Voice Analysis before and after Radioactive Iodine Ablation in Patients with Total Thyroidectomy (적갑상선 전절제술 환자의 방사성 동위원소치료 전.후 음성의 변화에 대한 연구)

  • Background and Objectives:This study is to objectively compare and analyze the acoustic changes in the patients with total thyroidectomy before and after RI therapy. Subjects and Methods:For this study, a total of 50 patients with total thyroidectomy were participated as subjects. Voice samples were obtained at the time of post-operation (Post-OP), before high-dose radioactive iodine therapy (Pre-RIT), and after high-dose radioactive iodine therapy (Post-RIT). Acoustic analysis, the maximum phonation time and K-VHI (Korea-Voice handicap index) were used for subjective evaluation. Results:According to the comparison analysis of the three periods, mFo (Hz) was significantly reduced in all of the vowels /a/ and /i/ as the hormone was discontinued. This can be related to the reduction in vocal range. As thyroid hormone was discontinued, Shim (%) and APQ (%) values, which are the parameters related to the degree of aggressiveness, showed a significant increase in the middle vowel /a/. As thyroid hormone was discontinued, emotional index was significantly decreased in VHI (voice handicap index). Conclusion:These results can be assumed that thyroid hormone suspension is related to the increased changes in the vocal intensity, the increase in noise and the reduction in vocal range. Emotionally, these data can be assumed that the responsive factors of one's own voice disorders were significantly decreased in the patients with vocal handicap.

  • In this paper, the concept and characteristics of the nuclear propulsion system were introduced and the state of the art for the nuclear-powered space propulsion in abroad were summarized. Since uranium used in nuclear propulsion has a very high energy density per unit mass, it has exceptional specific impulse performance compared to the existing chemical propulsion method and can reduce the amount of fuel loaded, thereby having advantage for long-distance exploration. For this reason, advanced countries in space development are recently spurring to the research of nuclear propulsion technology, and it is judged that the development of a propulsion engine using nuclear power is absolutely necessary in order to gain an competitive edge on the space development.

  • 하천으로 유입되는 오염물질은 유수의 흐름에 따라 이송되며 혼합된다. 이러한 오염물질의 해석을 위해서는 확산 또는 분산계수 산정이 필요하다. 오염물질의 거동과 관련된 실험적 연구는 방사선 동위원소와 형광성 물질을 이용하여 수행되어 왔으나, 추적자 실험은 많은 비용 및 인력을 요하며, 고정식으로 설치한 계측장비로부터 수집한 시계열 농도자료만을 이용하여 분석하기 때문에 공간적 분포에 대한 자료 취득은 어렵다는 한계가 있다. 하천의 오염물질을 모니터링하기 위해서는 공간을 이동하는 입자의 관점에서 물리량을 표현하는 Lagrangian 방식보다 특정 위치에서 물리량 변화를 표현하는 Eulerian 방식이 적합하다. 그러나 드론을 활용한 하천원격탐사 연구의 대부분은 이동식 플랫폼으로 활용되어 특정 시간에 공간적 분광특성의 분포 파악이 한정적이며, 동일 지점에서 분광특성의 시간적 변화를 파악할 수 없다. 따라서 본 연구에서는 기존의 면단위를 측정하는 이동식 플랫폼의 한계를 극복하고 하천 모니터링에 적합한 Eulerian 방식을 적용하기 위하여 CCTV 형태의 고정식 초분광촬영 플랫폼을 도입하였다. 본 연구에서는 하천으로 유입되는 오염물질의 거동을 분석하기 위하여 자연하천에서 형광성물질인 Rhodamine WT를 이용하여 추적자 실험을 수행하였으며, 접촉식 센서를 활용한 농도측정과 동시에 드론과 초분광센서를 활용하여 CCTV 형태의 고정식 초분광영상을 획득하였다. 실험결과 도출된 전통적인 방식의 분산계수 산정과 시공간 초분광영상을 활용한 분산계수 산정을 비교하여 오염물질 거동 분석에 초분광영상 활용의 가능성을 검토하였다. 본 연구에서 제시한 드론기반 시공간 초분광영상 기법을 교량이나 기타 하천구조물에 초분광 센서를 설치하여 CCTV형식으로 활용할 경우, 공단이나 하·폐수 처리장 등의 점오염원이 밀집해 있는 지역에 직접 설치하여 화학사고의 감지 및 오염물질의 유출 확인 및 조류, 부유사 등의 다양한 수질항목의 농도 변화 감지가 가능하고, 수심변화 감지로 장기적으로 활용할 경우 특정 지점에서의 하상변동 조사가 가능하다. 또한, 오염물질의 유출 사고 발생 등의 사람이 직접 접근이 불가능한 지역에 드론을 활용하여 초분광센서를 이용한 오염물질 감지가 가능할 것으로 판단된다.

