EFFECT OF CITRIC ACID AND CALCIUM ON DENTAL EROSION (구연산과 칼슘이 치아침식증의 발생에 미치는 영향)

  • The purpose of study was to observe the effect of calcium and citric acid on the dental erosion of human premolar enamel. Enamel specimens were demineralized in 0.1%, 0.3%, 0.5%, or 1.0% citric acid solutions with 0.05%, 0.1%, or 0.2% calcium for 5, 15, 30, and 60 minutes, and then the surface microhardness of the enamel was measured. The hardness decreased as the concentration of citric acid and the demineralization time increased. Hardness after 5 minutes was 76~90% in case of no calcium and the inhibition of dental erosion by calcium addition was 2??15%. Hardness after 15 minutes was 65~84% in case of no calcium and the inhibition of dental erosion by calcium addition was 3~17%. Hardness after 30 minutes was 53~72% in case of no calcium and the inhibition of dental erosion by calcium addition was 6~22%. Hardness after 60 minutes was 43~66% in case of no calcium and the inhibition of dental erosion by calcium addition was 7~19%. The inhibition was the highest in 1.0% citric acid and 0.2% calcium. In 0.1% citric acid the inhibition increased as the demineralization time increased, but in 0.3% to 1.0% citric acid the inhibition was most high at 30 minutes and decreased a little at 60 minutes. These results suggest that calcium has a inhibitory effect on the citric acid induced dental erosion.

  • To evaluate the cephalometric and dental characteristics of obstructive sleep apnea (OSA) patients, 23 OSA patients and 15 control, non-OSA, patients who visited the Sleep Disorder Clinic Center, Keimyung University were investigated. Patients who suffered from apnea-hypopnea episodes over 10times per hour were diagnosed as having OSA after polysomnograph testing, Impressions were taken with alginate. Cephalometric radiographs were taken at maximum intercuspation. The dental cast measurements, including transpalatal width, intercanine width, intermolar width and palatal depth did not differ between the control and OAS groups and did not have a positive correlation with the apnea-hypopnea index (AHI). Upper airway width was statistically narrower than the control group. Upper airway width had a low negative correlation with AHI, but, lower airway width had a low positive correlation, and, the higher the AHI score, the longer the mandibular border to hyoid distance.

  • Hallermann-Streiff syndrome is a rare disease. Approximately 150 cases have been reported, including 6 cases in Korea. The authors experienced a case of Hallermann-Streiff syndrome in a 6-year-old female with aphakia. The syndrome is characterized by a bird-like face, dental abnormalities, hypotrichosis, atrophy of the skin, bilateral microphthalmia, and proportionate dwarfism. A brief review of the literature was conducted.

  • The automated dental cavity detection program for a new concept intra-oral dental x-ray imaging device, an auxiliary diagnosis system, which is able to assist a dentist to identify dental caries in an early stage and to make an accurate diagnosis, was to be developed. The primary theory of the automatic dental cavity detection program is divided into two algorithms; one is an image segmentation skill to discriminate between a dental cavity and a normal tooth and the other is a computational method to analyze feature of an tooth image and take an advantage of it for detection of dental cavities. In the present study, it is, first, evaluated how accurately the DRLSE (Direct Regularized Level Set Evolution) method extracts demarcation surrounding the dental cavity. In order to evaluate the ability of the developed algorithm to automatically detect dental cavities, 7 tooth phantoms from incisor to molar were fabricated which contained a various form of cavities. Then, dental cavities in the tooth phantom images were analyzed with the developed algorithm. Except for two cavities whose contours were identified partially, the contours of 12 cavities were correctly discriminated by the automated dental caries detection program, which, consequently, proved the practical feasibility of the automatic dental lesion detection algorithm. However, an efficient and enhanced algorithm is required for its application to the actual dental diagnosis since shapes or conditions of the dental caries are different between individuals and complicated. In the future, the automatic dental cavity detection system will be improved adding pattern recognition or machine learning based algorithm which can deal with information of tooth status.

