A Study of Occurrence of Secondary Complications and Chronic Diseases due to Aging of Spinal Cord Injury (척수손상환자의 고령화에 따른 2차합병증 발생과 만성질환발병에 관한 연구)

  • The purpose of this study is to provide basic data for effective management and prevention of chronic diseases and secondary complications as elderly spinal cord injuries(SCI). The subjects were 200 spinal cord injuries admitted to S hospital from April, 2013 to April, 2018. We investigated the occurrence of chronic diseases and secondary complication through medical records. The results showed that SCI were affected chronic diseases and secondary complications over 50years. The prevalence of chronic diseases and secondary complications over 50 years of age had the odds ratio 11.8 times higher in hypertension and 6.7 times diabetes mellitus. Secondary complications had the odds ratio Osteoporosis 7.5 times, Pneumonia 5.2 times, and central pain 0.4 times. We suggest that continuous management and service of chronic diseases and secondary complications of elderly SCI are necessary. It will be necessary to expand the target population and to study various characteristics including.

Chronic Injuries and Types of Injuries Related to Adolescent Taekwondo Athletes (성장기 태권도 선수의 만성 통증과 손상 유형)

  • Purpose: This study was designed to analyze? the chronic pain of adolescent Taekwondo athletes, to research the understanding about chronic pain of coaches and athletes, to? reduce the development of chronic pain. Materials and Methods: From March. 2006 to June 2007, 210 athletics and 12 coaches of elementary, middle and high school in Kwang-ju city were surveyed. They was evaluated by physical examination, simple radiographs and MRI. Results: In 210 athletics, 162(77.1%) athletes had chronic pain. 19(11.7%) athletes had over three regions of pain, 74(45.7%) athletes had two regions of pain, 69(42.6%) athletes had one region of pain. In 274 cases of 164 athletes, the most common region was foot and ankle(145 cases, 52.9%), followed by hand(47 cases, 17.2%) and knee(38 cases,13.9%), hip(25 cases, 9.1%), and lumbar area(19 cases, 6.9%) The chronic pain was classified by injury type. Contusion of foot was 103 cases(most common), sprain of ankles was 40cases, Contusion of hands and wrists was 28 cases, and so on. The etiologies of injury that causes the chronic pain were match injury in training that is most common, intensive exercise and match injury in competition. Conclusion: The chronic pain was common in adolescent Taekwondo athletes. The ankle and foot were the most common region of chronic pain and the match injury was the most common injury, caused the chronic pain. Although athletes had considerable mental stress about chronic pain, the treatment of chronic pain and rest were insufficient. Therefore the variable effort to prevent injury and to treatment injury must be considered importantly.

The Incidence and the Patterns of Associated Injuries of Anterior Cruciate Ligament Tear (전방십자인대 파열 시 동반 손상의 빈도 및 양상)

  • Purpose: We purposed to analyze the incidence and the patterns of the injuries associated with anterior cruciate ligament(ACL) tears. Materials and Methods: We retrospectively reviewed 547 knees that had undergone ACL reconstructions and compared the associated injuries according to the time of injury(acute vs. chronic), the cause of injury(sports vs. non-sports) and the type of sports. Results: Medial collateral ligament(MCL) injury and bone contusion were more in acute injury group less than 3 months after trauma(p=0.001, 0.019) and the injuries of medial or lateral meniscus were more in chronic injury group more than 3 months after trauma(p=0.014, 0.029). The trochlea in acute injury group(p=0.027) and the medial femoral condyle in chronic injury group(p=0.011) had higher incidence of chondral injury. Grade I, II chondral injury was frequent in acute injury group(p=0.016) and grade III, IV chondral injury was frequent in chronic injury group(p=0.017). Lateral meniscus injury was frequent in sports injury group(P=0.035). Medial collateral ligament injury was frequent in ski(P=0.005), and chondral injury was in gymnastics(P=0.017). Conclusions: The incidences of medial and lateral meniscus tears and grade III, IV chondral defects were high in chronic injury group. According to the causes of the tears and the types of sports, some differences in the incidence and pattern of associated injuries were found.

  • Clinical conditions causing the lower leg injury and pain in athletes include acute injuries which are acute compartment syndrome, acute strain injury, contusion, muscle cramps, and chronic injuries which are chronic exertional compartment syndrome, achilles tendinitis, shin splints and stress fractures. Most injuries occur during running, jumping and playing soccer. Many of these conditions relate to overuse, increased strength, training errors and environment. It is necessary to know the cause of injuries and treatment methods to prevent the sports related lower leg injuries.

