The Reproductive Patterns and Clinical Application of Endangered Common Chimpanzees by Monitoring the Steroid Hormone Measurements in Fecal Samples (분변내 스테로이드 호르몬의 측정을 통한 멸종 위기 침팬지의 번식형태와 임상적용)

  • To monitor the reproductive patterns of endangered common chimpanzee including annual reproductive cycle, amenorrhea, breeding season, and pregnancy diagnosis, Time-Resolved Fluorescence Immuno Assay (TR-FIA) was used to trace MRH (estradiol, progesterone, testosterone) and human chorionic gonadotropin (HCG). In result of this research, age was not the important factor in determining the reproduction capability in common chimpanzee; it was rather greatly influenced by the combination of various factors such as individual's fertilizing ability, presence of obstetrical diseases, the pattern of mating behavior, and the mental communication under the introduction of a mating partner. This research will play an important role in operating conservation project for common chimpanzees and can be extended also for shedding new light on understanding human menopause and obstetrical diseases.

  • Objectives: Cytogenetic investigations were carried out on 770 women with primary (n=560) and secondary amenorrhea (n=210) to determine the frequency of chromosomal or genetic causes of amenorrhea. Materials and Methods: In 770 women with primary amenorrhea (n=560) and secondary amenorrhea (n=210), chromosomal analysis were performed. Results: 1) The most prevalent age group is 16-20 years of age group with primary amenorrhea and 26-30 years of age group with secondary amenorrhea. 2) Out of 560 cases of primary amenorrhea, 343 cases (61.3%) had the normal chromosome constitution and 217 cases (38.7%) had the abnormal chromosome constitution including 46,XY. 3) In 217 cases of abnormal chromosome of primary amenorrhea, 57 cases (26.3%) had 45,X and 34 cases (15.8%) had the 46,XY, 24 cases (11.0%) had 45,X/46,X,i (Xq), 23 cases (10.6%) had 45,X/46,X,+mar and 14 cases (6.6%) had 45,X/46,XY. 4) Out of 210 cases of secondary amenorrhea, 181 cases (86.2%) had the normal chromosome constitution and 29 cases (13.8%) had the abnormal chromosome. 5) In 29 cases of abnormal chromosome of secondary amenorrhea, 7 cases (24.1%) had 45,X/46, X,i (Xq), 4 cases (13.8%) had 45,X/46,XX. Conclusion: High percentage of chromosomal abnormalities was diagnosed in primary amenorrhea and most of them were sex chromosome anomalies. In secondary amenorrhea, the prevalence was lower than primary amenorrhea, so a preselection of patients with secondary amenorrhea for cytogenetic investigations seems to be necessary.

  • The high rate of chromosomal abnormalities in patients with primary amenorrhea implies the need for routine screening for chromosomal abnormalities among such patients. This study was designed for the cytogenetic analysis of 236 patients with primary amenorrhea, which was referred to Yonsei University Medical Center, from January, 1, 1974 to December, 31, 1985. The results were as follows: 1. Of the 236 patients, 145 cases (61.4%) showed normal karyotype, and 91 cases (38.6%) showed chromosomal abnormalities. 2. Gonadal dysgenesis was found in 56 cases, consisting of 42 cases, Turner's syndrome, 12 cases, pure gonadal dysgenesis, and 2 cases mixed gonadal dysgenesis. a) Turner's syndrome was found in 42 cases, consisting of 18 cases of 45, X and 24 cases of mosaicism. b) Pure gonadal dysgenesis was found in 12 cases, consisting of 10 cases of 46, XX and 2 cases of 46, XY. c) Mixed gonadal dysgenesis was found in 2 cases, consisting of 1 case of 46, XY and 1 case of 45, X/46, XY. 3. Intersex was found in 80 cases, consisting of 35 cases of 46, XX, and 45 cases of 46, XY. 4. Congenital anomalies of reproductive system was found in 82 cases and all cases were normal karyotype.

