Clinical Comparison of Complications Between Cervical and Thoracic Esophagogastrostomy After Resection of Esophageal Cancer (식도암 절제술시 식도 위 문합 위치에 따른 조기 합병증의 비교)

  • 배경: 식도암의 절제술에 있어 식도 위 문합술은 중대한 합병증, 즉 문합부 누출, 양성협착, 종양의 재발 등을 유발한다. 수술 후 재원기간동안 환자가 느끼는 주관적인 증상과 합병증이 식도와 위장관의 문합부 위치에 따라 달라질 수 있으므로 서로간에 비교분석할 필요가 있겠다. 대상 및 방법: 1995년 1월부터 1999년 5월까지 식도암 근치술로 식도 위 문합술을 시행 받은 55명의 환자를 대상으로 문합위치에 따라 경부문합한 23명의 환자와 흉부문합을 한 32명의 환자를 비교 분석하였다. 절제술 후 AJCC분류에 따라 I기 5명, II기 27명, III기 23명으로 판정되었으며, 종양이 상흉부에 위치한 경우 3명, 중흉부 34명, 하흉부 18명이었다. 조직학적으로 55명의 환자 2명의 선암을 제외하고 53명이 편평상피세포암이었다. 55명 전원이 남자였으며 평균연령은 경부문합의 경우 59세였고, 흉부문합은 55세였다. 경부 문합의 경우 1명의 staple봉합을 제외하고 나머지 22명이 수봉합을 하였으며, 흉부문합술의 경우는 9명이 수봉합, 23명이 staple봉합을 하였다. 결과: 수술 후 사망자는 경부문합 1명, 흉부문합 2명이었다. 경부 문합환자의 경우 23명 중 15명에서 호흡기, 소화기등의 합병증 46례가 발생하였고, 흉부 문합환자의 경우 32명중 13명에서 합병증 37례가 발생하였다. 경부문합환자는 중등도 혹은 심한 연하곤란을 나타내는 경우가 11명에서 있었으며, 흉부문합환자는 2명에서 나타났다. 또한 수술 후 20일 이후까지 재원한 경우는 경부문합환자 18명, 흉부문합환자는 13명이었다. 결론: 식도암에서 식도 위 문합술의 경우 호흡기, 소화기, 감염등의 합병증이 발생하였으며, 특히 호흡기 합병증에 의한 사망률이 높았다. 문합부 누출은 staple봉합보다 수봉합에서, 흉부문합보다 경부문합에서 빈도가 높았다. 경부문합의 경우 문합부 누출률이 높고, 연하곤란을 많이 호소하며 재원기간이 의미있게 길었다.

Interrupted Single-layer Suture Technique in Esophageal Anastomosis Using Monofilament Polypropylene Suture (Monofilament Polypropylene사를 이용한 단속단층 식도문합술)

  • Background: Although various anastomotic techniques and suture materials have been used in esophageal anastomosis, anastomotic leakage and stenosis are still somewhat frequent and serious complications when compared to other intestinal anastomoses. We have used interrupted single-layer suture technique using monofilament polypropylene suture in various esophageal anastomoses, including repair of the esophageal atresia, since 1990. Methods and method: We retrospectively evaluated the efficacy of this technique on postoperative leakage and stenosis in several esophageal reconstructions. The esophageal reconstructions using this technique were performed in 90 patients at Dong-A University Hospital from April 1990 through December 1996. Results: Anastomotic leakage occurred in 5 patients(5.6%) with one operative death. Stenosis at the anastomotic site occurred in 15 patients(n=86, 17.4%), which was most common in esophagogastrostomy(22%) and least common in esophagocolostomy (5%). This result was comparable to other methods including the autosuture technique. Conclusions: We concluded that this suture technique in esophageal anastomosis can be used with reasonable results in various esophageal reconstructions including correction of the esophageal atresia.

