Kinetic Differences between Normal-design Running Shoes and Spring-loaded Running Shoes (기능성 스프링신발과 일반운동화의 운동역학적 비교분석)

  • The purpose of this study is to examine the effect of the functional shoes through the kinetic comparison of normal-design running shoes and spring-loaded running shoes. For this, 12 healthy females from the age from 30 to 40 years participated in the EMG and ground reaction force experiment with testing kinetic variables. 12 subjects walked at the velocity of 1.7m/s. After analyzing variables in the spring-loaded running shoes and normal-design running shoes, the following conclusions were obtained; For the ground reaction force, spring-loaded running shoes have larger antero-posterior GRF than normal-design running shoes in the first and second apexes of antero-posterior ground reaction force. For the analysis of EMG, spring-loaded running shoes showed the higher muscle activation of rectus femoris muscle than norma-design running shoes. So the spring-loaded running shoes help improvement muscle strength of knee extensor.

The Effects of Kaltenborn Orthopedic Manual Therapy with Evjenth-hamberg Stretching on Range of Motion of Ankle Joint and Balance Ability in Patients with Chronic Stroke (칼텐본 정형도수 물리치료와 에비안스-함베르크 뻗침 운동의 결합이 뇌졸중 환자의 발목관절 가동범위와 균형능력에 미치는 영향)

  • PURPOSE: Stroke patients have limited ankle range of motion and balance problems. The purpose of this study was to determine the effects of Kaltenborn orthopedic manual therapy, Evjenth-hamberg stretching, and combination of both on ankle dorsiflexion range of motion and dynamic balance ability in stroke patients. METHODS: Thirty patients were placed in three groups, each of which received different treatments: Kaltenborn orthopedic manual therapy (15 mins), Evjenth-hamberg stretching (15 mins), or Kaltenborn orthopedic manual therapy with Evjenth-hamberg stretching (30 mins). Each group received three exercise sessions per week during four weeks. To determine its effectiveness of interventions an evaluation was carried out. ankle dorsiflexion range of motion was gauged using a tiltmeter application, and dynamic balance ability was measured using the Berg balance scale. RESULTS: All groups revealed significant improvements in ankle dorsiflexion range of motion, Berg Balance Scale (BBS) score before and after the intervention, and Kaltenborn orthopedic manual therapy with Evjenth-hamberg stretching group showed a significant improvement in ankle dorsiflexion range of motion and BBS score in comparison to other groups. CONCLUSION: We found out that each individual method was effective in ankle dorsiflexion range of motion, dynamic balance ability in stroke patients, and the combined method was more effective.

Surface Tribology of Total Ankle Joint Replacement (인공발목관절의 표면 마모 특성)

  • Total ankle replacement (TAR) is a visible option in the surgical treatment of degenerative or inflammatory diseases of ankle joint. it is attributed to the current TAR which has improvements in surgical technique, uncemented implant fixation and minimally constrained articulation. In the clinical result, they can show promised surgical result when compared to earlier attempts in TAR. However, TAR is still not as successful as total knee replacement (TKR) or total hip replacement (THR), it needs to be note that there are limitations in concerning of long term performance of TAR, the high failure rate still associated with wear of the PE (polyethylene) component that has related with their material property and surface roughness. The aim of this study was to introduce the tribology characteristics of total ankle joint prosthesis with one of TDR model which was fabricated to try multi-axis wear test as a region of motion in ankle joint. The wear specimen of TDR was prepared with Ti-6Al-4V alloy and UHMWPE (ultra-high molecular weight polyethylene) for tibia-talus and bearing component, respectively. A wear test was carried out using a Force 5 (AMTI, Massachusetts, US) wear simulator which can be allowed to move in three axis to flexion-extension ($+3^{\circ}{\sim}-6^{\circ}$), internal-external axial rotation (${\pm}5^{\circ}$), as well as sinusoidal compressive load (1.6 kN, R=10). All tests were performed following standard ISO 14243, wear rate was calculated with weight loss of UHMWPE bearing while the specimen has tested at certain cycles. As based on the preliminary results, wear rate of UHMWPE bearing was $7.9{\times}10^{-6}mg/cycles$ ($R^2=0.86$), calculated loss weight until $10^7cycles$ was 79 mg, respectively.