  • This investigation represents a cytogenetic study of 382 women with primary amenorrhea. 235 cases (61.5%) showed normal karyotype, 46, XX and l47 cases (38.5%) showed chromosomal abnormalities. Of 144 females with cytogenetic abnormalities, 44 cases (30.0%) showed a 45, X constitution: 37 cases (25.2%) with 45, X/46, XX: 3 (2.0%) 45, X/47, XXX: 11 (7.5%) 45, X/46, XX/47, XXX: 1 (0.7%) 45, X/46, XY: 3 (2.0%) 46, $XX_{p-}$: 2 (1.4%) 46, $XX_{q-}$: 6 (4.1%) 46, X, i ($X_q$): 1 (0.7%) 45, X/46, $XX_{q+}$: 1 (0.7%) 45, X/46, XX, inv(X): 1 (0.7%) 45, X, inv (9)/47, XXX, inv(9). 36 women (24.5%) had a 46, XY constitution and 1 case (0.7%) had a 47, XXX constitution.

  • Objectives: The purpose of this study is to report the effect of Haedoksamul-tang on three patients with heavy menstrual bleeding. Methods: Three patients took Haedoksamul-tang for 3 months 5 cycles and got electric needle and infrared therapy. The Pictorial Blood loss Assessment Chart was used to evaluate menstrual index that shows the menstrual bleeding loss. We also assessed dysmenorrhea using the Numerical Rating Scale during the treatment. Results: The Menstrual Index has decreased and the symptoms of dysmenorrhea have relieved after treating with Haedoksamul-tang. Conclusions: The case report shows that Haedoksamul-tang is effective on patients with heavy menstrual bleeding.

  • Purpose: The purpose of this study was to find out the correlation among Shift Rotation, dysmenorrhea and digestion function. Methods: We conducted a questionnaire survey of 161 nurses about their menstruation and digestion function from several hospitals from Oct to Nov in 2009. Using chi-square test and pearson correlation, statistical analysis was made by SPSS 12.0. We significance level p-value was determined as 0.05. Results: 1. There was no significant correlation between Shift Rotation and the severity of dysmenorrhea. 2. There was statistically mild correlation between Shift Rotation and digestion power. 3. There was statistically mild correlation between dyspepsia and the severity of menstruation. Conclusion: From above results, we could conclude that Shift Rotation, dyspepsia had mild relation to menstruation.

A Survey on Korean Medicine Doctors' Recognition and Clinical Fields of Treating Primary Dysmenorrhea for Developing Korean Medicine Clinical Practice Guideline for Dysmenorrhea (월경통 한의표준임상진료지침 개발을 위한 한의사의 인식과 원발성 월경통 치료에 관한 실태조사)

  • Objectives: This study is aimed to figure out Korean medicine doctors' recognition of Korean Medicine clinical practice guidelines (CPG) and clinical fields of treating primary dysmenorrhea before developing CPG for dysmenorrhea. Methods: We conducted a questionnaire survey targeting 515 Korean medicine doctors belonging to the Association of Korean Medicine by e-mail and analyzed the answers. Results: 81.2% of the respondents knew the concepts and contents of CPG, and 98.7% agreed about the necessity of CPG. 94.2% were willing to use CPG for dysmenorrhea in learning and treating. Average number of patients visiting the respondents' clinics for dysmenorrhea was 3.9, the main age group was 20s (63.1%), and the treatments the patients given before were mostly Western treatments such as pain killers and hormonal drugs. The respondents answered that they diagnosed patients with dysmenorrhea mainly with pattern diagnosis (41.6%), and treated them with herbal medicine (39.2%), acupuncture (31.6%) and moxibustion (22.6%) for 2-3 months. They answered that the acupoint they use most was San yin jiao, and the prescription was Gui-zhi-fu-ling-wan, They answered that the field considered to need further study was decoction of herbal medicine most (27.4%), and the field considered to need insurance coverage was also decoction of herbal medicine most (40.2%). Conclusions: We figured out Korean Medicine doctors' recognition of CPG, clinical diagnosis, treatment, cost for treating dysmenorrhea, and fields of clinical research and policy they required.