Radiation Absorbed Dose Calculation Using Planar Images after Ho-166-CHICO Therapy (Ho-166-CHICO 치료 후 평면 영상을 이용한 방사선 흡수선량의 계산)

  • Ho-l66 was produced by neutron reaction in a reactor at the Korea Atomic Energy Institute (Taejon, Korea). Ho-l66 emits a high energy beta particles with a maximum energy of 1.85 MeV and small proportion of gamma rays (80 keV). Therefore, the radiation absorbed dose estimation could be based on the in-vivo quantification of the activity in tumors from the gamma camera images. Approximately 1 mCi of Ho-l66 in solution was mixed into the flood phantom and planar scintigraphic images were acquired with and without patient interposed between the phantom and scintillation camera. Transmission factor over an area of interest was calculated from the ratio of counts in selected regions of the two images described above. A dual-head gamma camera(Multispect2, Siemens, Hoffman Estates, IL, USA) equipped with medium energy collimators was utilized for imaging(80 keV${\pm}$10%). Fifty-nine year old female patient with hepatoma was enrolled into the therapeutic protocol after the informed consent obtained. Thirty millicuries(110MBq) of Ho-166-CHICO was injected into the right hepatic arterial branch supplying hepatoma. When the injection was completed, anterior and posterior scintigraphic views of the chest and pelvic regions were obtained for 3 successive days. Regions of interest (ROIs) were drawn over the organs in both the anterior and posterior views. The activity in those ROIs was estimated from geometric mean, calibration factor and transmission factors. Absorbed dose was calculated using the Marinelli formula and Medical Internal Radiation Dose (MIRD) schema. Tumor dose of the patient treated with 1110 MBq(30 mCi) Ho-l66 was calculated to be 179.7 Gy. Dose distribution to normal liver, spleen, lung and bone was 9.1, 10.3, 3.9, 5.0 % of the tumor dose respectively. In conclusion, tumor dose and absorbed dose to surrounding structures were calculated by daily external imaging after the Ho-l66 therapy for hepatoma. In order to limit the thresholding dose to each surrounding organ, absorbed dose calculation provides useful information.

The Role of Intraluminal Brachytherapy in Management of Esophageal Cancer (식도암 치료에 있어 관내근접치료의 역할)

  • Purpose : To evaluate our clinical experience with the combination of teletherapy and intraluminal brachytherapy in patients with unresectable or inoperable esophageal cancers. Materials and Methods : From Nov 1989 to Mar 1993, twenty patients with esophageal cancer were treated with radical radiotherapy and intraluminal brachytherapy at Yonsei Cancer Center. All patients had squamous histolgy and stage distribution was as follows: stage II, 4($20{\%}$)patients; III, 15 ($75{\%}$)patients; IV, 1($5{\%}$)patients. A dose of S-12Gy/1-3weeks with intraluminal brachytherapy (3-5Gy/fraction) to 5mm from the outside of the esophageal tube using high dose rate Iridium-192 remotely afterloading brachytherapy machine was given 2 weeks after a total dose of 59-64Gy with external radiotherapy. Induction chemotherapy using cisplatin and 5-FU was performed in 13 patients with median 3 cycles(1-6 cycles), Response rate, local control rate, survival and complications were analysed retrospectively. Results : Two-year overall survival rate and median survival were $15.8{\%}$ and 13.5 months. Response rates were as follows complete remission(CR) 5($25{\%}$): partial remission a(PRa) 7($35{\%}$): partial remission b(PRb) 7($35{\%}$), no response(NR) 1($5{\%}$). Patterns of failure were as follows; local failure 13($65{\%}$), local and distant failure 3($15{\%}$), distant failure 0($0{\%}$). Ultimate local control rate was $20{\%}$. Treatment related complications included esophageal ulcer in two patients and esophageal stricture in one. Conclusion : Though poor local conrol rate, median survival was improved as compared with previous results of radiation therapy alone(8months) and chemoradiation combined treatment(11 months) in Yonsei Cancer Center High-dose-rate intraluminal brachytherapy following external irradiation is an effective treatment modality with acceptable toxicity in esophageal cancer.

