자가부식 접착제의 레진 Tag 형성 (RESIN TAG FORMATION OF SELF-ETCHING ADHESIVES)

  • 본 연구는 자가부식 접착제의 치질로의 침투와 레진 tag 형성을 알아보고 부가적인 산부식이 레진 tag 형성에 미치는 효과를 관찰하기 위하여 계획되었다. 3종의 자가부식 접착제(SE bond, AQ bond and L Pop)와 one bottle adhesive(Single bond)를 사용하였고 발거한 구치로 교합면 법랑질과 상아질 시편을 제작하였으며 각 군당 5개씩 네 개의 군으로 나눈 총 20개의 시편을 양분하여 각각 접착제를 적용하기 전 35% 인산으로 산부식하거나 산부식처리하지 않았으며 이후 레진을 적용시키고 중합하였다. 시편은 Silverstone microtome으로 절단한 후 HCl 용액과 NaOCl 용액으로 처리하고 건조시킨 후 이온으로 피복하였으며 주사전자현미경으로 관찰하여 다음과 같은 결과를 얻었다. 1. 부가적 산부식 처리한 상아질은 모든 자가부식 접착제군에서 산부식하지 않은 상아질보다 resin tag의 길이와 두께가 증가하는 것이 관찰되었다. 2. 법랑질에서는 L Pop을 제외하고 부가적 산부식한 군에서 레진 tag의 두께가 산부식하지 않은 군보다 컸고 보다 명확한 부식양상을 관찰할 수 있었다. L Pop에서는 부식법랑질과 비부식법랑질간의 차이가 없었다. 본 연구의 결과는 자가부식형 접착제가 법랑질을 부식시키지 않고 상아세관으로 침투하는 정도가 산부식군에 미치지 못하다는 것을 의미한다. 이와 관련하여 접착강도와 미세누출 및 레진 tag 형태에 대한 부가적 연구가 필요할 것이다.

Fluid transport model을 이용한 치근단 역충전 재료의 혈액오염시 미세누출평가 (EVALUATION OF MICROLEAKAGE WITH RETROGRADE FILLING MATERIALS IN BLOOD CONTAMINATION USING FLUID TRANSPORT MODEL)

  • Leakage studies have been performed frequently, since a fluid-tight seal provided by various dental fill-ing materials has been considered clinically important. The leakage of the various root-end filling materials has been widely investigated mostly dye penetration method. These dye studies cannot offer any information about the quality of the seal of a test material over a long period of time The purpose of this study was to evaluate the microleakage of root end cavities in blood contamination filed amalgam, intermediate restorative material(IRM), light cured glass ionomer cement(GI) and mineral trioxide aggregate(MTA) by means of a modified fluid transport model. Fifty standard human root sections, each 5mm high and with a central pulp lumen of 3mm in diameter, were and filled with our commonly used or potential root end fill ing materials after they were contaminated with blood. At 24h. 72h, 1, 2, 4, 8, and 12 weeks after filling, leakage along these filling materials was determined under a low pressure of 10KPa(0.1atm) using a fluid transport model. The results were as follows : 1 MTA group showed a tendency of decreasing percent of gross leakage (20m1/day) in process of time, whereas the other materials showed a tendency of increasing in the process time. 2. At the all time interval, GI group leaked significantly less than amalgam group and IRM group (p<0.05). 3. At the 4 weeks, the percentage of gross leakage in MTA group decreased to 0% thereafter, the low per-centage of gross leakage was maintained in MTA group until the end of the experiment, whereas the percentage in IRM group increased to 100% 4. At the 12 weeks, percentage of gross leakage was significantly low in MTA group(0%), comparison with GI group(40%), amalgam group(90%) and IRM group(100%), but there was no significant difference between latter two materials.

