Nursing Needs for Elderly Patients with Regional Anesthesia during Operation (부위마취 수술 노인환자의 수술 중 간호요구)

  • Purpose: The purpose of this study was to identify the nursing needs of elderly patients who got a surgical operation under regional anesthesia(spinal, epidural, nerve block). Methods: The participants were 126 elderly patients who completed a questionnaire. The data were collected October 1 and October 31, 2012, and analyzed using descriptive statistics, t-test and ANOVA, $Scheff{\acute{e}}$ test. Results: The mean score for the nursing needs of elderly patients administered regional anesthesia during surgery was $3.08{\pm}0.38$, The scores for specific nursing needs were as follows: educational needs ($3.47{\pm}0.50$), spiritual needs ($3.37{\pm}0.78$), physical needs ($3.31{\pm}0.46$), emotional needs ($2.72{\pm}0.50$), and environmental needs ($2.51{\pm}0.47$). There were significant differences in nursing needs relative to gender, religion, and spouse status. Additionally, there were significant differences in nursing needs according to surgery department, the length of time the surgery, and the ASA(American Society of Anesthesiologists) physical classification. of the operation-related characteristics. Conclusion: When caring for elderly patients during the surgery, nurses must provide adequate information about the surgery and anesthesia. Further studies are needed to develop and evaluate nursing interventions to provide quality surgical care for the elderly patients.

The Effect of Hand Holding and Nei-Guan Acupressure on Anxiety and Pain under Local Anesthetic Patients during Surgery (국소마취 환자의 수술시 손잡아주기, 내관지압이 불안과 통증에 미치는 효과)

  • This research is to figure out the influence of hand holding and Nea-Guan acupressure to anxiety and pain that local anesthesia patients feel in out patient operation room. Non equivalent control group pretest-posttest time series design was adopted and the data had been collected from Jul. to Oct. 2013. Total 78 local anesthesia patients of out patient operation room in the H Hospital were allotted by control group, experimental group 1( hand holding) and experimental group 2(Nea-Guan acupressure) which consisted of 26 patients each. Hand holding and Nea-Guan acupressure in this research was validated to relieve the anxiety and the pain of the local anesthesia patients in operation and can be adopted as an efficacious nursing interventions.

A Study on Anesthesia and Operating Room (OR) Nurses' Perception and Performance of Privacy Protection Behavior for Patients Undergoing General Anesthesia Surgery and Patients' Satisfaction with Operating Room Hospitalization Experience (프라이버시 보호 행동에 대한 전신마취 수술환자와 마취⋅수술실 간호사의 인식, 실천 정도 및 전신마취 수술환자의 입원경험 만족도 연구)

  • Purpose: This study aims to examine level of perception and performance of privacy protection behavior of anesthesia and operating room (OR) nurses for patients who underwent general anesthesia surgery. Methods: Data collection was conducted from August 2020 to January 2021 for a total of 101 participants, consisting of 49 patients and 52 nurses. Independent t-test and Pearson's correlation were conducted using SPSS 21. Results: Anesthesia and OR nurses showed the highest score in patient privacy, followed by patient information management, body privacy, and the lowest score in communication. There was a significant difference between the patient information and the communication. Conclusion: Anesthesia and OR nurses had the highest level of perception and performance of patient privacy protection behavior for body privacy, and the lowest for communication. In addition, there was a significant difference in patient information management and communication. In order to protect the privacy of patients undergoing general anesthesia surgery, efforts are needed to learn standardized nursing knowledge, attitudes, and practice.

Effect of favorite music therapy on anxiety and vital sign in patients undergoing gynecologic surgery using the general anesthesia (선호 음악요법이 전신마취 산부인과 수술 환자의 불안과 활력 징후에 미치는 효과)

  • This study examined the effects of favorite music therapy on the anxiety and vital signs at each point in gynecologic surgery using the general anesthesia. The research design was a non-equivalent control group non-synchronized design. The data were collected from May 1 to July 30, 2013 and the participants were 44 patients (experimental group, 21, control group, 23) received music therapy while waiting for anesthetic induction and PACU (Post Anesthesia Care Unit). Repeated measures ANOVA was performed to analyze the data by SPSS 18.0. Music therapy reduced the anxiety level at inducing the anesthetic time, and awakening time (p=.003; p=.011). The systolic blood pressure maintained stability at discharge from the PACU (p=.023), and pulse rate was stable at the awakening time (p=.016). This findings support the use of music as a nursing intervention to reduce anxiety and maintain the vital signs for gynecologic surgery patients under general anesthesia.

  • 무의식환자와 수술후환자의 구강청결방법으로 Milk of Magnesia를 사용하였을 때의 효과여부와 수술 환자에게 Milk of Magnesia와 함께 Chewing Gum을 사용했을 때의 효과여부를 측정하기 위하여 인디아나대학 Medical Center에 입원한 무의식환자 2명과 수술 후 마취에서 깨어난 환자 4명을 대상으로 하여 7-8시간 간격으로 2일간 구강청결을 실시했다. 본 연구는 1969년 가을학기에 시행했으며 분석방법으로는 표본 크기가 적은 경우에도 사용할 수 있는 방법의 T-test를 사용했으며 그 결과는 다음과 같다. 1. 무의식환자에서 Milk of Magnesia로 구강청결을 시행했을 때의 구강상태는 시행전보다 유의적으로 호전되지 못했다(p>0.1). 2. 수술 후 환자에서 Milk of Magnesia로 구강청결을 시행했을 때의 구강상태는 시행전보다 유의적으로 호전되었다(p>0.05). 3. 수술 후 환자에서 Milk of Magnesia와 Chewing Gum을 동반하여 사용했을 때의 구강상태는 유의적으로 호전되었다(p<0.05).

