장무지 굴곡근의 혈관성 기형에 동반된 발가락의 굴곡 변형: 증례 보고 (Vascular Malformation of Flexor Hallucis Longus Muscle Associated with a Flexion Deformities of Toes: A Case Report)

  • 혈관성 기형은 주로 무통성 종괴나 종창과 통증을 수반한 종괴의 형태로 나타난다. 드문 경우에서, 근육에 발생한 혈관성 기형은 침범한 근육의 위축을 유발하여 관절의 변형을 초래한다. 혈관성 기형이 하퇴부의 굴곡근에 발생한 경우 발목의 첨족 변형을 유발한다. 그러나, 발가락의 장족지 또는 장무지 굴곡근에 혈관성 기형이 발생하여 발가락의 굴곡 변형을 초래한 것에 대한 문헌 보고의 예는 없다. 이에 저자들은 40세 여자 환자에서 장무지 굴곡근에 발생한 혈관성 기형으로 인해 유발된 발가락의 굴곡 변형을 경험하고 수술적 절제술을 통해 치료하였기에 문헌 고찰과 함께 보고하는 바이다.

  • PURPOSE: This study was conducted to investigate the effects of pain, balance, and left / right step time difference between elastic taping and non-elastic taping in patients with acute ankle sprain. METHODS: The subjects were patients with acute ankle sprains who had been injured within 1 day. A total of 30 subjects were divided into three groups (CG: control group, EG1: experimental group 1, EG2: experimental group 2) of 10 people. The intervention period was 3 days. In the CG, only physical therapy (cryotherapy+pulsed ultrasound) was performed, while physical therapy and elastic taping were applied in EG 1 and physical therapy and non-elastic taping were applied in EG 2. RESULTS: Changes in pain, balance and left / right step time difference following intervention decreased significantly in all three groups. After intervention, the balance between the groups differed significantly between the CG and the EGs. In addition, the left / right step time difference differed significantly between the CG and EGs, as well as between the EG 1 and the EG 2. CONCLUSION: Physiotherapy is helpful for treatment of pain associated with acute ankle sprain. Taping is considered to be a way to provide more balance and gait ability.

치료적 운동이 앞 발목 충돌 증후군 환자의 통증, 관절가동범위, 근력 및 균형능력에 미치는 영향: 증례보고 (Effects of Therapeutic Exercise on Pain, Range of Motion, Strength and Balance Ability in a Patient with Anterior Ankle Impingement: A Case Study)

  • PURPOSE: The purpose of this study is to determine the effect of therapeutic exercise on pain, range of motion and strength, and balance ability in a patient with an anterior ankle impingement (AAI). METHODS: A-32-year-old male presented limited ankle motion and pain with forced dorsiflexion at both ankle joints. In response, a therapeutic exercise program consisted of ankle joint mobilization, strength exercises using an elastic band, and proprioceptive exercises including semi-squats and a one-legged standing exercise with open and closed eyes. The program was performed for 40 min/day, twice per week, for 8 weeks. Pain, range of motion (ROM), and muscle strength (ankle dorsiflexion, plantarflexion, inversion, and eversion), as well as a one-legged standing test of both ankles, were measured before and after 4 and 8 weeks of therapeutic exercise. RESULTS: VAS decreased in both ankles after 8 weeks of exercise, respectively, compared to baseline levels. Range of motion and strength increased in both ankles for dorsiflexion, plantarflexion, inversion, and eversion after 8 weeks of exercise compared to baseline levels. In addition, the ability to perform a one-legged standing test with eyes opened and closed improved in both legs after 8 week of exercise compared to baseline levels. CONCLUSION: These results suggest that therapeutic exercise improves pain, ROM, muscle strength, and balancing ability in patients with AAI.