상아질 접착제의 미세누출과 변연부 혼화층 (MICROLEAKAGE AND MARGINAL HYBRID LAYER OF DENTIN ADHESIVES)

  • The purpose of this study was to compare microleakage and marginal hybrid layer in class V restorations using two one-bottle adhesives and one self-etching adhesive. Class V cavity preparations with occlusal margins in enamel and gingival margins in dentin were pre-pared on buccal and lingual surfaces of 30 extracted human molar teeth. Prepared teeth were randomly divided into three treatment groups (n=30) and restored with three adhesives and composites: Single Bond/Filtek Z-250 (Group 1), Prime&BondNT/Esthet.X (Group 2), UniFil Bond/UniFil F (Group 3). For microleakage, samples were stored in room temperature water for 24 hours, thermocycled stained with 2% methylene blue dye, sectioned into halves, scored and analysed using Mann-whitney test and Wilcoxon signed rank sum test. For marginal hybrid layer, samples were sectioned into halves, treated with 10% phosphoric acid for 5 seconds, stored in 5% NaOCL solution for 24 hours, dried and gold coated. Occlusal and gingival margins of each sample were inspected under SEM. The results of this study were as follows ; 1. Microleakage at the occlusal margins was not evident in group 1 and group 2, but it showed in group 3 (p<0.05). 2. Microleakage in group 1 and group 3 was significantly lower than in group 2 at gingival margins (p<0.05). 3. Microleakage at gingival margins was greater than at occlusal margins in group 1 and group 2, but microleakage at occlusal margins was greater than at gingival margins in group 3 (p<0.05). 4. In group 1 and group 2, no gaps at occlusal margins showed. But gaps showed in group 3. Occlusal margins were free from a hybrid layer in all groups 5. The thickness of the marginal hybrid layers was 2.5~5 $\mu\textrm{m}$ thick in group 5 $\mu\textrm{m}$ thick in group 2 and 1.5 $\mu\textrm{m}$ thick in group 3. 6 There was no corelation between microleakage and thickness of marginal hybrid layer. In coclusion, the effect of dentin adhesives on microleakge in class V composite restorations was excellent when one-bottle adhesives were applied on enamel margin, and it was good when a self-etching adhesive was applied on dentinal margin. There was no corelation between microleakage and thickness of marginal hybrid layer.

자가 산부식 프라이머와 자가 산부식 접착제의 변연 미세누출 (MARGINAL MICROLEAKAGE OF SELF-ETCHING PRIMER ADHESIVES AND A SELF-ETCHING ADHESIVE)

  • This study evaluated the microleakage performance of four self-etcing primer adhesives(Clearfil SE Bond, Clearfil Liner Bond 2, UniFil Bond, and FL Bond) and one self-etching adhesive(Prompt L-Pop). Class V cavity preparations with occlusal margins in enamel and gingival margins in dentin were prepared on both buccal and lingual surfaces of 50 extracted human molar teeth. Prepared teeth were randomly divided into five groups and restored using one of five adhesives and composite resins: Prompt L-Pop/Filtek Z 250(Group 1), Clearfil SE Bond/Clearfil AP-X(Group 2), Clearfil Liner Bond 2/Clearfil AP-X(Group 3), UniFil Bond/UniFil F(Group 4), and FL Bond/Filtek Z 250(Group 5). Following one day storage in room temperature water, the restored teeth were thermocycled for 500 cycles between 5$^{\circ}C$ and 55$^{\circ}C$. Marginal microleakage was assessed by dye penetration using 2% methylene blue dye. After 24 hours, the teeth were sectioned longitudinally and evaluated for microleakage under steromicroscope The data were statistically analysed by Kruskal-Wallis Test, Mann-Whitney and Wilcoxon signed ranked tests. The results of this study were as follows ; 1. The microleakges at both enamel and dentinal mal$.$gins were the lowest in group 4. increasing among groups in the following order: group 2, follwed by group 5, follwed by group 1, and the highest in group 3. 2. At the enamel margins, the microleakage of group 3 was significantly higher than those of groups 2. 4 and 5(p<0.05), and also the microleakage of group 1 was statistically higher than those of groups 2 and 5(p<0.05). 3. At the dentinal margins, microleakage of group 3 was signincantly higher than microleakages of groups 1, 2, 4 and 5(p<0.05). 4. Compared with microleakages between the enamel and dentinal margins of each group, groups 1, 4 and 5 at enamel margin and group 2 and group 3 at dentinal margin were higher microleakage. But there was no significant difference between enamel and dentinal microleakages of each group(p>0.05).

