The Longitudinal Interrelationships of Multidimensional Social Exclusion among Married Immigrant Women in Korea (결혼이주여성이 경험하는 다차원적 사회적 배제 영역간의 종단적 상호관계)

  • The purpose of this study was to examine how various dimensions of social exclusion experienced by married immigrant women are affected longitudinally. Autoregressive cross-lagged analysis between sub-dimensions of social exclusion of married immigrant women was conducted to investigate the direction of interrelationships. The data were collected each year through a three-wave longitudinal survey from 2011 to 2013. The results showed that statically significant autoregressive effects appeared among economic exclusion, community exclusion, educational exclusion, and ethnic exclusion. The cross-lagged effects of the sub-dimensions of social exclusion showed that married immigrant women with higher levels of the economic exclusion tend to experience higher levels of the educational exclusion, and ethnic exclusion at the later measurement point, even when controlling for their previous level of social exclusion. It was found that economic exclusion as a more fundamental precedent factor positively predicted other dimensions of social exclusion. The results suggested that financial support and job-related services should be strengthened for active social inclusion of married immigrant women, and early intervention to prevent chronicization of social exclusion should be needed.

  • Degenerative disc disease has traditionally been thought of as low back pain caused by changes in the nucleus pulposus and annulus fibrous, in recent studies, however, changes in the upper and lower endplates cause degeneration of the disc, resulting in mechanical pressure, inflammatory reactions and low back pain. Recently, the bone marrow of the vertebral body-endplate-nucleus pulposus and annulus fibrous were considered as a single unit, and the relationship was explained. Once the endplate is damaged, it eventually aggravates the degeneration of the bone marrow, nucleus pulposus, and annulus fibrosus. In this process, the compression force of the annulus fibrosus increases, and an inflammatory reaction occurs due to inflammatory mediators. Hence, the sinuvertebral nerves and the basivertebral nerves are stimulated to cause back pain. If these changes become chronic, degenerative changes such as Modic changes occur in the bone marrow in the vertebrae. Finally, in the case of degenerative intervertebral disc disease, the bone marrow of the vertebral body-endplate-nucleus pulposus and annulus fibrous need to be considered as a single unit. Therefore, when treating patients with chronic low back pain, it is necessary to consider the changes in the nucleus pulposus and annulus fibrosus and a lesion of the endplate.

Phenotype Changes in Immune Cell Activation in Obesity (비만 환경 내 면역세포 활성화 표현형의 변화)

  • Immune and metabolic systems are important factors in maintaining homeostasis. Immune response and metabolic regulation are highly associated, so, when the normal metabolism is disturbed, the immune response changed followed the metabolic diseases occur. Likewise, obesity is highly related to immune response. Obesity, which is caused by an imbalance in energy metabolism, is associated with metabolic diseases, such as insulin resistance, type 2 diabetes, fatty liver diseases, atherosclerosis and hypertension. As known, obesity is characterized in chronic low-grade inflammation. In obesity, the microenvironment of immune cells became inflammatory by the unique activation phenotypes of immune cells such as macrophage, natural killer cell, T cell. Also, the immune cells interact each other in cellular or cytokine mechanisms, which intensify the obesity-induced inflammatory response. This phenomenon suggests the possibility of regulating the activation of immune cells as a pharmacological therapeutic strategy for obesity in addition to the common pharmacological treatment of obesity which is aimed at inhibiting enzymes such as pancreatic lipase and α-amylase or inhibiting differentiation of preadipocytes. In this review, we summarize the activation phenotypes of macrophage, natural killer cell and T cell, and their aspects in obesity. We also summarize the pharmacological substances that alleviates obesity by regulating the activation of immune cells.

Adult Attachment Styles and Insomnia (성인 애착유형과 불면증)

  • Introduction: Human attachment is known to be closely associated with psychophysiological phenomenon. However, there have not been enough researches on the relationship of the attachment with sleep, especially with insomnia. The objective of the present study was to investigate the relationship between adult attachment styles and insomnia in community-dwelling population. Methods: One hundred seventy seven community-dwelling adults (74 males and 103 females;mean age $41.23{\pm}8.44$) participated in the current study. To assess the attachment styles (secure, dismissing, preoccupied and fearful), self-reporting Relationship Style Questionnaires (RSQ) were completed by the participants. Presence, type, frequency and duration of insomnia in the last month were also investigated. Results: Compared to subjects without insomnia, subjects with insomnia had higher fearful attachment scores (t=2.87, p=0.005). Higher fearful attachment score were found in all subtypes of insomnia (sleep-onset insomnia, t=2.33, p=0.021;maintenance insomnia, t=2.92, p=0.004;terminal insomnia, t=2.89, p=0.004). Subjects with frequent (${\ge}3$ per week) insomnia had higher fearful attachment scores than subjects with infrequent (${\le}2$ per week) insomnia (t=2.57, p=0.012). In addition, subjects with chronic insomnia (${\ge}6$ months) had higher preoccupied attachment scores relative to subjects with transient insomnia (<6 months), (t=2.57, p=0.012). Conclusion: In the current study, attachment styles were different depending on the characteristics of insomnia. The fearful attachment was associated with the presence of insomnia, while the preoccupied attachment was associated with the chronicity of insomnia. These findings suggest that there may be some relationship between the adult attachment styles and the clinical features of insomnia.

