Reference Standard of Korean Carotid Intima-Media Thickness (한국인 경동맥내중막두께 참조표준 개발 및 응용)

  • 경동맥 내중막두께(intima media thickness: IMT)는 2000년 미국심장학회에서 관상동맥 질환이나 허혈성 뇌질환 위험도의 독립적인 인자임을 인정하여 일차 진단지표로 권장함에 따라 세계적으로 그 관심이 확산되고 있다. 최근 국내에서도 서구화되어 가는 식생활 패턴으로 인해 고혈압, 동맥경화, 뇌졸중, 관상동맥 질환 등의 심뇌혈관계 질환 발병률이 높아지고 있다. 서구의 연구결과에 의하면 경동맥 내중막 두께는 인종, 성별, 연령에 따라 많은 차이를 보이기 때문에 임상적 진단에 기초가 되는 한국형 경동맥 내숭막두께 정상치 확립이 요구된다. 경동맥의 내중막두께는 경동맥 팽대부에서 근위부 10 mm 부위의 far wall 총경동맥 부위에서 심혈관계 위험 인자와 가장 상관성이 많은 것으로 보고되고 있다.

Comparison of performance for classification arrhythmia with PCA, ICA, LDA using artificial neural network (신경망 분류법을 사용한 PCA, ICA, LDA에 따른 부정맥 판별 성능 평가)

  • 본 논문에서는 부정맥 판별을 위한 전처리 과정으로 PCA, LDA, ICA를 바탕으로 하여 정확도를 비교하여 보았다. 각각의 전처리는 고유의 특성을 가지고 있으며 본 논문의 목적은 부정맥 판별상 어떤 전처리가 더욱 정확성의 면에서 효과적인지를 알아보는 것이다. 본 논문의 데이터는 MIT-BIH에 기반하고 있으며, Beat의 분류는 정상(Normal), 좌각차단(Left Bundle Branch Block, LBBB), 우각차단(Right Bundle Branch Block, RBBB), 조기심실수축(Premature Ventricular Contraction, PVC), 조기심방수축(Atrial Premature Beat, APB), paced Beat, 심실보충수축(Ventricular Escape Beat)로 나누었다. 실험적 결과는 PCA-BPNN의 경우 95.53%, ICA-BPNN의 경우 93.95%, LDA-BPNN의 경우 96.42%로 LDA가 가장 ECG 부정맥 판별 응용에 있어 가장 효율적인 방법으로 나타났다.

  • This is a genuine case report of the Ross operation without the use of homografts or heterografts in reconstruction of the right ventricular outflow tract. A 8-year-old boy with congenital aortic stenosis underwent aortic valve replacement with a pulmonary autograft and right ventricular outflow tract reconstruction with a pericardial conduit bearing autologous aortic monocusp. The postoperative echocardiography and cardiac angiography revealed good ventricular function and competent neoaortic valve. He has been followed up for 19 months.

Sewage and Tissue Wastewater Treatment Using Quartz Porphyry and Natural Zeolite (맥반석과 천연제올라이트를 이용한 도시하수와 화장지폐수처리)

  • In this study, not only making an effort to find a solution for sewage and wastewater treatment, but also demonstrating a feasibility of wastewater disposal process which is treated with the cohesion and adsorption mechanism of sewage and wastewater using quartz porphyry and zeolite, which is a domestic natural resource. By using a jar test, we evaluated removal efficiency of $COD_{Cr}$ after reation. According to the result of experiment, when we injected 400~1,200 g/L of quartz porphyry and zeolite into a sewage as a coagulant, there was no difference of the rejection rates of $COD_{Cr}$, however, when quartz porphyry and zeolite, which were sintered at $600^{\circ}C$, were treated into urban sewage, removal efficiency of $COD_{Cr}$ had increased by about 15%, 3% respectively. In addition, we applied into a cosmetic tissue wastewater by using mixture of quartz porphyry and natural zeolite in order to analyze the applicability of other wastewater as well as Municipal Sewage.

  • Endovascular treatment of the aortic aneurysm with a stent graft is rapidly evolving. We describe here a case of hybrid TEVAR (thoracic endovascular aortic repair) in which the stent grafts were placed in the aortic arch after debranching of the arch vessels. The patient had undergone ascending aorta replacement for acute type I aortic dissection 2.5 years earlier. The aneurysmal change of the distal dissection progressed with time. A provisional bypass surgery from the ascending aorta to the innominate artery and left carotid artery was performed and then stent grafts were inserted via an antegrade route that covered the whole aortic arch and proximal descending thoracic aorta.

  • Carotid Intima-Media Thickness (CIMT) testing is a test that precisely assesses cerebrovascular and coronary heart diseases. According to many previous studies, CIMT predicts atherosclerosis and is highly correlated to cardiovascular disease risk factors. It has also been reported that CIMT is an independent predictor of risk factors for myocardial infarction and stroke. Therefore, the purpose of this study is to investigate CIMT and other independent factors through a correlation study with the clinical laboratory test results of a blood test. As a result, this study could not prove the correlation between CIMT and risk factors of cardiovascular disease (TC, TG, LDL cholesterol, and HDL cholesterol) due to an insufficient number of subjects. Nevertheless, a positive correlation was demonstrated between CIMT and ALT (p<0.05), GGT (p<0.05), Uric acid (p<0.05), and CEA (p<0.05) at a statistically significant level, suggesting a continuation of the study.

  • Nonocclusive mesenteric ischemia (NOMI) is a rare complication that occurs in about 0.05% of patients after openheart surgery, and NOMI refers to the mesenteric ischemia that's caused by splanchnic vasospasm without occlusion of the great intestinal vessels. In the presently reported case, NOMI developed to maintain the blood flow to the heart and brain after several minutes of a hypotensive status and the latter was caused by acute aortic dissection that complicated an aortic cannulation procedure. Unfortunately, the patient died even though the problems were diagnosed early and proper treatment was administered. Early diagnosis of NOMI by angiography and the selective infusion of vasodilators are thought to be the only way to improve survival for patients with clinically suspected NOMI.
