Anesthetic Management of the Oral and Maxillofacial Surgery in a Patient with End-Stage Renal Disease -A case report - (말기신부전 환자의 구강외과 수술 마취관리 -증례보고-)

  • Patients in end-stage renal disease (ESRD) and chronic renal failure present a number of challenges to the anesthesiologist. They may be chronically iii and debilitated and have the potential for multiorgan dysfunction. A 65-year-old male patient with ESRD was scheduled for oral cancer surgery under general anesthesia. He was in regular hemodialysis three times a week and secondary hypertension with left ventricular hypertrophy was accompanied. He also had chronic metabolic acidosis and hyperkalemia. The day after hemodialysis, general anesthesia was carried out. Uneventful anesthetic induction using thiopental and vecuronium and nasotracheal intubation were carried out. General anesthesia was maintained with isoflurane for 9 hours. During the anesthesia, he did not have any problem but persistently increasing serum potassium level. After anesthetic emergence, he was transferred to intensive care unit for mechanical ventilation. So we report this successful case of anesthetic management in a patient with ESRD for oral cancer surgery, which massive bleeding and long anesthetic time were inevitable in, from the preoperative preparation to anesthetic emergence.

  • In this paper, we propose a method of analysing the relation between the patient's face color and his(her) kidney disease using image processing technology. This method is based on the ocular inspection which is one of the most famous diagnosis methods used in the oriental medical system. The way of processing and analysing the face image, which is for visualization and objectification of the color difference, is included. The objects are selected from the patients who suffer the kidney disease and use the hemodialyzer. Their facial images and clinical data are collected. From these data, we propose a hypothesis that the color of the patient's face is changed according to the patient's kidney state. At the same time, we present two algorithms of extracting the specific part of face which can identify the state of the patient's kidney and tracing the history of the color's change. This proposed method is evaluated through the practical experiments and their analysis.

Pain and Its Major Influencing Factors in the Management of Terminal Cancer Patients (말기암 환자의 통증 양상과 통증관리에 영향을 미치는 요소들)

  • Background : Validity of WHO guideline of cancer pain management has been proven and many trials have been done to derive solutions for inadequate cancer pain management. We assessed the severity of pain of terminal cancer patients in a few different ways and patients' characteristics influencing inadequate pain management. Methods : This study was based on 100 adult oncological patients who were confirmed as terminal stage in our institution from 3/1998 to 11/1998. Medical records were reviewed and individual patients were interviewed to obtain demographic information and medical characteristics such as: daily activity performance, metastasis, and drug-adjusted pain severity. Adequacy of prescribed analgesics in accordance with WHO guidelines of pain management and patients' characteristics influencing adequacy of pain management were assessed. Results : Among those cancer patients diagnosed as terminal stage, 85% complained of pain, and 68% of those patients reported pain above moderate severity. 38% of those patients received inadequate pain management resulting in greater severity of pain; the less adequate pain relief was(p<0.01). Sex, age, primary cancer site metastasis, symptoms such as depression and anxiety, and daily activity performance were not significantly related. Conclusions : Despite WHO guidelines for pain management, majority of the terminal cancer patients received inadequate pain management. There is a necessity for education on proper pain evaluation and strict implimentation for WHO guidelines of pain management.

Comparison of Controlled-release Oral Morphine with Transdermal Fentanyl in the Management of Terminal Cancer Pain (말기암 환자의 통증 치료에 있어 서방형 몰핀과 경피형 펜타닐의 비교 연구)

  • Background: For terminal cancer pain management, controlled-release oral morphine (morphine sulfate tablet, MST) is a simple and convenient regimen. Recently, fentanyl transdermal therapeutic system (F-TTS, transdermal fentanyl) has been developed and became one of the alternative ways of providing adequate pain relief. This open prospective study was designed to compare the analgesic efficacy and safety of MST and transdermal fentanyl in the management of terminal cancer pain. Methods: In this open comparative and randomized study, 64 terminal cancer patients received one treatment for 15 days, controlled-release oral morphine (MST group) or fentanyl transdermal therapeutic system (F-TTS group). Daily diaries about the vital sign, visual analogue scale (VAS) for pain, opioids requirement, co-anagesics, adjuvant drugs and adverse effects were completed with 24 patients in MST group, 18 patients in F-TTS group. Results: The majority of patients in both treatment groups were late-stage cancer and their distribution was not different in both groups. Daily opioids requirement was 126.4 mg in MST uced in F-TTS group (P<0.05). The incidence of nausea, vomiting and constipation was lower in F-TTS group (P<0.05). Patients satisfaction was similar, but F-TTS patient group favored continous use of same treatment compared with MST group after the study was finished. Conclusions: Transdermal fentanyl seems to be safe and similar analgesic effect to controlled-release oral morphine for the control of the terminal cancer patients. However, transdermal fentanyl provides a simpler and more convenient especially in respect to constipation, nausea & vomiting. To determine the exact analgesic effect, cost-effectiveness and complications, controlled trials should be followed.

