Clinical Characteristics of Patients with Taste Disorders (미각 장애 환자의 임상적 특성에 관한 연구)

  • There is tremendous variability in the ways patients present with taste problems. Because of complex and multifactorial etiological background, it is not simple to evaluate patients with taste disorders. Accurate assessment of patients' status by prudent, thorough history taking and symptom analysis is the most essential for exact diagnosis of taste disorders. The aim of this study was to investigate the clinical characteristics of patients with taste problems as a primary complaint. Consecutive series of 50 patients (12 males and 38 females, mean age $53.6\;{\pm}\;14.7$ years) were included for the present study. All subjects were requested to complete a comprehensive questionnaire. Clinical evaluation procedures included oral examination, interview, questionnaire analysis, panoramic radiography, blood test and measurement of salivary flow rate. The obtained results were as follows: 1. Among the patients, 36 patients (72%) complained of oral mucosal pain or burning sensation. Of these patients, 18 patients (36%) were diagnosed as burning mouth syndrome. 2. Nineteen patients (38%) complained of subjective oral dryness. The flow rate of unstimulated whole saliva was less than 0.1 mL/min in 14 patients (28%) and 17 (34%) had a stimulated whole salivary flow rate of less than 0.5 mL/min. 3. Among the types of taste disorders, hypogeusia, the most frequently reported, was found in 25 patients (50%), dysgeusia in 18 patients (36%), phantogeusia in 15 patients (30%), hypergeusia in 10 patients (20%), and ageusia in 5 patients (10%). Nineteen patients (38%) reported more than one type of taste disorder and the most frequent combination was dysgeusia + hypogeusia (n=6, 12%). 4. Based on data from the medical and dental histories and examinations, the patients were assigned to 12 probable causal categories. Taste disorders due to oral mucosal diseases and idiopathic taste disorder were the most frequent (n=9; 18%, each), followed by psychogenic taste disorder (n=8; 16%), drug-induced taste disorder (n=7; 14%), and taste disorder due to dry mouth (n=6; 12%). These 5 categories of taste disorder accounted for 78% of all cases in this study.

  • Food is crucial for the nutrition and survival of humans. Taste system is one of the fundamental senses. Taste cells detect and respond to five basic taste modalities (sweet, bitter, salty, sour, and umami). However, the cortical processing of taste sensation is much less understood. Recently, there were many efforts to observe the brain activation in response to taste stimulation using functional magnetic resonance imaging (fMRI), magnetoencephalography (MEG), and optical imaging. These different techniques do not provide directly comparable data each other, but the complementary investigations with those techniques allowed the description and understanding of the sequence of events with the dynamics of the spatiotemporal pattern of activation in the brain in response to taste stimulation. The purpose of this study is the understanding of the brain activities to taste stimuli in sensory and affective aspects and the reviewing of the recent research of the gustotopic map by functional brain mapping.

Evaluation of Taste in the Patients with Recurrent Aphthous Ulceration by Electrogustometer (전기미각측정기를 이용한 재발성 아프타성 궤양 환자의 미각평가)

  • The purpose of this study was to investigate whether there were any changes in taste sensitivity with recurrent aphthous ulceration. Nintyseven subjects(25 males and 72 females) were included for the study and they were categorized into 2 groups(recurrent aphthous ulceration 27 persons, control 70 persons), recurrent aphthous ulceration group was investigated in the department of Oral Medicine, College of Dentistry, Pusan National University from April, 2005 to August, 2006 and control group was investigated in the clinic at Incheon city from June, 2006 to August, 2006. The electrical taste thresholds were measured using an electrogustometer of the 4 different sites(tongue tip, tongue lateral, circumvallate papilla and soft palate) in oral cavity. The results were as follows ; 1. The electrical taste threshold showed significant lower in the RAU group. 2. The electrical taste threshold showed significant lower in female group, and showed significant lower except soft palate in male group of the RAU group. 3. The electrical taste threshold showed a tendency to increase in all site of the multiple RAU group, but there were no significant differences. 4. The electrical taste threshold showed a tendency to increase in tongue lateral of the acute RAU group, and showed a tendency to increase in tongue tip, circumvallate papilla, soft palate of the chronic RAU group. 5. After treatment, electrical taste threshold was significant increase than initial visit in the RAU group. 6. After treatment, NAS showed a tendency to decrease in the RAU group.

