Diagnostic classification and clinical aspects of floppy infants in the neonatal and pediatric intensive care units (신생아 및 소아 중환자실에 입원한 늘어지는 영아(floppy infant)의 진단적 분류 및 임상적 고찰)

  • Purpose : The purpose of this study is to make a diagnostic classification and discuss a diagnostic strategy of floppy infants by investigating clinical, neurological, electrophysiological, and genetic analysis of infants admitted to intensive care units with the complaint of hypotonia. Methods : A retrospective study was performed from Jan. 1993 to Dec. 2005 in neonatal and pediatric intensive care units of Seoul National University Children's Hospital. Clinical features and all tests related to hypotonia were investigated. Results : There were 21 cases of floppy infants admitted to intensive care units. Final diagnosis was classified as centra (7 cases[33.3 percent]), peripheral (11 cases [52.4 percent]), and unspecified (3 cases [14.3 percent]). Among the central group, three patients were diagnosed as hypoxic ischemic encephalopathy, two patients as Prader-Willi syndrome, one patient as chromosomal disorder, and one patient as transient hypotonia. Among the peripheral group, four patients were diagnosed as myotubular myopathy, three patients as SMA type 1, two patients as congenital myotonic dystrophy, one patient as congenital muscular dystrophy, and one as unspecified motor-neuron disease. Motor power was above grade 3 on average, and deep tendon reflex was brisk in the central group. Among investigations, electromyography showed 66 percent sensitivity in the peripheral group, and muscle biopsy was all diagnostic in the peripheral group. Brain image was diagnostic in the central group, and Prader-Willi FISH or karyotyping was helpful in diagnosis in central group. Morbidity and mortality was more severe in the peripheral group Conclusion : Classification of diagnosis by clinical characteristics in this study, and application of investigations step by step, may provide an effective diagnostic strategy.

  • It is as result that study to apply mini magnet nerve curer in peripheral nervous system disease treatment. Design and embodied action power and Control Unit in cylinder form of magnet roof object firstly. Yielded service area about special quality of probe of roof object cylinder style and treatment area dimension and distance of treatment pulse secondly. Embody pulse forming course energy value by Probe's form by third, could embody treatment pulse by disease. Specially, through a special quality experiment, saved Damping pulse form and treatment pulse form etc. variously. Lately, embodied this to aid a little in disease treatment that follow that there is no invasion that there is no stimulation by medicine development. Go amplitude treatment pulse (traditional magneto-therapy of greatly great that strong) does curative effect greatest at short time and becomes thought that demand is magnified greatly at the future in this research.

Effectiveness of Non-Pharmacologic Interventions in Chemotherapy Induced Peripheral Neuropathy: A Systematic Review and Meta-Analysis (항암화학요법 관련 말초신경병증에 적용한 비약물적 중재의 효과: 체계적 문헌고찰 및 메타분석)

  • Purpose: This study was conducted to evaluate the effectiveness of non-pharmacologic interventions in chemotherapy-induced peripheral neuropathy (CIPN). Methods: PubMed, Cochrane Library CENTRAL, EMBASE, CINAHL, and several Korean databases (Until August 2017) were searched. The main search strategy combined terms for peripheral neuropathy and presence of neoplasms. The risk of bias was assessed using the Cochrane's Risk of Bias tool for randomized studies and the Risk of Bias Assessment tool for non-randomized studies. To estimate the effect size, a meta-analysis of the studies was performed using the Rev Man 5.3 program of the Cochrane Library random-effects models were used in the analyses. Results: Twenty-two studies with a total of 954 participants met the inclusion criteria. Of the 22 studies, 12 were used to estimate the effect size of the non-pharmacologic interventions. The non-pharmacologic interventions used in patients with CIPN were exercise, acupuncture, massage, and foot bath. The acupuncture significantly reduced CIPN symptoms and signs (d=-0.71) and CIPN pain (d=-0.73) (p<.001). Massage and foot bath were also effective in reducing CIPN symptoms (d=-0.68; 95% CI=-1.05, -0.30; p<.001; $I^2=19%$).Exercises were effective in improving muscle strength and endurance(d=-0.55) and quality of life (d=-2.96), but they were not significantly effective in improving CIPN. Conclusion: Although these results provide little evidence of the effectiveness of acupuncture, massage, and foot bath in the treatment of CIPN, they suggest that these interventions can reduce CIPN symptoms in patients with cancer. However, the findings of this study should be interpreted with caution as there is a relative lack of data in this field, and additional well-designed studies are needed. Development of stimulator for peripheral disturbance therapy using A variable Micro-electromagnetic (미약 전자기장을 이용한 말초장애 치료시스템 개발)