  • Surgical anterior ventricular endocardial restoration (SAVER) is a technique that improves hemodynamic status by excluding akinetic or dyskinetic portions of the left ventricle, restores the ventricle to normal elliptical shape and reduces ventricular wall tension to normal level in patients with acute anterior wall myocardial infarction that accompanies aneurysm. We performed redo-SAVER procedure in a 40-year old man with remodeled dilated ventricle who had already underwent LV aneurysmectomy 12 years earlier, and the results were satisfactory.

  • 간호사업은 1970년대 후반부터 이제까지의 병원중심-환자중심의 간호사업을 수행해 오다가 지역사회에 기반을 둔 간호사업인 건강간호사업으로 전환되기 시작하였다. 세계보건기구에서도 지역사회에 기반을 둔 간호사업인 일차건강관리사업을 중점적으로 지원하기 시작한 것이다. 이러한 역사적 전환의 배경과 실태에 관하여 문헌고찰을 하여 앞으로 국민건강사업 체계에 대한 논의에 참고자료가 되게 하고자 본 논문을 시도하였다. 본 논문에서 얻어진 결과를 다음과 같이 요약한다. 고도의 과학문명의 발달과 물질적 풍요로 특징짓는 현대사회속에서도 인류의 건강문제는 영원히 충족되지 못한 상태로 대두되고 있는 실정이다. 전통적으로 60년대 중반까지는 의료지식 및 기계공업의 발달과 이를 뒷받침할 재정적 지원속에 세계적으로 치료의학분야에 획기적인 발전을 가져왔다. 다른 한편으로는 건강을 태생의 권리라고 규정하여 여러나라에서 사회보장제대로써 의료시해의 균점을 위한 각가지 방도를 마련하기에 이르렀다. 그리하여 60년대말기부터 70년대로 넘어오면서 치료의학의 발달만으로는 의료수가가 앙등되어 모든 국민이 이용할 수 없을 뿐만 아니라 나아가서는 모든 국민의 건강문제를 해결하기는 어렵다는 사실을 발견하기 시작하였다. 또한 지역사회의 건강문제를 요인별로 따져보면 복잡하여 사회경제적, 문화적, 환경적, 유전적 제요인과도 밀접한 관계를 맺고 있기 때문에 신체적인 질병의 치료만이 지역사회 건강문제를 해결하는 길이 아님을 알게 되었다 그리고 지역사회의 건강문제를 해결하기 위해서는 일차보건의료사업에 중점을 두어 노력하는 것이 가장 합리적인 개선방안이라는 의견이 경험적으로 이론적으로 뒷받침되기 시작하였다. 이러한 사조는 지역사회 건강문제가 복잡한 것과 같이 그 접근방법에 있어서도 여러보건요원이 팀이 되어 임해야만 하며 그중에서도 간호팀의 활동이 중추적인 역할을 담당해야 한다고 여러 전문가들은 주장하고 있다. 따라서 앞으로의 간호사업의 방향은 지역 사회에 기반을 둔 간호사업이 실시되어야만 국민건강문제 해결에 기여하게 될 것으로 고찰되었다.

The Influencing Factors on the Degree of Nurse's Suffering Experience Caring for Terminal Cancer Patient (말기 암 환자를 간호하는 간호사의 고통경험 정도에 영향을 미치는 특성요인)