  • 색채의 공감각적 특성에 기초하여 대학생들의 미각에 대한 단색 연상 색채를 조사함으로써 현 시대의 대학생들이 느끼는 미각에 대한 대표적인 공감각적 색채를 찾아내기 위한 기초 연구를 진행하였다. 연상을 활용하여 미각에 대한 색채의 공감각에 대해 기초적인 접근이 가능하도록 하였으며, 연상과의 상호관계를 통해 기본 미각의 대표컬러로써 대학생들이 선택한 단맛은 빨강과 노랑, 핑크와 초컬릿 색, 짠맛은 흰색, 회색, 빨강과 검정 등이 추출되었다.

Changes of Taste Threshold after Smoking in Young Korean Men (한국인 청년 남성에서 흡연에 의한 미각역치의 변화)

  • The aim of this study is to determine a relationship between the smoking and the gustation. For that purpose, the survey is about the subjective taste change after smoking compared the electrical taste thresholds by EGM on healthy young Korean men before and after smoking. The results of the electrogustometry showed significant differences between smokers and non-smokers, also between before smoking and directly after smoking in smokers showed significant differences which indicated the immediate, but temporary effects of smoking on the taste thresholds. There were no difference of gustatory function by the amount of smoking and the degree of addiction. Based on the results of this study, further studies will be focused on the difference of gustatory function between before and after smoking using the chemical taste test, and the relationship between change of olfactory and gustation by smoking.

Suprathreshold Taste Intensities for Sucrose, NaCl, Citric Acid, and Quinine HCl in Young Koreans and the Influence of Sex, Taste Preference, and Smoking (청년 한국인의 초역치 미각강도에 대한 성, 미각기호 및 흡연의 영향)

  • The aim of this study was to measure the suprathreshold taste intensity for sucrose, NaCl, citric acid, and quinine HCl in Korean young people using a whole-mouth, sip-and-spit procedure, employing the method of magnitude matching. The results were analysed in terms of sex, taste preference, and smoker or nonsmoker. One hundred twenty three subjects (61 male and 62 female, mean age of 25.2$\pm$2.1 years) were included for the study. Subjects were instructed to give nonmodulus magnitude estimates to the intensities of five concentrations each of sucrose, NaCl, citric acid, and quinine HCl; distilled water; 6 loudness levels of a 1,000-Hz tone, using the same 9-point intensity scale. Each of the 21 taste stimuli and 6 auditory stimuli are presented in random order twice. The auditory function is used to assess the absolute intensity function of the subject's taste system. The results were as follows; 1. The perceived taste intensity for sucrose, NaCl, citric acid, and quinine HCl increased progressively as the concentrations of taste solutions were increased. Most of the subjects are standing within the zone of mean value $\pm$1standard deviation in their total perceived intensity score, and nobody is outside the limit of mean value - 2standard deviation. 2. There were not significant difference in total perceived intensities for sucrose, NaCl, citric acid, and quinine HCl between males and females. 3. There were not significant difference in total perceived intensities for sucrose, NaCl, citric acid, and quinine HCl according to the difference in taste preference. 4. There were not significant difference in total perceived intensities for sucrose, NaCl, citric acid, and quinine HCl between smokers and non-smokers in males.

Pain Threshold & Taste Threshold Variations across the Menstrual Cycle (월경주기에 따른 동통역치 및 미각역치의 변화)

  • 이 연구의 목적은 월경주기에 따른 동통역치와 미각역치의 변화를 평가하고자 하는 것이다. 안면부에 동통성 질환이 없고 정상 월경 주기를 가진 건강한 20대 여성 11명이 이 연구에 참여하였다. 2회의 연속적인 월경주기 동안, 월경기(menstrual phase), 여포기(follicular phase), 황체기(luteal phase)의 세 시기에서 압력통각역치, 전기미각역치, NaCl 미각역치를 각각 측정하였다. 각 역치를 월경주기의 시기별로 비교 분석하여 다음과 같은 결과를 얻었다. 압력통각역치와 전기미각역치는 월경주기의 시기에 따라 유의한 차이를 나타내었고, 황체기에서 월경기에서와 비교하여 통계적으로 유의하게 높은 역치를 나타내었다. NaCl 미각역치는 월경주기에서 통계적으로 유의한 변화를 나타내지 않았다.