구치부 제2급 와동에서 고흐름성 복합레진을 이용한 적층 충전술식의 미세누출에 미치는 영향 (THE EFFECT OF SANDWICH TECHNIQUE USING FLOWABLE COMPOSITE RESIN BASE ON THE MICROLEAKAGEIN CLASS II CAVITIES OF MOLAR)

  • The aim of this study was to evaluate the marginal adaptation of direct class II sandwich restoration with packable composites(P-60), resin modified glass ionomer cement(Fuji-II LC), flowable compomer(Dyract Flow), flowable composites(Filtek Flow) in comparison with total bond restorations. In addition, for sandwich restorations, influence of different sandwich techniques was also evaluated. Large butt-joint box typed class II cavites with cervical margins 1mm below the cemento-enamel junction were cut into 70 extracted human molars. The cavities(7 groups, n=10) were filled using a closed/open sandwich restoration or total bond restoration technique with materials according to the manufacturer's recommandation using the single-component bonding agent for each system. Teeth were thermocycled 500 times between 5$^{\circ}C$ and 55$^{\circ}C$ with 30-second dwell time. The teeth were then coated with nail polish 1mm short of the restoration, placed in a 2% methylene blue for 24 hours, and sectioned with diamond wheel. Sections were examined with a stereoscope to determine the extent of microleakage. Dentine /Cementum margins were analyzed for microleakage on scale of 0(no leakage) to 4(entire axial wall) and interface between materials, on scale of 0(no leakage) to 3(axial wall). Results were evaluated with Kruskal Wallis Test, corrected for ties, to determine whether there were statistically significant differences among the seven groups. Pairs of groups were analyzed using the Student-Newman-Keuls Method and Dunn s Method. The results were as follows : 1. All groups showed some micoleakage in cervical portion. But there were no microleakage in interface between materials. 2. Closed sandwich restorations with Fuji-II LC and Filtek Flow had significantly lower leakage rating than total restorations with only P-60. However, open sandwich restorations with Dyract Flow showed significantly higher (P<0.05) 3. Closed sandwich restorations had significantly lower leakage rating than total restorations. However open sandwich restoration s showed significantly higher (P<0.05). 4. Sandwich restorations with Fuji-II LC were iou$.$or leakage than only P-60. Filtek Flow, Dyract Flow. But there were no statistically differences among the materials. From the results above, it could be concluded, closed sandwich restorations was effective in reducing microleakage of class II restorations. The best results showing the least microleakage were for the closed sandwich technique with Fuji-II LC and Filtek Flow.

IRM 임시수복이 상아질 접착제의 변연 미세누출에 미치는 영향 (THE INFLUENCE OF IRM TEMPORARY RESTORATIONS ON MARGINAL MICROLEAKAGE OF DENTIN ADHESIVES)

  • This study investigated the influence of IRM on marginal microleakage of 5th generation adhesives. Class V cavities with gingival margins in dentin were prepared on both buccal and lingual surfaces of 60 extract-ed human molar teeth. Prepared teeth were randomly divided into six groups. Group 1 and 4 received no temporary restoration with IRM. Group 2 and 5 were covered with IRM mixed at P/L ratio(10g/1g). Group 3 and 6 were covered with IRM mixed at P/L ratio(10g/2g). The temporary restorations were removed mechanically with an ultrasonic scaler after one-week storage in distilled water. The cavities were restored using one of two adhesives and composites ; Single Bond/Filtek Z 250(Croup 1, 2 and 3), UniFil Bond/UniFil F(Group 4, 5 and 6). Following one day storage in distilled water, the restored teeth were thermocycled for 500 cycles(between $5^{\circ}C{\;}and{\;}55^{\circ}C$) and immersed in 2 % methylene blue for dye penetration testing. The results were analysed using Kruskal-Wallis Test, Mann-Whitney and Wilcoxon signed ranked test at a significance level of 0.05. The results of this study were as follows 1. Ranking of mean microleakage scores at the enamel margins was Group 10.05) 4. At the dentin margins, the microleakage of the group not pretreated with IRM was lower than that of the group pretreated with IRM. And the microleakage of UniFil Bond was lower than that of Single Bond. 5. Compared with microleakages between the enamel and dentin margins of each groups, Group 1, 2, 3, 4, 5 and 6 at dentin margin were higher microleakage than those at enamel margin. There were significant difference between enamel and dentin microleakage of Group 2 and 3(p<0.05).