  • Purpose: The study was undertaken to examine the degree of nurse's suffering experience and to identify the influencing factors on nurses' suffering experience in Korea. Method: Data were collected using a questionnaire for 271 nurses working at 5 general hospitals in Daegu and Kyung-book province from Sep. 1, to Sep. 30, 2003. The questionnaire consists of 54 items, general characteristics(10) and nurse's suffering experience(44). All surveys were sorted and studied by frequency analysis, mean score, standard deviation, range, independent t-test, one way ANOVA, Pearson's correlation coefficient and Multiple regression. Result: The findings of this survey indicate 1) The degree of suffering experienced by nurses caring for terminal cancer patients was 2.96; 2) Demographic variables affecting the degree of nurses' suffering experience were age(F=5.62, p=.000), marital status(F=20.53, p=.000), religion(F=5.44, p=.020), career of clinical experience(F=6.96, p=.000), and feelings of end-life care(F=3.11, p=.016); 3) There were slight correlation between the subitem of nurse's suffering experience and general characteristics of subjects. For 'expanding self consciousness', age, career duration, and position; for 'forming empathy with family', age and career duration ; for 'spiritual sublimation', age, and career duration were affected variables. 4) As a result of the multiple regression analysis for predictable variables affecting nurses' suffering, it was found that 'career of clinical experience' was most significant(F=23.100, p=.000). The explanatory power of this regression formula was 17.6%. Conclusion: This study can provide the basic data useful towards improvement of nursing services for terminal cancer patients and the health of the nurse.

  • Inferior vena cava filters are increasingly being used as an alternative to anticoagulation therapy for the prevention of pulmonary embolism. However, using an Inferior vena cava filter may result in clinically significant complications. Phlegmasia cerulea dolens is a rare disease that presents with acute complete venous occlusion due to extensive thrombosis in the lower extremity. It is characterized by intense pain, edema, decreased pulses and a cyanotic extremity. We report here on a case of phlegmasia cerulea dolens that was accompanied with disseminated intravascular coagulation (DIC) as a complication of the placement of an inferior vena cava filter in a patient who had been previously diagnosed with pulmonary embolism, and the patient had recently developed a cerebral hemorrhage due to a traffic accident.

A Comparison of Nursing Interventions with Terminal Cancer Patients in a Hospice Unit and General Units (호스피스 병동과 일반병동의 말기암환자의 간호중재 비교)

  • Purpose: The purpose of the study was to compare symptoms, medical therapies, and nursing interventions with terminal cancer patients during the last four weeks of their lives in a hospice unit and general units. Method: For the descriptive survey study, data were collected by reviewing the medical records of 243 patients who died of terminal cancer at K hospital in Seoul. The data was analyzed by using Chi-square test and t-test. Result: The study findings are summarized as follows: There were higher frequencies in physical symptoms of constipation, itching sensation, pain, sleeping disturbance, soreness and dysuria for those patients in the hospice unit than those patient in general units. All emotional symptoms were recorded significantly higher for those patients in the hospice unit than those in general units. Regarding the major medical interventions, pain management was used more significantly for those patients in the hospice unit, but antibiotic therapy and resuscitation were used more significantly for those patients in general units. Conclusion: The hospice unit provided more comprehensive nursing interventions including psychological, spiritual, and family cares as well as physiological care for terminal cancer patients. The facts showed that those patients who would need hospice care in general units should be referred to the hospice unit at an appropriate time.

End Stage Ankle Arthritis with Ankle Instability Patients Treatment Results Using Autograft Ligament Reconstruction with Total Ankle Arthroplasty (인대 불안정성이 동반된 말기 족근 관절염 환자의 자가 인대 재건술과 인공관절 치환술의 치료 결과)

  • Purpose: We report the clinical and radiographic result of ligament reconstruction using plantaris and total ankle replacement in end-stage ankle arthritis with ankle instability. Materials and Methods: The study is based on the 9 cases among total 48 patients of end-stage ankle arthritis that were treated with total ankle prosthesis and ligament reconstruction from 2007 to 2009 at least 12 months follow-up. We evaluated the VAS (Visual analogue scale) pain score, AOFAS (American orthopedic foot and ankle society) score and radiographic measurements. Results: Average age was 59.4 years (53~67 years) old. VAS pain score improved from preoperative average $8.2{\pm}0.9$ (range, 7~10) to $2.7{\pm}1.7$ (range, 0~6) and the AOFAS score improved from $46.4{\pm}14.6$ points (range, 23~69) to $80.1{\pm}9.3$ points (range, 65~95) at final follow-up. Anterior draw test improved $15.2{\pm}3.4$ mm (range, 12~23 mm) to $8.8{\pm}2.6$ mm (range, 6~13mm), varus stress test improved from $13.9{\pm}4.6^{\circ}$ (range, $10-18^{\circ}$) to $6.2{\pm}4.7^{\circ}$ (range, $2-18^{\circ}$) at final follow up. Conclusion: Plantaris ligament reconstruction is good option as part of the management of ankle instability with end-stage ankle arthritis. We achieved good clinical and radiographic results.
