarding differential diagnosis. pathogenesis and biologic behaviour of the lesion were discussed.

  • Ameloblastomas are benign epithelial neoplasm of the jaw comprising approximately 1-3.3% of all odontogenic cysts and tumors. Although most are microscopically benign, they are generally considered to be locally aggressive and destructive, exhibiting a high rate of recurrence. Treatments of them contain the conservative treatments on unilocular types and radical treatments on multilocular types. Classifications based on the histologic features of ameloblastoma generally included the follicular, plexiform, acanthomatous, basal cell, uncystic types. However, recently a new and unusual variant has been added by Eversole et. al in 1984, the histologic features of which are characterized by marked stromal desmoplastic proliferation. This is a case report of desmoplastic ameloblastoma in the anterior maxilla of 63 year-old female treated by block excision. This provides a brief review of the literature because of the rarity and unusual radiographic-histologic features of desmoplastic ameloblastoma.

  • Percutaneous transthoracic needle biopsy (PTNB) is a minimally-invasive procedure that is an indispensable tool for evaluating pulmonary lesions. Though extremely rare, tumor seeding of the pleura and chest wall can occur as a complication. Given that the breast is located anterior to the thorax, needle tracking through the breast is inevitable when PTNB is performed using the anterior approach. We describe tumor seeding of metastatic pulmonary ameloblastoma in the pectoralis muscle layer of the breast along the needle track of PTNB in a 51-year-old female presenting with a palpable lump in the right breast.