Need Assessment of Home-based Cancer Patients (재가암환자 요구도 조사)

  • Purpose : Cancer, one of the major causes of death in Korea, tends to become chronic due to the rapid development of diagnostic and therapeutic methods. As a result, the number of home-based cancer patients is in the increasing trend. However, on account of the insufficiency of continuous and comprehensive cancer patient management system, a number of cancer patients are left in a defenseless state. This study was designed for need assessment of home-based cancer patient to establish the community-based health care system for the comprehensive and continuous health care service to improve the quality of life of cancer patients and reduce rare burdens of their families. Methods : Through making a survey for needs assessment toward the health care service, the 455 respondents among home-based cancer patients answered the given enquetes to analyze the management status and problems of home-based cancer patients Results : 1) Unsatisfaction rates of pain control is 25.5 percent for mild cases, 46.5 percent for severe cases. 2) According to the needs assessment of home-based cancer patients, most of the respondents want to receive economical support, alleviation for the pain and symptoms, and the information of health care and consultation. So these needs account for the main contents of the home-based cancer patient management plan. 3) In the aspect of the satisfaction rate for basic care need, most items account for $20{\sim}30%$ of satisfaction. And the proportion of need for special case is under 5%, satisfaction rate for special care need is about 50% of satisfaction. So the home-based cancer patients are not being cared sufficiently. Conclusion : According to the result of need assessment, many home-based cancer patients received inadequate pain and symptom management. And Satisfaction rate for basic and special care need is low. So development of comprehensive and continuous health care service to improve the quality of life of cancer patients and reduce care burdens of their families is very necessary.

  • Temporomandibular disorder(TMD) is relatively prevalent disease, and quality of life may be impaired in TMD patients. Like general population, dental hospital workers are also exposed to the risk of TMD. But, many of them tend to overlook or tolerate their symptoms for lack of time and interest. Therefore, problems may become more serious, causing interference of performing task and decrease of quality of life. The aim of this study were to obtain data for TMD prevalence in dental hospital workers and to evaluate quality of life according to TMD symptoms. Subjects were recruited from Wonkwang University Dental Hospital. After consent, subjects completed quality of life questionnaire and were evaluated for subjective and objective signs and symptoms of TMD. Subjects were classified into 4 groups : (1) normal group (2) joint disorder group, (3) local myalgia group, and (4) myofascial pain group. The result of the study indicated that TMD negatively influences the quality of life in dental hospital worker. TMD symptoms can deteriorate quality of life in dental hospital worker. Future effort to make protocol for proper management is needed.

A Statistical Survey of Foreign Bodies in Air and Food Passage (Report V) (식도 및 기도이물의 통계적 고찰(제5보))

  • Foreing bodies of the esophagus and tracheobronchus are likely to occur whenever a child places an inedible object in the mouth and are common in the older age group particularily in edentulous individuals or preexisting disease. Our department had already reported the statistical survey at first in 1954, successively in 1962, 1963 and 1968, respectively according to the age, sex, variety of foreign bodies, location and duration of the lodgement. Now, we have experienced the new 186 cases during the consecutive 5 year period from January 1973 to December 1977 and reported as a 5th report comparing with previous report. The results were as follows. 1. In the incidence of foreign bodies, coins were the most common and pointed metal, disc, bony fragment were followed in order in the esophagus while the peanut and corn were common in the air passage. 2) 79 cases (56.4%) were males and 61 cases (43.6%) were females in the esophagus, which was not a significant difference to note. But in the air passage, males accounted for 33 cases (71.7%) compared to 13 cases (28.3%) of females. 3) In the aspect of age incidence, child 10 years or under accounted for almost all cases in the esophagus as well as in the air passage. 4) In the location of foreign bodies in the esophagus, the first narrowing was the most common, while in the air passage right bronchus had a high incidence but the difference of incidence between right bronchus and left bronchus was not remarkable. 5) The duration of lodgement was 24 hours or less in more than half of the all cases. 6) In the aspect of kind of coin, 10 won coins accounted for majority of the cases (67.0%) but the incidence of 100 won coins were tend to increase gradually in these years.

