Immuno-Electronmicroscopic Studies on the Expression of Tenascin in the Synovial Cells of Human Knee Joint (퇴행성관절염의 사람 무릎관절 윤활막에서 Tenascin 발현에 대한 면역전자현미경적 연구)

  • To observe the morphological changes and the expression of tenascin in synovium of human knee joint, between normal condition and degenerative arthritis, were processed by immunoelectron microscopic method. The results were summarized as follows. 1. In degenerative arthritis, the hyperplasia of synovial membrane was characterized by the increase of cell number in secretory synovial cells. 2. In normal condition, there was no marking of the immuno-gold for tenascin in synovial membrane. 3. In degenerative arthritis, the immuno-gold for tenascin were observed in endoplasmic reticulum of secretory synovial cells and extracellular matrix of synovial layer. On the basis of above findings, the hyperplasia of synovial membrane and the pathologic processes may be concerned with the increase of number of secretory synovial cells and of expression of tenascin, in degenerative arthritis.

The Effects of Betula Platyphylla on Cartilage Pratection, Anti-inflammatory and Analgesic Activity in Arthritis (관절염 유발 모델에서 화피가 연골 보호 및 소염 진통에 미치는 영향)

  • 목적 : 본 연구는 화피의 연골 보호 및 소염 진통 작용을 알아보고, 화피를 이용한 관절염치료 약침액 개발의 기초자료를 얻기 위해 고안되었다. 방법 : In vitro에서는 토끼 무릎관절에서 배양된 연골조직에 5ng/ml IL-1${\alpha}$ 처리 후, 화피의 연골보호 효과, 연골세포에 대한 독성을 조사하였다. In vivo에서는 토끼 무릎관절내 collagenase를 주입, CIA 유발 후, 28일간 매일 토끼의 구강으로 화피, 증류수, CEX를 투여하였으며, 연골보호, 소염, 진통에 대한 측정을 하였다. 결과 : 화피는 proteoglican 및 collagen분해 억제, MMPs 활성 억제로 연골 보호 효과가 있었으며, 연골 세포에 대한 독성이 없었다. 소염작용은 PGE2 생산 억제 및 COX-2발현 억제, carrageenan 유발 쥐 모델에서의 부종 억제로 확인되었다. 진통작용은 tail flick test에서의 latency time 증가, formalin test에서의 염증성 통증억제로 나타났다. 결론 : 화피가 퇴행성관절염에 대한 연골 보호 효과 및 소염 진통 작용이 있으므로, 이를 근거로 약침액을 개발 응용하면 퇴행성관절염 치료에 활용될 수 있다고 사료된다.

Effects of Radiation Mutant Perilla frutescens var. crispa and Atractylodes macrocephala Koidzumi Complex Extract on the Mediators Related to Degenerative Arthritis (방사선 형질전환 차조기와 백출 복합추출물이 퇴행성관절염 관련 매개체에 미치는 영향)

  • The present study aimed to evaluate the effects of radiation mutant Perilla frutescens var. crispa and Atractylodes macrocephala Koidzumi complex extract(Perilla frutescens var. crispa complex extract) on the mediators related to degenerative arthritis in a monosodium iodoacetate-induced rat model of degenerative arthritis. Perilla frutescens var. crispa complex extract was administered orally at doses of 25, 50 or 100 mg/kg/day for 2 weeks before direct injection of monosodium iodoacetate (3 mg/50 µl of 0.9% saline) into the intra-articular space of the rats' right knees. The rats subsequently received the same doses of oral Perilla frutescens var. crispa complex extract for another 4 weeks. It was evaluated that the treatment effects based on serum bio-markers, and morphological and histopathological analysis of the knee joints. Compared with those in negative control rats, the Perilla frutescens var. crispa complex extract treatments significantly reduced the serum levels of inflammation, bone metabolism markers (i.e., TNF-α, MMP-3, COX-2, PGE2, COMP, and Aggrecan). Otherwise, it was significantly increased the production of CTX-2 in cartilage absorption mediators. In addition, the Perilla frutescens var. crispa complex extract treatments effectively preserved the knee cartilage and synovial membrane. As a result, it indicates that the Perilla frutescens var. crispa complex extract improved degenerative arthritis symptoms. Thus, the Perilla frutescens var. crispa complex can be used in food material for the management of degenerative arthritis.

Effects of Nordic Walking Exercise on muscular strength, Flexibility, Balance and Pain in Older Woman with Knee Osteoarthritis (노르딕 워킹이 퇴행성 무릎 관절염 노인여성의 근력과 유연성, 균형 및 통증에 미치는 영향)

