  • 당뇨병은 단순히 혈당이 높은 병이 아니다. 혈당이 높은 상태로 오랫 동안 지속되면 우리 몸의 여러 곳에 문제가 생길 수 있는데 그 대표적인 곳이 눈의 망막과 콩팥(신장), 그리고 신경이다. 신경이란 뇌에서 나와 우리 몸의 구석구석까지 뻗어 있는 나뭇가지라고 생각하면 되는데 특히 뇌에서 우리 몸의 손끝, 발끝에 있는 근육에 명령을 보내 몸을 움직이게 하거나 반대로 손끝, 발끝에서 느끼는 여러 감각을 뇌에 전달하는 역할을 하는 신경을 말초신경이라고 한다. 말초신경에 문제가 생기면 뇌가 손, 발에 내리는 명령을 수행할 수 없거나(손, 발을 제대로 움직이지 못함), 손, 발에서 느끼는 여러 감각을 뇌로 전달하지 못해 감각을 느끼지 못하게 된다. 말초신경이 손상되는 이유는 당뇨병 외에도 영양결핍, 납중독, 약물, 신부전증, 갑상선기능 저하증 등 여러 가지가 있으나 말초신경이 손상되는 가장 흔한 원인은 당뇨병이 오래되고 혈당이 조절되지 않아서 생기는 '당뇨병으로 인한 신경합병증'이다.

Genetics of Hereditary Peripheral Neuropathies (유전성 말초신경병의 유전학)

  • Hereditary peripheral neuropathies can be categorized as hereditary motor and sensory neuropathies (HMSN), hereditary motor neuropathies (HMN), and hereditary sensory neuropathies (HSN). HMSN, HMN, and HSN are further subdivided into several subtypes. Here, we review the most recent findings in the molecular diagnosis and therapeutic strategy for hereditary peripheral neuropathies. The products of genes associated with hereditary peripheral neuropathy phenotypes are important for neuronal structure maintenance, axonal transport, nerve signal transduction, and functions related to the cellular integrity. Identifying the molecular basis of hereditary peripheral neuropathy and studying the relevant genes and their functions is important to understand the pathophysiological mechanisms of these neurodegenerative disorders, as well as the processes involved in the normal development and function of the peripheral nervous system. These advances and the better understanding of the pathogenesis of peripheral neuropathies represent a challenge for the diagnoses and managements of hereditary peripheral neuropathy patients in developing future supportive and curative therapies.

MR Neurography: Current Several Issues for Novice Radiologists (자기공명영상 신경조영술: 경험이 적은 영상의학과 의사가 이해해야 할 몇 가지 쟁점들)

  • Magnetic resonance neurography (MRN) has been increasingly used in recent years for the assessment of peripheral neuropathies. Fat suppression T2-weighted imaging (T2WI) and diffusion-weighted imaging (DWI) have typically been used to provide high contrast MRN. Isotropic 3-dimensional (3D) sequences with fast spin echo, post-processing imaging techniques, and fast imaging methods, among others, allow good visualization of peripheral nerves that have a small diameter, complex anatomy, and oblique course within a reasonable scan time. However, there are still several issues when performing high contrast and high resolution MRN including standard sequence; fat saturation techniques; balance between resolution, field of view, and slice thickness; post-processing techniques; 2D vs. 3D image acquisition; different T2 contrasts between proximal and distal nerves; high T2 signal intensity of adjacent veins or joint fluid; geometric distortion; and appropriate p-values on DWI. The proper understanding of these issues will help novice radiologists evaluate peripheral neuropathies using MRN.

Compressive Neuropathy in Upper Extremity (상지의 압박 신경병증)

  • Compressive neuropathy in the upper extremity can be clinically diagnosed by careful history taking, physical examination of the involved nerve. Electrodiagnosis for the suspected nerve informs severity of compression of the involved nerve and indicates specific site of the lesion. In the early stage of the disease, non-operative treatment generally cures the symptom, however, if the conservative treatment fails, confirmation of the exact site of the lesion should be preceded before the operation. Recently, ultrasonography, as a supportive tool for the diagnosis of compressive neuropathy has increasing popularity for its ability to find space occupying lesion, anatomical change of the nerve, and the pathologic change in the nerve itself. For the successful treatment of the compressive neuropathy, these various diagnostic tools have to be introduced in the orthopaedic clinic.

Imaging Findings of Breast Metastasis from Malignant Peripheral Nerve Sheath Tumor of Mediastinum: A Case Report (종격동 악성 말초 신경 초종으로부터 유방 전이의 영상 소견: 증례 보고)

  • Malignant peripheral nerve sheath tumor (MPNST), a rare soft-tissue sarcoma, is most commonly located in the trunk, extremities, and head and neck, but rare in the breast. We report a metastatic breast MPNST in a 27-year-old woman with neurofibromatosis type 1 (NF-1). Chest computed tomography revealed a well-defined, oval, mildly enhancing nodule in the right breast. US revealed a circumscribed, oval, heterogeneous echoic mass with vascularity and intermediate elasticity in the right upper outer breast. The breast mass was excised and diagnosed as MPNST on histopathology evaluation. Although rare, it should be included in the differential diagnosis of breast mass in NF-1 patient.