난황 유래 인지철로 합성한 리포좀의 물리적 특성과 안정성 (Physical Property and Stability of Liposome Prepared from Egg Yolk Phospholipids at Various Storage Conditions)

  • 난황 유래의 인지질을 주원료로 리포좀을 제조하여 서로 다른 저장조건에서의 리포좀의 물리적인 안정성을 연구하였다. 리포좀 용액의 pH를 달리하여 저장한 후 리포좀 입자크기와 리포좀 용액의 제타전위와 형광도를 측정한 결과, 리포좀은 pH 1-2에서 입자크기의 증가를 나타내었고 pH 4 이하에서 제타전위의 절대값이 감소하였으며 리포좀의 내부에 포집한 수용성물질의 손실량 지표인 형광도의 측정치 또한 pH 2-4에서 급격하게 증가한 것으로 보아 산성의 저장조건에서는 리포좀이 불안정함을 추론할 수 있다. 저장 온도별 리포좀의 안정성 평가에서는 $4^{\circ}C$에서 저장하였을 때 10일째부터 입자의 크기 변화가 있었고 20, $35^{\circ}C$에서 저장할 경우, 저장기간 40일째 이후부터 입자의 크기변화가 있었다. 제타전위는 3가지 온도조건에서 동일하게 저장 말기에 측정값의 절대값이 미세하게 감소하는 경향을 보였다. 또한, $50^{\circ}C$에서 저장된 리포좀의 형광도 측정실험에서 포집된 SRB의 누출량이 급격히 증가하는 양상을 나타내었다. 따라서 pH 6 이하의 산성의 조건에서 리포좀은 불안정하며 $50^{\circ}C$와 같이 높은 온도에서 인지질로 구성된 리포좀의 안정성을 유지하기 어렵다는 것을 알 수 있었다. 본 연구의 결과는 난황 유래 인지질로 구성된 리포좀을 이용한 진단용 시약 개발 및 포집 특성 연구에 유용한 기초 자료로 활용될 수 있을 것이다.

Formocresol에 의한 치조골 괴사의 치험례 (NECROSIS OF ALVEOLAR BONE BY FORMOCRESOL : CASE REPORT)

  • 유치의 치수절단술에 사용되는 약제로는 formocresol, ferric sulfate, 수산화칼슘 등이 있다. 그 중에서 formocresol은 치수조직의 고정능력과 높은 임상적 성공을 보임으로써 아직까지 널리 사용되고 있으나 자체독성이 매우 강하여 영구 계승치 치배의 변위나 결손, 법랑질 형성부전, 전신 흡수로 인한 돌연변이 가능성과 암유발 가능성 역시 보고되고 있다. 최근에는 formocresol의 근관내 적용시 불완전한 임시충전에 의한 미세누출로 주위 치은과 치조골의 괴사를 야기한 보고가 있으며 과잉의 formocresol을 적용하였을 경우에도 치조골의 괴사의 원인이 될 수 있다고 하였다. 본 증례는 개인치과에서 formocresol을 이용한 치수치료를 받은 후 지속되는 동통으로 인해 상악 좌측 제 2유구치를 발거하였으나, 증상이 개선되지 않아 본원에 의뢰되었다. 환아는 8세 남아로 심한 구취 및 계승 영구치 치관 주위로 환형의 치조골 괴사를 보여 부골절제술을 시행하고 1년 7개월간 관찰을 하였을 때 정상적인 치근 성장은 이루어졌으나 결손된 치조골 및 부착치은의 완전한 조직재생 소견은 관찰되지 않았다. Formocresol에 의한 치주조직에 대한 부작용은 비가역적이므로 formocresol의 사용에 있어 적응증 여부를 잘 판단해야 하며, 철저한 임시 가봉에 주의를 기울여야 할 것이다.