TREACHER COLLINS SYNDROME : A CASE REPORT (Treacher Collins 증후군 환아의 증례보고)

  • Treacher Collins syndrome(TCS) is a rare, incurable condition occurring in approximately 1 of 25,000 to 50,000 births. It may occur as a spontaneous mutation out of genetically normal parents or it may be manifested as an autosomal dominant pattern. TCS is characterized by facial deformities such as, underdevelopment of the maxilla, mandible and zygoma, malocclusion, bilateral deformities of auricles, antimongoloid slant of the palpebral fissures. The syndrome is often associated with cleft lip and palate, ear malformations and hearing loss, short stature, and anomalies of the heart and skeleton. Respiratory difficulty associated with air way obstruction may also be observed, and there is considerable difficulty in airway management during general anesthesia. It is necessary that dentists provide safe dental treatments and guidelines to TCS patients by providing adequate understanding about the characteristics of the syndrome and proper ways of managements. The purpose of this study is to report the dental and medical characteristics of the patient who visited the Department of Pediatric Dentistry, Yonsei University for multiple dental caries treatment and to review the literatures of TCS.

An Epidemiological Study on the Dental Treatment Needs of Adolescents in Yangsan (양산시 거주 청소년의 치과치료 필요도에 대한 조사연구)

  • To evaluate the oral health status and to assess the resultant dental treatment needs in adolescents, 2,062 adolescents aged 14-17 years attending middle and high schools in Yangsan were surveyed by clinical examination and questionnaires. The obtained results were as follows. In the dental caries examination based on WHO criteria, the treatment needs of 66.7% of the subjects were determined. Assessment of dental erosion by the VEDE system indicated the treatment needs in 27.8% of the subjects, while MIH examination based on EAPD criteria indicated the treatment needs in 14.7%. Assessment of malocclusion using the occlusal index showed good occlusion in 67.8%, no need for treatment in 19.7%, slight need for treatment in 6.8%, definite need for treatment in 4.5%, and worst occlusion in 1.1% of the subjects, showing 12.5% of average treatment need. The prevalence and treatment need for periodontal disease was found to be 71.2% by CPITN assessments. The assessment of temporomandibular disorders by Helkimo's anamnestic index showed no symptoms in 67.1%, mild symptoms in 13.4%, and severe symptoms in 19.5% of the examinees. The overall treatment need of TMD was 20.6%. Periodontal diseases were ranked the highest in treatment need, followed by dental caries, dental erosion, MIH, TMD, and malocclusion in order. The results of this study indicated the overall status of adolescents'oral health and dental treatment needs. These might hopefully provide fundamental data and contribute to establishing promotional projects for adolescent oral health in Korea.

A CLINICAL STUDY OF CONGENITAL MISSING TEETH (선천성 결손치에 관한 임상적 연구)

  • The congenital missing of teeth is common, which takes place since the proliferation and differentiation are not allowed in that tooth bud fail to start development. The purpose of this study is to research incidence rate, number, and missing part of congenital missing teeth, and to study whether a person who has missing teeth has other abnormality of teeth or not. For this study, 1,520 subjects(aged 2.9$\sim$17) who had visited pediatric dentist department of Chonbuk national university dental hospital within 2 years were examined with an panoramic radiograph; exempting third molar missing state. The obtained results are as follows. 1. 8.88% among total subjects show missing teeth; male 9.05%, female 8.64% 2. The most frequently missing permanent teeth were the mandibular second premolars(22.3%). The most frequently missing primary teeth are mandibular lateral incisors(50%). 3. 43.3% patients have one permanent missing tooth, 34.3% have two, and 10.4% have more than six, respectively. In primary teeth, 86.7% patients have one missing tooth, and 13.3% have two missing teeth. 4. 18 patients(13.3%) have missing teeth as well as hyperdontia, while some patients have microdont, ectopic eruption, and fusion teeth.