The Effect of Balance Training on Balance Ability and Ankle Joint Muscle Activity (불안정한 지지면에서의 균형훈련이 발목관절 근활성도에 미치는 영향)

  • Purpose : This study investigates the effects on balanced ability and ankle joint muscle following balance pad and trampoline training periods in order to report a clinical directing about this matter. Methods : A total of nine members participated in a balance training program that put a balance pad and trampoline to use. Participants trained three times a week for thirty minutes in groups of six. Resting time was set for two minutes. EMG (median frequency and root mean square) was used to measure muscle activity at 3 weeks and 6 weeks, respectively. The tibialis anterior and gastrocnemius muscles were measured. Participants were measured with eyes open and closed. Results : The median frequency showed a significant difference according to the change in time in both muscles. Also, results showed a significant interaction between eyes open and closed in the gastrocnemius muscle. The root mean square only showed a significant difference according to the change in time in the gastrocnemius muscle. Conclusion : We suggest that the balance pad and trampoline training influence a decrease in the recruitment rate of slow twitch muscles through an improvement in balance ability; however, the training did not show a statistically significant difference with regards to eyes open and closed.

Effect of the Instrument Assisted Soft Tissue Mobilization and Static Stretching on the Range of Motion and Plantar Foot Pressure of an Ankle Joint (도구를 이용한 연부조직가동술과 정적 스트레칭이 족저압과 발목관절 가동범위에 미치는 영향)

  • Background: The purpose of this study was to compare the effects of IASTM and static stretching techniques on ankle joint range of motion (ROM), static foot pressure. Methods: Twenty four subjects with gastrocnemius shortness participated in this study. The subjects were assigned randomly to one of two groups: The soft tissue mobilization technique (IASTM) group received intervention using a IASTM instrument for two minutes, and the stactic stretching group performed self stretching for 30 seconds, four times. The ROM of the ankle joint was measured by active ankle dorsi-flexion test, and a TPScan was utilized to collect the plantar foot pressure. This experiment was performed by two physical therapists. The significant level was set at ${\alpha}=.05$. Results: The results were as follows: 1) The ROM of the ankle joint and was significantly increased in both groups. 2) Plantar foot pressure was no significant in both groups. 3) There were no significant differences between the IASTM group and static stretching group for any variable. Conclusions: The results of this study suggest that static stretching is an effective and easy technique for restoring proper muscle length in subjects with gastrocnemius shortness. We recommend that static stretching technique be used for treat gastrocnemius shortness in clinical setting and home program.

Effect of Deep Breathing Exercise and Ankle Exercise on Blood Flow Velocity in the Femoral Vein (심호흡 운동과 발목관절 운동이 대퇴정맥의 혈류속도에 미치는 영향)

  • This study was conducted to determine the effects of deep breathing exercise and ankle exercise on blood flow velocity in the femoral vein. Sixteen healthy male students were recruited from Yonsei University, at Wonju. The blood flow velocities in the femoral vein were measured under three different conditions: resting, deep breathing, and ankle exercise. All subjects were given a 5-minute relaxing time in supine position prior to the study. Using a doppler ultrasound with a 8 MHz probe, the peak blood flow velocities were collected in a twenty-second-period at each condition. The subjects took a rest in between trials for the blood flow to return to its resting levels. The result showed a significant difference in peak blood flow velocities under those three conditions (p<.001). The peak blood flow velocity was highest in ankle exercise condition. The peak blood velocity was significantly higher in deep breathing condition compared with the resting condition. As a result, it is revealed that not only the muscular contractions but also the deep breathing exercises induced facilitating effects of venous return. Either of the exercise methods can be recommended to prevent blood stasis in patients with risk of deep vein thrombosis after cardiac or lower extremity surgery.

  • This paper proposed visualization tool for motion of snowboarding using Skeleton data obtained by the Microsoft's Kinect sensor. The BBP(Balanced Body Position) posture is a most basic motion in the Snowboarding. This posture is the primary technology for stable turns. The implementation of visualization tool to analyse the BBP posture of snowboard. comparative analysis with standard postures to the ankles, knees, hips and spine angle of joints and body tracking using coordinate information obtained by the Kinect Sensor. Analysis of the final results of the screen through the OpenGL library. This research result could be used to analysis for turn postures of snowboarding.