치면열구전색제의 도포 시 전색제에 따른 기포 발생 양태 (BUBBLE BEHAVIOR OF PIT AND FISSURE SEALANT)

  • 치면열구전색제의 도포 시 발생하는 기포는 전색제의 두께 감소에 의한 강도의 저하로 파절 및 마모를 일으킬 수 있으며 특히 계면부에 발생한 경우 미세 누출 등의 임상적인 문제점을 가져온다. 본 조사에서는 치면열구전색제 중 Teethmate $F-1^{(R)}$(Kuraray, Japan), Ultraseal $XTplus^{(R)}$(Ultradent, USA), $Clinpro^{(R)}$(3M-ESPE, USA), $Helioseal^{(R)}$(Ivoclar-Vivadent, Liechtenstein)의 기포 발생 양태를 알아보고자 하였다. 자연치 교합면의 열구를 모방한 표준화된 시편을 제작하고 치면열구전색제를 각 군별 20개의 시편에 전색하고 광중합시킨 후 열구 중앙을 절단하여 stereoscope(Olympus SZ61, Japan)으로 확대하여 관찰하였고 컴퓨터 영상분석 프로그램(Image Pro plus Express, Mediacybermetics Co., U.S.A)을 통하여 기포의 발생 양태를 관찰하여 다음과 같은 결론을 얻었다. 1. 기포의 발생 빈도는 Ultraseal $XT^{(R)}$ plus에서 사용 시간과 관계없이 $Helioseal^{(R)}$에 Clinprotip과 metal canula tip을 적용한 경우보다 더 높게 나타났다(p<0.05). 2. 기포의 단면적 당 개수는 Ultraseal $XT^{(R)}$ plus old가 $Clinpro^{(R)}$, $TeethmateF-1^{(R)}$, $Helioseale^{(R)}$에 brush tip을 적용한 경우보다 많이 발생하였다(p<0.05). 3. 기포의 평균 단면적은 Teethmate $F-1^{(R)}$가 다른 군에 비해 큰 값을 보였다(p<0.05). 4. $Clinpro^{(R)}$와 적용 tip을 달리한 $Helioseal^{(R)}$의 경우 기포의 발생 빈도, 평균 개수, 평균 단면적 모두 유의차를 보이지 않았다(p>0.05).

제 2급 와동에서 구치부 복합레진의 미세변연누출에 관한 연구 (MICROLEAKAGE OF POSTERIOR PACKABLE COMPOSITE RESIN hi THE GINGIVAL MARGINS OF CLASS II CAVITIES)

  • The use of flowable composite resins as liners in class II packable composite restoration has been suggested by some manufacturers. However, the contributions of this technique are unproven. The purpose of this study was to compare the gingival microleakage in class II packable composite restorations with or without the use of flowable composite resins as liners. Slot cavities were prepared on both proximals of 80 extracted human molars and randomly assigned to 8 groups of 20 each. The gingival margins were located at 1mm above CEJ in 80 cavities (group1-4) and 1mm below CEJ in 80 cavities (group5-8). The prepared teeth were mounted in the customized tray with adjacent teeth to simulate clinical conditions and metallic matrix band (Sectional matrix) and wooden wedges were applied. After acid etching and application of Single Bond, each group was restored with the following materials using incremental placement technique: Group 1,5 (Filtek P60), group 2, 3, 4 and group 6, 7, 8 (AeliteFlo, TetricFlow, Revolution/ Filtek P60). All specimens were thermocycled 500 times between 5$^{\circ}C$ and 55$^{\circ}C$ with 1 mimute dwell time, immersed 2% methylene blue dye for 24 hours and then rinsed with tab water. The specimens were embedded in clear resin and sectioned longitudinally through the center of restoration with a low speed diamond saw. Dye penetration at gingival margin was viewed at 20 magnification and analyzed on a scale of 0 to 4. Kruscal-Wallis One way analysis and Mann-Whitney Rank sum test were used to analyze the results. The results of this study were as follows. 1. The leakage values seen at the enamel margin were significantly lower than those seen at the dentin margin(P<0.05). 2. On the enamel margin, packable composite resins with flowable liners showed lower leakage than those without flowable liners, but there were no significant differences among the four groups(P>0.05). 3. On the dentin margin, four groups demonstrated moderate to severe leakage, and there were no significant differences in leakage values(P>0.05).