외측측부인대 손상을 동반한 발목염좌 환자에서 균형 훈련기를 이용한 훈련이 통증 및 균형능력에 미치는 영향 (Effect of Rehabilitation with Balance Trainer Machine on Pain and Postural Stability after Ankle Sprain)

  • Background: Instability due to ankle sprains will be accompanied by a problem of balance and pain change. Balance trainer is used to improve the ankle strength and balance ability. The purpose of this study was to evaluate the change of pain and postural balance ability in ankle joint after balance trainer application in patients with ankle sprain and instability. Methods: Twenty patients in K hospital in Incheon were enrolled. Balance trainer was applied to 10 subjects in the experimental group and 10 subjects in the Balance cushion under the same conditions as the experimental group to compare the pain and balance ability. Results: In the experimental group, there was a significant difference in the change of the pain variation. In the postural balance ability comparison, there was a significant difference in total and post - posterior comparison compared to the control, but there was no significant difference in the postural balance ability comparison. Conclusion: Pain and postural balance ability of patients with instability due to ankle sprain improved the pain and balance ability of the Balance trainer group compared to the Balance cushion training group.

흰쥐의 급성 발목삠에서 테이핑요법이 cFos 단백의 발현과 통증억제에 미치는 영향 (The Effect of Taping Therapy on the Expression of cFos Protein and Pain Suppression in Acute Ankle Sprain in Rats)

  • Objectives : The purpose of this study was to evaluate effects of taping therapy on recovery of behavioral symptoms and neural excitability of the lumbar spinal cord in rat model for ankle sprain. Methods : Adult Sprague-Dawley rats was used and divided into 3 experimental groups: normal group(n=6), ankle sprain(n=6), and ankle sprain with taping treatment(n=6). In order to induce ankle sprain the right ankle joint was injured with 4~5 repetitive over-flexions and over-extensions manually. The severity of joint pain was evaluated by measuring foot weight bearing force ratio(FWBRF) of the hind limb and the injury-induced edema formation by diameter of the joint following ankle sprain. The changes of neural excitability in the lumbar spinal cord was tested by observation of cFos protein expression, a metabolic marker for neural excitation. Results : Severity of ankle injury induced in this experiment coincided with Grade 1 ankle sprain. Compared with ankle sprain group, ankle sprain+taping showed a significant reductions of joint pain as well as of edema formation at the ankle joint following ankle sprain. There was significant upregulation of cFos-immunoreactive neurons in the lumbar spinal cord 24 hours after ankle sprain. In contrast, taping therapy resulted in significant inhibition of cFos-immunoreactive neurons in the lumbar spinal cord. Conclusions : Collectively, these results suggest that taping therapy may be an alternative therapeutic intervention for symptom recovery of the mild ankle sprain.

  • Background: Task-oriented training on an unstable support surface is an effective intervention for improving the ankle joint stability and muscle strength in patients with ankle instability. This study examined the effects of balance training on an unstable support surface in patients with ankle sprains with ankle instability. Methods: Forty-four patients with ankle sprains participated in this study. Screening tests were performed and assigned to an experimental group, who performed task-oriented training on an unstable support surface (n=22), and a control group, who performed task-oriented training on a support surface (n=22) using a randomization program. All interventions were applied 3 times per week for 4 weeks. The numeric rating scale (NRS), cumberland ankle instability tool (CAIT), balance ability, muscle activity, and muscle thickness were compared to evaluate the effects of the intervention. Results: Both groups showed significant differences in the NRS, CAIT, balance ability, and muscle activity between before and after the intervention (p<.05). In addition, there were significant differences in balance ability, muscle activity, and muscle thickness between the experimental and control groups (p<.05). Conclusion: Task-oriented training on an unstable support surface is an effective intervention for improving the balance ability, muscle activity, and muscle thickness during contraction.

볼기근 근력강화 운동과 병행한 발목관절 펌핑 운동이 엉덩관절 전치환술 환자의 허벅지 둘레, 보행능력, 통증 수준에 미치는 영향 (Effect of Adding Ankle-Joint-Pumping Exercise to Gluteal-Muscle-Strengthening Exercise on Thigh Swelling, Gait ability and Pain level in Patients With Total Hip Arthroplasty )