Assessment of Isokinetic Muscle Strength in the Knee Extensor Through the Ankle Joint Angles (발목관절 각도에 따른 무릎 폄근의 등속성 근력 평가)

  • Purpose:This study was designed to investigate difference in isokinetic muscle strength in the knee extensor muscle and characteristic differences in muscle strength between males and females through the ankle joint angles. Methods: Seventy-four subjects participated in this study. There were two groups: 36 males and 38 females. The mean age of the men was 24.58 years and women was 23.74 years. Subjects were seated on a CON-TREX LP (leg press) lean to back of chair, and there bodies were fixed by straps with the hip joint at an angle of $130^{\circ}$. After randomly fixing the ankle joint at $0^{\circ}$, $20^{\circ}$, and $40^{\circ}$ of plantar flexion (PF) in range of full extension of knee joint. We studied force max average, force max average/kg, power average, and total work through the angle of the ankle joint when the knee joint was extended from $90^{\circ}$ to $180^{\circ}$. Results: In the male group, all maximum measured value showed at the ankle joint $0^{\circ}$, all minimum measured value showed at $40^{\circ}$ PF (p<0.01). In the female group, all maximum measured value showed at the ankle joint $20^{\circ}$ PF, especially the power average increased significantly. All minimum measured value showed $40^{\circ}$ PF (p<0.01). Conclusion: There are differences between males and females in isokinetic muscle strength of the knee extensor through ankle joint angles in healthy adults. Males and Females have different characteristics of muscle strength through the ankle joint angles.

The Effect of Calf-Raise Exercise on Gastrocnemius Muscle Based on Other Type of Supports (지지면에 따른 발목관절의 족저굴곡 운동이 비복근 두께에 미치는 영향)

  • Purpose : The purpose of this study was to perform the calf-raise exercise on the stable support surface and unstable support surface for comparing deference of the gastrocnemius thickness changes that measured by ultrasonography. Method : Subjects stand on unstable support surface and a stable support surface with both feet. Keeping calf-raise position in the 3 seconds, and then had to return back to the starting position. Total of 3 sets of 15 repetitions performed and give a 30-second rest between each set. In this way, exercise performed 3 times a week, performed a total of 6 weeks. Result : 1. gastrocnemius muscle thickness before and after exercise in experimental group A was significantly different(p<.05). 2. gastrocnemius muscle thickness before and after exercise in experimental group B was significantly different(p<.05). 3. gastrocnemius muscle thickness in experimental group A and experimental group B was significantly difference(p<.05). Conclusion : In conclusion, calf-raise exercise on unstable support surface was more efficient than stable support surface for increasing thickness of gastrocnemius.

Effects of a Stim up Mat Walking Exercise Program on Balance, Gait Function and Joint Motion Range of the Frail Elderly (스팀업(Stim UP)매트 걷기운동 프로그램이 허약노인의 균형능력과 보행기능 및 관절 가동범위에 미치는 효과)

  • Purpose: This study was performed to evaluate effects of a stim-up matt walking exercise program on balance and gait of the frail elderly. Methods: A total of 37 elderly people recruited from S city were randomly assigned to the experimental group (n=22) and control group (n=15). The stim-up matt walking exercise program was offered twice a week for 8 weeks. Data were analyzed by SPSS 21.0. Results: The dynamic balance ability Timed Up and Go test of the experimental group was significantly faster than that of the control group (t=21.72, p<.001). The static balance ability open-eye standing test (t=44.15, p<.001) and close-eye standing test (t=9.01, p=.005) also showed increase in effects of the experimental group. In the walking ability, gait cycle (t=2.48, p=.018), cadence (t=-2.21, p=.034) and gait speed (t=-2.78, p=.009), positive effects were on. However, no statistically significant differences were found in stride length and double support. At the ankle joint range left ankle plantar flexion (t=3.92, p<.001) and left ankle dorsal flexion (t=4.51, p<.001) were higher in the experimental group than in the control group, and also right ankle plantar flexion (t=2.79, p=.008) and right ankle dorsal flexion (t=2.92, p=.006) increased in the experimental group. Conclusion: The significance of this study is that the stim-up matt walking exercise program for the frail elderly proves to be useful for improving balance and walking.