  • The purpose of this study is to examine whether the 12-week Nordic walking can improve the physical function and arthritis pain of elderly women with osteoarthritis This study were divided into randomly assigned Nordic Walking Exercise Group (n=9) and Control Group (n=7) for 16 Elderly women diagnosed with Osteoarthritis (age: 73±3.79 year, height: 154.3±4.09 cm). The exercise group used Nordic sticks to carry out 30 minutes of Nordic walking exercise three times a week for 12 weeks, and the kinetic intensity was set at 40-60% of HRR. The control group maintained daily life for the same period. Body composition (weight, percentage body fat, skeletal muscle mass), muscular strength, Flexibility (muscular strength of upper and lower limbs, flexibility of upper and lower limbs), balance ability (static balance, dynamic balance) and pain level were measured as subordinate variables. These indicators were measured twice before and after the exercise program. The study shows that percentage body fat and skeletal muscle mass in the body composition function over 12 weeks of Nordic walking exercise have significant effects after the exercise than before (p=004)(p=.003), and it also shows significant interaction effects between the groups and timings(p=.018)(p=.005). In muscular strength, Flexibility factors, there were significant effects between the groups and timings in the upper limb muscular strength and the lower limb flexibility (p=.009)(p=.036), and a significant difference between the exercise group and the control group(p=.006) in the lower limb muscular strength. In addition, in the upper limb flexibility, there was a more significant difference after the exercise than before(p=.020). There were improvement effects after the exercise than before in the balance ability and the static balance(p=.016), but no difference in the dynamic balance(p>.05). In pain, there was a significant improvement after the exercise than before(p=.022), and a significant difference between the exercise group and the control group(p=.013). In conclusion, the 12-week Nordic walking exercise has positive effects on the body composition functions of the elderly women with Osteoarthritis, and has a positive effect on the improvement of upper limb muscular strength and lower limb flexibility in the health fitness factors. These effects are believed to have contributed effectively to the improvement of the level of pain by contributing to the improvement of physical and motor functions of the elderly women with Osteoarthritis. Therefore, it is considered that Nordic walking exercise, which enhances stability and balance of the patients with Osteoarthritis by using poles, is an effective exercise method for the improvement of the body and motor functions by lowering the pain of the joints and reducing the muscular strength and percentage body fat.

The Effect of pain control and improve function of knee applied to osteoarthritis by carbon surface heating (퇴행성 관절염 환자에게 적용한 탄소면상발열체가 통증조절과 슬관절 기능 향상에 미치는 효과)

  • This study was to investigate the effects of pain control and improvement function to far infrared ray of carbon surface heating element applied on elderly women with knee osteoarthritis. The subjects for this study were forty-five subjects with osteoarthritis, who were divided convenice sampling into 3 groups, control(15 female, no intervention), ceramic far infrared ray(15 female, applied ceramic far infrared rays) and carbon surface-heating(15 female, applied carbon surface-heating) after treatment measuring VAS, PPT and K-WOMAC. These results represent decreased pain and improved function by applied carbon surface heating element and ceramic far infrared ray on osteoarthritis knee. Therefore, carbon surface heating element will be a good treatment for osteoarthritis.

Dynamic Stability Analysis of Patients with Degenerative Osteoarthritise during Walking (보행 시 퇴행성 관절염 환자의 동적 안정성 분석)

  • The purpose of this study was to investigate the variability to compare local dynamic stability via a linear and nonlinear analysis during walking. Twenty four elderly males, 12 healthy elderly and 12 patients with osteoarthritise walked on a treadmill for 100 consecutive strides. Lyapunov exponent and correlation dimension and coefficient variation were calculated for the kinematic parameters to determine the dynamic stability during walking. The linear measures indicated that the healthy elderly demonstrated significantly higher variability in the ankle joint displacement. The nonlinear analysis revealed that COD for the knee joint angle were higher in patient with osteoarthritise. There were no coincidence in results between linear and nonlinear techniques over two groups. In light of nonlinear analysis, it was concluded that patients with osteoathritise showed higher local instability during walking.

Compared with those in control rats, the radiation mutant Perilla frutescens var. crispa treatments significantly reduced the serum levels of inflammation, bone metabolism markers (i.e., COX-2, LTB4, MMP-3, and COMP), and the amount of fibrous tissue. Otherwise, it was significantly increased the concentration of TIMP-1 and calcitonin. In addition, the radiation mutant Perilla frutescens var. crispa treatments effectively preserved the knee cartilage and synovial membrane. As a result, it indicates that radiation mutant Perilla frutescens var. crispa prevented and alleviated osteoarthritis symptoms. Thus, radiation mutant Perilla frutescens var. crispa can be used in food and drug material for the management of osteoarthritis.

The Effects of Trunk Stability Exercise on Knee function, Balance, Gait in patients after Total Knee Arthroplasty (체간 안정화 운동이 무릎관절 전치환술 환자의 무릎기능, 균형, 보행에 미치는 영향)

  • This study was conducted to investigate the effects of trunk stability exercise on knee function, balance and gait in patients who underwent total knee arthroplasty. The subjects of this study were recruited from individuals diagnosed with degenerative arthritis who had undergone total knee arthroplasty. Overall, 24 patients were randomly divided into a control groups and an experimental groups (12 each). The trunk stability exercise was conducted for 4 weeks with three exercises developed in a previous study. The measurement tools used were knee function measurements based on the Lysholm knee score, balance ability measured using a test of TUG and OLS, and a walking ability test measured using a 10MWT. Pre and post test results were within groups were compared using the paired t-test, whole differences between groups were compared using the independent t-test. The experimental group showed significantly enhanced results relative to the control group (p<0.05). Based on these results, trunk stability exercise in parallel with knee joint therapy effectively improves the recovery of patients with total knee arthroplasty.