  • PURPOSE: This study was conducted to investigate the effect of gluteal muscle strengthening exercises (GMSE) with ankle joint pumping exercises (AJPE) on thigh swelling, gait ability, and pain level in patients who underwent total hip arthroplasty. METHODS: A total of 38 patients who had undergone total hip replacement surgery >1 week prior participated in this study. Participants were randomly assigned to a group that performed only GMSE (CG; n = 19) and a group that performed GMSE and AJPE (EG; n = 19). The CG group performed GMSE for 30 min, and the EG group performed GMSE for 30 min followed by AJPE for 15 min. Exercises were performed five times a week, for a total of 20 times over 4 weeks in both groups. Thigh swelling (thigh size), 10 m walking test (10MWT) and timed up and go test (TUG) results, pain level (visual analogue scale, VAS) scores, Short Form 36 health survey (SF-36), and hip outcome scale (HOS) scores were evaluated before and after the intervention. RESULTS: After 4 weeks of intervention, significant differences were observed in the thigh size, 10MWT, TUG, VAS, SF-36, HOS before and after intervention in both groups (p < . 05). However, only thigh size showed a significant interaction between group and measurement time (p < . 05). CONCLUSION: GMSE combined with AJPE might be effective in improving the gait ability and pain level in patients with total hip arthroplasty, and GMSE may be more effective in improving thigh swelling and gait ability than GMSE without AJPE.

정강종아리 관절가동술이 외측 발목염좌 환자의 통증, 관절가동범위 및 균형에 미치는 영향 (Effects of Tibiofibular Joint Mobilization on Range of Motion, Balance, and Pain in Patients with Lateral Ankle Sprain)

  • Background: Ankle sprains are a common clinical ankle disorder and alternations in tibiofibular joint biomechanics along with the talus are thought to contribute to its occurrence. During ankle joint dorsi flexion, proper movement requires the talus to glide posteriorly. Due to the wider front of the talus head, achieving the end range of dorsi flexion necessitates both superior and posterior glide of the distal fibula and anterior glide of the proximal fibula. The purpose of this study was to investigate the effects of tibiofibular joint mobilization on pain, range of motion, and balance in patients with lateral ankle sprains. Methods: Participants were randomly assigned to a control group (n=33) or an experimental group (n=31). Both groups underwent ankle joint mobilization three times a week for two weeks. Additionally, the experimental group received proximal and distal tibiofibular joint mobilization three times a week for two weeks. Measurements were obtained pre-intervention and post-intervention (after 2 weeks). Results: Evaluation parameters included the visual analog scale score (VAS), range of motion (ROM), and one-leg standing test (OLS). Post-intervention, both groups demonstrated significantly improved results for all assessments (p<.01). A significant intergroup difference was observed only in the ROM (p<.01) and OLS (p<.05). Conclusion: Our findings suggest that tibiofibular joint mobilization, combined with ankle joint mobilization, may be beneficial in enhancing outcomes for individuals with lateral ankle sprains.

원발성 하지정맥류의 임상적 고찰 - 209례 보고 - (Clinical Analysis of Primary Varicose Vein - review of 209 cases -)

  • 배경 : 하지정맥류는 주변에서 흔히 볼수 있는 혈관 질환으로 흉부외과 영역에서도 최근 관심의 대상이 되고있다. 대상 및 방법 : 본 연구는 1999년 4월부터 2000년 12월까지 본원에서 치료한 하지정맥류 환자 209명 혹은 269족의 임상기록을 후향적으로 연구 관찰하였다. 결과 : 남자는 52명, 여자는 157명으로 1:3의 비율이었고, 연령은 평균 42.2$\pm$9.7세 이었으며, 평균 발생기간은 12.2$\pm$9.7년이었고, 평균 추적간은 2001년 7월을 기준하여 평균 14.8$\pm$6.1개월이었다. 증상은 하지 통증이 122례(58.4%)로 가장 많았다. 장시간 서있는 직업(44명), 임신(37명), 가족력이 있는 경우(7명) 및 이들의 조합이 주원인으로 전체의 79.9%를 차지하였다. 주병소는 대복제정맥까지 침범한 경우가 126례, 소복제 정맥까지 침범한 경우가 18례, 망상혈관 및 모세혈관확장증만 있었던 경우가 65례 있었다. 치료는 대복제정맥 발거,분절 절제술, 대복재정맥-대퇴정맥 연결부위 결찰, 혈관 경화요법 및 보존적 치료를 시행하였다. 대복재정맥을 침범한 경우 발거를 시행한 군(A군)과 혈관경화요법을 시행한 군(B군)을 비교했을때, 시술후 합병증은 A군에서 많았고(p 0.05), 재발은 B군에서 많았다(p 0.05). 소복재정맥까지 침범한 경우 stab avulsion을 시행한 경우(C군)와 혈관 경화요법을 시행한 경우(D군)의 비교시 D군에서 2례의 재발이 있었으나 합병증 및 재발에서 통계적 차이는 없었다. 혈관 경화요법에서 대복재정맥을 침범한 경우(B군)와 망상정맥 및 모세혈관확장증이 있는 경우(E군)의 비교시 합병증은 차이가없었으나(p 0.05), 재발률은 B군에서 높았다(p 0.05). 발거후 합병증으로는 발목 무감각 및 저린 느낌(2례), 발목 통증(2례), 족부부종(2례),창상감염(1례)등이 있었고, 혈관 경화요법의 경우 정맥혈전염(1례), 피부궤양(1례)등이 있었다. 결론 : 대복재정맥의 혈관경화요법은 발거술 보다 재발률이 높았지만. 망상정맥 및 모세혈관확장증에서의 혈관경화요법은 대복재정맥을 침범한 정맥류의 혈관 경화요법을 시행한 경우보다 재발률이 낮았다. 하지만, 혈관경화요법은 수술 및 입원이 필요없는 간편한 치료이기에 대복재정맥의 경우에도 더욱 연구 보완하여 적용하는 것이 필요하리라 생각된다.