Convergence study on Effects of Music Therapy in Patients Undergoing Prostatectomy with Spinal Anesthesia (척추마취 전립선절제술환자의 음악요법효과에 대한 융합연구)

  • The purpose of this convergence study was to identify the effects of favorite music therapy on anxiety, fatigue, and vital signs of patients undergoing prostatectomy with spinal anesthesia. This study used a nonequivalent control group design. A sample of 45 patients was included. The experimental group was given music therapy during operation. The data were collected using a structured questionnaire and monitoring at 30 min before operation, at 20 min and 40min undergoing operation, and at arrival recovery room after operation. Data were analyzed using descriptive statistics, ${\chi}^2-test$, Fisher's exact test, t-test, repeated measures ANOVA. The experimental group reported significantly lower anxiety and lower fatigue than the control group(p=.001; p=.020). However there were no significant differences in the systolic blood pressure, diastolic blood pressure and pulse rate between groups(p=.821; p=.473; p=.782). This findings indicate that the tailored favorite music therapy can be an effective nursing intervention for patient undergoing prostatectomy with spinal anesthesia to reduce anxiety and fatigue related to operation.

Effects of the Heated-Humidified Breathing Circuit Applied on the Body Temperature, Shivering, and Thermal Comfort of General Anesthesia Patients (가온가습호흡회로 적용이 전신마취 환자의 체온, 전율, 온도 편안감에 미치는 효과)

  • Purpose: The purpose of this study was to identify the effects of the heated-humidified breathing circuit applied on the body temperature, shivering, and thermal comfort of general anesthesia patients. Methods: The participants were patients who received general anesthesia at University Hospital K located in City B, with 25 patients in the experimental group and 25 patients in the control group. The period of this study was from Mar 19 to Apr 26, 2019. The experimental equipment included a heated-humidified breathing circuit, which connects the intubation tube with the anesthesia machine for mechanical ventilation after airway intubation in general anesthesia patients. Results: The body temperature, shivering, and thermal comfort after surgery were significantly different between the two groups (p<.001). However, the body temperature during surgery was slightly different between the experimental group and the control group. Conclusion: A heated-humidified breathing circuit may be actively used in a warming method to prevent the hypothermia of general anesthesia patients.

The Effects of Relaxation Therapy on Pain and Anxiety in Spinal Anesthesia Surgery Patients (척추마취 수술환자의 불안감소를 위한 이완요법 적용 효과)

  • Purpose: The purpose of this study was to examine the effects of relaxation therapy to reduce anxiety level among spinal anesthesia surgery patients. Methods: The study employed a quasi-experimental pre-and-posttest design with nonequivalent control group. The experimental group was provided with the 15-min muscle relaxation therapy on the day before the surgery and an hour before the spinal anesthesia. Data were collected from 44 patients (22 in experimental and 22 in control group) who were planned to have spinal anesthesia surgery from an orthopedic surgery department at a general hospital in Seoul from January 13 to March 21, 2006. Results: The experimental group reported lower level of state anxiety after the surgery (23.18 vs. 33.64) than did control group but was not statistically significant. There were no significant differences in blood pressure or pulse rate between experimental and control groups after surgery. Conclusions: The further research should be conducted to polish the muscle relaxation therapy for various types of patients and to create more comfortable environment setting for application of the therapy. The muscle relaxation therapy may be a convenient and useful nursing intervention to reduce anxiety among spinal anesthesia surgery patients if more studies prove its effectiveness.

The Effect of Chest Meridian Massage on Post- Anesthetic Recovery of General Anesthesia Patients (흉부경락마사지가 충수돌기 절제술 환자의 마취 후 회복에 미치는 효과)

  • Purpose: This study aims at confirming the effects of the chest meridian massage on the post-anesthetic recovery of general anesthesia appendectomy patients. Method: The research was post-test designed as a nonequivalent control group among quasi-experiments tested. The data was collected from June 20, 2003 to October 14, 2003 at a hospital in P city. In the collected data, using SPSS Win 10.0 program, these general features were analyzed by real numbers and percentages; the homogeneity among variables by $X^2-test$ and t-test and the research hypothesis by the t-test. Result: Hypothesis 1; The experimental group receiving the treatment of the chest meridian massage will have a much higher post-anesthetic recovery score than that of the control group not receiving it was supported(t=2.544, p=.014). Hypothesis 2; The experimental group receiving the treatment of the chest meridian massage will have a much shorter time of stay in the recovery room than that of the control group not receiving it was supported(t=-4.919, p=.000). Conclusion: According to these results, it may be concluded that Chest Meridian Massage is effective in producing a higher level of post-anesthetic recovery score and helps appendectomy patients reduce the time of stay in the recovery room. Therefore the chest meridian massage can be considered as an intervention therapy for directly nursing general anesthesia appendectomy patients.

The Convergent Development of Nursing Activity Index for postoperative patient using Patient Controlled Analgesia (자가통증조절기를 사용하는 수술 후 환자를 위한 간호 활동 목록의 융합적 개발)

  • The purpose of this study was done to develop a nursing activity index for postoperative patient using Patient-controlled Analgesia(PCA). The study was developed in two steps: $1^{st}$ step) nursing activity index was developed through literature review and focus-group interview. $2^{nd}$ step) the nursing activity was ratified and Intervention Content Validity(ICV) was tested. Subjects were consisted of 5anesthesiologists, 6adult/fundamental nursing professors and 1acute pain management Clinical Nurse Specialist. We identified 12 items of nursing activity for postoperative patient using PCA. The Intervention Content Validity(ICV) score for each item ranged from 0.92 to 1.0. The nursing activity index for postoperative patient using PCA would be very useful in facilitating the nursing practice.
