The radiation dose received by the patient varies according to the tube current and time used during dental intraoral imaging. A large amount of tube current is required for image quality, but the radiation dose to the patient increases accordingly. Therefore, in this study, the optimal amount of tube current that can reduce the radiation dose received by the patient while securing the image quality was calculated through the evaluation of the image quality according to the tube current used during intraoral imaging through simulation. The average tube current, time, and tube voltage presented in the Guidelines for Diagnostic Reference Level for intraoral radiography were used as basic imaging conditions, and images were obtained when only the tube current was changed, and then the optimal tube current was compared and analyzed with the basic image quantity was calculated. Images were obtained by changing the tube current to 0.1, 0.5, 1, 2, 3, 4 and 5 mA under the basic conditions of 63 kV, 6 mA, and 0.29 s. The obtained image was evaluated for structural similarity index with the image taken under the condition of 6 mA using the ICY program. As a result, even under the condition of 0.5 mA tube current, the index of structural similarity with the image of 6 mA was evaluated to be high. Based on these results, it is considered that the radiation dose given to the patient can be greatly reduced if imaging is performed at 0.5 mA instead of 6 mA during dental intraoral imaging.
With the development of medical technology and radiation treatment equipment, the frequency of high-precision radiation therapy such as intensity modulation radiation therapy has increased. Image-guided radiation therapy has become essential for radiation therapy in precise and complex treatment plans. In particular, with the introduction of imaging equipment for diagnosis in a linear accelerator, CBCT scanning became possible, which made it possible to calibrate and correct the patient's posture through 3D images. Although more precise reproduction of the patient's posture has become possible, the exposure dose delivered to the patient during the image acquisition process cannot be ignored. Radiation optimization is necessary in the field of radiation therapy, and efforts to reduce exposure are necessary. However, when acquiring 3D CBCT images by changing the imaging conditions to reduce exposure, there should be no image quality or artefacts that would make it impossible to align the patient's position. In this study, Rando phantom was used to scan and evaluate images for each shooting condition. The highest SNR was obtained at 100 kV 80 mA 25 ms F1 filter 180°. As the tube voltage and tube current increased, the noise decreased, and the bowtie filter showed the optimal effect at high tube current. Based on the actual scanned images, it was confirmed that patient alignment was possible under all imaging conditions, and that image-guided radiation therapy for patient alignment was possible under the condition of 70 kV 10 mA 20 ms F0 filter 180°, which showed the lowest SNR. In this study, image evaluation was conducted according to the imaging conditions, and low tube voltage, tube current, and small rotation angle scan are expected to be effective in reducing radiation exposure. Based on this, the patient's exposure dose should be kept as low as possible during CBCT imaging.
목적 : 기존 구내 방사선 촬영에서 사용되었던 Film에서의 노출선량과 Digital Sensor를 이용한 구내 디지털 촬영에서의 노출선량을 비교하여 현재 광범위 하게 사용 되어지고 있는 Digital Sensor가 환자의 피폭선량을 감소하는데 기여하는 정도를 알아본다. 대상 및 방법 : 치아 우식증이 없는 5개의 구치부 치아를 선택하여 석고 블럭에 매식한 후 교합면과 인접면에 우식병소들을 형성하였다. 이를 필름(Kodak Insight; IS, Kodak Co, USA)과 Digital Sensor(Kodak RVG 6000; Kodak Co, USA)에 XCP Instrument(Rinn Co, USA)를 사용하고, 전면에는 조직등가물질인 Acrylic Resin Block 20mm를 설치하였다. Acrylic Resin Block의 관구 측면 에서는 조사선량계를 부착하여 단계별로 변화시키는 노출조건에 대한 조사선량을 측정하였다. 그리고 이렇게 얻어진 영상을 3명의 방사선학 전공의와 1명 의 보존과 전공의가 평가를 하였다. 결과 : Film과 Digital Sensor를 가지고 촬영한 영상을 분석한 결과 노출선량에서도 Digital Sensor는 Film 노출선량과 비슷한 결과를 도출해 낼 수 있었다. 그러나 Digital Sensor로 촬영된 영상의 경우에는 Film 경우보다 좀더 효과적으로 조사선량을 판단 할 수 있었으며, 또한 영상 판독시 필름보다 폭 넓게 응용 할 수 있었다. 결론 : 본 연구에서는 선량 변화에만 의존하는 것이 아니라 Digital Sensor로 촬영시 영상 조절을 병행한다면 좀더 정확하고 효과적인 진단 활동에 도움이 될 수 있을 것으로 사료된다.