Effects of periodic breast massage on the cyclical mastalgia of fertile women before their menstrual period (유방 마사지가 가임기 여성의 월경 전 주기적 유방통 완화에 미치는 효과)

  • This study was a randomized controlled trial to examine the effects of breast massage applied to women of childbearing age with cyclic mastagia on breast pain, breast hardness, nipple extensibility, breast size, and breast blood circulation. The study participants were 54 women with cyclic mastalgia who were recruited from Medical Center C, Women's Hospital M, department stores located in City D, and Internet cafes. The experimental treatment was to apply breast massage for a total of 30 minutes, 15 minutes each to the left and right breasts during the period of cyclic mastalgia. After experimental treatment, breast pain (F=49.16, p<.001), breast hardness (Rt t=8.93, p<.001; Lt t=-10.34, p<.001), nipple extensibility (Rt t=3.58, p<.001 ; Lt t=4.66, p<.001), breast size (Rt F=60.59, p<.001; Lt F=51.05, p<.001) and breast blood circulation (Rt t=-1.30, p=.201; Lt t=-2.82, p=.007) were significantly different between the two groups. In conclusion, breast massage performed in this study was effective in relieving breast pain, relieving breast hardness, and improving breast blood circulation in participants with cyclic mastalgia.

Effects of Young Sprouts Cutting Times and Nitrogen Split Application on Growth and Yield of Atractylodes japonica KOIDZ (삽주의 어린순 채취(採取)회수 및 질소분시(窒素分施)가 생육(生育)과 수양(收量)에 미치는 영향)

  • This Experiment was conducted to know effect of cutting times and nitrogen split application rates on shoot and root-related characters of Atractylodes japonica. The cutting time was forced on none, once and twice along with three levels of nitrogen split application, all basic 50 -30 -20% and 40 -30 -30%. The plant height declined with increased cutting time and dry matter weight was increased with nitrogen split application under none and once cutting. Average yield of cutted shoot as edible wild plant was 257kg/10a at once cutting condition, but twice cutting condition was little and unfavorable to use edible wild plant because shoot was coarse. Yield 0 dry rhizome was the highest at once shoot cutting und­er 50 -30 -20% nitrogen split application by 471kg/10a.Especially, culture of Atractylodes japonka needed once shoot cutting one the 10th-30th of May.

  • Hypothalamic-pituitary function in patients of 6 selected groups of amenorrhea was evaluated by performing premarin test. Selected amenorrheic patients were divided into 6 groups of Turner's syndrome(5), adrenogenital syndrome(3), Sheehan's syndrome(4), moderate hyperprolactinemia(3), severe hyperprolactinemia(9) and functional oligoamenorrhea(9) the diagnoses of which were performed according to modified our own protocol for management of amenorrheic patients. As control 20 normally cycling women in mid follicular phase determined by their symptothermal charts during last 6 months designed by WHO were compared. The premarin test which is one of the tests evaluating the hypothalamic-pituitary function by the principle of negative and positive feed back effect's of estrogen was performed by injecting 20 mg of premarin in volus intravenously. The levels of serum LH before, 24, 48, 72, 96 and 120 hours after injection of premarin were measured by double antibody technique radioimmunoassay the reagents of which were supplied by WHO. The results were as follows: 1. Both negative and positive feed back effects by exogenous estrogen were well preserved even in the patients of gonadal dysgenesis although the baseline levels were much higher than normal. 2. In the patients of Sheehan's syndrome one could observe the minimal response of feed back effect in the case with minimal pituitary function. 3, Androgens in adrenogenital syndrome and prolactin in hyperprolactinemia may suppress mainly the positive feed back effect rather than the negative one. The suppressive effect can be abolished by proper treatments which can eliminate those suppressive hormones. 4. This premarin test may be beneficial for predicting the result of clomiphene in ovulation induction.