미숙아에서 채혈을 위한 발뒤꿈치 천자시 두 종류 천자 기구의 비교 (A Comparative Study of Two Different Heel Lancet Devices for Blood Collection in Preterm Infants)

  • 목적: 효과적인 미숙아에서의 채혈방법을 모색하기 위해 미숙아에서 발뒤꿈치 천자시 새로운 자동화 천자기구(automatic lancet device)와 기존에 사용되던 고식적 수동식 천자침(conventional manual lancet device) 의 효과를 비교하여 임상에서의 유용성을 알아보고자 한다 방법: 재태 연령 37주 미만의 미숙아 총 100명을 대상으로 하였다. 연구 대상아들은 발뒤꿈치 천자시 무작위로 각 50명씩 고식적 수동식 천자 기구를 사용한 군(50명)과 자동화 천자 기구를 사용한 군(50명)으로 나누었으며, 발뒤꿈치 천자시 통증의 정도 및 천자 횟수, 검사 시간, 출혈의 여부 등을 평가하였다. 통증의 평가는 Premature Infant Pain Profile (PIPP) 점수와 최고 심장 박동수를 측정하였고, 검사시간은 발뒤꿈치 천자 시작으로부터 1개의 microtube에 채혈을 완료하는 시간까지를 측정하여 비교하였다. 천자횟수는 1개의 microtube를 혈액으로 채우기 위해 발뒤꿈치를 천자한 횟수를 측정하였다. 그리고 검사 12시간 이내에 발뒤꿈치 및 발목 주위에 출혈이나 멍 등을 관찰하였다. 결과: 통증을 나타내는 PIPP 점수의 평균은 자동화 천자기구군에서 4.91로 고식적 천자기구 군 5.84에 비해 유의하게 낮게 측정되었다(P=0.0255). 검사에 소요되는 시간은 고식적 천자군에서 평균 48.92초, 자동화 천자군에서 평균 30.69초로 자동화천자기구를 사용한 군에서 통계적으로 유의하게 짧았다(P<0.0001). 천자 횟수의 평균값은, 자동화 천자군에서 1.02로, 고식적 천자군 1.35에 비해 통계적으로 의미 있게 낮았다(P=0.0001). 검사후 출혈반점이나 멍 등은 고식적 천자기구 군에서 15례(30%), 자동화 천자기구 군에서 3례(6%)로 자동화 천자기구 사용 군에서 의미 있게 적게 나타남을 관찰할 수 있었다(P=0.0024). 결론: 자동화 절개형 천자기구(BD QuickHeel)는 검사 실패율이 낮으며, 짧은 시간에 채혈이 가능하고, 멍이나 출혈 같은 부작용이 적어 신생아, 특히 최근 생존율이 향상되어 증가되고 있는 미숙아에서 발뒤꿈치 천자에 의한 채혈에 유용한 도구가 될 수 있겠다.
