  • A case of inflammatory pseudotumor of the entire lung In a 61-ycar-old man is prcscntcd. The respiratory symptoms developed 2 months ago and progressed rapidly and the diagnosis of chronic pneumonia with ateletectasis of the entire lung, destroyed lung by tuberculosis and sepsis hAd to be ruled out The operative finding was different from our expectation. This case suggests that the Inflammatory pseudotumor can manifest as a whole lung-involving ass. Inflammatory pseudotumor is a nonneoplastic reactive pulmonary mass lesion that resembles tumor but shows little or no growth. Thc inflammatory pseudotufor usually present as a solitary round lung mass but in this casts progressed rapidly and destroyed the whole lung, which is rare. The patient was discharged with no problem and with outpatient followup.

Effect of praziquantel treatment on pulmonary lesions of rots infected with Payagonimus iloktsuenensis (이락촌폐흡충에 감염된 흰쥐의 폐 병변에 대한 프라지콴텔의 투여 효과)

  • An experimental pathological study was performed to observe the effect of prasiquantel treat- ment on the pulmonary lesions of the rat lung cuke, Paragonimus ilektsuenensis. The metacercariae were obtained from the freshwater crab, Sesarma dehaani, and 40 rats (wistar) were fed each with 10 metacercariae. On 20 rats prasiquantel treatment ($100mg/kg/day{\times}5$ days) was done at 5 weeks after the infection while remaining 20 rats were kept untreated for use as controls. The drug.treated rats and the untreated ones were sacrificed 3, 7, 14, 21 or 28 days later for the observation of lung pathology. The rats infected with P. iloktsuenensis showed remarkable pulmonary changes; gross features of hemorrhagic and nodular worm capsules protruded on to the surface of the lung, and histologically local atelectasis, inflammatory cell infiltration, and egg granuloma around the worm capsules each containing one or two worms. Praziquantel treatment of the rats was shorn to be highly effective in killing the worms and to lead them to degenerate, as early as in 3 days post-treatment. Almost all worms in the lung were dead and absorbed by the host cells in 21 days post-treatment, except a few living ones seen in a rat of 14-day post-treatment group. In most of the rats treated the pulmonary lesions showed the signs of resolution; regression of worm capsules with mummification of worms, decrease of inflammatory cell infiltration, improvement in the degree of atelectasis, and decreases in the frequency and size of the egg granuloma. From the results it is concluded that prasiquantel is highly effective for the treatment of rat P. iloktsuenensis infection in the lung, not only by its direct killing effect of the worms but also due to the excellent resolution capacity of the pulmonary tissues.

  • Carcinosarcoma is an uncommon pulmonary malignancy characterized by carcinomatous parenchyma and sarcomatous stroma. Pulmonary carcinosarcoma represented about 1% of the resected lung tumors. The predominant clinical features are productive cough, chest pain, dyspnea, hemoptysis, bronchiectasis, and atelectasis, but alternate atelectasis is rare. We report a case of pulmonary carcinosarcoma associated with alternate atelectasis of the right upper and lower lobe.

A case of a bronchial foreign body (기관이물 1례)

  • This is a case of a left lower bronchial foreign body in a 11 years old boy. This boy was admitted to E.N.T. clinic, Chonnam University Hospital with the complaints of paroxysmal attack of cough and dyspnea for 45 days. He had been treated at a local clinic under the diagnosis of active pulmonary tuberculosis, however, no significant response was achieved by the treatment. The chest X-ray film showed atelectatic shadow, however, no density of suspicious foreign body was observed. On the clinical points of view and the patient's history, bronchoscopy was carried out under the general anesthesia and a plastic gun bullet was removed from the left main bronchus. The boy was discharged from the hospital with good aeration of the left entire lung field and good recovery.

  • Plastic bronchitis is a rare disorder characterized by the formation of extensive, obstructing endobronchial casts. It is associated with asthma and complex cardiac defects such as those requiring the Fontan procedure. The treatment of plastic bronchitis comprises conventional therapy involving spontaneous expectoration and bronchoscopic removal and specific therapy with several new drugs. Herein, we describe the cases of 2 patients diagnosed with plastic bronchitis accompanied with a different underlying disease, which were treated with inhaled corticosteroid and low-dose oral clarithromycin.

Radiation Effect on Airway Obstruction from Lung Cancer (폐암으로 유발된 무기폐에 대한 방사선 치료의 효과)

  • Total 21 patients with airway obstruction from lung cancer treated with radiotherapy at Department of Therapeuctic Radiology, Yeungnam University College of Medicine, between April 1986 and December 1988 are retrospectively analysed by means of roentgenologic findings. Obtained results are as follows. 1. 15 out of 21 patients(71%) showed complete or partial response. 2. Patients with small cell lung cancer showed 100% response in spite of low dose(30GY/10 fractions). 3. Patients with non-small cell lung cancer treated with 50GY or over showed better response than below 45GY or below. 4. There is no relationship between the response and site of airway obstruction. These data suggested that high dose irradiation is more effective in the management of airway obstruction from lung cancer and meticulous radiotherapy planning with appropriate protection of normal lung and critical organs should be investigated in order to maximize radiation effect and minimize side effect, complication or sequelae.

Endobronchial Leiomyoma Combined with Pulmonary Sequestration -A case report - (폐격리증을 동반한 기관지 내 평활근종 -1예 보고-)

  • A 42-year-old female was admitted to our hospital complaining of a dyspnea. Chest X-ray showed left atelectasis. A mass was detected in left main bronchus by computed tomography and bronchoscopy. The mass was diagnosed as a endobronchial leiomyoma by biopsy exam. After open thoracotomy and bronchotomy, mass removal was done and middle lobe was ventilated normally. Aberrant arterial supply from descending aorta to left lower lobe of the lung was detected and left lower lobectomy was done. The lower lobe of the left lung was pathologically diagnosed as intralobar pulmonary sequestration. Herein we report a rare coexistent case of endobronchial leiomyoma and intralobar pulmonary sequestration.