Systematic Review on Effect of Peripheral Sensory Nerve Stimulation on Upper Extremity Function for Stroke Patients (뇌졸중 환자의 상지 기능 향상을 위한 말초감각신경자극의 효과에 관한 체계적 고찰)

  • Objective: This study is executed systematic review targeted at international journals intended to investigate on effect of peripheral sensory nerve stimulation on upper extremity function for stroke patients. Method: After literature search, researchers selected for 10 studies registered up to October 2015 based on PubMed database, using the following search terms: peripheral nerve stimulation, electrical stimulation, sensory stimulation, somatosensory stimulation, stroke, hemiplegia, hemiparesis and hand, arm, upper limb. Result: There were significant improves of upper extremity function and positive effect on the cortical activation in the use of peripheral sensory nerve stimulation. Conclusion: domestic studies in future requires a study of the method for measuring more accurately the effect of peripheral sensory nerve stimulation in RCT studies applying various intervention.

Effect of Transplantation of Intravascular Cultured Neural Stem Cell upon Peripheral Nerve Regeneration (혈관내에 배양한 신경줄기세포의 이식이 말초신경 재생에 미치는 영향)

  • The ultrastructural change of sciatic nerve and immunohistochemical changes of NGF, PCNA were studied at the transplanted segment of intravascular cultured neural stem cell in the rat sciatic nerve by 5 months after the sciatic nerve transection. The transplanted intravascular neural stem cells were differentiated into Schwann reals at the 20th day and these cells began to regenerate by the proliferation and hypertrophy. There were many remyelinating Schwann cells in the transplanted nerve in term of stimulation. According to NGF finding, we suggest preexisting Schwann cells may induce the differentiation of neural stem cells into regenerating Schwann cells. Electron microscopic changes were the remyelinating appearance, the increase of intraaxonal microtubules and enlarged mitochondria and contacting tell processes each other.

Effects of swimming on functional recovery and brain-derived neurotrophic factor (BDNF) mRNA expression after sciatic crushed nerve injury in rats

  • 말초신경은 외상이나 질병 등 여러 가지 원인으로 손상되기 쉬우며, 손상의 정도가 심하거나 치료가 지연되는 경우에는 심각한 기능 소실을 초래할 수 있다. 본 연구에서는 수영이 말초신경손상후 운동기능의 회복과 뇌유인성 신경영양인자 (brain-derived neurotrophic factor, BDNF) mRNA의 발현에 미치는 효과를 알아보기 위하여, 흰쥐 좌골신경에 압박 손상을 가하고 수영을 적용한 후 보행궤적분석 (walking track analysis)과 역전사연쇄반응 (reverse transcription-polymerase chain reaction, RT-PCR)을 실시하였다. 그 결과, 좌골신경 압박손상된 쥐는 특징적인 보행패턴을 나타내어 좌골신경기능지수 (sciatic function index, SFI)가 현저히 낮아졌으며, BDNF mRNA의 발현이 증가하였다. 좌골신경 압박 손상후 수영을 한 쥐에서는 SFI가 현저히 향상되었으며, BDNF mRNA의 발현은 억제되었다. 이러한 결과는 말초신경손상후 수영이 BDNF mRNA의 발현을 조절함으로써 기능 회복을 촉진시키는 효과적인 치료방법이 될 수 있음을 제안하고 있다.

The Effect of Acrylamide on the Ultrastructures of Nervous System of the Mouse (생쥐 신경계의 미세구조에 미치는 Acrylamide의 영향)

  • The effed of acrylamide on the nervous system has heen morphologically studied using light and electron microscopes. The light micrographs on central and pedpheral nervous tissues of mouse treated with acrylamide monomer showed total vacuolation of spinal cord, cell degradation containing neuron and neuroglia, and distal nerve fiber degeneration. The electron micrographs showed ultrastrudural changes. Abnormal mitochondria in neuron, splitting of myelin sheath in lumbar ventral root nerve, partial disintergration of myelin sheath and axoplasmic degeneration in sciatic nerve, and overafl polyneuropathies in nervous system were observed. These results suggest that acrylarnide intoxicated mouse shows distal behavioral neuropathy as an earlist clinical sign, but the initial effect of acrylamide on the nervous system seems to appear at nearly the same time in both central and peripheral nervous systems.

Effect of Electroacupuncture on the Regeneration of Injured Peripheral Nerve (손상된 말초신경의 재생에 미치는 전자침의 효과)

  • To confirm the effect of electroacupuncture on the regeneration of injured peripheral nerve, the change of evoked potential in the sciatic nerve, the change of enzyme activity in the spinal cord, and morphological change of injured sciatic nerve were examined comparatively in acupuncture group (AG) and control group (CG) after sciatic nerve of guinea pig was injured by purpose. The value of evoked potential after injury of the sciatic nerve was increased in both AG and CG, but the increase rate of that was higher in AG than CG. Acid phosphatase activity of the spinal cord was increased in 1CG and 2AG, but shown are tendency to return to the normal state as time went by. Ultrastructural recovering rate of the injured sciatic nerve was higher in AG than CG. Also, there was developed only adipose tissue in sciatic nerve of AG. As mentioned above, the effect of electroacupuncture on the regeneration of injured peripheral nerve was confirmed experimentally by change of evoked potential, acid phosphatase and ultrastructure. Especially, the effect of electroacupuncture was appeared clearly in an early stage than other treatment stages.
