Clinical Experiences of Trabeculectomy with Mitomycin C (Mitomycin C를 사용한 섬유주절제술의 임상경험)

  • The use of intraoperative application of Mitomycin C at the filtration site has been known to improve the surgical outcome in glaucomatous eyes with high risk for failure of trabeculectomy. The author performed trabeculectomies with intraoperative Mitomycin C on 25 eyes of 20 patients with poor surgical prognosis to study the efficacy and safety of this technique in glaucomatous patients with high risk for failure of trabeculectomy. After the preparation of a scleral flap, 0.2mg/ml solution of Mitomycin C was applied between Tenon's capsule and the sclera for 2 minutes. The exposed area was then irrigated with 200 ml of balanced salt solution. The follow-up period was from 1 to 7 months (mean 3.8 months). The mean preoperative intraocular pressure (IOP) was $38.6{\pm}6.6mmHg$. The mean final postoperative IOP was $11.7{\pm}3.8mmHg$. Twenty three (88%) of the 25 eyes were successfully controlled with the IOP of less than or equal to 20 mmHg without glaucoma medication. There were early postoperative complications of hyphema in 5 eyes (20%), shallow anterior chamber in 4 eyes(16%), punctate keratopathy in 3 eyes (12%), aqueous leaking from conjunctival wound in 2 eyes (8%), encapsulated bleb in 1 eye (4%) and choroidal detachment in 1 eye (4%), and 4 eyes had long term hypotony lasting more than 2 months. Although Mitomycin C is simple to use and effective adjunct to trabeculectomy, further study will be needed to determine the mechanism of action, indication, dosage and optimal exposure time of Mitomycin C.

  • Ultrafiltration (UF) tests performed on traditional soy sauce (kanjang) using UF flat membrane test cell unit with thin film laminar flow similar to the spiral type membrane module in batch operations revealed reduction in permeate flux is proportional to the logarithm values of volume reduction ratio of the retentate kanjang at different feed rate of kanjang. Feed rate of 1.5 L/min was found to be adequate for long-term UF operation of kanjang using the test unit attached with MW cut-off size of 200,000 dalton polyoleffin plastic membrane in batch operation with the least concentration polarization. The higher the feed rate of kanjang, the lower the permeability of total nitrogen and NaCl, resulting in lower optical density at 500 nm and lower permeability of minerals such as Cu, Mn, and Mg. Microbial cells were completely rejected regardless of the feed rate, whereas most free amino acids were not.

Trends in Recovering Dissolved Boron from Wastewater and Seawater (폐수와 해수로부터 용존 붕소를 회수하는 연구동향)

  • A lot of researches have been carried out on the recovery of resources from the seawater all over the world. The boron concentration in seawater is low about at 4.5ppm, but considering the volume of seawater, the total weight of dissolved boron amounts to about 5.4 trillion tons. The boron is an essential resource in about 300 kinds of industries. Korea has imported all of the boron and spent more than 700 billion won each year. In this article, we introduce the domestic and international research trends and technologies for removing or recovering the boron from wastewater and seawater. Most of the researches have been conducted to remove the boron from the desalination process, and to recover the boron mainly from wastewater and brine. The technique for the recovery of the dissolved boron includes the ion exchange, which is the most representative, the adsorption membrane filtration (AMF), solvent extraction, and so on.

Relationship of Glomerular Basement Membrane Alterations to Epithelial Cell Structure and Clinical Parameters in Alport Syndrome (Alport 증후군에서 사구체 기저막의 형태학적 변화와 사구체 상피세포의 구조 및 임상지표와의 관계)

  • Purpose : This study was performed to evaluate the relationship between glomerular basement membrane (GBM) alterations to epithelial cell (EpC) structure and renal function in Alport Syndrome (AS) patients. Methods : Fifteen patients diagnosed with AS (4-26yrs) were examined. The GBM in AS was categorized as : C1) normal, C2) minor alterations (widening of lamina rara interna or externa without lamina densa change), C3) nonspecific splitting of lamina densa, C4) basket-weaving pattern of lamina densa splitting. The length of each GBM portion along the epithelial side was measured on the systematically obtained electron microscopic photographs. Furthermore to obtain an objective assessment of the degree of glomerular EpC foot process change, the number of slit pores along $10\;{\mu}m$ of peripheral GBM in each category was obtained. Results : The percentage of normal GBM portion (C1) correlated inversely with daily protein excretion (g/day/$m^2$, P<0.05) and sum of the percentage of abnormal GBM portion (C2+C3+C4) had direct correlation with daily protein excretion (g/day/$m^2$, P<0.05). There were no significant relationships between the percentages of other categories of GBM alterations and creatinine clearance or protein excretion. There were no significant relationships between of creatinine clearance in relation to normal GBM(C1) portion as well as that in relation to sum of the percentage of abnormal GBM portion (C2+C3+C4). GBM abnormality did not correlate with age at biopsy. Conclusion : The extent of GBM structural abnormality is related to proteinuria in AS but the epithelial response is uniform even though the GBM ultrastructural lesions are not.