A Study on Neutron Resonance Energy of 180Ta below 1eV Energy (1 eV 이하 에너지 영역에서의 180Ta 동위원소의 중성자공명에 대한 연구)

  • In this study, the neutron capture cross section of $^{180}Ta$(natural existence ratio: 0.012 %) obtain by measuring has been compared with the evaluated data for the capture data. In generally, the neutron capture resonance is defined as Breit-Wigner formula. The formula consists of the resonance parameters such as neutron width, total width and neutron width. However in the case of $^{180}Ta$, these are very poor experimental neutron capture cross section data and resonance information in below 10 eV. Therefore, in the study, we analyzed the neutron resonance of $^{180}Ta$ with the measuring the prompt gamma-ray from the sample. And the resonance was compared with the evaluated data by Mughabghab, ENDF/B-VII, JEFF-3.1 and TENDL 2012. Neutron sources from photonuclear reaction with 46-MeV electron linear accelerator at Research Reactor Institute, Kyoto University used for cross section measurement of $^{180}Ta(n,{\gamma})^{181}Ta$ reaction. $BGO(Bi_4Ge_3O_{12})$ scintillation detectors used for measurement of the prompt gamma ray from the $^{180}Ta(n,{\gamma})^{181}Ta$ reaction. The BGO spectrometer was composed geometrically as total energy absorption detector.

The Expression of Plasma Progesterone and Estradiol-17$\beta$ Level before Parturition in the Recipients Pregnant by Hanwoo SCNT Embryos (체세포 복제란 이식 한우의 분만 전 혈장 Progesterone과 Estradiol-l7$\beta$ 농도 변화)

  • This study was performed to analyze the characterization of plasma hormonal levels during pregnancy in the Hanwoo recipients pregnant by artificial insemination (AI) or somatic cell nuclear transfer (SCNT) embryos. The synchronized recipients pregnant by SCNT embryos produced by Hanwoo fetal fibroblast cells (n=8) and by AI (control, n=5) were used. The plasma hormonal levels were measured by RIA (P4 and E2) and ELISA (cortisol), respectively. In control, the increase of E2 and the decrease of P4 were occurred immediately before the initiation of parturition. The expression pattern of plasma P4 was similar in both groups from 50 to 10 days before parturition, however, it did not decrease even at the expected date of labor in the SCNT recipients. The plasma cortisol was expressed a lower level during pregnancy in the SCNT recipients. But, the cortisol was increased in the cow aborted around 100 days of pregnancy (n=1). Based on these results, it can be postulated that the failure of the hormonal changes immediately before parturition in the SCNT recipients may be one of the most important reasons for a delayed parturition in clone calving.

A Study on the Measurement of Half-life for the 179Re Isotope Produced by a 100 MeV Proton Nuclear Reaction (100 MeV 양성자 핵반응에 의해 생성된 179Re 동위원소의 반감기 측정에 관한 연구)

  • This study accurately measured the half-life of the 179Re radioactive isotope using the 100 MeV proton and high-purity HPGe gamma ray measurement system generated from a high-energy proton linear accelerator at the Korea Atomic Energy Research Institute. The result obtained is 19.64 ± 0.26 min. Compared with the past results of the half-life of the 179Re radioisotope known to date, the results of B. Harmatz published in 1960 and B. J. Meijer published in 1975 measured the median value higher than the results obtained in this study. In the case of K. J. Hofstetter, published in 1966, a very large error is also characteristic. This result showed a tendency that the center value was very consistent with the result of this study. The results of Coral M. Baglin's 19.5 min published in 2009 are very consistent with the error range. The obtained measured result was compared with the result of ENSDF (Evaluated Nuclear Structure Data File). Through this study, more reliable values were measured for the results of the half-life of the 179Re, which was previously incorrect, and the validity of the recently published results of Coral M. Baglin was confirmed.