Anastomotic Leakage and Stricture Relating to Anastomotic Level and Methods in Esophageal Resection and Reconstruction for Esophageal Cancer (식도암에 있어서 식도 절제술 및 재건술 후 문합 위치와 방법에 따른 문합 부위 누출과 협착)

  • Background: The prevalence of anastomotic complication is related to anastomotic procedure or site in esophageal cancer operation. We studied the anastomotic leakage and stricture related to the anastomotic procedure & site in patients who received the esophageal resection and reconstruction for esophageal cancer. Material and Method: The anastomotic procedure, site and complication of 321 patients who received the esophageal reconstruction from August 1993 to May 2003 were investigated. Mean age was 64.5$\pm$4.9 (37${\~}$94) years, 300 patients ($93.5\%$) were male and 21 patients were female ($6.5\%$). Result: There were 7 anastomotic leakages ($2.2\%$) and no difference in anastomotic site (cervical anastomosis $4.1\%$, thoracic anastomosis $1.6\%$) and procedure (stapler technique $1.6\%$, semi-staple technique $9.1\%$, hand-sewn technique $0.0\%$). There were 52 anastomotic strictures ($16.2\%$), differences in sites (cervical anastomosis $2.7\%$, thoracic anastomosis $20.2\%$) (p < 0.001) and procedure (stapler technique $20.0\%$, semi-stapler technique $3.0\%$, hand-sewn technique $4.7\%$). And the stapler technique showed higher stricture rate (p < 0.001). Conclusion: Anastomotic technique was less related to anastomotic leakage in esophageal reconstruction for esophageal cancer. However, stapler technique had higher stricture rate than other techniques. Therefore, we suggest that the anastomotic technique be improved to reduce anastomotic stricture.

Assessment of the Quality of Esophago-gastric Anastomosis by Endoscopic Examination -Cervical Versus Intrathoracic Anastomosis- (내시경을 통한 식도-위 문합술의 질 평가-경부와 흉부 문합의 비교-)

  • Background: Mortality and morbidity of anastomotic complications after esophagectomy have gradually decreased in recent years. However, swallowing difficulties and reflux symptoms after esophagogastrostomy continue to be a burden jeopardizing the quality of life. In the present study, we evaluated the quality of esophagogastrostomy by analyzing anastomotic stenosis and reflux esophagitis. Material and Method: A retrospective analysis was made in 74 patients who underwent esophagogastrostomy after esophagectomy by one surgeon between January 1995 and December 2004. 53 patients of them received endoscopic examination during follow-up($29{\pm}23.6$ months, range $5{\sim}111$ months). Reflux esophagitis and stenosis at anastomostic site were analyzed according to the techniques and locations of esophagogastrostomy. Result: The median age at the time of repair was $60.3{\pm}8.87$ years(range $39{\sim}81$ years). 23 patients received a hand-sewn esophagogastric anastomosis and 30 patients a circular stapled one. There was no significant statistical difference in terms of anastomotic stenosis(p=0.64) and reflux esophagitis(p=0.41) between the two groups. Cervical anastomosis was peformed in 26 patients and intrathoracic anastomosis in 27 patients. No significant statistical difference in anastomotic stenosis between the two groups was found(p=0.44), but reflux esophagitis was noted in 3 patients in the cervical anastomosis group and 14 patients in the intrathoracic anastomosis group(p=0.003). Conclusion: Cervical anastomosis was supposed to have a better quality of esophagogastrostomy by lowering the risk of reflux esophagitis. In the future, the comprehensive study including a patient's subjective symptom and Barrett's metaplasia should be performed in larger cases.

Evaluation of Anastomotic Strength and in-vitro Degradability with Microvascular Anastomosis Coupler Based on Injection Molding Condition made by Biodegradable Polycaprolactone(PCL) (생체분해성 폴리카프로락톤(PCL) 미세혈관 문합커플러의 사출성형조건에 따른 문합강도 및 in-vitro 분해능 평가)

  • The use of mechanical anastomosis coupler instead of sutures has been increasing in microvascular anastomosis surgery. However, non-biodegradable anastomosis coupler has problems such as not only inflammatory reaction but also remaining permanently in operation wound. Therefore, we fabricated biodegradable anastomosis coupler using injection molding process to overcome the limitation of non-biodegradable anastomosis coupler. In various injection molding process conditions, the shrinkage was calculated with different cylinder temperatures and injection molding pressures and anastomotic strength was measured. As a result, changes in shrinkage hole part larger than the pin part. In addition, the shrinkage in the cylinder at higher temperatures increase. However, the higher the injection pressure, shrinkage tends to decrease, respectively. In-vitro degradation behavior of PCL anastomotic coupler evaluated for 12 weeks, water uptake was increased and molecular weight was accompanied by a reduction in mass of the biological degradation and reduction of anastomotic strength was confirmed. However, decreased levels of anastomotic strength enough to exceed the requirements of preclinical surgery, PCL microvascular anastomosis coupler suitable candidate materials that could identify.

