THE DENTAL CARIES, CARIOGENIC MICROORGANISMS AND LEVELS OF SALIVARY IMMUNOGLOBULIN IN SUBJECTS WITH DOWN'S SYNDROME (다운증후군 환자의 치아우식증과 치아우식 원인균 및 타액내 면역항체의 연관성)

  • This study investigated the relationship between dental caries and various oral factors in subjects with Down's syndrome. We compared 25 subjects with Down's syndrome with 63 healthy control. The dental caries index and plaque index were examined, and the total salivary immunoglobulin A and S. mutans specific salivary immunoglobulin A concentration were measured using ELISA. The S. mutans counts, Lactobacillus counts and buffer capacity were measured with Dentocult test medium. The decayed and filled surface index of deciduous teeth in subjects with Down's syndrome was lower than in controls(p<0.001). The plaque index and total salivary immunoglobulin A concentration showed no difference, S. mutans specific salivary immunoglobulin A concentration and buffer capacity in subjects with Down's syndrome were lower than in controls(p<0.001). There was no significant difference between two groups in the S. mutans counts and Lactobacillus counts. In 9-11 year age group, S. mutans counts in subjects with Down's syndrome was lower than in controls(p<0.001) and S. mutans specific salivary immunoglobulin A concentration was lower(p<0.05). There was a high correlation among deciduous dental caries index and buffering capacity and S. mutans counts.

  • The aims of this study were to compare the status of dental caries between mental retarded persons(MR) and normal persons and investigated the association among dental caries and oral factors(plaque index, S. mutans, Lactobacillus, buffering capacity, salivary total IgA and anti-S. mutans IgA titers). DMFS index and plaque index were significantly greater in the MR person group than in the normal person group. The concentration of S. mutans-specific IgA was significantly greater in the normal person group than in the MR person group, but that of salivary total IgA was not show the significant difference. Salivary buffering capacity was significantly greater in the normal person group than in the MR person group, but the counts of salivary S. mutans and Lactobacillus were not significantly different. By age group(I: 9-11Y, II: 12-14Y, III: 15-18Y), DMFS index and plaque index were significantly greater in the MR person group than in the normal person group at III. The S. mutans counts and Lactobacillus counts were significantly greater in the normal person group than in the MR person group at I, but those were contrary at II, III. There was a high correlation among caries index and buffering capacity, level of S. mutans and Lactobacillus, plaque index at III.

RELATIONSHIP BETWEEN SALIVARY CARIES-RELATED TESTS AND DENIAL CARIES EXPERIENCE IN KOREAN DENTAL COLLEGE STUDENTS (타액 우식 관련 검사와 치아 우식 경험과의 관계에 관한 연구)

  • The aim of this study was to analyze the relationship between salivary flow, salivary buffer capacity, level of mutans streptococci and dental caries experience in Korean dental college students. A total of 81 students of the school of dentistry, Chonbuk national university, mean age of 26.1 years, were subjected to oral examination to establish the DMFT and DMFS according to WHO guidelines. Saliva samples from the students were collected for quantitating stimulated and unstimulated salivary flow rate. Buffering capacity was estimated using a commercial colorimetric strip test, and number of mutans streptococci was determined from stimulated saliva using a strip mutans test. The means of DMFT and DMFS were 6.57 and 12.65, respectively. The stimulated salivary flow rate was correlated with DMFT(${\acute{o}}\;=\;-0.219$) and high levels of salivary mutans streptococci were significantly correlated with higher DMFT and DMFS scores(P<0.05). Level of mutans streptococci was significantly correlated with dental caries experience, in both DMFT and DMFS score, and buffering capacity was inversely correlated to DMFT score. However, unstimulated salivary flow rate was not correlated with caries experience.