Surface Dose Evaluation According to the Environment Around the Patient after Nuclear Medicine Examination (핵의학 검사 후 환자의 주위 환경에 따른 표면 선량 평가)

  • The purpose of this study was to investigate changes in surface dose due to increased scattering of gamma rays from patients injected with 99mTc and 18F, which are radioactive isotopes, in close contact with materials with high atomic number such as the walls of the stable room. Prepare 99mTc and 18F by injecting 20 and 10 mCi respectively into the NEMA phantom, and then measuring the surface dose for 60 minutes by positioning the phantom at a height of 1 m above the surface, at a distance of 0, 5 and 10 cm from the wall, and at the same location as the phantom facing the wall. Each experiment was repeated five times for reproducibility of the experiment and one way analysis of variability (ANOVA) was performed for significance testing and Tukey was used as a post-test. The study found that surface doses of 220.268, 287.121, 243.957, and 226.272 mGy were measured at 99mTc, respectively, in the case of empty space and in the case of 0, 5 and 10 cm, while those of 18F were measured at 637.111, 724.469, 657.107, and 640.365 mGy, respectively. In order to reduce changes in surface dose depending on the patient's location while waiting, it is necessary to keep the distance from the ground or the wall where the patient is closely adhered to, or install an air mattress, etc., to prevent the scattered lines as much as possible, considering the scattered lines due to the wall etc. in future setup of the patient waiting room and safety room, and in addition to the examination, the external skin width may be reduced.

Interstitial Vaginal Needle Implantation in Gynecological Tumors : Design and Construction of Applicator (부인과암에서 조직내 삽입 방사선치료 - Applicator의 고안 및 제작-)

  • Purpose : It is not a simple task to achieve the ideal isodose curve with a standard vaginal applicator or sing1e plane needle impant in the paravaginal tissue when primary or recurrent gynecological neoplasms(cervical cancers, vaginal cancers and vulvar cancers) are treated as a boost following external beam radiotherapy. The authors introduce the development and construction of a simple, inexpensive, customized applicator for volume implant to maximize the radiation dose to the tumor while minimizing the dose to the rectum and the bladder. Materials and Methods : Nine patients underwent Ir-192 transperineal interstitial implantation for either recurrent(5 cases) or primary(3 cases) cervical cancers or primary vaginal cancer(1 case) between August 1994 and February 1998 at Ajou university hospital. First 3 cases were performed with a sing1e plane implant guided by digital palpation. Because of inadequate isodose coverage in the tumor volume in first 3 cases, we designed and constructed interstitial vaginal applicator for volume implant to improve tumor dose distribution and homogeneity while sparing the surrounding normal tissue. Our applicators consist of vaginal obturator and perineal template that made of the clear acrylamide and dental mold material$(Provil^{(R)})$. The applicators were customized individually according to the tumor size and its location Both HDR and LDR irradiation were given with these applicators accomodating 6 Fr needles(Microselectron Nucletron). The pretreatment planning prior to actual implant was performed whenever possible. Results : Needles can be inserted easily and evenly into the tumor volume through the holes of templates, requiring less efforts and time for the implant procedure. Our applicators made of materials available from commercial vendors. These have an advantage that require easy procedure, and spend relatively short time to construct. Also it was possible to fabricate applicators to individualize according to the tumor size and its location and to achieve the ideal isodose coverage. We found an accurate needle arrangement and ideal dose distribution through the CT scan that was obtained in 3 cases after needle implant. Three patients with primary cervical and vaginal cancers were controlled locally at final follow up. But all recurrent cases failed to do so. Conclusion : The authors introduce inexpensive, simple interstitial vaginal templates which were self-designed and constructed using materials available from commercial vendors such as acrylanide and dental mold material $(Provil^{(R)})$.