  • Forefoot injuries are common in runners or martial art athletes, but due to the anatomical stability, collateral ligament injury of the hallucal interphalangeal joint has been rarely reported. We report a college Taekwondo athlete with chronic varus instability of the hallucal interphalangeal joint due to chronic lateral collateral ligament rupture. The patient had been treated with lateral ligament reconstruction and achieved good clinical outcome.

  • Background : Toxic oxygen free radicals have been implicated as important pathologic mediators in many clinical disorders. Enhancing the intracellular content of antioxidant enzymes(superoxide dismutase, glutathione peroxidase, and catalase) can provide means of limiting biological damage caused by oxygen free radicals. The oxygen free radicals and changes of antioxidant enzymes are though to play a role in the pathogenesis of chronic obstructive pulmonary disease. Method : To investigate the pulmonary oxygen radical injury and the protective role of antioxidant enzymes in Chronic obstructive pulmonary disease(COPD), author measured the amount of thiobarbituric acid reactants, the activities of antioxidant enzymes and the sulfhydry1 groups of glutathione in serum and red blood cells from the patients with COPD(COPD patients) and the normal controls. Results : The thiobarbituric acid reactant in serum and red blood cells of COPD patients was increased than those of the normal controls, and the superoxide dismutase activity in red blood cells was no statistical difference in both groups. But the glutathione peroxidase and catalase activities in red blood cel1s of COPD patients were significantly lowered than those of the normal controls. The sulfhydry1 groups in serum and red blood cells were no statistically difference in both groups. Conclusion : These results suggest that the increased thiobarbituric acid reactants in serum and RBCs of chronic obstructive pulmonary disease mean oxygen radical toxicity, and the decreased glutathione peroxidase and catalase activities in RBC could take pan in pathogenesis of chronic obstructive pulmonary disease.

  • Injury of articular cartilage can be classified into acute injury and chronic degenerative osteoarthritis Acute mechanical trauma on articular cartilage causes injuries that are divided into three distinct types based on the depth of injury: microdamage, chondral fracture, osteochondral fracture and each type has different potential of healing response and long-term prognosis. Articular cartilage undergoes degradation in response to a number of stimuli and eventually degenerative osteoarthritic changes will progress. The extent of initial injury to the articular cartilage is the most important factor affecting the long-term outcome of the healing response and other variables such as the size of lesion, site, age, activity level, obesity, limb alignment are also important factors. In this review, the pathophysiology that occurs within articular cartilage after different injuries and the effect of nonsurgical treatment mainly in physicochemical aspect and biological aspect will be discussed.

  • In this paper, we evaluated the performance of the automatic classifier applied for the discrimination of acute alcoholic liver injury and chronic liver fibrosis. The classifier uses the discriminant peaks of the preprocessed Raman spectrum as a feature set. In preprocessing step, we subtract baseline and apply Savitzky-Golay smoothing filter which is known to be useful at preserving peaks. After identifying discriminant peaks from the spectra, we carried out the classification experiments using MAP and neural networks. According to the experimental results, the classifier shows the promising results to diagnosis alcoholic liver injury and chronic liver fibrosis. Classification results over 80% means that the peaks used as a feature set is useful for diagnosing liver disease.

  • With the increase of participation in the sport activities, there has been a commensurate rise in the number of sport injuries. A more commonly encountered injured region in the upper-extremity is the wrist. Sport injuries are often characterized as overuse and traumatic. Traumatic injuries include fractures, dislocations, and ligament tears often seen in contact or collision sports. Overuse injuries, represented damages by a level of repetitive microtrauma sufficient to overwhelm the tissues$^{\circ}{\O}$ ability to adapt, include inflammatory conditions such as De Quervain$^{\circ}{\O}s$ disease, extensor carpi ulnaris tendinitis, intersection syndrome. Also included is a traumatic problem such as stress fractures of the hook of the hamate, subluxation of the extensor carpi ulnaris tendon, chronic scapholunate instabilities, and degenerative triangular fibrocartilage tears. This review will focus on both overuse and traumatic injuries of the wrist in the athletes. A significant emphasis will be placed on the evaluation, pearls and pitfalls of conservative and operative treatments.
