요즈음은 인터넷이나 대중매체를 통해 일반인들도 다양한 질병에 대한 많은 정보를 얻을 수 있다. 이에 속하는 것으로 당뇨병은 대표적 만성질환이며 누구나 한번쯤은 들어 본 적이 있는 질환일 것이다. 현대와 같이 고도로 의학이 발달한 상황에서도 만성질환은 완치되는 것이 아니고 평생 관리를 해야하는 질병이기 때문에 많은 환자들이 살아가는 동안 서서히 지치게 되고 관리를 소홀히 하게 됨으로써 당뇨병성 신경병증, 망막증, 신증, 대혈관병증(뇌졸중, 심혈관질환, 말초혈관질환) 등과 같은 합병증의 발생으로 일상생활에 지장을 받게 된다. 따라서 당뇨병환자는 일생 주기적으로 검사와 함께 자신에게 맞는 약을 복용하고 경우에 따라서는 인슐린 주사까지 맞음으로써 지속적인 혈당관리를 하여 합병증의 발생을 최소화 하는 것이 필요하다. 이상에 언급한 다양한 합병증 중 가장 중요한 사망원인은 심혈관질환으로 40세 이후에 발생한 당뇨병환자에서 전체 사망 원인의 50% 이상을 차지한다. 또한 대부분의 당뇨병성 만성합병증이 적극적인 혈당관리로 효과적인 예방이 가능한 반면 심혈관질환과 같은 대혈관합병증은 혈당조절이 잘 되고 있는 환자에서도 당뇨병 발생 기간과 무관하게 발생하고 또 여러 혈관에 걸쳐 다발적이고 광범위하게 발생하므로 원인이 되는 다른 위험요소들에 대한 관리가 함께 이루어져야 한다.

Objectives We report a series of 2 treatment cases with Hyeongbangdojok-san which is based on Sasang Constitutional Medicine for peripheral facial palsy in Soyangin. Methods We diagnosed these two cases as a Soyangsangpung. And we prescribed Hyeongbangdojok-san for their physical symptoms. The improvement of their peripheral facial palsy were evaluated. Results After the Hyeongbangdojok-san was given, the patient's peripheral facial paralysis and physical symptoms were improved. Conclusions These case-studies showed an efficient result of using Hyeongbangdojok-san in the peripheral facial paralysis of Soyangin.

Recently, there have been researches to automate processing and analysing images in the medical field using image processing technique, a fast communication network, and high performance hardware. In this paper, we propose a system to be able to analyze morphological abnormality of red-blood cells for peripheral blood image using image processing techniques. To do this, we segment red-blood cells in the blood image acquired from microscope with CCD camera and then extract UNL fourier features to classify them into 15 classes. We reduce the number of multi-variate features using PCA to construct a more efficient classifier. Our system has the best performance in recognition rate, compared with two other algorithms, LVQ3 and k-NN. So, we show that it can be applied to a pathological guided system.

Analysis of intraepidermal nerve fibers using skin biopsy is a recently developed technique, providing diagnostic information on small fiber neuropathies. The specimens are obtained by 3 mm punch biopsy, which is safe and minimally invasive. Immunohistochemical staining by Protein gene product (PGP) 9.5 demonstrate not only intraepidermal nerve fibers but dermal structures, such as sweat gland and erector papillae. Up to now, many studies agree that intraepidermal nerve fiber density is dramatically reduced in various sensory neuropathies. The utility of density measure was confirmed with high sensitivity in the diagnosis of sensory neuropathy, comparable to sural nerve biopsy or quantitative sensory testing. Besides quantitative methods, morphological changes like axonal swelling and fragmentation can be used as predegenerative markers. This article reviews the technique of skin biopsy and clinical and experimental usefulness of skin biopsy in diagnosing and monitoring peripheral neuropathies.

편측무시는 뇌졸중 환자에게서 나타나는 지각 손상중의 하나로 말초 운동 및 감각 신경의 손상과 상관없이 손상된 대뇌반구의 반대편의 공간과 신체의 지각이 감소된 상태로 양방향에서 동시에 주어지는 자극에 대해서 한쪽 자극만을 지각하며 뇌손상 반대편의 신체 움직임의 인식 부족과, 무시된 공간쪽으로의 적은 눈 움직임을 보인다. 이와 같은 편측무시를 측정하는 기존 방법으로는 Albert Test, Line bisection Test, Star Cancellation Test 등이 있다. 하지만, 기존 편측무시 평가 방식에는 여러 가지 단점들이 발생한다. 항상 새로운 평가용지가 필요, 검사시간이 오래 소모되고, 모든 작업을 수작업으로 진행, 종이로 데이터를 관리, 수작업이므로 인력낭비 발생한다. 따라서 본 논문에서는 이러한 아날로그 방식에서 나오는 문제점들을 누구나 사용하고 있는 스마트 디바이스를 이용해 디지털방식으로 전환하여 기존의 비효율적이던 방식을 개선시키고자 평가시스템을 개발하고자 한다.

저자들은 복통과 고혈압으로 입원하여 급성간헐성 포르피리아증으로 진단된 47세 여자 환자에서 고혈압, 말초 신경장애, 항이뇨호르몬 부적절분비증, 뇌경색을 동반한 다양한 임상양상을 보인 1례를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Malignant peripheral nerve sheath tumors (MPNSTs) usually arise in soft tissues; they are rarely found in the bone. This paper reports a case of MPNST in the foot and ankle joint involving the distal tibia, talus, calcaneus, navicular, medial intermediate, and lateral cuneiform, cuboid, and 2nd to 4th metatarsal bone. Palliative treatment was performed. The authors encountered a patient with intraosseous MPNST of the midfoot who presented with nonspecific clinical and radiologic findings. This case shows that a high index of suspicion and a histopathology examination, including immunohistochemistry, will be necessary for an accurate diagnosis.

In this case report, we present a rare case of lipoma of the tendon sheath localized to the wrist which caused median entrapment neuropathy and was successfully treated with surgical excision. Dynamic examination using ultrasonography revealed the exact location of the lipoma. Electrodiagnostic study (EDX) was done before surgery to elucidate combined neuropathy, and surgery for ulnar neuropathy around elbow was also performed simultaneously. Diagnostic ultrasound can be used for dynamic examinations with real-time visualization.

Objective We report a case about a Soyangin patient with diabetic neuropathy. His symptoms were alleviated with Sasang Constitutional medicine. Methods A Soyangin patient with diabetic neuropathy was treated by herbal medicine, acupuncture, pharmacopuncture according to Sasang Constitutional Medicine. The progress was evaluated with numerical rating scale(NRS) and blood sugar test(BST). Results Sasang Constitutional Medicine seems to be effective for the symtoms of the patient with diabetic neuropathy in this case. Conclusions This case study shows that Sasang Constitutional Medicine may be an effective treatment for Soyangin's diabetic neuropathy.