Prospects of Triticale as Fodder and Feed in Farming of Bangladesh (방글라데시 농업에서 트리티게일의 조사료 및 곡물사료이용 전망)

  • This paper reviews the present situation of Triticale cultivation and examines the potentiality of contribution to livestock as well as poultry sector in Bangladesh Agriculture. Triticale is a human-made cross between rye and durum wheat that has the ability to produce quality green fodder, and then re-grow after first and second cutting to produce grain. In Bangladesh, it is a non-traditional cereal that grows well during the cool and dry Rabi season (November-March) when fodder and feed scarcity is a major limiting factor for ruminant livestock. In Bangladesh Triticale was started to grow in the late Ninety's. The scientists of Bangladesh Agricultural Research Institute (BARI) were first introduced triticale in Bangladesh. Still now the situation of Triticale is grown as fooder and feed in Bangladesh within the scientists under trial. High quality grass fodder was obtained by cutting green triticale plants twice, at 35 and 50 days after seeding, while later the ratooning tillers produced grain to a yield of 1.1-2.4 t/ha of grain for poultry feed or human food. Triticale straw was twice as nutritious as rice or wheat straw and its grain contained more protein than other cereals. Researchers and farmers have also successfully made triticale hay and silage from a mixture of triticale green cuttings, rice straw and molasses. A feeding trial at Bangladesh Livestock Research Institute(BLRI), Savar station showed a large(46%) increase in cow live weight gain and a 36% increase in milk yield (but no change in milk quality or dry matter intake) in cows fed triticale silage compared with those fed rice straw over a period of nine weeks. In another feeding trial, it was found that triticale grain was a good replacement for wheat in the feed blend for chickens in Bangladesh. So it will be a good chance to alive our livestock as well as poultry sector if triticale enters to our existing cropping system as fodder cum grain. The challenge in Bangladesh is to identify fodder technologies that match existing small-scale farmer cropping patterns without needing major inputs or increasing risks. Preliminary field experiments revealed that triticale is a crop with good potential to produce quality fodder and grain for small scale farmers in Bangladesh.

The Effect of Self-Efficiency and Instrumental Activities of Daily Living of Local Community Stroke Patients on Quality of Life in Caregiver (지역사회 거주 뇌졸중환자의 수단적 일상생활동작과 자기 효능감이 보호자의 삶의 질에 미치는 영향)

  • Objective : This study is aimed to examine the degree of independence in stroke patients residing in a community based on their self-efficacy and Instrumental Activities of Daily Living and also correlation between community-residing stroke patients and their guardians by figuring out the life quality of their guardians. Methods : For total two weeks between May 12th and 23rd, 2014, this researcher collected data by using a survey (K-IADL, a scale to measure self-efficacy, WHOQOL-BREF) targeting 138 stroke patients who received over 24 points in MMSE-K among the outpatients of three hospitals located in Busan Metropolitan City and two hospitals located in Geoje-si, Gyeongsangnam-do, and total 125 sheets were finally used for analysis. Results : Among the 125 patients, 70 were males, and 55 were females, and their average age was 58.72 (16 to 82). The subjects' general self-efficacy (p<0.01) and detailed self-efficacy (p<0.01) indicated significant correlation with their guardian's life quality, and there was no significant correlation found between their Instrumental Activities of Daily Living and their guardian's life quality. Conclusion : Their general self-efficacy and detailed self-efficacy indicated correlation with their guardian's life quality, and among their general characteristics, the factor of their financial situation indicated correlation with their guardian's life quality. In conclusion, when stroke patients' self-efficacy is higher, their guardian's life quality increases, too, and if their financial situation is more favorable, their guardian's life quality gets higher. However, stroke patients' Instrumental Activities of Daily Living did not indicate correlation with their guardian's life quality. When looking at the chronic trend of outpatients, therapeutical plan must be established about the self-efficient recovery for the development of those who are not satisfied.

Diagnostic Methods Used in the Bone Infections in Children (소아기 골 감염의 진단방법에 관한 조사)

  • Purpose: To prevent residual physical disability and chronic infection, prompt diagnosis and adequate treatment are important in the skeletal infections in children. Although radioisotope scanning is knwon as the method of choice for early diagnosis of bone infection, we conducted a study on twenty nine children who had skeletal infections to reevaluate the most appropriate way in diagnosis and management. Methods: A retrospective study was conducted on twenty nine children, who were admitted to the departments of Pediatrics and Orthopedic Surgery and who had acute osteomyelitis or septic arthritis, through review of medical records, radiologic & radioisotope study results. Their diagnoses were confirmed by bacteriologic cultures on the aspirated specimens from suspected bony lesions. Results: 1) Among twenty nine patients, there were 6 infants including 5 newborn infants, and 23 children were aged between 1 and 15 years. Male to female ratio was 1.4 to 1. 2) Point tenderness was noted in all cases, and the common physical signs were swelling, limitation of motion, fever and local heat in the order of frequency. 3) Fifty two percents of the patients were diagnosed within a week after onset of symptoms and all cases were within 15 days. 4) Leukocytosis was noted in only 58.6% of cases but erythrocyte sedimentation rate was increased in all cases except only one case. Staphylococcus aureus was revealed as the most common etiologic agent. 5) Radioisotope scans showed hot uptake in five of six cases(83.3%) who had no abnormal finding on plain skeletal radiolograms. Conclusions: Although radioisotope scan and MRI are helpful in early diagnosis before radiologic finding was detected on plain X-ray film, the antimicrobial therapy can be started after bacteriologic study of the aspirated specimens from the suspected skeletal lesions if skeletal infection is highly suspected clinically.
