Cauda Equina Syndrome Occurred by Adhesive Arachnoiditis of the Lumbar Spine with an Unknown Cause (원인 미상의 요추의 유착성 지주막염에 의해 발생한 마미 증후군)

  • Spinal adhesive arachnoiditis is an inflammation and fibrosis of the subarachnoid space and pia mater caused by infection, trauma, spinal vascular anomalies, and iatrogenic (surgery and/or puncture). Adhesive arachnoiditis develops various symptoms and signs (gait disturbances, radiating pain, paralysis, and incontinence). On the other hand, adhesive arachnoiditis associated with cauda equina syndrome has not been reported in Korea until now. The authors experienced cauda equina syndrome caused by adhesive arachnoiditis of the lumbar spine with satisfactory results following decompression. We report this case with a review of the relevant literature.

  • Percutaneous balloon kyphoplasty is an option for pain relief in pathological vertebral compression fractures. Complications related to cement leakage through cortical defects have been reported. On the other hand, dural compression due to retropulsion of the tumor mass is rarely reported. The authors report a case of a 65-year female patient who had cauda equina syndrome after a percutaneous balloon kyphoplasty in a pathological vertebral fracture, even though epidural compression were not found prior to surgery. Magnetic resonance imaging revealed retropulsion of the tumor mass into the spinal canal through the disrupted posterior vertebral cortex.

  • Spinal epidural hematoma (SEH) can occur naturally or traumatically and is most common in patients with an underlying disease of the vascular structure or coagulation disorder. Most SEHs occur naturally for no apparent reason, and epidural hematoma caused by trauma is less common, comprising 1.0%-1.7% of total spinal injuries. Few reports of SEH induced cauda equine syndrome resulting from low-energy injury caused by osteoporotic vertebral compression fractures are available. The authors experienced a case of delayed SEH after hemorrhage due to a low-energy injury in an elderly patient. No cases in Korea have been reported; therefore, this case is reported with a review of the relevant literature.

  • Cauda Equina Syndrom is caused by compression of nerve root resulted from trauma or neoplasm and so forth in the lumbasacral lesion, so the patients suffer from the incontinence or urgency in urination and feces. We are extended to find out the effect of serveral kinds of Herbal Medicine that administrated to the patient suffering from Cauda Equina Syndrom. This patient was differenciated as defiency of Yang. When administrated Palmijihwang-Hwan gami-Bang, there was dominent improvement in both of symptom and dermatome test. From this case it is thought that Herbal Medicine treatment according to differenciation of symptom is very effective to Cauda Equina Syndrom and that more studies and examinations are needed in futher.

  • Objectives : The purpose of this study is to evaluate clinical effect of Korean medical treatments for the patient who were diagnosed as the Cauda Equina Syndrome. Methods : The patient was treated by Korean medical treatments including acupuncture, pharmacopuncture, herbal medication. The Improvement of clinical symptoms was assessed by Visual Analog Scale(VAS), Oswestry Disability Index(ODI) and Muscle Manual Test(MMT). Results : After treatments, VAS and ODI were generally decreased, MMT was improved in case. Conclusions : The Oriental medical treatments might be effective methods to treat the patient who were diagnosed as the Cauda Equina Syndrome.

  • We report a case of cauda equine syndrome following caudal anesthesia possibly caused by metastatic spine tumor. Male, 80-year-old, who had prostatic carcinoma with $L_3$ and $L_4$ spine metastasis was scheduled for bilateral orchiectomy. Twenty two-gauge needle was introduced at sacral hiatus and 15 ml of 2% lidocaine administered. The next morning, patient complained of perineal numbness and urination difficulty. During the next several day patient had episodes of fecal incontinence and motor weakness on both lower extremities. This case reminded us that neuroaxial blocks such as spinal, epidural and caudal anesthesia, should be used with extreme care in patients having neoplasm with high incidence of spine metastasis.

  • Lysis of epidural adhesion has been done in patients with refractory lumbar radiculopathies. Cauda equina syndrome is a rare complication of epidural block. We report on a case of cauda equina syndrome following epidural adhesiolysis in a patient with spinal stenosis. The patient complained of numbness of the perineum, weakness of the left leg, an inability to void and fecal incontinence. She was treated with medication, bladder training and physical therapy, and finally recovered 1 month after the procedure without any sequelae. We suggest that the causative factors are osmotic damage produced by the subarachnoid injection of large doses of hypertonic saline and temporary neural compression due to spinal stenosis.

A Clinical Case Study of Operation-delayed Cauda Equina Syndrome Patient Improved by Korean Traditional Treatment (한방요법으로 호전된 수술이 지연된 마미 증후군 환자의 증례보고)

  • Objectives The objective of this study is to report clinical effect of korean traditional treatments for Operation-delayed Cauda Equina Syndrome patient. Methods The patient was treated by korean traditional treatments including acupuncture, physical treatment, herbal medication. The improvement of the clinical symptoms was observed by Modified Visual analog scale (Modified VAS), SSCES (Scoring System for Cauda Equina Syndrome), function of urination and function of defecation. Results After treatments, Modified VAS, SSCES, function of urination and function of defecation were improved in this case. Conclusions On the basis of these results, we suggest that korean traditional treatment might be an effective method to improve the clinical symptoms of Operation-delayed Cauda Equina Syndrome patient.

  • Objectives : This study was to investigate the effect of the Carthami Semen Herbal acupuncture therapy on a Cauda equina syndrome patient who has a complex of low back pain, bilateral sciatica, saddle anesthesia and motor weakness in the lower extremity and paraplegia with bladder and bowel incontinence. Methods : Oriental Medical Therapy was performed on the Cauda equina syndrome patient from July 15th 2004 to July 29th 2004. The patient was treated with Carthami Semen Herbal acupuncture at BL22, BL23, BL25, BL28 and GV3 in combination with herbal medicine and conventional body acupuncture. We evaluated The Visual Analog Scale(VAS), Improvement index, The Oswestry Diability Index(ODI), gaiting, dyschezia, bladder incontinence, duration of urination and area of anesthesia, Digital Infrared Thermographic Imaging(DITI) before and after treatment. Results : 1. After treatment, VAS, Improvement index, ODI were improved each from 10 to 2, from 21 to 73, from 333 to 166. 2. After treatment, gaiting, bladder incontinence, duration of urination and area of anesthesia and DITI were improved well, but dyschezia was remained. Conclusions : From this case it is thought Carthami Semen herbal acupuncture therapy is very effective to Cauda Equina Syndrome and further study is needed for the confirmation of the effect of Carthami Semen Herbal acupuncture.

Methods : The patient was treated by Oriental medical treatments including acupuncture, ShinBaro pharmacopuncture treatment, herbal medication. The improvement of the clinical symptoms was observed by Visual analog scale(VAS) and Oswestry disability index(ODI). Results : After treatments, VAS and ODI. were improved in case. Conclusions : Oriental medical treatment might be an effective method to improve the clinical symptoms of Cauda Equina Syndrome Patient induced spinal anesthesia.