뇌기능 자기공명 영상장치를 이용한 인간 미각에 대한 기초 연구

  • 목적: 본 연구의 목적은 미각자극에 대한 인간 뇌의 활성화 영역을 측정하고 가시화하여 미각의 뇌 활성화 부위에 대한 기초자료를 마련하는데 있다. 대상 및 방법: 5명의 남자 대학생 (평균 24.8세)을 뇌기능 연구의 실험 대상자로 선정하였고, 대상자들은 미각에 이상이 없고 구강수술 경험이 없는 오른손잡이를 대상으로 3번에 걸쳐 EPI 혈액산 소농도의존(blood oxygen level dependent)법을 이용하여 미각자극에 의한 기능적 자기공명영상 실험을 수행하였다. 미각 자극을 위하여 5%의 saline을 사용하였으며 자체적으로 제작한 Stimulator를 사용하여 자극하였다. 자극은 3회의 휴식기간과 2회의 자극기간에 걸쳐 시행하였으며, 각 자극기간은 30초씩 5초 간격으로 이스 당 42개의 영상을 획득했다. Post-processing은 SPM99 (Statistical Parametric Mapping 99, The Wellcome Department of Cognitive Neurology, Oxford 1999)의 correlation법을 사용하여 threshold 0.4∼0.7의 범위에서 통계처리 하였으며, 활성화 영상은 EPI영상과 같은 부위의 T1 강조영상에 overlapping시켰다. 이렇게 얻어진 fMRI 영상으로 활성 영역의 위치를 분석하였다.

ROLE OF NITRIC OXIDE AND DISTRIBUTION OF NITRIC OXIDE SYNTHASE IN THE GUSTATORY SYSTEM (미각계에서 산화질소의 역할과 산화질소 합성효소의 분포)

  • 말초 미각계 및 중추 미각계에서 산화질소의 역할과 그것의 합성효소의 존재는 아직 규명되지 않고 있다. 본 연구는 말초미각계인혀와 미각구심성신경 그리고 중추미각계인 뇌간고속핵에서 산화질소 합성효소의 분포 및 면역조직화학 방법과 고삭신경의 extracellular recording 뇌간고속핵 절편 whole cell patch 방법으로 조사하였다. 신경성 산화질소 합성효소는 혀의 전방에 위치한 심상유두와 유곽유두에 약하게 존재하였으며 미뢰주위와 결체조직에 존재하는 신경섬유 및 혀의 상피층에 풍부하게 존재하였다. 혀에 소금물을 가하여 증가된 고삭신경의 복합전위는 산화질소 유리제인 SNP에 의해 증가되었으며 내인성 산화질소 합성효소 억제제인 L-NAME와 soluble guanylate cyclase 억제제인 ODQ에 의해 억제되었다. 문측 연수에 존재한 문측 고속핵과 진전핵에서 nNOS가 풍부하게 존재하였다. 문측 고속핵의 신경들은 안정막전위가 $-48{\pm}52mV$였고 활동전위의 크기는 $74{\pm}11mV$였다. SNP에 의해 뇌간 고속핵 신경들이 탈분극되었으며 current clamp하였을 때 활동전압의 빈도가 증가하였다. 또한 SNP에 의한 문측 고속핵의 탈분극과 활동전압 빈도증가는 L-NAME와 ODQ에 의해 감소되었다. 이상의 실험결과는 산화질소 합성효소가 혀와 뇌간고속핵에 존재하며 여기서 유리된 내인성 산화질소가 말초성 및 중추성 미각기전에 관여하리라 사료된다.