  • It has increased that peripheral disturbance(blood flow, nerve, Raynaud's phenomenon) and finger rheumatoid arthritis which is caused by the diabetic complications. To improve these pain issues, we proposed new method for the Finger Disease Therapy(FDT). In this paper, we manufactured solenoid cylindrical coil which was only for the FDT using a variable micro-electromagnetic. Also, we designed the Finger Disease Therapy System(FDTS) which could select three stimulation modes(N_pulse, S_pulse, N/S_pulse) and frequency(0.25hz, 0.5hz, 1hz). We used a Teslameter to measure magnetic flux inner solenoid, and measured magnetic flux as distance(0 ~ 3cm) inner solenoid with stimulation modes and frequency. In the results, magnetic flux was the highest in center of solenoid(0cm) for all stimulation modes. Also, the highest magnetic flux was measured as N_pulse(294.3mT), S_pulse(293.8mT) in 1Hz and N/S_pulse (275.4mT) in 0.25Hz, respectively. Therefore, we developed the FDTS using various pattern and intensity for finger diseases therapy, and checked therapy clinic application possibility of the FDTS as measuring magnetic flux inner solenoid.

A Case Report of Chronic Chemotherapy-Induced Peripheral Neuropathy Treated by Korean Traditional Medicine (항암화학요법으로 유발된 만성 말초신경병증 환자의 한의학적 치험 1례 : 2년 이상 지속된 항암화학요법 유발 말초신경병증 치험)

  • Objective: This study reports the effect of Korean traditional medicine on persistent chemotherapy-induced peripheral neuropathy (CIPN). Methods: The patient was treated with Korean traditional medicine that included acupuncture, moxibustion, and herbal medicine. The effectiveness of the treatment was evaluated by the Quality of Life Questionnaire-Core 30 (QLQ-C30), the Quality of Life Questionnaire Chemotherapy-Induced Peripheral Neuropathy (QLQ-CIPN20), the Medicinae Doctor Anderson Symptom Inventory (MDASI), and a numeral rating scale (NRS). We also used the Beck Depression Inventory (BDI) to evaluate the patient's mood change after relieving the paresthesia. Results: After treatment, the symptoms were improved. Paresthesia decreased 80%p after Korean medicine treatment. Conclusions: According to this study, Korean traditional medicine can be an effective treatment for paresthesia in patients suffering from CIPN over 2 years.

Effects of Nitric Oxide Synthase Inhibitor on Hindlimb Muscles in Rats with Neuropathic Pain Induced by Unilateral Peripheral Nerve Injury (산화질소 합성효소 억제제가 일측성 말초신경 손상에 의해 유발된 신경병증성 통증 쥐의 뒷다리근에 미치는 영향)

  • Purpose: The purpose of this study was to examine effects of nitric oxide synthase (NOS) inhibitor on muscle weight and myofibrillar protein content of affected and unaffected hindlimb muscles in rats with neuropathic pain induced by unilateral peripheral nerve injury. Methods: Neuropathic pain was induced by ligation and cutting of the left L5 spinal nerve. Adult male Sprague-Dawley rats were randomly assigned to one of two groups: The NOSI group (n=19) had NOS inhibitor (L-NAME) injections daily for 14 days, and the Vehicle group (n=20) had vehicle injections daily for 14 days. Withdrawal threshold, body weight, food intake and activity were measured every day. At 15 days all rats were anesthetized and soleus, plantaris and gastrocnemius muscles were dissected from hindlimbs. Muscle weight and myofibrillar protein content of the dissected muscles were determined. Results: The NOSI group showed significant increases as compared to the Vehicle group for body weight at 15 days, muscle weight and myofibrillar protein content of the unaffected soleus and gastrocnemius. The NOSI group demonstrated a higher pain threshold than the vehicle group. Conclusion: NOSI for 14 days attenuates unaffected soleus and gastrocnemius muscle atrophy in neuropathic pain model.

  • The purpose of this experimental study was to examine and compare the regeneration capacity between crushed nerve & transected nerve. For this study, 20 Sprague-Dawley female albino rats were used as experimental animals and divided into two groups. In group 1, the sciatic nerves were crushed 6mm. in length for 1 min. using maximum force with a needle holder. In group 2, the sciatic nerves were resected 6mm. in length and the gaps were encased by inserting the proximal and distal stumps into each end of silicone tubes. The animals were sacrificed 1 month & 2 months after the experiment. All specimens were fixed in 2.5% glutaraldehyde and 1% Osmium tetroxide solution then embedded in epon 812 and were cross-sectioned at $1{\mu}m.$ After these procedures, specimens were observed under Light microscope. The results obtained were as follows. 1. Group 1 showed greter diameters of regenerating nerves than group 2. 2. Group 1 showed greater number of axons than group 2.