A Simultaneous Fluid -Structure Modeling of the End-to-End Vascular Graft Anastomosis (인조혈관의 단단문합에 대한 유동과 구조의 동시해석)

  • 수축절정기에 0.2mm, 0.4mm 두께의 소구경 인조혈관의 내경3.2mm의 동맥에 문합한 단단문합 주위의 유동과 구조해석을 동시에 수행하였다. 유동해석 결과 속도분포 및 벽단전률은 타 연구결과와 매우 유사하며 이론해와 매우 유사함을 보였다. 유동에 의해서 문합부 주위에 작용하는 응력은 문합부에 집중되며 이는 수십만 pa에 달했다. 또한 인조혈관과 동맥에 작용하는 원주방향의 응력을 이론해와 비교한 결과 서로 유사함을 보였으며 두께가 얇은 인조혈관을 사용하는 경우 문합부의 compliance mismatch 는 개선되나 반대로 응력은 더 많이 받음을 알 수 있었다. 본 연구를 통해서 유체에 의하여 혈관이 영향을 받는 심혈관계 현상을 연구하는데 있어서 이와 같은 유체-구조 상호작용을 고려하여 동시에 해결 하는 방법은 매우 유용할 것으로 생각된다.

Experience with a Safe Anastomotic Method for Ivor Lewis Operation (Ivor Lewis 수술 후 발생되는 문합 부위 합병증을 예방하기 위한 술식 평가)

  • Background: When it comes to esophageal cancer operations, the prevalence of anastomotic complications that adversely affect quality of life is related to the type of anastomotic procedure and the operative site. We studied outcomes related to a safe anastomotic method used in Ivor Lewis esophagogastrectomy for preventing anastomotic leakage and stricture formation. Material and Method: Between May 2003 and April 2007, 18 patients with esophageal cancer underwent this type of esophagogastrectomy. Four people were lost to follow-up. There were 17 men (94.4%) and 1 woman. The mean patient age was 61 years (range, $46{\sim}73$ years). Result: The mean follow-up period was 17.2 months (range, $1{\sim}45$ months). There was no anastomotic leakage. There was one benign anastomotic stricture (5.6%) requiring esophageal balloon dilatation, which was accomplished with a 25 mm circular stapler. Conclusion: We experienced relatively good postoperative results using a safe anastomotic method in the Ivor lewis operation for preventing anastomotic complications. These results suggest that this anastomotic method is effective in reducing the incidence of benign anastomotic complications.

Cervical Esophagogastric Anastomosis with Endo Stapler (흉강경용 봉합기를 이용한 경부 식도위 문합술)

  • Although esophagogastric (EG) anastomosis with a circular surgical stapler (EEA or ILS) is a safe find convenient proc dure with less anastomotic leakage, a concern for the anastomotic stricture still remains, especially in patients with small esophagus. We modified cervical EG anastomotic technique using straight thoracoscopic endostapler to prevent EG anastomotic stricture. Prospective clinical study was performed to determine the feasibility of our modification using Endo-GIA (US Surgical Corp., Worwalk), during the period from October, 1994 to July, 1995, in thirteen patients with carcinoma of the thoracic esophagus. A stomach tube was reanastomosed to the cervical esophagus utilizing a 30 mm Endo-GIA after esophagectomy and node dissection. There was one early mortality due to respiratory failure and pulmonary tuberculosis. Anastomotic leakage with resultant stricture was noticed in one patient, and it was re- lated to ischemic necrosis of the stomach tube. The overall incidence of stricture was 7.6 % (1113). During the 8 month follow-up period, the remaining 11 patients did not show any clinica evidence of stricture such as dysphagia. All patients were on a regular diet. We conclude that our new technique for cervical EG anastomosis with GIA-Endo stapler is a safe and convenient procedure in preventing anastomotic stricture.