Clean effect of a cetylpyridinium chloride-based mouthwash on removable orthodontic appliances (염화세틸피리디늄 계열 구강세정제의 가철성 교정장치에 대한 세정효과)

  • Objectives: Cetylpyridinium chloride CPC-based mouthwashes are well known to have no harmful ingredients in the mouth and can be used for a long time. The purpose of this study was to evaluate the effect of using CPC-based mouthwashes to suppress the biofilm formation and antibiotics for handling orthodontic appliances. Methods: To measure the antibacterial effect, Streptococcus mutans (S. mutans) cultured orthodontic appliances were precipitated in Gargreen and Polident for 5 minutes, incubated at 37℃ for 24 hours(h). In order to measure the biofilm removal effect, the degree of biofilm formation on the orthodontic appliances was stained with a methylene blue and the difference before and after was compared using image J software program (NIH Image J; NIH, Bethesda, MD). Results: The viability of S. mutans according to the concentration showed that Gargreen and Polident inhibited colony formation compared to the control, respectively (p<0.01). The degree of biofilm formation was significantly higher in the control, however both Gargreen and Polident significantly reduced it compared to the before and after condition on removable orthodontic appliances (p<0.01). Conclusions: This study suggests that the use of Gargreen, a cetylpyridinium chloride based oral cleaning cleanser, could be replaced by Polident for antibacterial effect and biofilm formation on removable orthodontic appliances.

RELATIONSHIP BETWEEN DENTOCULT-SM TEST, MICROBIAL ANALYSIS AND DENTAL CARIES IN THE PRE-SCHOOL CHILDREN (미취학아동들의 유치우식경험도와 개량형 Dentocult-SM 검사 및 치면세균막 세균활성과의 관련성)

  • Dental caries has a multifactorial aetiology in which there is an interplay of three principal factors: the host(saliva and teeth), the microflora(dental plaque) and the substrate(diet), and a fourth factor time. There is no single test that takes into consideration all these factors and can accurately predict an individual's susceptibility to dental caries. The risk of dental caries can be evaluated by analysing and integrating several causative factors. The objective of the study was to clarify the relationship between caries activity and the status of caries of primary teeth and the relationship between the quantity of S. mutans in the dental plaque and the status of dental caries. Forty nine children ages 5 were examined with caries indices, Dentocult-SM and quantity of S. mutans. The results were as follows: All the survey results were similar with national means of 5-6 years Korean preschool children. The deft index of subjects was 5.46 and defs index was 9.81. There was 8.6% of negative, 10.9% of mild, 28.1% of moderate and 52.3% of active results at screening strip test. 8.6% was negative, 17.2% was mild, 33.6% was moderate and 40.6% was active in site strip test. The Spearman correlation coefficient was 0.775, and overall kappa index was 0.496 between screening strip test and site strip test. There also was a high correlation between caries incidence, Dentocult-SM test, and quantity of S. mutans. Dentocult-SM test as a caries activity test is a reliable method for measuring the status of dental caries in primary teeth.

ANTIBIOTIC SUSCEPTIBILITY IN MUTANS STREPTOCOCCI AND STREPTOCOCCUS ANGINOSUS ISOLATED FROM DENTAL PLAQUE (치면세균막에서 분리한 뮤탄스 연쇄상구균 및 Streptococcus anginosus의 수종 항생제에 대한 감수성 조사)

  • The aim of this study was to investigate the susceptibility of mutans streptococci (S. mutans and S. sobrinus) and Streptococcus anginosus, for seven antibiotics, penicillin G, amoxicillin, ciprofloxacin, cefuroxime, erythromycin, bacitracin, and vancomycin. The minimum inhibitory concentration (MIC) of seven antibiotics against 3 species (type strains) of mutans streptococci and S. anginosus, 10 strains (wild type) of S. mutans, 7 strains (wild type) of S. sobrinus, and 11 strains (wild type) of S. anginosus, were measured by broth dilution method. All of the type strains of mutans streptococci and S. anginosus had the same susceptibility for penicillin G, amoxicillin, cefuroxime and bacitracin. Type strain of S. anginosus was sensitive in ciprofloxacin, but those of mutans streptococci were not. All of the clinical isolates of mutans streptococci and S. anginosus had the same susceptibility for the seven antibiotics. Our data reveal that mutans streptococci and S. anginosus have similar antibiotic-resistant character. In addition. these results may offer the basic data to verify the antibiotic-resistant mechanism of mutans streptococci and S. anginosus.