The purpose of this study was to compare the compression force and thickness of the compressed breast between mediolateral oblique and craniocaudal mammography. This population consisted of 204 paired mediolateral oblique and craniocaudal mammography obtained on one mammographic unit by certified mammography technologists during 2 month period. The women examined were $30{\sim}59$ years old with breast implants, the patients were classified into 3 groups according to age, $30{\sim}39$, $40{\sim}49$, and $50{\sim}59$, prior breast cancer, mastectomy or breast deformity were excluded. The digital readout of compressed breast compression force and thickness was recorded. Mammographic positioning was assessed using standard criteria. The mean compression force of the compressed breast on the craniocaudal projection was less than the mean compression force on the mediolateral projection(14 versus 13.88 daN, p<0.05). The mean thickness of the compressed breast for mediolateral projection was 41.46 mm and that for the craniocaudal projection was 39.50 mm(p<0.05). The compressed breast is higher or thicker on mediolateral oblique than on craniocaudal mammography.
Since the spine radiography were explained differently at every several hospitals and textbooks. the technique has not been accurately defined and interfered each other. We would like to define the most appropriate positioning for clinical cases, and reference books, by comparing scoliotic angle and lordotic angle. From Mar 2009 to Sep 2011, 85 patient cases were studied, who had not been undergone surgical treatment among spondylopathy patients. Scoliotic angle and lordotic angle were measured, using Cobb's method. We analyzed statistically using t-test(SPSS 18), and evaluated spine general radiography position. Moreover, we researched on the actual condition at 10 university hospitals in Seoul. The results of scoliotic angle measurement, the value at erect position showed 20.98% higher than supine position, and it has statistical significance (p<.01). In lordotic angle measurement, the value at neutral holding position represented 29.3% higher than supine position, and it also has statistical significance(p<.01). The results of clinical survey, supine posine(70.0%) took much higher possession than erect position(30.0%). In conclusion, compare to supine position, erect position shows increased scoliotic and lordotic angle. It was agreed with the importance of clinical erect position radiography, which gravity affects. So clinical radiologist must recognize the difference, and conduct an accurate study.
Computed tomography (CT) has the problem of having more radiation exposure compared to other radiographic apparatus. There is a low-dose imaging technique for reducing exposure, but it has a disadvantage of increasing noise in the image. To compensate for this, various noise reduction algorithms have been developed that improve image quality while reducing the exposure dose of patients, of which the median modified Wiener filter (MMWF) algorithm that can be effectively applied to CT devices with excellent time resolution has been presented. The purpose of this study is to optimize the mask size of MMWF algorithm and to see the excellence of noise reduction of MMWF algorithm for existing algorithms. After applying the MMWF algorithm with each mask sizes set from the MASH phantom abdominal images acquired using the MATLAB program, which includes Gaussian noise added, and compared the values of root mean square error (RMSE), peak signal-to-noise ratio (PSNR), coefficient correlation (CC), and universal image quality index (UQI). The results showed that RMSE value was the lowest and PSNR, CC and UQI values were the highest in the 5 x 5 mask size. In addition, comparing Gaussian filter, median filter, Wiener filter, and MMWF with RMSE, PSNR, CC, and UQI by applying the optimized mask size. As a result, the most improved RMSE, PSNR, CC, and UQI values were showed in MMWF algorithms.
This study was conducted to analyze factors Influencing Protective Behavior against Radiation Exposure using questionnaires for 231 radiological technologists working in Computed Tomography(CT) examination room with high radiation dose in diagnostic radiology field. Statistical analysis of the collected data revealed that the reasons for partially shielding the examination part in the CT scan were the lack of protective equipment, securing of radiation justification, being annoying and maybe not being harm to adults in order. It was also revealed that the variables influencing the protective behavior were protective behavior against radiation harm, self-efficacy, protective environment, organization culture, protective knowledge and protective instrument in order. The higher the radiological protective environment(${\beta}=0.245$) and the lower the radiological protective knowledge(${\beta}=-0.034$), the more influential the protective behavior against radiation harm was. In this study, it was shown that non examination parts were not shielded in the CT scan. Therefore, it is necessary to improve the level of protective environment, to cultivate knowledge to improve the protective behavior against radiation harm and to have an intervention strategy for concrete action.