  • In this study transmembrane pressure (TMP) was measured with respect to operational time by applying the sinusoidal flux continuous operation (SFCO) for the hollow fiber membrane. The hollow fiber module which has $100cm^2$ of effective area and $0.45{\mu}m$ nominal pore size was submerged in the activated sludge solution of MLSS 5,000 mg/L. The critical permeate flux was measured as $26.6L/m^2{\cdot}hr$ by the method of continuous flux step change. TMPs of the filtration/relaxation (FR), FR with backwashing (FR/BW) and SFCO modes were measured. The SFCO mode was more effective than FR and FR/BW modes below the critical permeate flux such as 15, 20 and $25L/m^2{\cdot}hr$. However, the FR/BW was confirmed as more effectively fouling controlled mode than SFCO mode above the critical permeate flux.

  • In general, whey obtained from various cheese batches is being reused, so as to improve the texture and to increase the yield and the nutrient value of the various final milk-based products. In fact, re-usage of whey proteins, including whey cream, is a common and routine procedure. Unfortunately, most bacteriophages can survive heat treatments such as pasteurization. Hence, there is a high risk of an increase in the bacteriophage population during the cheese-making process. Whey samples contaminated with bacteriophages can cause serious problems in the cheese industry. In particular, the process of whey separation frequently leads to aerosol-borne bacteriophages and thus to a contaminated environment in the dairy production plant. In addition, whey proteins and whey cream reused in a cheese matrix can be infected by bacteriophages with thermal resistance. Therefore, to completely abolish the various risks of fermentation failure during re-usage of whey, a whey treatment that effectively decreases the bacteriophage population is urgently needed and indispensable. Hence, the purpose of this review is to introduce various newly developed methods and state-of-the-art technologies for removing bacteriophages from contaminated whey and whey products.

  • Angiotensin-I-converting enzyme (ACE) inhibitory peptides have functional and potential properties of casein hydrolysates that are used in the development of food ingredients and anti-hypertensive hydrolysates derived from sodium caseinate enzymatic hydrolysates. Sodium caseinate could be treated by various kinds of commercial proteases, and then could be treated with the enzyme combination. Ultrafiltration treatment can be used to generate hydrolysates that can be used to determine ACE inhibitory activity. In general, hydrolysate quality can be evaluated by changes in hydrolysis characteristics, ACE inhibitory activity, as well as functional properties such as solubility, foam capacity, cytotoxicity, free radical-scavenging effects, and sensory evaluation. In this review, we present an overview of the ACE inhibitory peptides obtained by performing enzymatic hydrolysis on various sources to identify food ingredients and functional foods that reduce hypertension.

  • The determination of uranium and its isotopes in spent nuclear fuels by isotope dilution mass spectrometry (IDMS) has been studied. The spent fuel samples were dissolved in 8 M $HNO_3$ or its mixture with 14 M $HNO_3-0.05M$ HF. The dissolved solutions were filterred on membrane filter with $1.2{\mu}m$ pore size. The uraniums in the spiked and unspiked sample solutions were quantitatively adsorbed by anion exchange resin, AG 1X8 and eluted with 0.1 M HCl. The contents of uranium and its isotopes ($^{234}U$, $^{235}U$, $^{236}U$$^{238}U$) in the spent fuel samples were determined by isotope dilution mass spectrometric method using $^{233}U$ as spike. The spike reference solution was standarized by reverse isotope dilution mass spectrometry (R-IDMS) using natural and depleted uranium. The results from IDMS were in average relative difference of 0.34% when compared with those by the potentiometric titration method.

The Influence of Various Factors upon the Membrane Filter Technique on Raw Water of the Nak-Dong River (낙동강 원수에 대한 대장균군 막여과시험법에 있어서 여러 인자가 결과에 미치는 영향)

  • In this study, the membrane filter method was compared to the MPN method for the analysis of total coliforms from raw water using raw test waters and controls including 6 standard strains of coliforms, and the various factors were analyzed on the detection of general and fecal origin coliforms. The range of error rate for the detection of 5 standard strains using the membrane filter and the MPN methods was 0 to 6% and 45 to 133%, respectively. The error rate of the membrane filter method was lower than that of the MPN method. The membrane filter method (m-Endo) showed 10% (11 out of 111) of difference for the detection sensitivity of coliforms isolated from raw water compared to the MPN method (BGLB). The membrane filter method was less affected by the factors including temperature, turbidity, charcoals of powder form, contamination, and reverse pressure. In conclusion, the membrane filter method is a better method for the analysis of total coliforms from raw water than the MPN method, considering the accuracy of detection and the tolerance to various experimental factors.

Pilot-scale preparation and physicochemical characteristics of microbiological agar from Gelidium amansii in Korea (국내산 우뭇가사리로부터 미생물 배지용 한천의 pilot규모 정제와 특성)

  • Agar for microbiological medium was prepared with pilot-scale for industrial application by the process of microfiltration ($0.4 {\mu}m\;pore size$) in $40{\~}50{\circ}C$, washing with sot water, and treatment with $0.25\;N NaOH\;at\;70{\circ}C$. Transparency, gel strength, viscosity, sulfate content, and syneresis ratio of agar prepared from Gelidium amansii was compared with commercial agar for microbiological medium. Gel strength and transparency were increased with processing, however, it's viscosity, sulfate content, and syneresis ratio were reduced. The final agar product was superior to commercial agar for microbiological medium.