Relationship between Streptococcus mutans levels and dental caries experience (치아우식경험과 Streptococcus mutans의 관련성)

  • Objectives : The aim of the present study was to measure the level of Streptococcus mutans(S. mutans), the major causative agent in dental caries, and to examine the relationship between S. mutans level and dental caries experience. Methods : The present study was carried out in 41 university students. S.mutans plaque was detected in the saliva using Dentocult$^{(R)}$-SM and was measured in absorbance. The data were analyzed in the relevance for number of dental caries experience. Results : The group of S. mutans value has progressive caries, which was higher than that of non-suffering caries(p<0.05). More caries experience showed higher value of S. mutans(p>0.05). According to the relationship with DMFT, level of S. mutans and absorbance, correlation with the level of salivary S. mutans is shown statistically highly significant (p<0.01). The level of S. mutans in saliva and plaque and absorbance is statistically significant(p<0.05). Conclusions : The levels of S. mutans in both group of high caries experience and progressive caries was highly detected, continuous oral health management should be carried out to prevent the possible dental caries.

Antimicrobial activity of jaborandi extract and sorbitol to oral microbes (Jaborandi extract와 Sorbitol이 구강미생물 증식에 미치는 영향)

  • Objectives : The aim of the study is to investigate the antimicrobial activity of jaborandi and sorbitol to various oral microbes. Methods : Jaborandi leaves contain pilocarpine. The conditions for extraction were optimized on the basis of substances, temperatures and pHs. Total pilocarpine content after extraction was analyzed by HPLC. The herbal antimicrobial activity of jaborandi and sorbitol were evaluated for oral microbes containing ATCC 25175 S. mutans, ATCC 13419 S. salivarius, ATCC 6249 S. mitis, ATCC 33398 S. equi, ATCC 29213 S. aureus, ATCC 14053 C. albicans. Results : The optimum conditions for highest yielding extraction were pilocarpine content after boiling at $100^{\circ}C$ for 1 hour at pH 3. The level of total pilocarpine content was analyzed at 833 mg/kg by HPLC. The most effective antimicrobial activity was obtained by combination of pilocarpine and sorbitol rather than pilocarpine, menthol and sorbitol, respectively. Conclusions : This results supported the preventive oral health care using safe and convenient jaborandi herb.

Antimicrobial Effect of Oleanolic Acid and Ursolic Acid against Streptococcus downei (Oleanolic acid 및 Ursolic Acid의 Streptococcus downei에 대한 항균작용)

  • Oleanolic acid (OA) and ursolic acid (UA) are triterpenoid compound present in many plants. This study examined the antimicrobial activity of OA and UA against Streptococcus downei. The antimicrobial activity was evaluated by the minimal inhibitory concentration (MIC) and time kill curves. The MIC values of OA and UA for S. downei isolated from the Korean population were $8{\mu}g/ml$. OA and UA had a bactericidal effect on S. downei ATCC $33748^T$ above $2{\times}MIC$, $16{\mu}g/ml$ and $8{\mu}g/ml$, respectively. The results suggest that OA and UA can be used in the development of oral hygiene products for the prevention of dental caries.

Identification of mutans streptococci isolated from dental plaque between the bracket and tooth surface in orthodontic patients (교정환자의 브라켓과 치아 경계부에 존재하는 치면세균막내 mutans streptococci 종 및 생물형의 식별)

  • The aim of this study was to compare the species and biotypes of mutans streptococci isolated from dental plaques sampled from the interfaces between the bracket aid tooth surface and smooth tooth surfaces In orthodontic patients. Dental plaque was collected from the interfaces between brackets aid teeth (test group), and from smooth tooth surfaces distant from brackets by more that 2mm (control group). The dental plaque collected by a sterilized curette was transferred into a vial of 1 X PBS. The sample in the vial was vigorously vortexed for 1 min and plated ou mitis-salivarius bacitracin (MSB) agar plate using cotton tips. The agar plates were incubated at $37^[\circ}C$ in a candle jar for 2 days, and again incubated for 1 more day at anambient temperature Individual colonies were cultured in TH broth at $37^[\circ}C\;CO_2$ incubator. The PCR-RFLP based on dextranase gene was performed for the identification of mutans streptococci at the species-level For biotyping of mutans streptococci, biochemical tests were performed There was no significant difference of the species of mutans streptococci isolated from both test and control groups However, the biotypes of the mutans streptococci isolated from test and control groups were different. These results may offer the basic data to verify the relationship between the mutans streptococci biotype and enamel decalcification or dental caries in orthodontic patients with fixed appliances.