MULTIPLE MYELOMA OF MANDIBLE : REPORT OF A CASE (하악골에 발생한 다발성골수종의 증례보고)

  • 다발성 골수증은 다양한 증세를 보이는 형질세포로부터 기원하는 악성종양으로 40세이상의 남성에서 호발되며 alkylating agent 단독, 혹은 corticosteroid와 병용하는 항암제 치료로 생존율이 크게 증가되기는 하였지만 치료후 평균생존기간의 20개월 내외로 완치율이 그다지 높지않은 형편이다. 악골에서의 발생은 극히 드물고 대개 늑골, 편평골, 척추, 골반, 두개골등에 광범위한 patchy destruction의 형태로 나타나는데 악골에 발생할 경우 임상증상은 장기적인 무통성 종창이 가장 흔하며 때로는 동통, 악골의 팽창, 감각이상, 치아의 동요, 악골골절등을 보이기도 한다. 전신적증상으로 무력감, 체중감소, 빈혈, 신부전, hypercalcemia, hyperiviscosity syndrome을 나타내기도 하는데 약 10%의 환자에서 전신적인 amyloidosis를 보인다. 진단은 골수생검상 골수의 plasmacytosis가 10% 이상이면서 혈장이나 뇨전기영동법에서 monoclonal globulin peak이 있을 때 가능하다. 여러 학자들은 처음에는 독립적인 고립성병소로 나타났다가 장기적으로 다발성골수종으로 이행됨을 주장하면서 장기적인 추적을 권하고 있지만 골내 고립성 형질세포종(solitary plasmacytoma in bone), 수질외 형질세포종(extramedullary plasmacytoma) 등은 다발성골수종과는 다른 질환이라 주장하는 등 아직 많은 논란이 있는 형편이다. 치료는 $4000{\sim}6000$ rad의 방사선치료와 여러 가지 약제의 복합적인 항암요법이 사용되고 있으며 국소적인 병소의 경우 외과적 절제술이 추천되기도 한다. 저자들은 59세 여자환자에서 하악골에 발생된 다발성골수종을 치험하였기에 문헌고찰과 함께 보고하는 바이다.

COMPARISON OF SEVERE EARLY CHILDHOOD CARIES PREVALENCES BY TWO DIAGNOSTIC CRITERIA (진단 기준에 따른 취학전 어린이의 중증 유아기 우식증 유병률 비교)

  • The purpose of study was to compare the prevalences of severe early childhood caries(S-ECC) using two different diagnostic criteria. The subjects of this study included 401 preschool children from 36 to 71 months of the randomly selected seven nurseries in Iksan city. Severe early childhood caries was defined as (1) 1 or more cavitated, missing (due to caries), or filled smooth surfaces in primary maxillary anterior teeth or (2) a decayed, missing, or filled score of ${\geq}$4 (age 3), ${\geq}$5 (age 4), or ${\geq}$6 (age 5) surfaces. The prevalence determined by maxillary anterior caries and the prevalence determined by dmfs were 26.7%, 28.9% in 3 years, 32.4%, 30.4% in 4 years, 39.4%, 41.3% in 5 years, and 35.4%, 35.9% in all, respectively. The results suggest that the two diagnostic criteria of the severe early childhood caries yield almost the same results.

  • Chemoradiation therapy used on pediatric oncology patients often causes dental developmental anomalies that affect future dental care. Defects noted include tooth and root agenesis, root thining and shortening, and localized enamel defects. The effect of radiotherapy usually are confined to the radiation site, but the effects of chemotherapy may be more wide spread because of its systemic distribution and structures and organs unrelated to the primary tumor may be affected. Many pediatric cancers are treated with a combination of radiation and multiagent chemotherapy to create synergic and additive effects. Dental treatment affected by chemoradiation damage to developing teeth includes orthodontic tooth movement, prosthetic abutment considerations, periodontal health, space maintenance, requirements for home fluoride regimens to protect hypomineralized areas, restoration options for hypoplastic/hypomineralized teeth, and endodontic procedures. The following case demonstrate chemoradiation therapy effects on the dental development.
